Biliary tract tumors are a heterogenous group of cancers that includes extrahepatic cholangiocarcinoma,hilar cholangiocarcinoma,gallbladder tumor as well as intrahepatic cholangiocarcinoma(ICCA)(1).Biliary tract tumors are uncommon tumors representing 2%of all newly diagnosed cancers in the United *** incidence of ICCA is increasing worldwide and associated with a dismal prognosis,and the 5-year overall survival(OS)rate is not exceeding 8%(1).

BACKGROUND As a consequence of the economic crisis,the sociopolitical instability and the advent of the coronavirus disease-19 pandemic,nested challenges faced the Lebanese healthcare *** have resulted in critical shortages of essential resources,including medications vital for oncologic *** To assess the ramifications of the ongoing economic crisis on oncology patient care focusing on our outpatient oncology *** A questionnaire was distributed during the month of February 2022 to oncology patients in Hôtel Dieu de France University Hospital in Beirut during their outpatient *** primary objective was to assess the far-reaching impact of the economic crisis on patient care and the resulting psychological *** Among 182 interviewed patients,31.87%experienced treatment interruption mainly due to acute drug *** 87.91%of the patients benefiting from third-party coverage,69.60%had to self-pay for their medications leading to 69.78%of patients perceiving that healthcare was more difficult to access after ***,one-third of the patients exhibited symptoms of anxiety and/or depression,with 7 patients reporting suicidal ***,37.93%of patients who interrupted cancer treatment reported a history of comorbidities,and 89.66%who altered their treatment cited financial *** Lebanese cancer patients face complex challenges spanning economic,healthcare,and psychological *** inequalities exacerbated by the economic crisis hindered healthcare access.

化疗引起的恶心呕吐(CINV)是众多抗癌药物治疗中常见的不良事件,不仅会对患者生活质量带来负面影响,还有可能影响化疗效果。目前,指南建议的止吐方案可以预防大部分癌症患者的CINV。但临床医师并不能始终遵循指南建议,患者也常常难以坚持医师处方的治疗。因此需要采取一些提高指南依从性的方法。奈妥匹坦帕洛诺司琼(NEPA)是首个也是唯一一个固定剂量复方止吐药,由奈妥匹坦(口服)/福奈妥匹坦(静脉给药)与帕洛诺司琼(PALO)组成。NEPA可以联合地塞米松组成三联止吐方案,这是用于高致吐性化疗(HEC)患者和部分中致吐性化疗(MEC)患者预防CINV的推荐方案。因此,NEPA止吐治疗操作简单便捷,可能有助于提高指南依从性。本综述对CINV进行了概括,评价了在临床试验和真实实践中积累的NEPA止吐效果及安全性方面的证据,同时也评价了在关键临床试验之外的环境下,即日常临床环境中使用NEPA进行止吐的初步证据。此外,本文还评述了化疗期间使用NEPA控制恶心症状和提高患者生活质量,这是癌症患者管理中面临的两个大挑战。

The PI3K/Akt/m TOR pathway in ovarian cancer: therapeutic opportunities and challenges

The phosphatidylinositol 3 kinase(PI3K) pathway is frequently altered in cancer, including ovarian cancer(OC). Unfortunately, despite a sound biological rationale and encouraging activity in preclinical models, trials of first-generation inhibitors of mammalian target of rapamycin(m TOR) in OC have demonstrated negative results. The lack of patient selection as well as resistance to selective m TOR complex-1(m TORC1) inhibitors could explain the disappointing results thus far. Nonetheless, a number of novel agents are being investigated, including dual m TORC1/m TORC2, Akt, and PI3 K inhibitors. Although it is likely that inhibition of the PI3K/Akt/m TOR pathway may have little effect in unselected OC patients, certain histological types, such as clear cell or endometrioid OC with frequent phosphatidylinositol-4,5-biphosphate 3-kinase, catalytic subunit alpha(PIK3CA) and/or phosphatase and tensin homolog(PTEN) alterations, may be particularly suited to this approach. Given the complexity and redundancy of the PI3 K signaling network, PI3 K pathway inhibition may be most useful in combination with either chemotherapy or other targeted therapies, such as MEK inhibitors, anti-angiogenic therapy, and hormonal therapy, in appropriately selected OC patients. Here, we discuss the relevance of the PI3 K pathway in OC and provide an up-to-date review of clinical trials of novel PI3 K inhibitors alone or in combination with cytotoxics and novel therapies in OC. In addition, the challenges of drug resistance and predictive biomarkers are addressed.

Brain macrophages include microglia in the parenchyma,border-associated macrophages in the meningeal-choroid plexus-perivascular space,and monocyte-derived macrophages that infiltrate the brain under various disease *** vast heterogeneity of these cells has been elucidated over the last decade using revolutionary multiomics *** such,we can now start to define these various macrophage populations according to their ontogeny and their diverse functional programs during brain development,homeostasis and disease *** this review,we first outline the critical roles played by brain macrophages during development and healthy *** then discuss how brain macrophages might undergo reprogramming and contribute to neurodegenerative disorders,autoimmune diseases,and ***,we speculate about the most recent and ongoing discoveries that are prompting translational attempts to leverage brain macrophages as prognostic markers or therapeutic targets for diseases that affect the brain.

OBJECTIVE:To determine the difference in prevention of chemotherapy-induced fatigue between time-acupointsspace acupuncture(ATAS)administered weekly compared to sham acupuncture and non-acupuncture in patients with early breast cancer receiving adjuvant chemotherapy by epirubicine-cyclophosphamide(EC)followed by paclitaxel,as measured by the multidimensional fatigue inventory(MFI)over the previous week and Visual Analogue Scale measuring fatigue(VAS-F),and to evaluate the effects of ATAS on selfreported neuropathy pain,sleep,anxiety and ***:In this multicenter clinical trial,we have randomized patients into 3 groups:ATAS,Sham and nonacupuncture with an unequal randomization of 2∶1∶1.A cloud related electronical clinical report form and smartphone platform was established for data *** with a history of stageⅠ-Ⅲbreast cancer scheduled to receive adjuvant *** will be delivered once a week during chemotherapy with VAS-F *** order to qualify and quantify the mechanism of fatigue induced by chemotherapy with or without acupuncture,an evaluation of immune profiling was incorporate in this ***:The presence and seriousness of chemotherapy-induced fatigue should be considered in therapeutic programs for early breast cancer *** induced by adjuvant chemotherapy in patients with breast cancer remains a major concern affecting the quality of life ***,we do not have effective pharmacological interventions *** clinical trials of acupuncture in preventing chemotherapyinduced fatigue in patients with early breast cancer have not been ***:The findings of the trial will allow us to determine the effects of acupuncture treatment *** will also be able to confirm whether ATAS is better than sham acupuncture and non-acupuncture treatments.
