Treatment of cryptococcal meningitis with low-dose amphotericin B and flucytosine

Background Amphotericin B （0.7 mg/kg） with flucytosine is the standard treatment for cryptococcal ***,the long treatment course can induce adverse reactions in patients; therefore,reducing the dose may decrease such *** performed a retrospective analysis of treatment effects and adverse reactions when amphotericin B （0.4 mg/kg or 0.7 mg/kg per day） and flucytosine were used together to treat HIV-negative patients with cryptococcal *** Retrospective analysis was conducted on inpatients at the First Affiliated hospital,College of medicine,Zhejiang University （January 2005 to December 2009）.Low- or high-dose amphotericin B （0.4 or 0.7 mg/kg per day,respectively） plus flucytosine was *** negative conversion rate of Cryptococcus in the cerebrospinal fluid （CSF）,patient mortality,and the incidence of side effects for the two groups （low- ***-dose） were compared immediately after treatment and 2 and 10 weeks *** were analyzed by the Student＇s t test,chi-square tests using SPSS 12.0 statistical *** Two weeks post-treatment,Cryptococcus negative CSF rates were 78% （18/23） in the low-dose group and 87% （13/15） in the high-dose group （P=0.28）.Ten weeks post-treatment,both groups were *** mortality rate was 8% （2/25） in the low-dose group and 17% （3/18） in the high-dose group （P=-0.25）.There was a statistically significant difference in the incidence of adverse events between the groups,48% （12/25） and 78% （14/18） in the low- and high-dose groups,respectively （P=0.04）.Adverse events that required a change in treatment program in the low-dose group were 12% （3/25） compared to 39% （7/18） in the high-dose group （P=-0.04）.Conclusion Low-dose treatment regimens were better tolerated

Background Polymer coating on coronary stents induces vascular inflammatory response, reduces re-endothelialization, and affects long-term outcome after percutaneous coronary intervention （PCI）. The SERY-1 registry aimed to determine whether a novel polymer-free paclitaxel-eluting microporous Yinyi stent could improve 1-year outcome after index procedure in real-world clinical practice. Methods Clinical and angiographic data and follow-up outcome were collected in 1045 patients who underwent PCI with implantation of 〉1 Yinyi stents between June 2008 and August 2009 at 27 medical centers. The primary endpoint was the cumulative rate of composite major adverse cardiac events （MACE） and the secondary endpoint was the incidence of stent thrombosis at 1 year. Results Overall, 1376 lesions were treated successfully with 1713 Yinyi stents, and 1019 （98.7%） patients received dual antiplatelet therapy for at least 12 months. During 1-year follow-up, 8 patients （0.78%） had cardiac death, 6 （0.58%） suffered non-fatal myocardial infarction, and 46 （4.46%） underwent repeat PCI due to recurrence of angina, resulting in 1-year MACE-free survival of 94.09%. Stent thrombosis occurred in 10 （0.97%） patients, and the rate of Academic Research Consortium （ARC） definite or probable stent thrombosis was 0.78%. Conclusions Polymer-free paclitaxel-eluting microporous Yinyi stent is effective and safe for interventional treatment of coronary artery disease in real-world clinical practice, without recourse to carrier polymer. Potential long-term clinical advantages of this stent deserve further investigation.

Background Neuroblastoma （NB） is one of the most common malignant solid tumors of *** is still not clear whether the apoptosis of tumor cells or the non-tumor cells contributes to the increase of concentration of cytochrome c （Cyt c） in the serum of the cancer *** aim of this research was to identify the source of the Cyt c in the serum when the tumor grows up by subcutaneous inoculation of human NB cells into nude *** We subcutaneously inoculated human NB cells （KP-N-NS） into nude mice and collected the sera of tumor-bearing mice （n=14） and control mice （n=25） 4 weeks later in order to screen for and identify differentially expressed proteins in the *** expressed proteins in the serum were screened by surface-enhanced laser desorption/ionization-time-of-fiight （SELDI-TOF） mass *** The relative intensity of a protein having a mass-to-charge ratio （m/z） of 11 609 was 3338.37±3410.85 in the tumor group and 59.84±40.74 in the control group,indicating that the expression level of this protein in the tumor group was 55.8 times higher than that in the control *** proteins were separated and purified by high-performance liquid chromatography （HPLC）.Liquid chromatography coupled with tandem mass spectrometry （LC-MS/MS） was performed to produce peptide mass fingerprints （PMFs）.Spectrum analysis and a database search revealed that the highly expressed protein （m/z=11605.4） from the serum of tumor-bearing mice was the mouse Cyt *** Increased concentration of Cyt c in the serum of tumor-bearing nude mice might be partially attributed to the secretion of this protein by non-tumor cells.

To the editor： Pulmonary arteriovenous malformation （PAVM） is abnormal communications between the pulmonary arteries andveins, and has been reported to be sporadic or to occur in association with hereditary hemorrhagic telangiectasia （HHT）.1 The patients of PAVM may be asymptomatic, present with hypoxemia, paradoxical thromboembolism, or hemoptysis. Ference and coworkers 2 reported only 6 patients with hemoptysis in a review of 143 patients with PAVM.

To the editor： Systemic sclerosis （SSc） is an autoimmune or multisystem connective tissue disease. It is characterized by autoimmunity, microangiopathy, and fibrosis in skin and internal organs. Sarcoidosis is a systemic inflammatory disease of unknown cause that affects the skin, respiratory tract, heart, eyes, lymphatic system, nervous system, kidney and endocrine systems.2 Coexistence of these two diseases had been reported, but is rare.3 Here, we describe a case of chinese woman who developed sarcoidosis 20 years after diagnosis of limited cutaneous SSc （IcSSc）.

Background Funnel chest has a negative effect on adolescents and it has a strong effect on adolescents＇ psychological and behavior. This study aimed to investigate the psychological characteristics and factors that affect adolescents with funnel chest and to evaluate the relationship between the patients＇ age and their physiological and psychological health. We aimed to establish an age model for maximum surgery benefits for funnel chest patients to provide an objective basis for choosing surgery. Methods The study adopted a general evaluation approach to assess the risk and benefits of minimally invasive surgery for funnel chest. The funnel chest index, the Symptom Checklist-90, and the Eysenck Personality Questionnaire were used as assessment tools to observe physiological and psychological features in funnel chest patients. A sample of 234 adolescents with funnel chest was selected from a third-grade class-A hospital in Beijing. Age groups were adopted as an independent variable, and other factors in funnel chest patients were dependent variables. Results There was a significant difference in the relapse rate for funnel chest in the different age groups （X2=11.883, P=-O.008）. There was a higher relapse rate in patients of 〈10 or 〉-19 years old than in patients of 11-18 years old. There was a significant difference in the SCL-90 total score in the different age groups （F=12.538, P=-0.0001）, the patients older than 13 years had a higher score than those younger than 13 years in the SCL-90. There was a significant difference in the standard score of E （introversion/extraversion） in the different age groups （F=10.06, P=0.0001）. There was also a significance in the funnel chest index before surgery in the different psychological scales （P〈0.01）, with a higher funnel chest index score associated with more obvious psychological trauma. Background The infection of Kaposi＇s sarcoma-associated herpes virus （KSHV） is most likely the cause of clinical Kaposi＇s sarcoma,primary effusion lymphoma,and multi-center Castleman＇s *** infection has very limited epidemiological survey data in China,and its definite mode of transmission remains *** study aimed to determine the infection status and the main transmission route of KSHV in chinese *** An enzyme-linked immunosorbent assay （ELISA） utilizing KSHV ORF65 recombinant protein was employed to analyze the antibody response to KSHV ORF65 in sera from 122 healthy physical examination people,107intravenous drug users,135 non-intravenous drug users,211 hepatitis B （HBV） patients infected via blood transmission,107 kidney transplant recipients,and 72 female sex workers in Zhejiang Province in Southeast *** KSHV infection occurred relatively common （13.1%） in healthy population in Zhejiang,*** rate was 16.7% in female sex workers,but significantly elevated in intravenous drug addicts （58.9%）,blood-transmitted HBV patients （28.0%） and kidney transplant patients （41.1%）.Conclusion Blood borne transmission of KSHV is probably the main route of infection in Zhejiang Province.

Reconstruction of segmental bone defects in the rabbit ulna using periosteum encapsulated mesenchymal stem cells-loaded poly（lactic-co-glycolic acid） scaffolds

Background Repair of large bone defects remains a challenge for clinicians. The present study investigated the ability of mesenchymal stem cells （MSCs） and/or periosteum-loaded poly（lactic-co-glycolic acid）（PLGA） to promote new bone formation within rabbit ulnar segmental bone defects. Methods Rabbit bone marrow-derived MSCs （passage 3） were seeded onto porous PLGA scaffolds. Forty segmental bone defects, each 15 mm in length, were created in the rabbit ulna, from which periosteum was obtained. Bone defects were treated with either PLGA alone （group A）, PLGA ＋ MSCs （group B）, periosteum-wrapped PLGA （group C） or periosteum-wrapped PLGA/MSCs （group D）. At 6 and 12 weeks post-surgery, samples were detected by gross observation, radiological examination （X-ray and micro-CT） and histological analyses. Results Group D, comprising both periosteum and MSCs, showed better bone quality, higher X-ray scores and a greater amount of bone volume compared with the other three groups at each time point （P 〈0.05）. No significant differences in radiological scores and amount of bone volume were found between groups B and C （P 〉0.05）, both of which were significantly higher than group A （P 〈0.05）. Conclusions Implanted MSCs combined with periosteum have a synergistic effect on segmental bone regeneration and that periosteum plays a critical role in the process. Fabrication of angiogenic and osteogenic cellular constructs or tissue-engineered periosteum will have broad applications in bone tissue engineering.

Hererin we present a case of hemoptysis treated with bronchial artery embolization which was complicated by myocardial infarction.
