Changes in the clinical features, treatments, and outcomes of patients with systemic light chain (AL) amyloidosis in Western China, 2010-2022: A multicenter, retrospective, real-world study

Systemic light chain(AL) amyloidosis, a rare and deadly disease affecting approximately 10 individuals per million each year worldwide, is marked by misfolded, aggregated immunoglobulin light chains produced by dangerous plasma cell or B cell clones. Diagnosis of AL amyloidosis often faces obstacles due to non-specific symptoms. Owing to a deeper understanding and therapeutic advancements of AL amyloidosis, patients' overall survival(OS) has significantly improved globally. However, data of China's situation remain limited, especially since the novel therapeutic agents were included in the national medical insurance reimbursement list(NMIRL) in late 2017, systemic light chain is listed in the first national list of rare diseases released, and the neoplasm multidisciplinary teams(MDTs) was established by the National Health Commission of China in 2018. Therefore, a multicenter, retrospective, real-world study(AMY4002) was conducted in Western China to report the clinical baselines, treatment patterns, and outcomes of Chinese AL amyloidosis patients before and after 2018.

Correlation of the expression of neurotrophins family and its receptors with cell proliferation and invasion in ovarian cancer

Objective:To investigate the correlation among the expression of neurotrophins (NTs) family and its receptors with cell proliferation and invasion in ovarian ***:Ninety patients with ovarian cancer who underwent surgical treatment in guangyuan First People's hospital between December 2015 and October 2017 were selected as ovarian cancer group, and 67 patients with ovarian chocolate cyst were selected as benign lesion group. The differences in the expression of NTs family and its receptors, proliferation genes and invasion genes in lesions were compared between the two groups, and Pearson test was used to further evaluate the correlation among the expression of NTs family and its receptors with cancer cell malignancy in ovarian cancer ***:NT-3 mRNA expression in ovarian cancer group was lower than that in benign lesion group whereas NT-5, TrkA, TrkB and TrkC mRNA expression were higher than those in benign lesion group;proliferation genes FUNDC1, GRP78, PTTG and SMG-1 mRNA expression were higher than those in benign lesion group whereas shSASH1 and ESRP1 mRNA expression were lower than those in benign lesion group;invasion genes Clusterin, EFEMP1, Twist and IFITM1 mRNA expression were higher than those in benign lesion group whereas DUSP10 mRNA expression was lower than that in benign lesion group. Pearson test showed that the NT-5, TrkA, TrkB AND TrkC mRNA expression in ovarian cancer were directly correlated with cancer cell proliferation and invasion ***: The expression levels of NTs family and its receptors are abnormal in ovarian cancer and the specific expression levels are directly correlated with the proliferation and invasion activity of cancer cells.

Correlation of MyD88 expression in the peripheral blood and placenta with the inflammatory response and insulin signal transduction in the placenta of patients with GDM

Objective: To study the correlation of MyD88 expression in peripheral blood and placenta with the inflammatory response and insulin signal transduction in the placenta of patients with gestational diabetes mellitus (GDM). Methods: The patients with GDM and healthy pregnant women who accepted antenatal care and gave birth in guangyuan First People's hospital between March 2015 and June 2017 were selected as the research subjects and enrolled in the GDM group and control group for the study respectively;the peripheral blood was collected before delivery to determine the MyD88 mRNA expression, and the placental tissue was collected after delivery to determine the mRNA expression of MyD88, inflammatory response molecules and insulin signal transduction molecules. Results: MyD88 mRNA expression levels in the peripheral blood and placenta of GDM group were significantly higher than those of control group, and the MyD88 mRNA expression in the peripheral blood was positively correlated with the MyD88 mRNA expression in the placenta;IL-1β, IL-6, RBP4, Chemerin, Resistin and PTP1B mRNA expression levels in the placenta of GDM group were significantly higher than those of control group whereas IRS1, ISR2, p-PI3K and GLUT4 protein expression levels were significantly lower than those of control group;IL-1β, IL-6, RBP4, Chemerin, Resistin and PTP1B mRNA expression levels in the placenta of GDM group of patients with high MyD88 expression were significantly higher than those of patients with low MyD88 expression whereas IRS1, ISR2, p-PI3K and GLUT4 protein expression levels were significantly lower than those of patients with low MyD88 expression. Conclusion:The expression of MyD88 in peripheral blood and placenta increase in patients with GDM and the change of MyD88 expression in peripheral blood could reflect the abnormality of inflammatory response and insulin signal transduction in the placenta.

OBJECTIVE: To assess the efficacy and safety of Sancai powder in patients with type 2 diabetes mellitus(T2DM) inadequately controlled with single oral metformin in a randomized controlled trial(RCT).METHODS: A total of 132 patients with T2 DM were enrolled in the study, who only took metformin(500-1000 mg/day) for at least three months and with inadequate glycemic control(7.0% ≤ hemoglobin A1 c ≤ 9.0%) in the past three *** patients stopped taking metformin with lifestyle interventions for three weeks, and 105 patients qualified for the program. They were randomly divided into the Sancai powder group and the metformin group(1500 mg/day). The follow-up period was for 12 weeks. Comparisons of several variables were ***: No significant differences were found between the two groups in hemoglobin A1c(Hb A1c),fasting plasma glucose(FPG) and 2 h post-meal glucose(2h PG), although they had decreased significantly(P < 0.01). Homeostasis model assessment of beta cell function index was significantly improved in Sancai powder group(P < 0.01), and there were significant differences in the changes of homeostasis model assessment of insulin resistance and insulin sensitivity index in the two groups(P < 0.05). Sancai powder significantly reduced triglyceride level(P < 0.05), although there was no significant difference in the body weight and body mass index in the two ***: In this 12-week study, Sancai powder could significantly reduce hemoglobin A1 c,FPG and 2h PG levels, improved beta-cell function and insulin resistance of the T2 DM inadequately controlled with metformin.

Analysis of the levels of in vivo immunological indicators in patients with different types of malignanat lymphoma

Objective:To analyze the levels of in vivo immunological indicators in patients with different types of malignanat ***: A total of 69 patients with different types of malignanat lymphoma who were admitted in our hospital were included in the study, among which 31 had primary extranodular malignant lymphoma (extranodular group), and 38 had primary nodular malignant lymphoma (nodular group). Moreover, 34 patients with pure lymphnoditis were served as the control group. FCM and full-automatic speed scattering turbidimetry were used to detect T lymphocyte subsets proportion, IgG, IgA, IgM, C3, and C4 in the three ***: The comparison of CD3+, CD4+, CD8+, NK cell proportion, and CD4+/CD8+ among the three groups was significantly different. When compared with the control group, CD3+, CD4+, CD8+, NK cell proportion, and CD4+/CD8+ in the extranodular group and nodular group were significantly reduced;moreover, CD3+, CD4+, CD8+, and CD4+/CD8+ in the extranodular group were significantly lower than those in the nodular group, while NK cell in the extranodular group was significantly higher than that in the nodular group. The comparison of IgG, IgA, and C3 levels among the three groups was statistically significant. When compared with the control group, IgG and IgA levels in the extranodular group and nodular group were significantly elevated, while C3 level was significantly reduced, and the comparison between the extranodular group and nodular group between the two groups was statistically significant. IgM level in the extranodular group and nodular group was elevated, C4 level was reduced, but the comparison with the control group was not statistically significant. The change of serum folic acid, homocysteine and its relationship with cognitive function in first episode schizophrenia

Objective To observe the change of serum folic acid,homocysteine(Hcy)and its relationship with cognitive function in first episode schizophrenia(SZ). Methods Forty-five patients with first episode schizophrenia treated in our hospital from March 2014 to June 2015 were selected as the observation group,and 50 healthy adults were selected as the control group. The serum level of folic acid,Hcy and cognitive function were observed in two groups. The correlation between them was analyzed by Pearson correlation analysis. Results The level of folic acid of the first episode SZ patients was significantly lower than that of the control group,while the Hcy level was significantly higher than that of the control group,and the difference was statistically significant(P<0.05);The scores of orientation,instantaneous memory,attention,word immediate memory and delayed recall of the observation group were significantly lower than those of the control group(P<0.05);The level of folic acid was positively correlated with orientation,instantaneous memory and attention and calculation,while the Hcy level was negatively correlated with the indexes mentioned above;the folic acid,homocysteine(Hcy)level had no obvious correlation with language ability,visual and spatial perception and word immediate memory. Conclusions The level of serum folic acid of the patients with first episode SZ are lower and the level of serum Hcy is higher,and the above indexes are closely related with some cognitive functions of the patients.

To know the nutritional status among students in guangyuan, to explore its major issues and influencing factors, so as to provide scientific basis for the the government on nutritional intervention of school students, a stratified cluster random sampling investigation was conducted among 1879 9-15 years old students from 7 primary schools and 8 high schools in guangyuan. Daily diets of the students were surveyed and their weight, height and hemoglobin were measures to assess their nutritional status. The prevalence rates of undernutrition and underweight were respective 2.3% and 41.1% respectively. The prevalence rates of overweight and obesity were 5.4% and 8.5% respectively. Undernutrition and overnutrion exist simultaneously in the students investigated. Undernutrition is particularly prominent. There was a high anaemia rate among the students. The ratios of students who often drank milk, ate fish, ate bean products, ate animal innards 1-2 times a week were 60.2%, 35.3%, 23.9%, and 10.8% respectively. Nutritional status of students of guangyuan isn’t satisfactory. Effective nutritional intervention on school students should be taken, such as improving the dietary pattern, strengthening the publicity of nutritional knowledge, developing the healthy dietary behavior.
