Impact of Vascular Variations on Living Donor Kidney Transplantation

Background: Kidney transplantation is the best treatment for end-stage chronic kidney disease. However, its realization is confronted by several difficulties among which are anatomical variations. Objective: The objective of our work was to describe the impact of renal pedicle variations on the operative procedure as well as the complications. Method: We conducted a retrospective study on living kidney donors and their recipients in the period from 2012 to 2017. Several variables were studied, in particular socio-demographic, operative and progression. Results: We identified 49 living donors whose mean age was 37.59 years with a male-to-female ratio of 3.9 and 45 ± 10.75 years in the recipients. The prevalence of renal vascular abnormalities was higher with a proportion of 46.94% in recipients. The left kidney was most frequently removed (75.51%) and then kept mainly in HTK (95.92%). These vascular anomalies were associated with a longer operating time but this difference was not statistically significant (p = 0.5804). They had no effect on hot and cold ischemia times (p = 0.9838, p = 0.8389). Complications were observed in 11 patients, all recipients, i.e. 11.22%, and were not related to the presence of vascular abnormalities (p = 0.086). We observed that 4.08% of deaths were all recipients. Conclusion: It seems that kidney transplantation with multiple renal arteries and/or veins does not significantly lengthen the operating time and does not promote the onset of complications.

Anal Pathologies: What Management at the Reference Health Center of Bamako’s Commune V?

Anal pathologies are conditions affecting the anal margin and the anal canal. The aim of our study was to update data on anal diseases. Non-specific, it can reveal an emergency, a serious illness, a chronic disease or a benign condition. It was a prospective, descriptive and analytical study from April 2022 to March 2023, carried out in the hepato-gastroenterology unit of the Centre de Santé de Référence de la commune V du District de Bamako in Mali. Out of seven hundred and forty-nine (749) patients seen in consultation, 98 had anal pathology, i.e. a prevalence of 13.08%. The mean age was 40 ± 15.15 years and the sex ratio was 1.5. Hemorrhoidal disease and anal fissure were the most frequent pathologies in 66.4% and 24.4% of cases respectively. Medical treatment was initiated in 91.9% of patients with hemorrhoidal disease and 91.7% with anal fissure. Anal pathologies are common in young male patients. In our context, they are dominated by benign conditions.

Irrigation and nitrogen fertiliser optimisation in protected vegetable fields of northern China:Achieving environmental and agronomic sustainability

Globally,sub-optimal use of nitrogen (N) fertiliser and elevated N irrigation groundwater have led to high leached nitrate (NO_(3)^(–)) losses from protected vegetable field *** fertiliser and irrigation management in different soil types is crucial to reduce future N loads from such *** present 4-year study examined leached N loads from lysimeter monitoring arrays set up across 18 protected vegetable system sites encompassing the dominant soil types of northern *** treatments applied at each field site were:1) a high N and high irrigation input treatment (HNHI);2) a low N but high irrigation input treatment (LNHI) and 3) a low N with low irrigation input treatment (LNLI).Results showed that the mean annual leached total nitrogen loads from the HNHI,LNHI and LNLI treatments were 325,294 and 257 kg N ha^(–1) in the fluvo-aquic soil,114,100 and 78 kg N ha^(–1) in the cinnamon soil and 79,68 and 57 kg N ha^(–1) in the black soil,*** N dissolved in irrigation water in the fluvo-aquic soil areas was 8.26-fold higher than in the cinnamon areas.A structural equation model showed that N fertiliser inputs and leaching water amounts explained 14.7 and 81.8%of the variation of leached N loads,***,reducing irrigation water by 21.5%decreased leached N loads by 20.9%,while reducing manure N and chemical N inputs by 22 and 25%decreased leached N loads by only 9.5%. This study highlights that protected vegetable fields dominated by fluvo-aquic soil need management to curtail leached N losses in northern China.

With increasing life expectancy， neurodegenerative diseases have become one of the leading causes of ill-health in the elderly. Preventive strategies include following healthy diets， such as the Mediterranean diet， which is particularly rich in polyphenols， bioactive compounds with neuroprotective properties. The aim of this study was to assess the association of microbial phenolic metabolites (MPM) with cognition. This cross-sectional analysis was performed with 200 participants of the PREDIMED trial (Barcelona-Clinic recruitment center). A novel method based on liquid chromatography coupled to mass spectrometry was used to identify urinary MPM (protocatechuic acid， enterodiol glucuronide， enterolactone glucuronide， urolithin B glucuronide， and vanillic acid glucuronide)， and cognitive function was evaluated with neuropsychological tests. Multivariable-adjusted ordinary least squares regression was used to assess the associations between cognitive function and MPM， and a score was calculated as the weighted sum of MPM. A higher MPM score was associated with better frontal lobe function. Among individual metabolites， vanillic acid glucuronide was correlated with frontal cognitive performance. Participants with higher concentrations of vanillic acid glucuronide and urolithin B glucuronide obtained better scores in the Color Trail Test part 2. A higher score for urinary multi-MPM was associated with better frontal cognitive performance in an older Mediterranean population.

Immunogenetic biomarkers in inflammatory bowel diseases:Role of the IBD3 region

Many studies have demonstrated the linkage between the IBD3 region (6p21.1-23), an area which encompasses the famous human leukocyte antigen (HLA) complex, and Crohn’s disease (CD) or ulcerative colitis (UC). IBD3 is the only region that meets genome-wide significance, and provides stronger evidence of the linkage than 16p13.1-16q12.2 (IBD1), the locus that contains the susceptibility gene CARD15. However, despite these findings, IBD3 susceptibility genes remain elusive and unclear due to the strong linkage disequilibrium, extensive polymorphism, and high gene density that characterize this area and also due to varying allele frequencies in populations around the world. This area presents an extremely high abundance of genes, including the classical and non-classical major histocompatibility complex (MHC) class I and II genes, and other genes, namely MHC class III genes tumor necrosis factor (TNF)-α and -β, and Hsp, whose proteins play key functions in immunological processes. To date, it is not clear which genes within the MHC family contribute to the IBD pathogenesis, although certain HLA alleles have been associated with IBD. Recent insights into the biological function of other genes encoded within the IBD3 region, such as the MHC class I chain-related (MIC) genes, have led investigators to a more comprehensive exploration of this region. MHC class I chain-related molecule A (MICA) is highly polymorphic and interacts with NKG2D, its receptor on the surface of NK, Tγδ and T CD8+ cells. Increased expression of MICA in intestinal epithelial cells and increased expression of NKG2D in CD4+ T cells (lamina propria) in patients with CD have also been reported.
