Sinusoidal endotheliitis as a histological parameter for diagnosing acute liver allograft rejection

AIM To investigated the feasibility of using sinusoidal endotheliitis(SE) as a histological marker for liver allograft rejection.METHODS We compared the histological features of 88 liver allograft biopsies with acute cellular rejection(ACR) and 59 cases with no evidence of ACR. SE was scored as:(1) focal linear lifting up of the endothelial cells by lymphocytes with no obvious damage to adjacent hepatocytes;(2) focal disruption of the endothelial lining by a cluster of subendothelial lymphocytes(a group of > 3 lymphocytes); and(3) severe confluent endotheliitis with hemorrhage and adjacent hepatocyte loss.RESULTS The sensitivity and specificity of SE was 81% and 85%, respectively. Using SE as the only parameter, the positive predictive value for ACR(PPV) was 0.89, whereas the negative predictive value for ACR(NPV) was 0.75. The correlation between RAI and SE was moderate(R = 0.44, P < 0.001)(Figure 3A), whereas it became strong(R = 0.65, P < 0.001) when correlating SE with the venous endotheliitis activity index only.CONCLUSION Our data suggest that SE scoring could be a reliable and reproducible supplemental parameter to the existing Banff schema for diagnosing acute liver allograft rejection.

CD200:CD200R interactions regulate immune responses. Since CD200Rs show extensive homology in their extracellular region, generating anti-CD200R specific antibodies is a challenge. We report below on the generation of mAbs specific for murine (m)R1/R2 and evidence that mR2 is expressed on the cell surface in the absence of the adaptor protein Dap12. Despite homology between mR1 and mR4, the unexpected reduction in the molecular mass (i.e. 90kDa vs 48kDa) between the two receptors suggested that the TM and cytoplasmic region of mR4 regulated glycosylation. Substitution of the TM and cytoplasmic region of mR1 and mR2 with that of mR4 reduced glycosylation of the chimeric receptors mR1r4 and mR2r4 implying that these regions regulated the glycosylation of mCD200Rs. In activation experiments, phosphorylation of Dap12 following interaction with CD200 occurred on cells expressing mR2V5 but not mR4V5. Similar experiments with the chimeric receptors mR1r2 and mR1r4 also produced phosphorylation of Dap12. Our data suggest that the TM and cytoplasmic region of mCD200Rs dictate their state of glycosylation and provide further evidence that both mCD200R1 and mCD200R2 bind CD200 as ligand with functional consequences for down-stream signaling.

Liver cirrhosis is a common and growing public health problem globally.The diagnosis of cirrhosis portends an increased risk of morbidity and mortality.Liver biopsy is considered the gold standard for diagnosis of cirrhosis and staging of fibrosis.However,despite its universal use,liver biopsy is an invasive and inaccurate gold standard with numerous drawbacks.In order to overcome the limitations of liver biopsy,a number of non-invasive techniques have been investigated for the assessment of cirrhosis.This review will focus on currently available non-invasive markers of cirrhosis.The evidence behind the use of these markers will be highlighted,along with an assessment of diagnostic accuracy and performance characteristics of each test.Non-invasive markers of cirrhosis can be radiologic or serum-based.Radiologic techniques based on ultrasound,magnetic resonance imaging and elastography have been used to assess liver fibrosis.Serum-based biomarkers of cirrhosis have also been developed.These are broadly classified into indirect and direct markers.Indirect biomarkers reflect liver function,which may decline with the onset of cirrhosis.Direct biomarkers,reflect extracellular matrix turnover,and include molecules involved in hepatic fibrogenesis.On the whole,radiologic and serum markers of fibrosis correlate well with biopsy scores,especially when excluding cirrhosis or excluding fibrosis.This feature is certainly clinically useful,and avoids liver biopsy in many cases.

The global coronavirus disease 2019(COVID-19)pandemic is currently ongoing.It is caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2).A high proportion of COVID-19 patients exhibit gastrointestinal manifestations such as diarrhea,nausea,or vomiting.Moreover,the respiratory and gastrointestinal tracts are the primary habitats of human microbiota and targets for SARS-CoV-2 infection as they express angiotensin-converting enzyme-2(ACE2)and transmembrane protease serine 2(TMPRSS2)at high levels.There is accumulating evidence that the microbiota are significantly altered in patients with COVID-19 and post-acute COVID-19 syndrome(PACS).

BACKGROUND Recently,neonatal abstinence syndrome(NAS)emerged as a significant global concern with a dramatic increase in healthcare expenditures.The incidence of the NAS has increased notably in the past decade and emergence as a global public health problem.AIM To evaluate the development and trend of global NAS research from 1958 to 2019 by bibliometric analysis.METHODS Analyzed aspects included publication output per year,language,document types,journals,countries/territories,h-index,authors,and top research priorities.The VOSviewer was used to determine the top research priorities,and trends,and to present bibliometric networks concerning various dimensions,such as coauthorship,authors,and countries.RESULTS A total of 1738 articles were retrieved in the Scopus database from 1958 to 2019.It was found that the great majority of the total NAS documents(n=1295)were original articles followed by reviews(n=268)and letters(n=48).The most productive countries in the NAS field were the United States(n=833),Canada(n=112),the United Kingdom(n=111),and Germany(n=77).Treatment and hospital outcomes in NAS,evidence-based nurse-driven interventions for the care of newborns with NAS,and a systematic reviews and network meta-analysis for therapeutic approaches of NAS were found in recent years(after 2010),compared with terms such as pathophysiology,mechanisms of NAS,and signs and symptoms in the early years.CONCLUSION Treatment and pediatric outcomes and the effectiveness of pharmacological treatment may be frontiers in the NAS field,and continued efforts from researchers are needed in those topics.

Background:Osteoarthritis is one of the leading causes of pain and disability worldwide,and a large percentage of patients with osteoarthritis are individuals who are also obese.In recent years,a series of animal models have demonstrated that obesity-inducing diets can result in synovial joint damage(both with and without the superimposition of trauma),which may be related to changes in percentage of body fat and a series of low-level systemic inflammatory mediators.Of note,there is a disparity between whether the dietary challenges commence at weaning,representing a weanling onset,or at skeletal maturity,representing an adult onset of obesity.We wished to evaluate the effect ofthe dietary exposure time and the age at which animals are exposed to a high-fat and high-sucrose(HFS) diet to determine whether these factors may result in disparate outcomes,as there is evidence suggesting that these factors result in differential metabolic disturbances.Based on dietary exposure time,we hypothesized that rats fed an HFS diet for 14 weeks from weaning(HFS Weanling) would demonstrate an increase in knee joint damage scores,whereas rats exposed to the HFS diet for 4 weeks,starting at 12 weeks of age(HFS Adult) and rats exposed to a standard chow diet(Chow)would not display an increase in knee joint damage scores.Methods:Male Sprague-Dawley rats were fed either an HFS diet for 14 weeks from weaning(HFS Weanling) or an HFS diet for 4 weeks,starting at 12 weeks of age(HFS Adult).At sacrifice,joints were scored using the modified Mankin Criteria,and serum was analyzed for a defined subset of inflammatory markers(Interleukin-6,leptin,monocyte chemoattractant protein-1,and tumor necrosis factorα).Results:When the HFS Weanling and HFS Adult groups were compared,both groups had a similar percent of body fat,although the HFS Weanling group had a significantly greater body mass than the HFS Adult group.The HFS Weanling and HFS Adult animals had a significant increase in body mass and percentage of body fat when compared to the Chow g

Transforming growth factor-beta（TGF-β）/bone morphogenetic protein（BMP） plays a fundamental role in the regulation of bone organogenesis through the activation of receptor serine/threonine kinases. Perturbations of TGF-β/BMP activity are almost invariably linked to a wide variety of clinical outcomes, i.e., skeletal, extra skeletal anomalies, autoimmune, cancer, and cardiovascular diseases. Phosphorylation of TGF-β（I/II） or BMP receptors activates intracellular downstream Smads, the transducer of TGF-β/BMP signals. This signaling is modulated by various factors and pathways, including transcription factor Runx2. The signaling network in skeletal development and bone formation is overwhelmingly complex and highly time and space specific.Additive, positive, negative, or synergistic effects are observed when TGF-β/BMP interacts with the pathways of MAPK, Wnt, Hedgehog（Hh）, Notch, Akt/m TOR, and mi RNA to regulate the effects of BMP-induced signaling in bone dynamics. Accumulating evidence indicates that Runx2 is the key integrator, whereas Hh is a possible modulator, mi RNAs are regulators, and b-catenin is a mediator/regulator within the extensive intracellular network. This review focuses on the activation of BMP signaling and interaction with other regulatory components and pathways highlighting the molecular mechanisms regarding TGF-β/BMP function and regulation that could allow understanding the complexity of bone tissue dynamics.
