Hoarding is defined as the acquisition and failure to discard a large number of possessions,resulting in significant cluttering and distress to themselves and *** the severity of hoarding behaviour increases with age,the risk of vulnerability increases,especially in older people with *** pathological comportment tends not to be identified until the patient is hospitalized and needs a home visit for safe discharge planning or when major concerns arise from family or the *** hoarding disorders from secondary conditions with hoarding behaviour can be challenging.A number of assessments and intervention strategies for hoarding can be implemented to improve the health and safety of these patients.A case of a patient with hoarding behaviour in a patient with dementia is described.

Background:Brunei has a rapidly ageing population,with a high rate of non-communicable *** is expected that there will be an increasing number of people with dementia locally.A review of clinical encounters for clinical services by people with dementia is ***:The Brunei Health Information Management System(Bru-HIMS)is a nationwide medical electronic record used in all government health *** clinical encounters require manual entry of ICD-10 codes by the *** encounters related to all ICD-10 codes for dementia were identified for the years 2015 to 2021(up to 15th October 2021).Results:From 2015 to 2019,there was an annual progressive increase in the number of patients and clinical encounters related to an ICD-10 diagnosis of a dementia ***,there was a plateau or decrease in the number of encounters in 2020 and 2021,possibly due to the COVID-19 *** mean number of clinical encounters for dementia was 500 annually,with a mean of 115 new patients *** most common ICD-10 dementia diagnosis given was unspecified dementia(39.8%),followed by Alzheimer’s disease(37.5%),vascular dementia(16.6%)and Parkinson’s disease dementia(5.7%).In 2015,the majority of clinical encounters were with psychiatry(72.2%),followed by neurology(17.2%)and geriatrics(5.7%).However,by 2020,geriatrics provided 47.2%of the clinical encounters for dementia,followed by psychiatry(24.8%)and neurology(21.5%).Conclusion:This study showed an increase in dementia diagnosis in older people over time,as evidenced by the increase in geriatrics consultations for dementia.A dementia registry is warranted to observe the demographic change of the increasing number of older people with dementia locally over time to assist with planning for service provision for people with dementia.

Objectives To estimate the prevalence of malnutrition in elderly cancer patients and its association with frailty and primary cancer treatment recommendations in patients with the two most prevalent cancers(colorectal cancer,CRC and breast cancer,BC)in the Centre-Val de Loire region of *** The entire cohort of 704 patients aged 75 years or older presenting with cancer who underwent comprehensive geriatric assessment(CGA)between 2014 and 2017 was *** status,frailty(defined by the Balducci classification system based on CGA parameters and comorbidity),and pathological criteria were analyzed in terms of the cancer treatment recommendations suggested by geriatricians both in the whole cohort and in those with CRC and *** In the whole group of 704 patients(84.3+/-4.8 years),the prevalence of malnutrition was 62.9%,and was higher in CRC than in BC patients(71.1%vs 55.4%,Ppalliative care to CRC patients presenting with malnutrition(15.4%vs 2.7%,P=0.006)than to BC patients(9.8%vs 5.4%,NS).Conclusion Malnutrition in elderly cancer patients is prevalent,especially in those with colorectal cancer,where malnutrition is frailty-related and may strongly impact on cancer treatment strategies.

A 90-year-old man was diagnosed with primary gastric diffuse large B-cell lymphoma(PGDLBL)by PET/CT examination,gastroscopy,biopsy and histopathological analysis at a regular physical check in April,*** patient received R-CO chemotherapy(rituximab,cyclophosphamide,and vincristine)and radiotherapy subsequently,with enteral nutritional treatment through 3-cavity nasogastric tube due to development of pyloric *** satisfy patient's strong desire of eating by himself,we performed surgery of exploratory laparotomy and Roux-en-Y gastric bypass(RGB)to relieve pylorus ***,the patient resumed oral feeding,supplemented by nasogastric tube feeding at 1350-1550 Kcal *** is now 94 years old with fairly well nutrition and normal *** outcome of 4 year follow-up suggests that nutritional treatment and palliative medicine are important for improving prognosis and life-quality of very elderly patients with end-stage tumors apart from the effective chemotherapy,radiotherapy,and surgery.

Objectives The in-hosptial palliative care consultation(PCC) is emerging as a routine service in some medical center in China. The current study evaluated how physicians in primary care team and consultation team perceive the PCC service for the purpose of investigating the effectiveness of this consultation model in a general *** In-hosptial palliative care consultations have been carried out at Peking Union Medical College Hosptial by a dedicated consultation team, and 37 consultations were completed in 2016. A questionnaire was designed for physicians in terms of its benefits to patients,their family as well as the primary care team. Physicians who applied for consultation in 2016 formally(requested from the department other than the geriatrics) and informally(by rotating residents and unemployed visiting doctors in geriatric department) were invited to participate in the survey by scanning a two dimentional code on social networking *** There were 103 physicians participated in the survey, including primary care physicians from the department of Internal Medicine(n=8), Gynaecology(n=16) and Surgery(n=13), rotating residents(n=30), visiting doctors(n=16) in Geriatric department, and PCC team members(n=20). 94.0% of the non-PCC physicians agreed that PCC relieved the suffering of patients; 89.2% thought PCC improved the quality of patients' life; there were 91.6%, 95.2%, 90.4% physicians who felt it relieved the anxiety of patients, of family members and of care providers, respectively. There were 96.4% physicians who felt it could ease the tension in physician-patient relationship; 97.6% felt it lower the risk for medical negligence, and 96.4% of doctors who applied for PPC felt satisfied with PCC service in terms of process and achieving objectives of consultation.
