Enterovirus A71 (EV-A71) is a significant hand-foot-mouth disease (HFMD) etiology. The inactivated EV-A71 vaccines were approved in China in 2016. However, the seroprevalence of EV-A71 after the vaccine application and its potential association with the EV-A71 epidemic in the population are rarely studied. In this study, we analyzed the incidence of EV-A71 infection and seroepidemiology in guangzhou City, China. From 2019 to 2021, 167,920 clinically confirmed HFMD cases were reported in guangzhou. In 6,868 enterovirus-positive samples, Coxsackievirus A6 and Coxsackievirus A16 were dominant genotypes, and only 3 EV-A71-positive samples were detected, highlighting the deficient epidemic activity of EV-A71. Microneutralization assay was performed on 1,000 representative serum samples. Notably, the seroprevalence and geometric mean titer (GMT) decreased significantly in 2020, and that in the guangzhou. Furthermore, a moderate decline of GMT level was observed following the vaccination, but the seropositive serums were still detected for 49 months after second immunization, suggesting the long-term persistence of the immunity. Our seroepidemiology study revealed relatively higher neutralizing antibody activity in the susceptible population after the EV-A71 vaccine was adopted in 2016 in guangzhou. It may be one of the reasons for the lower epidemic activity of EV-A71 in guangzhou from 2019 to 2021.

China has been continuously improving its monitoring methods and strategies to address key infectious diseases(KIDs).After the severe acute respiratory syndrome epidemic in 2003,China established a comprehensive report-ing system for infectious diseases(IDs)and public health *** relatively lagging warning thresholds,limited warning information,and outdated warning technology are insufficient to meet the needs of comprehensive monitoring for modern *** early monitoring and warning capabilities to enhance the public health system has become a top priority,with increasing demand for early warning thresholds,information,and tech-niques,thanks to constant innovation and development in molecular biology,bioinformatics,artificial intelligence,and other identification and analysis technologies.A panel of 31 experts has recommended a fourth-generation comprehensive surveillance system targeting KIDs(41 notifiable diseases and emerging IDs).The aim of this surveil-lance system is to systematically monitor the epidemiology and causal pathogens of KIDs in hosts such as humans,animals,and vectors,along with associated environmental *** integrating factors influencing epidemic spread and risk assessment,the surveillance system can serve to detect,predict,and provide early warnings for the occurrence,development,variation,and spread of known or novel ***,we recommend comprehensive ID monitoring based on the fourth-generation surveillance system,along with a data-integrated monitoring and early warning platform and a consortium pathogen detection technology *** series of considerations is based on systematic and comprehensive monitoring across multiple sectors,dimensions,factors,and pathogens that is sup-ported by data integration and *** expert consensus will provides an opportunity for collaboration in various fields and relies on interdisciplinary application to enhance comprehensive monitoring,prediction,and ea

The reaction of carbonyl-to-imine/hemiaminal conversion in the atmospheric aqueous phase is a critical pathway to produce the light-absorbing N-containing secondary organic compounds(SOC).The formation mechanism of these compounds has been wildly investigated in bulk solutions with a low ionic ***,the ionic strength in the aqueous phase of the polluted atmosphere may be *** is still unclear whether and to what extent the inorganic ions can affect the SOC *** we prepared the bulk solution with certain ionic strength,in which glyoxal and ammonium were mixed to mimic the aqueous-phase *** characterization by High-resolution Mass Spectrometry was performed to identify the N-containing products,and the light absorption of the mixtures was measured by ultraviolet-visible ***-nine N-containing compounds were identified and divided into four categories(N-heterocyclic chromophores,high-molecular-weight compounds with N-heterocycle,aliphatic imines/hemiaminals,and the unclassified).It was observed that the longer reaction time and higher ionic strength led to the formation of more N-heterocyclic chromophores and the increasing of the lightabsorbance of the *** added inorganic ions were proposed to make the aqueous phase somewhat viscous so that the molecules were prone to undergo consecutive and intramolecular reactions to form the *** general,this study revealed that the enhanced ionic strength and prolonged reaction time had the promotion effect on the lightabsorbing SOC *** implies that the aldehyde-derived aqueous-phase SOC would contribute more light-absorbing particulate matter in the industrial or populated area where inorganic ions are abundant.

Is surgical axillary staging necessary in women with T1 breast cancer who are treated with breast-conserving therapy?

Background:In the post-Z0011 trial era,the need to perform surgical axillary staging for early-stage breast cancer patients,who are treated with breast-conserving therapy(BCT),is being *** conducted a retrospective cohort study using the Surveillance,Epidemiology,and End Results(SEER)database to evaluate the safety of waiving surgical axillary staging in patients with T1 breast cancer treated with ***:A total of 166,615 eligible patients diagnosed between 2000 and 2012 were divided into staging(sentinel lymph node biopsy or axillary lymph node dissection)and non-staging(no lymph node examined or only needle aspiration biopsy of lymph nodes)*** score matching(PSM)was performed to balance disparities between the two *** analysis with the Cox proportional hazards model was used to assess factors related to breast cancer-specific survival(BCSS).Results:Although the tumor size at time of presentation was decreasing over years,the rate of surgical axillary stag-ing increased from 93.3%to 96.9%.The 5-year BCSS rates of the whole cohort(before PSM)and matched cohort(after PSM)were 98.0%and 97.5%.Within the matched cohort,the BCSS was significantly longer in the staging group than in the non-staging group(Pdisease,or favorable histological types(tubular/mucinous/papillary)in stratified analyses(P>0.05).Race,marital status,hormone receptors,and chemotherapy were not associated with the favorable impact of surgical axillary staging on BCSS(P>0.05).Conclusion:Although surgical axillary staging remains important for T1 breast cancer patients treated with BCT,it might be unnecessary for patients with old age,small tumor,grade I disease,or favorable histological types.

Domain Ⅲ of Dengue Virus Serotype 2 Envelope: Expression at High Levels in Escherichia coli and Competitive Inhibition of Virus Entry

Obejective The domainⅢof dengue virus type 2 envelope was cloned and expressed in Escherichia coli and the inhibited effects of recombinant protein on virus was detected. Methods In this study, the domainⅢ(DⅢ) protein of the dengue virus type-2 (DENV-2) envelope (E) antigen was expressed in Escherichia coli by fusion with a carrier protein. The protein was puriifed using enzymatic cleavage and afifnity puriifcation. Rabbit immunization and antibody detection was carried out. Inhibition of DENV-2 infection was observed by DENV-2 EDⅢprotein and its immunity rabbits serum. Results The recombinant expression DENV-2 EDⅢ protein plasmid was constructed successfully. After isopropyl thiogalactoside induction, a speciifc soluble 29 kD protein was obtained, and the expression product accounted for 68.87%of the total protein of the cell lysate. Western blot demonstrated the reactivity of the recombinant protein with his-tag and DENV (Ⅰ-Ⅳ) monoclonal antibodies. The protein was puriifed using enzymatic cleavage and affinity purification. The purified recombinant EDⅢ protein inhibited the entry of DENV-2 into BHK-21 cells. DENV-2 plaque neutralization assays were carried out using serially diluted antibodies against EDⅢprotein. At a 1︰16 dilution, the antibodies produced at least 90%neutralization of the DENV-2 virus. Furthermore, the antibodies continued to exhibit high neutralization effects (approximately 80%) until the anti-EDⅢantibody titer reached 1︰1 024. Conclusions DENV-2 EDⅢwas cloned and expressed successfully. DENV-2 EDⅢprotein could be useful in the development of inexpensive dengue vaccine. The data also suggested that DENV-2 employed an attachment molecule or receptor for its entry into C6/36 mosquito cells.

Trends of incidence rate and age at diagnosis for cervical cancer in China,from 2000 to 2014

Objective： To analyze the trends of incidence rate and age at diagnosis for cervical cancer incidence in China using population-based cancer registration data from 2000 to 2014. Methods： Data were from National Central Cancer Registry of China. Crude incidence rates （CIRs）, age-specific incidence rate, age-standardized incidence rates （ASIRs）, age percentage distribution, standardized age percentage distribution, mean age at diagnosis and standardized mean age at diagnosis for cervical cancer in all areas of China, urban China and rural China were calculated separately. The world Segi＇s population was applied to remove the age structure influence. Joinpoint regression was performed to obtain average annual percent change （AAPC） and age- period-cohort analysis was used to examine the incidence trends. Results： CIRs and ASIRs for cervical cancer increased in China from 2000 to 2014. The AAPC of ASIRs in China was at 9.2% [95% confidence interval （95% CI）： 7.0%-I 1.5%, P〈0.05], and the AAPC in rural areas was relatively high. The age-specific incidence rate in groups aged 0-69 years have significantly increased over time. Groups aged 40-69 years showed the highest incidence risk, and the annual percent changes （APCs） of incidence rate in groups aged 40-59 years in urban China and groups aged 0-49 years in rural China were more than 10%. For each age group, the urban-to-rural incidence rate ratios （IRRs） got close to 1 over time. There were clear birth cohort effects in successive generations born from 1940 to 1970 in China. In rural China, the standardized mean age at diagnosis had significantly declined by 5.18 years. In China, the main peak and secondary peak of standardized age percentages appeared in the groups aged 45-49 and 40-44 years, respectively. Background:Identifying factors associated with cardiovascular disease(CVD)is critical for its prevention,but this topic is scarcely investigated in Kashgar prefecture,Xinjiang,northwestern *** thus explored the CVD epidemi-ology and identified prominent factors associated with CVD in this ***:A total of 1,887,710 adults at baseline(in 2017)of the Kashgar Prospective Cohort Study were included in the *** candidate factors,including seven demographic factors,4 lifestyle factors,and 5 clinical factors,were collected from a questionnaire and health examination *** was defined according to International Clinical Diagnosis(ICD-10)*** first used logistic regression models to investigate the association between each of the candidate factors and ***,we employed 3 machine learning methods-Random Forest,Random Ferns,and Extreme Gradient Boosting-to rank and identify prominent factors associated with *** analyses by sex,ethnicity,education level,economic status,and residential setting were also performed to test the consistency of the ***:The prevalence of CVD in Kashgar prefecture was 8.1%.All the 16 candidate factors were confirmed to be significantly associated with CVD(odds ratios ranged from 1.03 to 2.99,all p values<0.05)in logistic regression *** machine learning-based analysis suggested that age,occupation,hypertension,exercise frequency,and dietary pattern were the five most prominent factors associated with *** ranking of relative importance for prominent factors in stratification analyses showed that the factor importance generally followed the same pattern as that in the overall ***:CVD is a major public health concern in Kashgar ***,occupation,hypertension,exercise frequency,and dietary pattern might be the prominent factors associated with CVD in this *** the future,these factors should be given priority in preventing CVD in future.

Experimental study on EPO treatment of model rats with infection-induced acute liver injury

Objective: To study the effect of EPO therapy on liver injury, inflammation, oxidative stress and cell apoptosis in model rats with infection-induced acute liver injury. Methods: SD rats were selected and randomly divided into Sham group, sepsis group (CLP group) and EPO intervention group (EPO group), the cecal ligation and puncture was adopted to establish sepsis models and 5000IU/kg human recombinant EPO was provided for intervention. 24 hours after intervention, serum levels of liver injury molecules, inflammatory factors and oxidative stress molecules as well as liver tissue levels of oxidative stress molecules and cell apoptosis molecules were detected. Results: Serum ALT, AST, TNF-α, IFN-γ, IL-1β, IL-6, IL-8, MDA and AOPP levels, liver tissue Nrf2, ARE, MDA and AOPP levels as well as Bax, Caspase-9 and Caspase-3 mRNA expression of CLP group were significantly higher than those of Sham group while liver tissue HO-1 and SOD levels as well as Bcl-2 mRNA expression were significantly lower than those of Sham group;serum ALT, AST, TNF-α, IFN-γ, IL-1β, IL-6, IL-8, MDA and AOPP levels, liver tissue MDA and AOPP levels as well as Bax, Caspase-9 and Caspase-3 mRNA expression of EPO group were significantly lower than those of CLP group while liver tissue Nrf2, ARE, HO-1 and SOD levels as well as Bcl-2 mRNA expression were significantly higher than those of CLP group. Conclusions: EPO therapy could reduce liver injury and inhibit inflammation, oxidative stress and cell apoptosis in model rats with infection-induced acute liver injury.

目的:分析本辖区24年来甲乙丙类传染病发生及流行趋势,探讨其演变规律,为制定防控措施提供参考依据。方法 :采用描述性流行病学方法对本辖区1985～2008年间各类法定传染病疫情报告资料进行统计学分析。结果 :24年来共报告法定传染病31种51910例,其中甲类1种97例,乙类22种35278例,丙类8种16535例。24年间有9年出现甲类传染病报告,其中1998年发病率最高(13.52/10万)。乙类传染病1985～2003年发病率呈逐年下降趋势,但从2004年开始又逐年上升,甚至超过1985年水平,发病率维持在一个相对较高的波动区间(99.41/10万—649.09/10万)。丙类传染病发病率高峰出现在2005年,在此之前呈低水平流行趋势,以后逐年回落,但回落幅度不明显。24年间各类传染病构成比主要以肠道传染病和呼吸道传染病为主,近年来新发传染病和性传播疾病又有逐年增多的趋势。虫媒和自然疫源性疾病波动较小。此外,每年前5名的传染病病种排序中24年间共有16种传染病出现过,其中病毒性肝炎出现过23次,13年排第一,肠道传染病和呼吸道传染病每年的排序最前。结论 :由于本辖区人口构成比较复杂,流动人口较多且流动频繁,规范性预防接种难以实现。加上经常有输入性传染病病例的发生,导致传染病防控难度加大,需要政府加大对传染病防控机构人力和财力的投入。
