OBJECTIVE:To synthesize the evidence from randomized controlled trials(RCTs)to assess the efficacy and safety of chinese patent medicine(CPM)on atherosclerosis(AS)or with a high risk of ***:All RCTs in three databases(Pub Med,EMBASE,and Cochrane Library)were included from the inception of the database to September 20,*** methodological evaluation of the included trials was carried out using the Cochrane Collaboration Risk of Bias ***-analysis was conducted using Rev Man 5.3 *** Grading of Recommendations Assessment,Development and Evaluation(GRADE)methodology was used to evaluate the quality of ***:Eighteen RCTs were included,involving a total of 3885 patients with AS or with a high risk of *** trials had favorable ***-analysis suggested significant differences in clinical endpoint(n=1938,RR 0.53;95%CI 0.40 to 0.69,P<0.00001;I^(2)=0%);the change in carotid artery IMT(n=1723,MD-0.09,95%CI-0.14 to-0.04,P<0.001;I^(2)=40%);change in FMD(n=239,MD 0.87,95%CI 0.52 to 1.21,P<0.00001;I^(2)=0%);change in high sensitive C-reactive protein(hsCRP)(n=1527,MD-1.89,95%CI-3.36 to-0.42,P=0.01;I^(2)=94%)and incidence of total adverse events(RR 0.76,95%CI 0.62 to 0.93,P=0.009;I^(2)=40%)in favor of the experimental ***,meta-analysis showed no significant differences in the change in lowdensity lipoprotein-C(LDL-C)(n=2419,MD-0.19,95%CI-0.50 to 0.12,P=0.22,I^(2)=94%)between the experimental and control ***:CPM could have certain clinical efficacy in the treatment of ***,more double-blinded placebo-controlled RCTs are required in further evaluations to provide stronger evidence.

Antipyretic Effect of Herba Ephedrae-Ramulus Cinnamomi Herb Pair on Yeast-Induced Pyrexia Rats: A Metabolomics Study

Objective： To investigate the antipyretic mechanism of Herba Ephedrae （Eph）-Ramulus Cinnamomi （RC） herb pair on yeast-induced pyrexia in rats. Methods： Totally 30 qualified male SD rats were randomly assigned to the normal control （NC） group, the pyrexia model （model） group, the Eph, RC and Eph-RC treatment groups by a random digital table, 6 rats in each group. Each rat received a 20% aqueous suspension of yeast （10 mL/kg） except the NC group. The 3 treatment groups were administered 8.1, 5.4 and 13.5 g/kg Eph, RC and Eph-RC respectively at 5 and 12 h after yeast injection, the NC group and the model groups were administered equal volume of distilled water. Rectal temperatures were measured at 0, 6, 8, 10, 12, 15, 18, 24 and 30 h and urine was collected prior to yeast injection and at 6, 10, 18, 24, 30, and 36 h after yeast injection. Then urine metabolomic profiling by gas chromatography tandem mass spectrometry, coupled with multivariate statistical analysis and pattern recognition techniques were used to explore the antipyretic effects of Eph-RC. Partial least squares discriminate analysis was used to analyze the metabolomics dataset including classification and regression in metabolomics plot profiling. Results： Compared with the NC group, rectal temperatures were significantly higher in the model group （P〈0.01）, while 3 treatment groups decreased significantly compared with the model group （P〈0.05 or P〈0.01）. Rectal temperatures of Eph-RC-treated rats started to go down at 6 h, and markedly decreased at 8, 12, 15, 18 and 24 h （P〈0.05 or P〈0.01）, while those of the Eph and RC groups had decreased firstly at 8 h and were markedly lower at 12 h （P〈0.05 or P〈0.01）. Seventeen potential biomarkers related to pyrexia were confirmed and identified, including pyruvic acid, L-phenylalanine, L-tyrosine, phenylacetic acid, hippuric acid, succinic acid, citrate and so on. There are several malignancies of the digestive system(including gastric, pancreatic and colorectal cancers, and hepatocellular carcinoma), which are the most common types of cancer and a major cause of death worldwide. MicroRNA(miR)-7 is abundant in the pancreas, playing an important role in pancreatic development and endocrine function. Expression of miR-7 is downregulated in digestive system malignancies compared with normal tissue. Although there are contrasting results for miR-7 expression, almost all research reveals that miR-7 is a tumor suppressor, by targeting various genes in specific pathways. Moreover, miR-7 can target different genes simultaneously in different malignancies of the digestive system. By acting on many cytokines, miR-7 is also involved in many gastrointestinal inflammatory diseases as a significant carcinogenic factor. Consequently, miR-7 might be a biomarker or therapeutic target gene in digestive system malignancies.

Objective:To explore the protective effect and the underlying mechanism of silibinin(SIB),one of the active compounds from Silybum marianum(L.)Gaertn in ***:Mouse peritoneal macrophage were isolated via intraperitoneally injection of BALB/c mice with thioglycolate *** viability was assessed using the cell counting kit-8,while cytotoxicity was determined through lactate dehydrogenase cytotoxicity *** protein expressions of interleukin(IL)-1α,IL-1β,and IL-18 were determined by enzyme-linked immunosorbent *** lipopolysaccharide(LPS)levels were measured by employing both the limulus amoebocyte lysate assay and flow ***,proximity ligation assay was employed for the LPS and caspase-11 *** were divided into 4 groups:the control,LPS,high-dose-SIB(100 mg/kg),and low-dose-SIB(100 mg/kg)groups(n=8).Zebrafish were divided into 4 groups:the control,LPS,high-dose-SIB(200μmol/L),and low-dose-SIB(100μmol/L)groups(n=30 for survival experiment and n=10 for gene expression analysis).The expression of caspase-11,gasdermin D(GSDMD),and N-GSDMD was determined by Western blot and the expressions of caspy2,gsdmeb,and IL-1βwere detected using quantitative real-time *** observation was performed through hematoxylineosin staining,and protein levels in bronchoalveolar lavage fluid were quantified using the bicinchoninicacid protein ***:SIB noticeably decreased caspase-11 and GSDMD-mediated pyroptosis and suppressed the secretion of IL-1α,IL-1β,and IL-18 induced by LPS(P<0.05).Moreover,SIB inhibited the translocation of LPS into the cytoplasm and the binding of caspase-11 and intracellular LPS(P<0.05).SIB also attenuated the expression of caspase-11 and N-terminal fragments of GSDMD,inhibited the relative cytokines,prolonged the survival time,and up-regulated the survival rate in the endotoxemia models(P<0.05).Conclusions:SIB can inhibit pyroptosis in the LPS-mediated endotoxemia model,at l

Medium-and long-term efficacy of ligustrazine plus conventional medication on ischemic stroke: a systematic review and meta-analysis

OBJECTIVE： To evaluate the medium- to long-term efficacy of ligustrazine plus conventional medicine treating ischemic stroke. METHODS： Randomized controlled trials （RCTs） testing ligustrazine in the treatment of acute isch- emic stroke were retrieved from Cochrane Library, PubMed, Excerpta Medica Database, chinese Medi- cal Journal Database, chinese Biomedical Data- base, China National Knowledge Infrastructure Da- tabase, and chinese Clinical Trial Register, and then identified by the inclusive and exclusive criteria. The quality of trials was assessed with the Co- chrane Handbook 5.1, a risk of bias assessment toot. RevMan 5.1 was used for ***： Three RCTs involving 643 patients were included. Compared to conventional medicine treatment alone, ligustrazine plus conventional medicine treatment showed significant difference in reduction of stroke recurrence either at the end of 1-year follow-up [RR=0.42, 95% CI （0.18, 0.94）, P〈 0.05] or 3-years observation [RR=0.48, 95% C/（0.27, 0.83）, P〈0.05]. The ligustrazine group also showed higher survival rate [RR=1.67, 95% CI （1.02, 0.2.71）, P〈0.05] and significantly better effective rate [RR= 1.28, 95% CI（1.10, 1.50）, P〈0.05] than that of the control group at the end of 1 year visit. Only one tri- al conducted safety assessment and no adverse events were reported. The methodological quality of all the trials included was generally poor. CONCLUSION： The findings provided evidence that the combination of ligustrazine and conven- tional medication was medium- and long-term ben- eficial to the patients suffering ischemic stroke. But more RCTs of high quality are needed to further prove the efficacy and safety of using ligustrazine for ischemic stroke.

Ventricular remodeling （VR） can be induced by myocardial injury, leading to progressive cardiac dysfunction and heart failure, and is associated with high morbidity and mortality. Despite being studied for more than 3 decades, current therapeutic strategies still remain unsatisfactory in efficacy, expensive, and with side effects and drug resistances. chinese medicine （CM） has been used to treat heart diseases for thousands of years. This article reviews the published studies on the mechanisms and therapeutic applications of CM in VR. The major aspects include： mechanistic studies of VR, molecular biology and myocardial functional studies of CM therapies on VR, and mechanism of CM therapies on VR.
