Purpose:The injury severity score(ISS)and new injury severity score(NISS)have been widely used in trauma ***,which scoring system is better in trauma outcome prediction is still *** purpose of this study is to evaluate the value of the two scoring systems in predicting trauma outcomes,including mortality,intensive care unit(ICU)admission and ICU length of ***:The data were collected retrospectively from three hospitals in Zhejiang province,*** comparisons of NISS and ISS in predicting outcomes were performed by using receiver operator characteristic(ROC)curves and Hosmer-Lemeshow ***:A total of 1825 blunt trauma patients were enrolled in our ***,1243 patients were admitted to ICU,and 215 patients died before *** ISS and NISS were equivalent in predicting mortality(area under ORC curve[AUC]:0.886 vs.0.887,p=0.9113).But for the patients with ISS>25,NISS showed better performance in predicting *** was also significantly better than ISS in predicting ICU admission and prolonged ICU length of ***:NISS outperforms ISS in predicting the outcomes for severe blunt trauma and can be an essential supplement of *** the convenience of NISS in calculation,it is advantageous to promote NISS in China's primary hospitals.

BACKGROUND The treatment outcome of transarterial chemoembolization(TACE)in unresectable hepatocellular carcinoma(HCC)varies greatly due to the clinical heterogeneity of the ***,several prognostic systems have been proposed for risk stratification and candidate identification for first TACE and repeated TACE(re-TACE).AIM To investigate the correlations between prognostic systems and radiological response,compare the predictive abilities,and integrate them in sequence for outcome *** This nationwide multicenter retrospective cohort consisted of 1107 unresectable HCC patients in 15 Chinese tertiary hospitals from January 2010 to May *** Hepatoma Arterial-embolization Prognostic(HAP)score system and its modified versions(mHAP,mHAP2 and mHAP3),as well as the six-and-twelve criteria were compared in terms of their correlations with radiological response and overall survival(OS)prediction for first *** same analyses were conducted in 912 patients receiving re-TACE to evaluate the ART(assessment for re-treatment with TACE)and ABCR(alpha-fetoprotein,Barcelona Clinic Liver Cancer,Child-Pugh and Response)systems for post re-TACE survival(PRTS).RESULTS All the prognostic systems were correlated with radiological response achieved by first TACE,and the six-and-twelve criteria exhibited the highest correlation(Spearman R=0.39,P=0.026)and consistency(Kappa=0.14,P=0.019),with optimal performance by area under the receiver operating characteristic curve of 0.71[95%confidence interval(CI):0.68-0.74].With regard to the prediction of OS,the mHAP3 system identified patients with a favorable outcome with the highest concordance(C)-index of 0.60(95%CI:0.57-0.62)and the best area under the receiver operating characteristic curve at any time point during follow-up;whereas,PRTS was well-predicted by the ABCR system with a C-index of 0.61(95%CI:0.59-0.63),rather than ***,combining the mHAP3 and ABCR systems identified candidates suitable for TA

Cognitive neuropsychology has made great contributions to our understanding of tile neural basis of language since Broca＇s seminal work; the primary neural circuitries for language processing have been documented. Unfortunately, these achievements were almost exclusively from studies on alphabetic languages. Recent behavioral and imaging studies suggest that the neural network involving processes of Chinese is not entirely identical to that in alphabetic languages. In the present study, reading performance in a dyslexic patient, whose native language is Chinese, was systematically examined by a series of cognitive neuropsychological experiments, and theoretical implications of this study was discussed in comparisons with studies in dyslexic patients who use alphabetic scripts.

Background A novel anti-rheumatic drug, T-614, has been shown to have an anti-inflammatory effect and to improve abnormal immunological findings in rheumatoid arthritis （RA）. To assess the safety and efficacy of T-614 versus placebo in patients with active RA we conducted a 24-week clinical study in 280 Chinese patients. Methods In a multicenter, randomized, double blind, placebo controlled study, 280 patients were randomly assigned to receive placebo （n=95） or T-614 at 50 mg （n=93） or 25 mg （n=92） daily. Active disease was defined by 4 of the following 5 criteria：〉5 tender joints, 〉3 swollen joints, morning stiffness lasting for 〉60 minutes, and Westergren erythrocyte sedimentation rate （ESR）〉28 mm/h, the assessment of pain at the rest by patient as moderate or severe. Clinical and laboratory parameters were analyzed at baseline, 2, 4, 6, 12, 18 and 24 weeks. The primary efficacy variable at week 24 was the American College of Rheumatology （ACR） response rate using the intent-to-treat population. Results The ACR response rate was significantly higher in the T-614 treatment group compared with the placebo group within 8 weeks after the initiation of treatment. After 24 weeks, the 25 mg/d and 50 mg/d dosage groups and the placebo group showed 39.13%, 61.29% and 24.21% in ACR20 and 23.91%, 31.18% and 7.37% in ACR50, respectively. A time-response in ACR response was observed, with clear superiority for the 25 mg/d and 50 mg/d dosage groups compared to placebo （P〈0.0001）, and the 50 mg/d dose compared to the 25 mg/d dose （P〈0.05） when using the ACR response analyses after 24 weeks. ESR and c-reactive protein （CRP） were significantly different in the treatment groups after 24 weeks. The incidence of adverse events （AEs） was not significantly higher with T-614 than with placebo, but upper abdominal discomfort, leucopenia, elevated serum alanine aminotransferase （sALT）, skin rash and/or pruritus were more common in the 50 mg and 25 mg dosage groups. BACKGROUND No guideline recommends antiviral therapy for hepatitis B e antigen(HBeAg)-positive chronic hepatitis B patients with persistently normal alanine aminotransferase levels and a high hepatitis B virus(HBV)DNA viral *** To evaluate the feasibility and safety of a Chinese herbal formula as a therapeutic option for chronic HBV *** In total,395 patients(30–65 years old)with confirmed HBeAg-positive chronic hepatitis B infection and persistently normal alanine aminotransferase were randomized to receive either Chinese herbal formula or placebo for 96 *** to evaluate therapeutic efficacy included:(1)HBV DNA levels decreased to less than 4 log10 IU/mL at weeks 48 and 96;and(2)HBeAg clearance and seroconversion rates at weeks 48 and *** HBV DNA levels≤4 log10 IU/mL were 10.05%at week 48 and 18.59%at week 96 in the treatment *** HBeAg clearance and conversion rates were 8.54%and 8.04%at week 48 and 16.08%and 14.57%at week 96,***,HBV DNA levels≤4 log10 IU/mL were 2.55%and 2.55%at weeks 48 and 96,respectively,and the HBeAg clearance rates were 3.06%and 5.61%at weeks 48 and 96,respectively,in the control *** quantitative hepatitis B surface antigen and HBeAg levels at baseline and changes during the treatment period as well as the alanine aminotransferase elevation at weeks 12 and 24 were strong predictors of HBeAg *** High rates of HBV DNA reduction,HBeAg clearance and seroconversion could be achieved with Chinese herbal formula treatments,and the treatments were relatively safe for HBeAg-positive chronic hepatitis B-infected patients with persistently normal alanine *** ability of the compound to modulate host immune function probably contributed to this effect.

Post-transplant diabetes mellitus(PTDM)increases the risk of graft failure and death in liver transplant(LT)*** studies have indicated that enteric dysbiosis mediated by immunosuppressive tacrolimus(TAC)could contribute to glucose disorders,but no data on human recipients with PTDM have been ***,by combining high-throughput shotgun metagenomics sequencing and metabolomics profiling,we characterized the intestinal microbiome(IM)in LT recipient cohort with or without PTDM and deciphered the potential relationship among IM,TAC dosage,and *** comparing with both non-PTDM and classical type 2 diabetes mellitus(T2DM),we identified microbial signatures of PTDM,which was characterized by the enriched Proteobacteria and decreased ***,the altered microbes,as well as the microbial metabolomics,correlated with the dosage of ***,the levels of beneficial microbes associated with PTDM were lowered in recipients with the high TAC trough concentrations(>5 ng·mL^(-1))than those with low ones(<5 ng·mL^(-1)),which was accompanied by reduced faecal metabolites involved in the biosynthesis of a-linolenic acid and arachidonic acid-lowering factors of developing ***,these microbial signatures linked with the extent of glucose disorders in LT *** summary,the faecal microbiome and metabolome differed between PTDM and non-PTDM patients,which were linked with TAC *** study was the first to explore taxonomic alterations and bacterial gene functions to better understand the contribution of the IM to PTDM.

Regulatory T （Treg） cells are necessary for immune system homeostasis and the prevention of autoimmune diseases. Foxp3 is specifically expressed in Treg cells and plays a key role in their differentiation and function. Foxp3＋ Treg cells are consisted of naturally occurring, thymus-derived Treg （nTreg） and peripheral-induced Treg （iTreg） cells that may have different functional characteristics or synergistic roles. AII-trans retinoic acid （atRA）, a vitamin A metabolite, regulates a wide range of biological processes, including cell differentiation and proliferation. Recent studies demonstrated that atRA also regulates the differentiation of T helper （Th） cells and Treg cells. Moreover, atRA also sustains nTreg stability under inflammatory conditions. In this review, we summarize the significant progress of our understanding of the role（s） and mechanisms of atRA in Treg biology.

Objective To identify factors that likely contribute to potentially inappropriate prescriptions(PIPs)among older adults in primary care settings,as well as barriers to medicines optimisation and recommended potential *** Systematic *** criteria Quantitative studies that analysed the factors associated with PIPs among older adults(≥65 years)in primary care settings,and qualitative studies that explored perceived barriers and potential solutions to medicines optimisation for this *** sources PubMed,EMBASE,Scopus,CINAHL,PsycINFO,Web of Science,CNKI and *** Of the 13167 studies identified,50 were included(14 qualitative,34 cross-sectional and 2 cohort).Nearly all quantitative studies examined patient-related non-clinical factors(eg,age)and clinical factors(eg,number of medications)and nine studies examined prescriber-related factors(eg,physician age).A greater number of medications were identified as positively associated with PIPs in 25 quantitative studies,and a higher number of comorbidities,physical comorbidities and psychiatric comorbidities were identified as patient-related clinical risk factors for ***,other factors showed inconsistent associations with the *** to medicines optimisation emerged within four analytical themes:prescriber related(eg,inadequate knowledge,concerns of adverse consequences,clinical inertia,lack of communication),patient related(eg,limited understanding,patient non-adherence,drug dependency),environment related(eg,lack of integrated care,insufficient investment,time constraints)and technology related(eg,complexity of implementation and inapplicable guidance).Recommended potential solutions were based on each theme of the barriers identified accordingly(eg,prescriber-related factors:incorporating training courses into continuing medical education).Conclusions Older adults with more drugs prescribed and comorbidities may have a greater risk of receiving PIPs in t
