The feasibility and safety of intracardiac echocardiography(ICE)-guided catheter ablation for atrial fibrillation(AF)using a minimal/zero-fluoroscopy approach have recently been *** approach helps to reduce ionizing radiation exposure and orthopedic complications resulting from using lead *** objectives of this planned prospective,multicenter randomized controlled trial(RCT)(paroxysmal AF(PAF)-ICE trial;ChiCTR2000033624)are to evaluate the efficacy and safety of ICE-guided minimal-fluoroscopy ablation in patients with PAF and the impact on occupational hazards among lab *** will be randomized in a 1:1 ratio to 2 groups:minimal fluoroscopy group(n=216)and traditional approach group(n=216).In the minimal fluoroscopy group,an ICE catheter will be used for geometry/anatomic construction,transseptal puncture,catheter tracking,and effusion *** vein isolation(PVI)will be performed using an open-irrigated radiofrequency SmartTouch Surround Flow or SmartTouch catheter(Biosense Webster,Diamond Bar,California,USA),and confirmed by a multipolar Lasso or PentaRay catheter(Biosense Webster).In the traditional approach group,an ICE catheter will not be *** puncture will be performed under fluoroscopic guidance,with all geometries constructed by mapping the *** primary efficacy endpoint is freedom from AF recurrence(without antiarrhythmic medications)at 12months after *** endpoints include duration of lead apron use,measures of intra-procedural efficiency,and peri-procedural *** RCT will evaluate the efficacy and safety of ICE-guided minimal-fluoroscopy ablation in patients with PAF,also evaluate the benefits to lab staff(regarding reducing occupational hazards)related to this“minimal/zero-fluoroscopy”and“leadless”mode.

Relationship of RANTES and MCP-1 with Immune-associated Interstitial Fibrosis of Renal Allografls

Objective To investigate the expression of RANTES and monocyte chemoattractant protein 1 in renal allografts with chronic antibody-mediated rejection, and explore their relationship with immune-associated interstitial fibrosis of renal allografts. Methods Immunohistochemical assay and computer-assisted genuine colored image analysis system were used to detect the expression of RANTES, monocyte chemoattractant protein 1 in renal allografts with chronic antibody-mediated rejection. The relationship between their expression levels and the grade of inflammatory cell infiltration, interstitial fibrosis and tubular atrophy in renal allografls were analyzed. Results Compared to the normal tissues, the expressions of RANTES, monocyte chemoattractant protein 1 were significantly higher in the renal tissues with chronic antibody-mediated rejection (p < 0.001), and the expressions tended to increase along with the pathological grade of IF/TA, the expression of RANTES and monocyte chemoattractant protein l were positively correlated with the pathological grades of IF/TA (r=0.940 and 0.954 respectively, p <0.001 for both), and the expression of RANTES and monocyte chemoattractant protein 1 were also positively correlated with the inflammatory cellular infiltration (r=0.939 and 0.952 respectively, p<0.001 for both ). Conclusion In renal allograft tissue with chronic antibody-mediated rejection, the up-regulated expressions of RANTES and monocyte chemoattractant protein 1may be related to the progressive of the chronic renal allografts dysfunction and allografts fibrosis.

Clinical efficacy of ventriculo-peritoneal shunt in the treatment of acquired immunodeficiency syndrome complicated with cryptococcal neoformans meningitis

Objective To investigate the clinical efficacy of ventriculo-peritoneal shunt (VPS) in acquired immunodeficiency syndrome (AIDS) patients with cryptococcal neoformans meningitis (CNM). Methods Patients with AIDS and CNM who were hospitalized in guangzhou eighth People’s hospital,guangzhou Medical University from January 2015 to June 2020 were included and divided into VPS group and conventional treatment group. The data including symptoms and signs of meningitis,cerebrospinal fluid (CSF) pressure,CSF routine examination,ink staining,Cryptococcus culture and Cryptococcus culture negative time were obtained,and the clinical efficacy was compared between the two groups after six and 48 weeks of treatment. Two independent samples t test or chi-square test was used for statistical analysis.
