Neutrophil depletion-but not prevention of Kupffer cell activation-decreases the severity of cerulein-induced acute pancreatitis

AIM： To determine whether neutrophil depletion and Kupffer cell inhibition might combine their protective effects to decrease the severity of acute pancreatitis. METhODS： Nice had cerulein administration to induce acute pancreatitis and were pretreated with either anti-mouse neutrophil serum or gadolinium chloride （GdCh） to prevent Kupffer cell activation, or both treatments. Injury was assessed in pancreas and lungs. Myeloperoxidases （MPO） assessed neutrophil infiltration. Interleukin-6 （IL-6） and IL-10 were measured in serum, pancreas, lungs and liver. RESULTS： In mice with acute pancreatitis, neutrophil depletion reduced the severity of pancreatitis and pancreatitis-associated lung injury. Kupffer cell inactivation by GdCh had less protective effect, although IL-6 and IL-10 concentrations were significantly decreased. The protective treatment brought by neutrophil depletion was not enhanced by Kupffer cell inactivation and both treatments did not combine their protective effects. CONCLUSION： Our results confirm the role of activated neutrophils in aggravating organ injury in acute pancreatitis while the role of Kupffer cell activation is less obvious.

Mechanisms and significance of liver steatosis in hepatitis C virus infection

The pathogenesis of liver damage associated with chronic hepatitis C virus （hCV） infection is thought to be largely immunomediated. however, some frequent histoo pathological features, such as steatosis, suggest a direct cytopathic effect of hCV. The direct responsibility of hCV in the pathogenesis of steatosis is shown by：（1） the association with hCV genotype 3 infection, suggesting that some viral sequences are involved in the intracellular aco cumulation of lipids; （2） the correlation between severity of steatosis and hCV replication levels; （3） association between response to treatment and disappearance of steatosis. Experimental studies have shown that the nuo cleocapsid protein of hCV （core protein） is capable and sufficient to induce lipid accumulation in hepatocytes. Moreover, the observation that chronic hepatitis C pao tients have reduced serum levels of ApoB suggests an interference with the very-low density lipoprotein （VLDL） assembly, although other mechanisms are possible. In patients with sustained virological response induced by antiviral therapy, such levels are normalized. Other obo servations suggest that the pathogenesis of steatosis in chronic hepatitis C is not solely due to hCV. The origin of the mild steatosis observed in most patients may be metabolic, since its severity correlates with body mass index and insulin resistance. Most studies have shown a correlation between presence and/or severity of steatosis and fibrosis stage, but it is unclear whether this effect is direct or mediated by the associated insulin resistance, increased susceptibility to apoptosis, or by inflammao tory cytokines. Finally, steatosis negatively influences the rate of response to antiviral treatment, as confirmed by large clinical trials. Management of steatosis in chronic hepatitis C requires knowledge of its pathogenesis and may involve both life-style changes and pharmacological interventions, although the latter remain largely experio mental.

Endoscopic treatment of biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants: Lessons learned from single-balloon enteroscopy-assisted ERCP

Background: Endoscopic treatment of biliopancreatic pathology is challenging due to surgically altered anatomy after Whipple's pancreaticoduodenectomy. This study aimed to evaluate the feasibility and safety of single-balloon enteroscopy-assisted endoscopic retrograde cholangiopancreatography(SBE-ERCP) to treat biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants. Methods: We retrospectively analyzed 106 SBE-ERCP procedures in 46 patients with Whipple's variants. Technical and clinical success rates and adverse events were evaluated. Results: Biliary SBE-ERCP was performed in 34 patients and pancreatic SBE-ERCP in 17, including 5 with both indications. From a total of 106 SBE-ERCP procedures, 76 were biliary indication with technical success rate of 68/76(90%) procedures and clinical success rate of 30/34(88%) patients. Mild adverse event rate was 8/76(11%), without serious adverse events. From a total of 106 SBE-ERCP procedures, 30 were pancreatic indication with technical success rate of 24/30(80%) procedures( P = 0.194 vs. biliary SBEERCP) and clinical success rate of 11/17(65%) patients( P = 0.016 vs. biliary SBE-ERCP). Mild adverse event rate was 6/30(20%)( P = 0.194 vs. biliary SBE-ERCP), without serious adverse events. After SBE-ERCP failure, endoscopic ultrasound-guided drainage, percutaneous drainage and redo surgery were alternative therapeutic options. Conclusions: Biliopancreatic pathology after Whipple's pancreaticoduodenectomy variants can be treated using SBE-ERCP without serious adverse events. Technical and clinical success rates are high for biliary indications, whereas clinical success rate of pancreatic indications is significantly lower. SBE-ERCP can be considered as first-line treatment option in this patient group with surgically altered anatomy.

Endothelial nitric oxide synthase regulation is altered in pancreas from cirrhotic rats

AIM： To determine whether biliary cirrhosis could induce pancreatic dysfunction such as modifications in endothelial nitric oxide synthase（eNOS） expression and whether the regulation of eNOS could be altered by the regulatory proteins caveolin and heat shock protein 90 （hsp90）, as well as by the modifications of calmodulin binding to eNOS. METhODS： Immunoprecipitations and Western blotting analysis were performed in pancreas isolated from sham and cirrhotic rats. RESULTS： Pancreatic injury was minor in cirrhotic rats but eNOS expression importantly decreased with the length （and the severity） of the disease. Because coimmunoprecipitation of eNOS with both hsp90 and caveolin similarly decreased in cirrhotic rats, eNOS activity was not modified by this mechanism. In contrast, drrhosis decreased the calmodulin binding to eNOS with a concomitant decrease in eNOS activity. CONCLUSION： In biliary cirrhosis, pancreatic injury is minor but the pancreatic nitric oxide （NO） production is significantly decreased by two mechanisms： a decreased expression of the enzyme and a decreased binding of calmodulin to eNOS.

We read with great interest the paper from Jena(Germany)recently published in hepatobiliary Surgery and Nutrition by Deeb et al.,which caught our attention,given its clinical *** fact,post-hepatectomy liver failure(PhLF)remains nowadays a significant clinical challenge,occasionally leading to death or potentially requiring,in highly selected cases,rescue liver transplantation(1).Within quaternary care centers,the incidence of PhLF fluctuates from 5%to 20%,depending on the criteria employed,the underlying liver parenchyma quality and the type of hepatectomy(2).hence,liver regeneration(LR)holds paramount importance in hepatic surgery as it directly impacts post-operative outcomes(3):it is a strictly orchestrated phenomenon that entails the activation of hepatocytes and hepatic progenitor cells,as well as the regulation of cell-cycle genes and growth *** contributing to PhLF include pre-operative,intra-operative and post-operative elements,leading to careful patient selection(4).

Artificial ground freezing of underground mines in cold regions using thermosyphons with air insulation

Current practice of underground artificial ground freezing(AGF)typically involves huge refrigeration systems of large economic and environmental *** this study,a novel AGF technique is proposed deploying available cold wind in cold *** is achieved by a static heat transfer device called thermosyphon equipped with an air insulation layer.A refrigeration unit can be optionally integrated to meet additional cooling *** introduction of air insulation isolates the thermosyphon from ground zones where freezing is not needed,resulting in:(1)steering the cooling resources(cold wind or refrigeration)towards zones of interest;and(2)minimizing refrigeration *** design is demonstrated using well-validated mathematical models from our previous work based on two-phase enthalpy method of the ground coupled with a thermal resistance network for the *** Canadian mines are considered:the Cigar Lake Mine and the Giant *** results show that our proposed design can speed the freezing time by 30%at the Giant Mine and by two months at the Cigar Lake ***,a cooling load of 2.4 GWh can be saved at the Cigar Lake ***,this study provides mining practitioners with sustainable solutions of underground AGF.
