Study Objectives: To determine the association of excessive daytime sleepiness（EDS） with the conversion of neurodegenerative diseases in patients with idiopathic REM sleep behavior disorder（iRBD）.Methods: A total of 179 patients with iRBD（79.1% males,mean age = 66.3 ± 9.8 years） were consecutively ***-five patients with Epworth Sleepiness Scale score ≥14 were defined as having ***,clinical,and polysomnographic data were compared between iRBD patients with and without *** risk of developing neurodegenerative diseases was examined using Cox proportional hazards ***: After a mean follow-up of 5.8 years（SD = 4.3 years）,50（27.9%） patients developed neurodegenerative *** was a significantly higher proportion of conversion in patients with EDS compared to those without EDS（42.2 % vs.23.1%,p =.01）.EDS significantly predicted an increased risk of developing neurodegenerative diseases（adjusted hazard ratios [HR] = 2.56,95% confidence interval [CI] 1.37 to 4.77） after adjusting for age,sex,body mass index,current depression,obstructive sleep apnea,and periodic limb movements during *** analyses demonstrated that EDS predicted the conversion of Parkinson’s disease（PD）（adjusted HR = 3.55,95% CI 1.59 to 7.89） but not dementia（adjusted HR = 1.48,95% CI 0.44 to 4.97）.Conclusions: EDS is associated with an increased risk of developing neurodegenerative diseases,especially PD,in patients with *** findings suggest that EDS is a potential clinical biomarker of α-synucleinopathies in iRBD.

Purpose: The purpose of this study was to investigate the impact of maternal, fetal and obstetric parameters in twin pregnancies due to chorionicity, perinatal morbidity and early neonatal mortality. Methods: Early neonatal outcome parameters were retrospectively analysed in 240 twin pregnancies (51 monochorionic [MC], 189 dichorionic [DC] twins) over a 7.5 years period. Beside chorionicity, we focused on risk factors affecting perinatal morbidity and early neonatal outcome in the overall study cohort and subgroups 1) late preterm and 2) pregnancies conceived by artificial fertilization (IVF/ICSI). Mixed effects logistic regression models were used for multivariate risk analyses. Results: MC vs DC pregnancies showed significantly lower birth weights (p 25% and amniotic inflammation (amniotic infection syndrome [AIS]). To the Editor:Anemia is a common finding among patients admitted to intensive care units(ICUs),nearly 60%of ICU patients had serum hemoglobin levels<12 g/dL at baseline,30%of whom had serum hemoglobin levels<9 g/dL,over 95%of critically ill patients were anemic within 3 days of ICU admission.[1]Moreover,anemia may be aggravated or newly developed in critically ill patients during their ICU stay.

Aim: This study sought to develop a process and methodology that could be a useful clinical and research tool for successfully completing functional magnetic resonance imaging (fMRI) scanning in children with Cerebral Palsy. Method: Six children with CP (mean age of 8.83 years;five with spastic hemiplegia, one with spastic quadriplegia) and three children with typical development (mean age of 9.33 years) completed an fMRI scanning protocol that used real-time motion feedback as a means of minimizing head and trunk motion. Anatomical, resting-state, and motor-task scans were sequentially obtained from each subject. Precentral “hand-knob” regions were identified on the anatomical scan and served as seed regions to reveal the functional connectivity of each subject’s brain as associated with hand movement. Results: Real-time motion feedback aided children in successful completion of resting state scans. Functional connectivity and brain activity mapping were obtained based on anatomical landmarks, and laterality indices were developed based on the obtained functional-connectivity map to specify a dominant side of brain activity that was matched to a clinical profile, despite anatomical variations that occur with Cerebral Palsy. Interpretation: Real-time motion feedback and the development of laterality indices can improve the clinical and research utility of fMRI scanning. What this paper adds: 1) Presents a real-time imaging protocol for fMRI to help children complete scanning;2) Presents an fMRI methodology to obtain laterality indices in the presence of abnormal anatomy;3) Provides findings of LI that match clinical diagnosis.

Human uterine leiomyosarcoma (LMS) develops more often in the muscle tissue layer of the uterine body than in the uterine cervix. The development of gynecologic tumors is often correlated with female hormone secretion;however, the development of uterine LMS is not substantially correlated with hormonal conditions, and the risk factors are not yet known. Importantly, a diagnostic-biomarker, which distinguishes malignant uterine LMS from benign tumor leiomyoma (LMA), is yet to be established. Accordingly, it is necessary to analyze risk factors associated with uterine LMS, to establish a clinical treatment method. Protea some β-ring subunit LMP2/β1i-deficient mice spontaneously develop uterine LMS, with a disease prevalence of ~40% by 14 months of age. We found LMP2/β1i expression to be absent in human uterine LMS, but present in human LMA. Therefore, defective-LMP2/β1i expression may be one of the risk factors for human uterine LMS. LMP2/β1i is a potential diagnostic-biomarker under the combination of candidate molecules, for instance cyclin B1, cyclin E and calponin h1 and ki-67/MIB1 counts for uterine mesenchymal tumors, especially human uterine LMS, and may be a targeted-molecule for a new therapeutic approach.

Human Immunodeficiency Virus (HIV) patients on Antiretroviral Therapy (ART) have shown impressive improvement and death rates have drastically reduced, even though complications still exist. This research carried out a baseline study to determine the nutritional interplay between HIV-drugs and kidney, liver, and heart indices among subjects undergoing HIV treatment and attending the Medical Out-Patient Department of a Federal Medical Center in the North-Eastern part of Nigeria, using a sample size of 50 individuals both male and female, who have been shown to be HIV positive and have been on ART for over 12 months. We analyzed prevalence, risk factors and hepatitis C virus(HCV) genotype distribution in different population groups in Croatia in the context of HCV epidemiology in Europe, with the aim to gather all existing information on HCV infection in Croatia which will be used to advise upon preventive measures. It is estimated that 35000-45000 of the Croatian population is chronically infected with HCV. Like in other European countries, there have been changes in the HCV epidemiology in Croatia over the past few decades. In some risk groups(polytransfused and hemodialysis patients), a significant decrease in the HCV prevalence was observed after the introduction of routine HCV screening of blood/blood products in 1992. Injecting drug users(IDUs) still represent a group with the highest risk for HCV infection with prevalence ranging from 29% to 65%. Compared to the prevalence in theCroatian general population(0.9%), higher prevalence rates were found in prison populations(8.3%-44%), human immunodeficiency virus-infected patients(15%), persons with high-risk sexual behavior(4.6%) and alcohol abusers(2.4%). Low/very low prevalence was reported in children and adolescents(0.3%) as well as in blood donors(0%-0.009%). In addition, distribution of HCV genotypes has changed due to different routes of transmission. In the general population, genotypes 1 and 3 are most widely distributed(60.4%-79.8% and 12.9%-47.9%, respectively). The similar genotype distribution is found in groups with high-risk sexual behavior. Genotype 3 is predominant in Croatian IDUs(60.5%-83.9%) while in the prison population genotypes 3 and 1 are equally distributed(52.4% and 47.6%). Data on HCV prevalence and risk factors for transmission are useful for implementation of preventive measures and HCV screening.
