Delivering high quality dietary protein at an affordable price is a major aim of the EU-funded CHANCE project. Foods have been formulated with this aim and as part of their nutritional assessment;the bioaccessbility of nutrients following simulated gastroduodenal digestion is being investigated. Nutrimetabolomics approaches can be used to comprehensively and quantitatively analyse nutrients and metabolites. They have been applied to monitor nutrient release from ham, formulated in the CHANCE project, during in vitro digestion. SDS-PAGE analysis shows that constituent ham proteins were broken down to lower molecular weight polypeptides (Mr ≤ 10 kDa) after 120 min simulated gastric digestion which was digested further by subsequent duodenal digestion. Digestion of porteins resulted in the appearance of coalesced lipid droplets associated with the loss of the muscle protein matrix of the ham. Important nutrients, such as choline, creatine, carnosine, sucrose, cholesterol, triacylglyceride and fatty acids (saturated and unsaturated) were identified using 1H NMR. Chance ham is a good source of dietary protein and the combined approach can provide representative data on the bioaccessibility of all detectable nutrients contained in CHANCE ham to human digestive system.

Introduction: Studies conducted in Benin have often not emphasized the living environment underlying hypercholesterolemia. The objective was to study the prevalence and factors associated with hypercholesterolemia in the Mono and Couffo departments in 2015 with consideration of the living environment. Methods: Descriptive cross-sectional study with an analytical aim that involved 2490 subjects aged 18 - 69 years, selected using a three-stage randomized sampling technique. data were collected using the Personal Digital Assistant (WHO STEPS instrument) and analyzed according to STEPS recommendations using Epi-Info7.***.5.0 and SPSS20 software. The Chi-square test was used to compare proportions and the difference was considered significant for p Results: Out of 2490 respondents, 60.20% were women. The mean age was 36.14 ± 12.82 years. The prevalence of hypercholesterolemia was 4.7% (95% CI: [3.88 - 5.54]) and lower in the Couffo. Factors associated with hypercholesterolemia after multivariate analysis were harmful alcohol consumption, high blood pressure, type 2 diabetes, obesity and overweight. Conclusions: Living environment was found to be one of the important factors to consider in strategies to control hypercholesterolemia in the Mono and Couffo departments.

It is predicted that the distribution on score of children＇s motor performance show abnormal curve, because their physical fitness shows decline and gap between good and poor. The aim of present study is to type pattern of distribution curve on physique and motor performance （PMP） in preschoolers, and to investigate gender difference, and change in season and age on distribution of PMP. Seven hundred and seven preschoolers participate in measurement on 21 items of PMP. And mix-longitudinal data which made from 6 terms （2 seasons of spring and autumn for 3 years） is completed. The histograms of 10 sections are trained for each gender, term, and test item based on range of data including mean ± 3 standard deviation. By cluster analysis and multi-correspondence analysis, ＂physique ＆ jumping＂ show closely normal curve, but ＂manipulation （MP）＂ is skewed to poor and ＂running ＆ prompt （RP）＂ is skewed to good. By Hotelling＇s T2-test and Mahalanobis＇ Distance, gender difference is shown that boys＇ RP is skewed to good, and their ＂throwing＂ and ＂weight ＆ flexibility （WF）＂ are skewed to poor. About season, WF is skewed to poor and RP is skewed to good, in spring. In annual change about pattern of distribution, skewing poor in MP and good in ＂running straight ＆ prompt＂ are changed to normal curve with aging.

The characteristics of typical AE signals initiated by mechanical component damages are analyzed. Based on the extracting principle of acoustic emission(AE) signals from damaged components,the paper introduces Wigner high-order spectra to the field of feature extraction and fault diagnosis of AE signals. Some main performances of Wigner binary spectra,Wigner triple spectra and Wigner-Ville distribution (WVD) are discussed,including of time-frequency resolution,energy accumulation,reduction of crossing items and noise elimination. Wigner triple spectra is employed to the fault diagnosis of rolling bearings with AE techniques. The fault features reading from experimental data analysis are clear,accurate and intuitionistic. The validity and accuracy of Wigner high-order spectra methods proposed agree quite well with simulation results. Simulation and research results indicate that wigner high-order spectra is quite useful for condition monitoring and fault diagnosis in conjunction with AE technique,and has very important research and application values in feature extraction and faults diagnosis based on AE signals due to mechanical component damages.

Therapeutic Itinerary and Causes of Delay in Specialized Consultation of Patients Admitted for Diabetic Foot

Introduction: Amputation remains a common practice in patients admitted for diabetic foot. The delay in consultation is one of the main reasons. The objective of this work was to analyze patients’ itinerary care and to identify the causes of delay in consultation in patients admitted for diabetic foot. Method: This is a descriptive cross-sectional study, including all patients hospitalized for diabetic foot. A wait of at least seven (7) days before consulting the Endocrinology department of Hubert Koutoukou Maga Teaching Hospital (CNHU-HKM) after the foot injury occurred was considered as delay in consultation. The data were recorded and analyzed in the EPI INFO software. The description of the sample was made according to the usual statistics (frequency, average, standard deviation). Results: Seventy-six (76) diabetic foot cases were admitted during the period of the study. The mean age of the patients was 56.64 ± 10.71 years with extremes of 29 years and 84 years. Patients were most often received at an advanced stage of the foot injury, 3D stage of Texas classification in 87% of cases and Wagner grades 4 and 5 in 52% of cases. The amputation rate was 38.16%. An analysis of the first care places reveals that 90.8% of patients had consulted conventional medicine, 13.2% in traditional medicine;1.2% had religious practices and 18.7% had declared self-medication at home. Consultation period at CNHU HKM was more than 7 days in 80% of cases and less than 7 days in 20% of cases. The average consultation time was 44.08 ± 26.43 days with extremes of 1 and 480 days. The main reasons for the delay in seeking hospital care cited by diabetic patients were fear of amputation (47.8% of cases), economic difficulties (32.5% of cases) and awareness lack of the situation seriousness (22.4%). Conclusion: The therapeutic itinerary of patients with diabetic foot was particularly long. The main reasons of late appeal to specialized care were fear of amputation and economic difficulties.

BACKGROUND Per-oral pancreatoscopy(POP) plays a role in the diagnosis and therapy of pancreatic diseases. With recent technological advances, there has been renewed interest in this *** To evaluate the efficacy and safety of POP in management of pancreatic stone disease and pancreatic ductal *** To determine the safety and efficacy of POP in the management of pancreatic diseases, a systematic search was conducted in MEDLINE, EMBASE and *** in languages other than English and case reports were excluded. All published case series were eligible. data specific to POP were extracted from studies, which combined cholangiopancreatoscopy. Ten studies were included in the analysis of POP therapy for pancreatic stone disease, and 15 case series satisfied the criteria for inclusion for the role of POP in the management of pancreatic ductal neoplasia. The examined data were subcategorized according to adjunctive modalities, such as direct tissue sampling, cytology, the role of intraoperative POP, intraductal ultrasound(IDUS) and POP combined with image-enhancing *** The success rate for complete ductal stone clearance ranged from 37.5%-100%.Factors associated with failure included the presence of strictures, multiple stonesand the inability to visualize the target area. Although direct visualization can identify malignant and premalignant conditions, there is significant overlap with benign diseases. Visually-directed biopsies provide a high degree of accuracy,and represent a unique approach for tissue acquisition in patients with ductal abnormalities. Addition of pancreatic fluid cytology increases diagnostic yield for indeterminate lesions. Protrusions larger than 3 mm noted on IDUS are significantly more likely to be associated with malignancy. Introduction: There has been an increase in the proportion of cesarean deliveries, especially in developing countries. Methods: Cross-sectional study of all hospital live births from mothers living in Sao Paulo Metropolitan Area, Brazil. data were obtained from the Live Birth Information System, which is linked to the National database of health Establishments. Hospitals were classified as public, private or mixed. Descriptive analysis and exponential regression were conducted to evaluate time trends. Poisson regression was applied to analyze each hospital type to identify risk factors and the attributable risk fraction for cesarean section. Results: There was an annual increase (1.4%) of cesarean deliveries between 2000 and2013. In2009, the percentage of cesarean deliveries was 53% overall and 83% in private hospitals. The primary risk factor for cesarean delivery was delivery in a private hospital. Other risk factors that were stronger in public hospitals included the following: advanced maternal age, high maternal education, nulliparity, high number of prenatal visits, multiple pregnancy, hospital with low annual volume of deliveries, birth outside the city of residence and white skin color. Discussion: These results may be explained by the obstetric care model of private hospitals, which is not multiprofessional. Prenatal care and delivery are conducted by the same doctor and rarely include the input of another professional, such as a midwife. In addition, the mode of delivery influences the professional’s payment.

Several studies investigated associations of IFN-γ rs2430561 T/A,IL28 B rs12979860 C/T and ERα rs2077647 T/C gene polymorphisms with outcomes of hepatitis B virus（HBV） infection,but the results were ***,we performed a meta-analysis of all published observational studies to address this *** was searched in online database and a systematic review was conducted based on the search results.A total of 24 studies were included and dichotomous data were presented as odds ratio（OR） with a 95%confidence interval（CI）.The rs2430561 T allele was associated with reduced persistent HBV infection risk（T vs.A：OR,0.690;95%CI,[0.490,0.971]）,while the rs2077647 T allele significantly increased the risk of persistent HBV infection（T vs.C：OR.1.678;95%CI,[1.212,2.3231）.Rs 2077647 CC might play a role in protecting individuals against HBV persistence（TT ***：OR,4.109;95%CI,[2.609,6.473]）.Furthermore,carriers of the rs2430561 TT genotype were more likely to clear HBV spontaneously compared with those of the AA genotype（TT ***：OR,0.555;95%CI,[0.359,0.856]）.For rs12979860 C/T polymorphism,no significant correlation with HBV infection outcomes was *** subgroup analyses,the results were similar to those of overall ***,for rs2077647 TT ***＋CC,significantly increased risks were observed in the Asian and hospital-based population,but not in the overall ***-γrs2430561 T/A and ERα rs2077647 T/C genetic polymorphisms were associated with outcomes of HBV infection,but no association was found between IL28 B rs12979860 C/T and HBV infection.
