D-transposition of the great arteries(d-TGA)is surgically repaired with the arterial switch operation(ASO)with excellent results,however short and long-term morbidities still develop including neurocognitive *** significant central sleep apnea is uncommon in non-premature infants,but when present indicates immature autonomic control of respiration likely due to a neurologic *** report the unanticipatedfinding of central sleep apnea in four-term neonates with d-TGA after uncomplicated ASO,with the short-term complication of delayed hospital discharge and long-term concerns regarding this early marker of brain immaturity and its hindrance to normal *** this report,we will review each patient’s clinical course and then examine the literature on pediatric central sleep apnea,neurodevelopmental outcomes after ASO,and the important overlap of these entities in the care of patients going forward.

Background and objective EGFR-tyrosine kinase inhibitors(EGFR-TKIs) were used to treat non-small cell lung cancer(NSCLC) patients with EGFR mutation positive. This study aims to compare the effectiveness of first line TKIs;gefitinib, erlotinib, and afatinib in the treatment of advanced stage NSCLC patients with EGFR mutation positive in the Indonesian *** A retrospective cohort study of 88 NSCLC patients with EGFR mutation positive treated with gefitinib(n=59), erlotinib(n=22), and afatinib(n=7) was performed in national cancer hospital in *** criteria were stage IIIb or IV NSCLC with adenocarcinoma subtype. Subjects less than 18 years or with a history of other malignancy were excluded. Outcomes were treatment response, progression-free survival(PFS), and mortality rate. Results Complete response, partial response, and stable disease were shown in 1.1%, 35.2%, and 31.8% of subjects, respectively. There were 31.8% of subjects developed progressive disease during treatment. Regarding EGFR mutation positive profile, a total of 56.8% subjects had deletion in exon 19, 42% subjects had mutation in exon 21, and rare mutation in exon 18 was found in 3.4% of total subjects. Demography and clinical characteristics had no significant association with the risk of progressive disease. The median PFS of subjects was 11 months(95%CI: 6.8-15.2 months). There was no statistical difference of PFS between treatment *** Gefitinib, erlotinib, and afatinib have similar effectiveness in advanced stage NSCLC with EGFR mutation positive. Afatinib tends to be associated with longer PFS but further investigation is required.

Background Several children are affected by airway foreign body aspiration(FBA)resulting from life-threatening *** has been considered the major symptom and is defined as airway blockage by a foreign body(FB),leading to marked morbidity or *** retrospective study indicates the possibility of misdiagnosis or rigid bronchoscope(RB)failure,which is the standard gold method for extracting FB in the *** Six children with airway FBA who failed treatment using RB between 2018 and 2019 were retrospectively *** inclusion criterion was a history of failure to extract FB using RB followed by flexible fiberoptic bronchoscopy(FFB).Results In the present study,among 63 patients who had undergone rigid bronchoscopy,airway FBs were successfully removed in 57(90.48%).Rigid bronchoscopy failed in 6(9.52%)*** age of cases at the time of bronchoscopy ranged from 11 months to 13 *** was performed to extract missing or remaining FBs and was done successfully in all *** patients made an uneventful recovery following FB extraction using the FFB *** It is not easy to diagnose and treat airway FB in *** bronchoscopy has been approved as a method to manage airway FB,but a negative bronchoscopy result must usually be interpreted *** is applicable as a proper and relatively safe diagnostic and therapeutic tool in managing airway FBs among the pediatric population,especially in cases where rigid bronchoscopy was performed but missed or failed to extract the FB.

Summary: The upper airway narrowing and changes in head posture and their relationship with apnea seventy in patients with obstructive sleep apnea (OSA) were investigated. In 86 male OSA patients and 37 healthy men, one-night polysomnographic examination was performed and a lateral cephalogram by digital image processing system was taken in each subject. Fifteen variables concerning the upper airway dimensions, area and head postures were measured by using a computer software (NIH Image). The results showed that upper airway dimensions in the OSA group at all levels were significantly smaller than those in the control group and the results hold true when the age and body mass index were well controlled in these two groups. Significant forward inclination of the cervical column was found in the patients with an apnea index (AI) greater than 35 episodes/h. And changes in the head posture variables in the whole study group were significantly correlated with AI and airway dimensions at various levels. It was suggested that there exist significant and extensive upper airway narrowing in OSA patients even in upright position and awake state; And as the apnea seventy progresses, patients may assume certain compensatory head postures in an attempt to maintain an adequate airway patency.

BACKGROUND Noninvasive ventilation(NIV)reduces intubation rates,mortalities,and lengths of hospital and intensive care unit stays in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Helmet-based NIV is better tolerated than oronasal mask-based ventilation,and thus,allows NIV to be conducted for prolonged periods at higher pressures with minimal air *** SUMMARY A 73-year-old man with a previous diagnosis of COPD stage 4 was admitted to our medical intensive care unit with chief complaints of cough,sputum,and dyspnea of several days’*** 10 mo,he had been on oxygen at home by day and had used an oronasal mask-based NIV at *** intensive care unit admission,he breathed using respiratory accessory *** and signs of infection were detected,and infiltration was observed in the right lower lung field by chest ***,we diagnosed AECOPD by communityacquired *** admission,respiratory distress steadily deteriorated and invasive mechanical ventilation became ***,the patient refused this option,and thus,we selected helmet-based NIV as a salvage *** 3 d of helmet-based NIV,his consciousness level and hypercapnia recovered to his pre-hospitalization *** Helmet-based NIV could be considered as a salvage treatment when AECOPD patients refuse invasive mechanical ventilation and oronasal mask-based NIV is ineffective.

Anemia and Hematologic Characteristics in Newly Diagnosed Pulmonary Tuberculosis Patients at Diagnosis in Kinshasa

Context: The burden of TB in Africa tends to be exacerbated by the socio-economic situation and the high prevalence of intercurrent infections such as HIV, malaria and non-specific bacterial infections. These factors often result in anemia, making patients at high risk for anemia. Objective: We aimed to gain insights into the characteristics of anemia, hematologic variations and socio-economic status in untreated pulmonary TB patients (PTB) in Kinshasa, the Democratic Republic of Congo. Methods: We conducted a cross-sectional analysis of 200 smear-positive pulmonary TB patients (PTB) recruited at the initiation of TB treatment. Complete Blood Count, Iron profile, BMI, CRP and albuminemia were assessed. Data were analyzed using Student t or Mann Whitney tests as appropriate, and logistic regression was performed to assess the strength of associations. Results: Anemia was a regular finding in (69%). This anemia was mostly moderate (92.2%) and with iron deficiency pattern (48%). Hypoalbuminemia was observed in half of the subjects and appears to be correlated with the severity of anemia. Surprisingly, the severity of inflammation, as reflected by the CRP, was inversely correlated with the anemia. In the multivariate analysis, alcohol intake (OR: 2.38;IC 95%: 1.05 - 5.38), hypoalbuminemia (OR: 1.98;IC 95%: 1.02 - 3.82) and CRP rate were significantly associated with the presence of anemia among pulmonary tuberculosis at the diagnostic. Conclusion: This study demonstrates the heavy burden of the iron responsive anemia and risky life conditions in newly diagnosed TB patients, and underscores the potential usefulness of iron supplementation in the Congolese context.

Thirteen parameters of respiratory function including spiro-gram, pulmonary capacities, V-V curve and respiratory resistancewere measured in 52 cases with chronic obstructive pulmonarydisease, of whom 27 cases were simple chronic blonchitis and 25cases were chronic bronchitis complicated with ***-four healthy subjects were taken as control. A new discriminantformula was derived from stepwise regression .Eight indices includingAge, FEV, %MVV, MVV, Rrs, V50/H,RV and RV/TLC% weretaken into consideration in the discriminant formula. It seemedthat the formula might be applied to evaluate respiratory functionproperly in patients with chronic bronchitis complicated withemphysema, however, it has little significance in assessing thatin patients with chronic bronchitis, because the above changes inrespiratory function were more obvious and rersistent in theformer group of patients than in the latter group.
