A monocloual antibody, SZ-2, reacts specifically on human platelets and megakaryocytes. The plate-lets from 10 normal donors are bound to 15,200±4,100 SZ-2 molecules/platelet. The antigen recognizedby SZ-2 is chymotrypsin-sensitive but neuraminidase-insensitive, and has been identified as glycoproteinIb (GPIb) by an affinity chromatography technique. SZ-2 is different from other monoclonal antibodiesto GPIb. It inhibits not only platelet aggregation induced by ristocetin, but also platelet aggregation in-duced by collagen (type Ⅰ) and by PAF. SZ-2 also inhibits platelet serotonin and β-thromboglobulinrelease in response to these stimuli.

Presepsin teardown- pitfalls of biomarkers in the diagnosis and prognosis of bacterial infection in cirrhosis

AIM To evaluate the diagnostic and prognostic value of presepsin in cirrhosis-associated bacterial infections. METHODS Two hundred and sixteen patients with cirrhosis were enrolled. At admission, the presence of bacterial infections and level of plasma presepsin, serum C-reactive protein(CRP) and procalcitonin(PCT) were evaluated. Patients were followed for three months to assess the possible association between presepsin level and short-term *** Present 34.7 of patients had bacterial infection. Presepsin levels were significantly higher in patients with infection than without(median, 1002 pg/m L vs 477 pg/m L, P 1206 pg/m L sensitivity, specificity, positive predictive values and negative predictive values were as follows: 87.5%, 74.5%, 61.8% and 92.7%. The accuracy of presepsin, however, decreased in advanced stage of the disease or in the presence of renal failure, most probably because of the significantly elevated presepsin levels in non-infected patients. 28-d mortality rate was higher among patients with > 1277 pg/m L compared to those with ≤ 1277 pg/m L(46.9% vs 11.6%, P < 0.001). In a binary logistic regression analysis, however, only PCT(OR = 1.81, 95%CI: 1.09-3.01, P = 0.022) but neither presepsin nor CRP were independent risk factor for 28-d mortality after adjusting with MELD score and leukocyte *** Presepsin is a valuable new biomarker for defining severe infections in cirrhosis, proving same efficacy as PCT. However, it is not a useful marker of short-term mortality.

Seventy-eight burned patients （53 males and 25 females） were classified into 4 groups （mild type: 14 cases, moderate type: 13 cases, severe type: 17 cases and super-severe type: 34 cases） in accordence with criteria established by the Chinese Society for burns. Twenty-two parameters related to platelet, coagulant and anticoagulant factors, fibrinolysis system and hemorrheology were determined at first 24 hours, on third and fifth post-burn days. The results were as follow: Fibrinogen antigen, von Willebrand factor antigen, plasminogen activitor inhibitor activity, thromboxane B2 and hematocrit and whole blood viscosity significantly increased, however antithrombin Ⅲ, protein C antigen, free protein S and 6-keto-prostaglandin F1α markedly decreased compared with normal control at first 24 hours post burn. In addition, fibrinogen antigen, von Willebrand factor antigen, plasminogen activitor inhibitor activity, thromboxane B2 and whole blood viscosity were persistently elevated, however, antithrombin Ⅲ, protein C, free protein S and 6-keto-prostaglandin F1α declined on third and fifth post burn days. These changes resulted in a prethrombotic state, being an important factor in genesis of microthrombosis on burn site and the cause of the deepening and aggravation of burn wound on the third and fifth days in burned patients.

Background von Willebrand factor （vWF） mediates the initial capture of platelets to vascular subendothelium and is essential for platelet aggregation under high fluid shear stress as in arterial stenosis. On release from endothelial cells, vWF is rapidly cleaved by ADAMTS13/vWF-cleaving protease （vWF-CP）. We investigated the clinical significance of changes in plasma vWF and vWF-CP activities in chronic renal disease.Methods Plasma vWF and vWF-CP activities were measured using enzyme-linked immunosorbent assay （ELISA） and residual collagen binding assay respectively in patients with lupus nephritis （n=31）, primary nephritic syndrome （n=25）, diabetic nephropathy （n=45）, chronic glomerulonephritis （n=38） and 40 normal controls. The relation of their levels with pathological and renal status was analyzed.Results In all diseased patients the levels of vWF were significantly higher and vWF-CP activity significantly lower than the controls （both P〈0.01）. vWF in the four subgroups did not correlate with the stage of disease but correlated negatively with vWF-CP activity, vWF-CP activity was not changed two weeks after renal transplantation. Renal biopsy demonstrated that the vWF level in stage IV was higher than in stages II and III while vWF-CP activity was lower in patients with lupus nephritis. After eight-week treatment, the vWF level significantly decreased and the vWF-CP activity significantly increased in systemic lupus erythema, disease activity index 〈9, but not with index 〉9. Even though the vWF-CP activity was significantly lower in membranous nephropathy than in minimal change disease, mesangial proliferative glomerulonephritis or IgA glomerulonephritis, the vWF level was not significantly different. Conclusions The alterations of plasma vWF and vWF-CP activities were associated with different renal pathologies. Acceptance of COVID-19 vaccine and its related determinants in Nigeria:An online survey

＜正＞ Eicosanoids are metabolites derived from arachidonic acid （AA）. Different enzymatic pathways transform AA into either prostanoids （prostaglandins （PG） and thromboxane）, leukotrienes （LT）, epoxyeicosatrienoic acids （EETs）, hydroxye-icosatetraenoic acids （ HETEs） and lipoxins. In addition, free-radical oxidation products of AA （E2 isoprostanes） may also be formed. However, for these numerous metabolites only some fifteen eicosanoid receptors present on the cell membranes have been described. The prostanoids activate 8 receptors （IP, EP1-4, DP, EP, IP）, isoprostanes act also on the TP receptor and the LTs activate 4 receptors （CysLTl, 2;BLT1, 2）. In addition, lipoxin, 5-oxo-ETE and PGD2 stimulate the ALX, OXE and CRTH2 receptors, respectively. Finally, at the nuclear level, the eicosanoids are also potent activators of gene transcription via the PPAR receptors. Activation of the eicosanoid receptors may result in vasoconstric-tion. vasodilatation, angiogenesis, migration and proliferation of the vascular or blood cells. A characterization of the eicosanoid receptors present in the vascular wall from human tissues will be presented in order to highlight and appreciate their role during vascular pathologies.

BACKGROUND Vaccine hesitancy is a major challenge in the fight against the coronavirus disease 2019(COVID-19)*** the sociodemographic factors associated with vaccine acceptance among Nigerians is crucial for improving vaccine *** To assess the acceptance rate of COVID-19 vaccine and its related determinants among *** An online cross-sectional survey(observational study)was conducted between February 2021 and May 2021,using a questionnaire hosted on *** invitation to take part in the poll was sent out to participants through social networking platforms.A logistic regression was used to determine which sociodemographic factors were associated with vaccine acceptance *** A total of 1800 persons responded to the survey,a larger proportion of whom were males(53.9%)and within the age group of 21-30 years(29.4%)and earned an average income of less than$500 per month(43.3%).Only 0.56%of participants had a high perceived risk of COVID-19 infection,while only 1.11%had a perceived risk of dying from *** perception rate of the COVID-19 vaccine among participants was 51.1%,while the acceptance rate was 63.9%.There was no significant association between the COVID-19 vaccine acceptance rate and related determinants assessed,particularly age(χ²=3.049,P=0.550),sex(χ²=0.102,P=0.749),average income(χ²=3.802,P=0.875),and religion(χ²=2.819,P=0.420).Participants with chronic conditions demonstrated a higher acceptance rate compared to the general *** Despite the positive perception observed and substantial vaccine acceptance rate among the study participants,more public health interventions are still needed to enhance vaccine acceptability in Nigeria.
