Sildenafil and tadalafil are efficacious and well tolerated in Chinese men with erectile dysfunction （ED）. Recent study results indicate that men with ED in China who were naive to phosphodiesterase inhibitor type 5 （PDE5） therapy prefer tadalafil 20-mg （on-demand） versus sildenafil 100-mg （on-demand）. Differences in psychosocial outcomes may help to explain treatment preference in favor of tadalafih This open-label, randomized, crossover study compared psychosocial outcomes and drug attribute choices between tadalafil and sildenafil in Chinese men with ED na＇（ve to PDE5 inhibitor therapy. Eligible patients were randomized to sequential 20-mg tadalafU/lOO-mg sildenafil （n = 190） or 100-mg sildenafil/20-mg tadalafil （n = 193） for 8 weeks each and were asked which treatment they preferred to take for the 8-week extension phase. Psychosocial outcomes were assessed using the Psychological and Interpersonal Relationship Scale （PAIRS）, Drug Attributes Questionnaire （DRAQ）, and Sexual Life Quality Questionnaire （SLQQ）. When taking tadalafil versus sildenafil, men had a higher mean endpoint score on the PAIRS Spontaneity Domain （tadalafil = 2.86 vs sildenafil = 2.72; P 〈 0.001）, and a lower mean endpoint score on the Time Concerns Domain （tadalafil = 2.41 vs sildenafil = 2.55; P 〈 0.001）. A numerical increase in the Sexual Self-Confidence Domain was observed when taking tadalafil versus sildenafil （tadalafil -- 2.76 vs sildenafil = 2.72; P= 0.102）. The most frequently chosen drug attributes explaining treatment preference were able to get an erection long after having drug, and ability to get an erection every time. SLQQ results were comparable between treatment groups. These psychosocial outcomes may explain why more Chinese men preferred tadalafil versus sildenafil for the treatment of ED in this clinical trial.

Background Many factors interfering with a listener attempting to grasp speech in noisy environments. The spatial hearing by which speech and noise can be spatially separated may play a crucial role in speech recognition in the presence of competing noise. This study aimed to assess whether, and to what degree, spatial hearing benefit speech recognition in young normal-hearing participants in both quiet and noisy environments. Methods Twenty-eight young participants were tested by Mandarin Hearing In Noise Test （MHINT） in quiet and noisy environments. The assessment method used was characterized by modifications of speech and noise configurations, as well as by changes of speech presentation mode. The benefit of spatial hearing was measured by speech recognition threshold （SRT） variation between speech condition 1 （SC1） and speech condition 2 （SC2）. Results There was no significant difference found in the SRT between SC1 and SC2 in quiet. SRT in SC1 was about 4.2 dB lower than that in SC2, both in speech-shaped and four-babble noise conditions. SRTs measured in both SC1 and SC2 were lower in the speech-shaped noise condition than in the four-babble noise condition. Conclusion Spatial hearing in young normal-hearing participants contribute to speech recognition in noisy environments, but provide no benefit to speech recognition in quiet environments, which may be due to the offset of auditory extrinsic redundancy against the lack of spatial hearing.

Management of unresectable urothelial cancer(UC)has been a clinical challenge for *** drug resistance is a key issue,precise understanding of biology of UC metastasis is another challenge for the improvement of treatment outcome of UC *** of the cell biology concepts including epithelial-mesenchymal transition(EMT)and cancer stemness seems to explain UC *** genetics based on gene expression profiling,next generation sequencing,and explosion of non-coding RNA world has opened the door to intrinsic molecular subtyping of *** steps include,based on the recently accumulated understanding,the establishment of novel disease models representing UC metastasis in various experimental platforms,particularly in vivo animal ***,novel knowledge molecular genetics has not been fully linked to the modeling of UC *** understanding of bladder carcinogenesis is needed particularly with regard to cell of origin related to tumor characteristics including driver gene alterations,pathological differentiations,and metastatic *** we will be able to establish better disease models,which will consequently lead us to further understanding of biology and eventually the development of novel therapeutic strategies for UC metastasis.

AIM: To clarify the value of combined use of markers for the diagnosis of gallbladder cancer and prediction of its prognosis. METHODS: Serum cancer antigens (CA) 199, CA242, carcinoembryonic antigen (CEA), and CA125 levels were measured in 78 patients with gallbladder cancer (GBC), 78 patients with benign gallbladder diseases, and 78 healthy controls using electrochemiluminescence. CA199, CA242, CEA, and CA125 levels and positive rates were analyzed and evaluated pre-and post-operatively. Receiver operator characteristic curves were used to determine diagnostic sensitivity and specificity of GBC. Survival time analysis, including survival curves, and multivariate survival analysis of a Cox proportional hazards model was performed to evaluate independent prognostic factors. RESULTS: Serum CA242, CA125, and CA199 levels in the GBC group were significantly higher when compared with those in the benign gallbladder disease and healthy control groups (P < 0.01). With a single tumor marker for GBC diagnosis, the sensitivity of CA199 was the highest (71.7%), with the highest specificity being in CA242 (98.7%). Diagnostic accuracy was highest with a combination of CA199, CA242, and CA125 (69.2%). CA242 could be regarded as a tumor marker of GBC infiltration in the early stage. The sensitivity of CA199 and CA242 increased with progression of GBC and advanced lymph node metastasis (P < 0.05). The 78 GBC patients were followed up for 6-12 mo (mean: 8 mo), during which time serum CA199, CA125, and CA242 levels in the recurrence group were significantly higher than in patients without recurrence (P < 0.01). The post-operative serum CA199, CA125, and CA242 levels in the non- recurrence group were significantly lower than those in the GBC group (P < 0.01). Multivariate survival analysis using a Cox proportional hazards model showed that cancer of the gallbladder neck and CA199 expression level were independent prognostic factors. Maturation arrest （MA） refers to failure of germ cell development leading to clinical nonobstructive azoospermia. Although the azoospermic factor （AZF） region of the human Y chromosome is clearly implicated in some cases, thus far very little is known about which individual Y-chromosome genes are important for complete male germ cell development. We sought to identify single genes on the Y chromosome that may be implicated in the pathogenesis of nonobstructive azoospermia associated with MA in the American population. Genotype-phenotype analysis of 132 men with Y-chromosome microdeletions was performed. Protein-coding genes associated with MA were identified by visual analysis of a genotype-phenotype map. Genes associated with MA were selected as those genes within a segment of the Y chromosome that, when completely or partially deleted, were always associated with MA and absence of retrievable testicular sperm. Expression of each identified gene transcript was then measured with quantitative RT-PCR in testicular tissue from separate cohorts of patients with idiopathic MA and obstructive azoospermia. Ten candidate genes for association with MA were identified within an 8.4-Mb segment of the Y chromosome overlapping the AZFb region. CDY2and HSFYwere the only identified genes for which differences in expression were observed between the MA and obstructive azoospermia cohorts. Men with obstructive azoospermia had 12-fold higher relative expression of CDY2transcript （1.33__.0.40 vs. 0.11＋_0.04; P=O.O003） and 16-fold higher expression of HSFYtranscript （0.78__.0.32 vs. 0.05_0.02; P=O.O005） compared to men with MA. CDY2 and HSFYwere also underexpressed in patients with Sertoli cell only syndrome. These data indicate that CDY2and HSFYare located within a segment of the Y chromosome that is important for sperm maturation, and are underexpressed in testicular tissue derived from men with MA. Objective: The relationship between peripheral blood CK-20 mRNA expression and tissue telomerase activity in bladder transitional cell carcinoma (TCCB) was investigated to evaluate the feasibility of their combined detection in early-stage diagnosis and prognosis estimation of TCCB. Methods: the blood CK-20 was detected by semi-nested RT-PCR and telomerase activity in tumor tissue was examined with silver-stained TRAP reaction. Results: the blood CK-20 expression and tissue telomerase activity in TCCB were 41% and 93% respectively. No statistical significance was detected among pathological grading and clinical staging (P>0.05). Positive correlation was shown between CK-20 expression and telomerase activity with the pathologic grade or clinical stage. Conclusion: combined use of blood CK-20 and tissue telomerase activity detections might be of great importance for clinical diagnosis, treatment and prognosis evaluation.

Paraesophageal hernia(PEH)repair is one of the most challenging upper gastrointestinal *** high rate of recurrence is due mostly to the low quality of the crura and size of the hiatal *** an attempt to diminish the recurrence rates,some clinical investigators have begun performing meshreinforced cruroplasty with nonabsorbable meshes like polypropylene or *** main problem with these materials is the occurrence,in some patients,of serious mesh-related morbidities,such as erosions into the stomach and the esophagus,some of which necessitate subsequent esophagectomy or *** meshes can be synthetic or biological and were introduced in recent years for PEH repair with the intent of diminishing the recurrence rates observed after primary repair alone but,theoretically,without the risks of morbidities presented by the nonabsorbable *** current role of absorbable meshes in PEH repair is still under debate,since there are few data regarding their long-term efficacy,particularly in terms of recurrence rates,morbidity,need for revision,and quality of *** this opinion review,we analyze all the presently available evidence of reinforced cruroplasty for PEH repair using nonabsorbable meshes(synthetic or biological),focusing particularly on recurrence rates,mesh-related morbidity,and long-term quality of life.
