A Registry-Based Observational Study on the Maternal and Fetal Outcomes of COVID-19 Patients in Hong Kong

Objective: To report the clinical maternal and fetal outcomes of pregnant women with coronavirus disease 2019 (COVID-19), along with any associated pregnancy complications, in Hong Kong, China, and to assess the impact of COVID-19 vaccination on these ***: This prospective registry-based observational study included pregnant women who were recruited through convenient sampling and had a laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection with a cycle threshold (Ct) value result available on admission to eight local hospitals in Hong Kong, China. Data on clinical symptoms, laboratory results, medical treatments, delivery timing and mode, and pregnancy complications were extracted from the Hospital Authority’s electronic medical record system. Maternal, fetal, and pregnancy outcomes were compared between unvaccinated pregnant women with COVID-19 and those who had received at least one dose of COVID-19 vaccine before diagnosis. Nonparametric continuous variables and categorical variables were analyzed using the Mann-WhitneyU test and the Pearson’s chi-squared test respectively. AP value less than 0.05 was considered statistically ***: A total of 164 pregnant women were included, of whom 78 (47.56%) were nulliparous. COVID-19 was diagnosed before 28 weeks’ gestation in 30 (18.29%), while 134 (81.71 %) were diagnosed at or after 28 weeks’ gestation. Sixty-two (37.80%) women received at least one dose of COVID-19 vaccine. There were no significant differences between vaccinated and unvaccinated groups in the time interval between COVID-19 diagnosis and delivery, the Ct value, and the gestational age at infection onset or delivery (P > 0.05). The majority of women were symptomatic at diagnosis regardless of vaccination status (55 (88.71%) in vaccinated groupvs. 78 (76.47%) in unvaccinated group (P = 0.052). Objective: To investigate the presence of trichomoniasis among women attending the Obstetrics and gynaecology Clinic, Universiti Kebangsaan Malaysia Medical ***: A total of 139 high vaginal swabs were taken from the subjects and sent to the laboratory in Amies gel transport media. The specimens were examined for the presence of Trichomonas vaginalis using wet mount, Giemsa staining and cultured in Diamond's medium. Sociodemographic characteristics and gynaecological complaints were obtained in private using structured questionnaire applied by one ***: The median age was 32 years, with an interquartile interval of 9.96. Most of the subjects were Malays(76.9%) and the remaining were Chinese(15.1%), Indians(2.2%)and other ethnic groups(5.8%). One hundred and thirty eight(99.3%) of the women were married and 98.6% had less than 6 children. More than half(75.5%) of the women's last child birth was less than 6 years ago. Forty seven percent of them were involved in supporting administrative work and 64.7% of the women gave a history of previous or current vaginal ***: The present study reported zero incidence rate of trichomoniasis. The low incidence rate was postulated due to all women who participated in this study were categorized into a low-risk group.

Transvaginal Natural Orifice Translumenal Endoscopic Surgery (TVNOTES): Concerns of gynaecology Outpatients in a Tertiary Care Centre

Objective: (TVNOTES) [1], currently the most popular of the NOTES procedures being performed, offers the safest access point while avoiding a breach in the gastrointestinal tract to gain intraperitoneal access. But are women ready to embrace TVNOTES? Although similar studies have been conducted in developed nations, the authors aimed to seek the opinions of Indian women towards TVNOTES. Design: This study presents the results of a survey on attitudes of Indian women towards TVNOTES. 300 women attending a gynaecology out patients’ clinic were administered a questionnaire which sought out the percentage of Indian women concerned with surgical scars, their preference between TVNOTES and conventional laparoscopic surgery and apprehensions towards the vagina being used as an entry site for surgery. Results: 85.67% of the participants opted for scarless intra-abdominal surgery. Upon revealing that the surgery would be performed transvaginally (TV), reservations were unveiled, with 67.33% of all respondents being either unhappy or unsure about undergoing TV surgery. Results were initially equivocal when asked to choose between TVNOTES and conventional laparoscopic surgery, but when informed that TVNOTES may be as safe and efficacious as conventional laparoscopic surgery, the scales tipped in favour of TVNOTES, with three-fourths of the respondents opting for the latter. Conclusions: Although TVNOTES has captured the imagination and attention of surgeons, existing data suggest that the beneficiaries (i.e. women) remain to be convinced. This study suggests that Indian women will embrace TV surgery once its safety is proven, improved cosmesis being a predominant factor in making this choice.

Attitude and willingness on gamete donation among medical students:An experience from a state university in Sri Lanka

Objective:To assess the attitude and willingness of medical students of the Faculty of Medicine,University of Jaffna,regarding gamete ***:An institutional-based descriptive cross-sectional study was conducted at the Faculty of Medicine,University of Jaffna,from September 2022 to May 2023 among undergraduate medical students who gave their voluntary participation.A self-administered questionnaire was used as a study instrument to collect data regarding their attitude and willingness toward gamete ***:A total of 345 participants were recruited and their sociodemographic data revealed that 56.8%of the participants were female,62.3%aged between 26 and 30 years,and 92.2%were *** of them received information regarding gamete donations during their clinical *** half(67.8%)of them showed a negative attitude towards gamete *** willingness,only 39.7%of participants had a positive approach for being a gamete donor;among them,84.7%preferred anonymous *** and ethnicity had a significant influence on their attitudes and *** addition,male was also found to be more willing to donate ***:Most medical students have negative views about gamete *** awareness and knowledge of assisted reproductive technology and gamete donation within medical students'sociocultural and ethical backgrounds might facilitate a change in attitude towards gamete donation amongst future medical practitioners.

Fetal Urinary Obstruction and Intervention for Sacrococcygeal Teratoma: A Case Report From Kenya

Fetal lower urinary tract obstruction is a rare condition commonly associated with significant perinatal morbidity and mortality, particularly when detected in the second trimester. The concurrent presentation of a recto-urethral fistula with membranous urethral atresia and sacrococcygeal teratoma has not been reported. We describe a case of extrinsic fetal lower urinary tract obstruction related to a recto-urethral fistula, diagnosed at 33 weeks gestation and managed by serial vesicocentesis until 38 weeks. This case, notable for its absence of hypoplasia despite late diagnosis, resulted in a term delivery of a male infant. After delivery, pediatric surgeons performed an abdominal perineal resection of the tumor. Fetal vesicocentesis remains a viable management option for patients without access to intrauterine surgical interventions.

An update on hormonal regulation of early embryo-fallopian tube interactions: insights for improvements of assisted reproductive technology outcomes

Human fallopian tubes (FTs) play important roles in the whole reproductive process. FTs are the site for the final maturation of gametes, fertilization, transport of preimplantation embryos, and early embryonic development before entering the uterus. All these functions are critical in the establishment of a successful pregnancy. When the embryo is in transit through the FT, the FT microenvironment changes through endocrine, paracrine, autocrine, and juxtacrine mechanisms which involve various hormones and bioactive substances mainly from the neighboring cells. Therefore, the interactions between the embryo and the FT may be the earliest embryo-maternal communications that are essential for the establishment of pregnancy. This review critically discusses the evidence of various hormonal and bioactive mediated control of the microenvironment in the FTs during the earliest embryo-maternal communications. Moreover, we highlight existing gaps in the knowledge and the importance of exploring more of the physiology of FTs which will hold the key to producing high-quality embryos for patients seekingin vitro fertilization (IVF) treatments.
