A Physics-informed Deep-learning Intensity Prediction Scheme for Tropical Cyclones over the Western North Pacific

Accurate prediction of tropical cyclone(TC)intensity is challenging due to the complex physical processes ***,we introduce a new TC intensity prediction scheme for the western North Pacific(WNP)based on a time-dependent theory of TC intensification,termed the energetically based dynamical system(EBDS)model,together with the use of a long short-term memory(LSTM)neural *** time-dependent theory,TC intensity change is controlled by both the internal dynamics of the TC system and various environmental factors,expressed as environmental dynamical *** LSTM neural network is used to predict the environmental dynamical efficiency in the EBDS model trained using besttrack TC data and global reanalysis data during 1982–*** transfer learning and ensemble methods are used to retrain the scheme using the environmental factors predicted by the Global Forecast System(GFS)of the National centers for Environmental Prediction during 2017–*** predicted environmental dynamical efficiency is finally iterated into the EBDS equations to predict TC *** new scheme is evaluated for TC intensity prediction using both reanalysis data and the GFS prediction *** intensity prediction by the new scheme shows better skill than the official prediction from the China Meteorological Administration(CMA)and those by other state-of-art statistical and dynamical forecast systems,except for the 72-h *** at the longer lead times of 96 h and 120 h,the new scheme has smaller forecast errors,with a more than 30%improvement over the official forecasts.

The high morbidity and mortality rate of individuals with human immunodeficiency virus(HIV)/acquired immunodeficiency syndrome(aids)coinfected with Mycobacterium tuberculosis(MTB)is a tough challenge for current global tuberculosis prevention and control ***/MTB coinfection is more complex than a single infection,and the interaction between the two diseases aggravates the deterioration caused by the disease,resulting in increased hospitalizations and *** screening and early diagnosis facilitate the timely initiation of anti-tuberculosis treatment in HIV/MTB coinfected individuals,thereby reducing transmission and the incidence of adverse *** date,pathogenic detection has remained the gold standard for diagnosing tuberculosis,but its sensitivity and specificity are greatly affected by the body's immune status,which limits its application in the diagnosis of HIV/MTB ***,immunology and molecular detection technology has developed *** detection technologies,such as interferon-γ release assays,interferon-gamma inducible protein 10,and GeneXpert MTB/RIF assay have overcome the limitations of traditional detection methods,significantly improved the sensitivity and specificity of tuberculosis diagnosis,and brought new hope to the detection of HIV/MTB *** this article,the principle,scope of application,and latest research progress of relevant detection methods are reviewed to provide a reference for the early diagnosis of HIV/MTB coinfection.

CircEPSTI1 in peripheral blood as a novel potential biomarker for childhood-onset systemic lupus erythematosus

Background:Circular RNA (circRNA) plays an important role in the pathogenesis of many diseases and can be used as a biomarker for diagnosis or disease monitoring. However, reports on circRNA in childhood-onset systemic lupus erythematosus (cSLE) are limited. Therefore, this study aimed to investigate circEPSTI1 expression in cSLE and evaluate its potential as a biomarker for diagnosing ***:This study included 70 children diagnosed with cSLE, 20 diagnosed with juvenile idiopathic arthritis (JIA), 20 diagnosed with juvenile dermatomyositis (JDM), and 50 healthy children at the Rheumatology Department of Beijing Children's Hospital from January 2019 to December 2019. Quantitative polymerase chain reaction was used to determine circEPSTI1 expression in the children. Correlations between circEPSTI1 and clinical features were assessed using Spearman's correlation test. Additionally, we calculated the receiver operating characteristic curve to assess the diagnostic ***:We found that circEPSTI1 expression was higher in children with cSLE (4.62 ± 3.55) than that in healthy children (1.00 ± 0.45), those with JDM (1.06 ± 0.76), and those with JIA (0.96 ± 0.48). The area of the curve of circEPSTI1 was 0.892 (95% confidence interval [CI]: 0.832-0.952, p < 0.001) to discriminate children with SLE from healthy children, with a specificity of 0.814 and a sensitivity of 0.922. Children with lupus nephritis showed a higher circEPSTI1 expression than healthy children, those with JDM, and those with JIA. In addition, circEPSTI1 expression in children with SLE showed significant correlations with the SLE Disease Activity Index ( p < 0.0001) and C3 concentrations ( p = 0.001). Conclusion:Our study suggests that circEPSTI1 is a promising biomarker for the diagnosis and monitoring of cSLE.

Antibiotic Resistance Profile of Salmonella Strains Isolated at the National Clinical Biology and Public Health Laboratory in Bangui, Central African Republic

In Africa, each year, there are estimated to be more than 91 million cases of salmonellosis and 137,000 cases of death. The problem of antibiotic resistance in Salmonella strains is a threat to public health. The objective of this study is to evaluate the antibiotic resistance profile of Salmonella strains isolated in biological products analyzed at the National Laboratory of Clinical Biology and Public Health (NLCBPH) in Bangui. This is, therefore, a cross-sectional study with a descriptive aim, running from January to December 2022. It focused on the strains of Salmonella isolated and identified in stools, urines, and blood samples. For each strain of Salmonella isolated, an antibiogram was carried out following the recommendations of the French Society of Microbiology (CASFM, 2022). A total of 93 strains of Salmonella have been recorded. The age group 0 - 9 years was 29% and that of >50 years was 11%. The median age of patients was 30 years with a minimum of 1 and a maximum of 78 years. The female gender was more represented at 52.69% than the male gender at 47.31%, i.e. a sex ratio of 0.89 (M/F). Salmonella strains were much more isolated in stools at 62% followed by urines at 29% and blood at 6%. Salmonella arizonae strains were more represented with 52%. Salmonella strains have a resistance rate to Tetracycline of 62.37% followed by Penicillins of 50%. The rate of multi-antibiotic resistance of the Salmonella strains isolated represented 48.38%. Salmonella spp. strains were multi-resistant at 58.69% followed by Salmonella arizonae strains at 47.91%. There is a significant association between the different families of antibiotics and Salmonella strains (p < 0.05). According to the results obtained, Penicillins, Phenicoles, and Cyclins had a high rate of resistance on Salmonella strains. No strain-producing Broad Spectrum Beta-lactamase has been isolated. CT Thresholds for Spinal Bone Mineral Attenuation to Prioritize Eligible Older Women for Osteoporosis Screening

Introduction: Computed tomography (CT) measurements of bone mineral attenuation may be a useful means to identify older women who should be prioritized for bone mineral density screening. Methods: We compared bone mineral attenuation as measured in the L1 vertebra of CT studies to the results of dual-energy x-ray absorptiometry (DEXA) studies to determine what CT attenuation thresholds might yield a reasonable combination of sensitivity and specificity for the detection of osteoporosis. The study was limited to women between the ages of 65 and 75 years who had a DEXA study and a CT that included the L1 or adjacent vertebra performed within 3 years of the DEXA study. Results: There were 1226 women in this study, of whom 452 (38%) had osteoporosis based on a T-score ≤ −2.5 by DEXA. There were 830 CT studies performed with contrast and 396 studies which were performed without contrast. There was a statistically significant difference in the mean HU of those studies performed without contrast compared to those with contrast (unenhanced mean 103 HU versus 125 HU, p < 0.001). Different CT attenuation thresholds provided the most appropriate combination of sensitivity and specificity for the detection of osteoporosis when comparing CT studies performed without or with IV contrast and when all the CT data were used in aggregate. Conclusion: Different thresholds appear necessary when using the mean CT vertebral attenuation to identify older women for preferential referral for DEXA studies.

Effect of the Initial Vortex Structure on Intensity Change During Eyewall Replacement Cycle of Tropical Cyclones:A Numerical Study

This study investigates the effect of the initial tropical cyclone(TC)vortex structure on the intensity change during the eyewall replacement cycle(ERC)of TCs based on two idealized simulations using the Weather Research and Forecasting(WRF)*** show that an initially smaller TC with weaker outer winds experienced a much more drastic intensity change during the ERC than an initially larger TC with stronger outer *** is found that an initially larger TC vortex with stronger outer winds favored the development of more active spiral rainbands outside the outer eyewall,which slowed down the contraction and intensification of the outer eyewall and thus prolonged the duration of the concentric eyewall and slow intensity *** contrast,the initially smaller TC with weaker outer winds corresponded to higher inertial stability in the inner core and weaker inertial stability but stronger filamentation outside the outer *** led to stronger boundary layer inflow,stronger updraft and convection in the outer eyewall,and suppressed convective activity outside the outer *** resulted in the rapid weakening during the formation of the outer eyewall,followed by a rapid re-intensification of the TC during the *** study demonstrates that accurate initialization of the TC structure in numerical models is crucial for predicting changes in TC intensity during the ***,monitoring the activity of spiral rainbands outside the outer eyewall can help to improve short-term intensity forecasts for TCs experiencing ERCs.
