Profiles of Major Cognitive Disorders in Elderly People Followed at the Geriatric Clinic of Fann National University Hospital in Senegal

Introduction: The aging of the world’s population is accompanied by an increase in the prevalence of chronic pathologies, including major neurognitive disorders (MNCD), which are a true global public health problem. The aim of this study was to update the epidemiological, diagnostic and therapeutic aspects of MNCD in elderly subjects in the first geriatric university department in Senegal and the sub-region. Materials and Methods: This was a retrospective and descriptive study from January 1, 2020 to January 1, 2021. It concerned subjects aged 65 years and over received in the department with MNCD diagnosed by using the Senegal test and the IADL score of Lawton. Results: Out of a total of 510 patients, major cognitive impairment was diagnosed in 61, representing a prevalence of (11.9%). The mean age was 70 years ±7 (extremes 65 - 98 years), with a male predominance (52%), (sex ratio 1.1). Sixty-four percent (64.10%) of the subjects were married, (25%) illiterated, (93.44%) lived with their families and fifty percent (50.81%) were well cared for. Antecedents and comorbidities included arterial hypertension (59.02%). Neurocognitive manifestations were dominated by memory disorders (62.30%), followed by anxiety and/or hallucinations (52.45%). MNCD were severe in (63.93%). The etiologies of CMND were dominated by Alzheimer’s disease (46%), followed by vascular causes (31%). Geriatric syndromes associated with dementia were dominated by loss of autonomy (88.52%) and frailty (65.57%). Memantine was the most prescribed anti-dementia treatment (72.13%). Psychosocial care in the form of home assistance was provided in 88.52% of cases. Conclusion: The prevalence of MNCD remains high compared to data from the African region. This indicates the importance of organizing the management of cases with serious consequences for caregivers. The etiologies remain dominated by Alzheimer’s disease and vascular MNCD.

Combined use of extended-release niacin and atorvastatin： safety and effects on lipid modification

Background Cholesterol-lowering therapy with statins has been reported to reduce the morbidity and mortality of cardiovascular diseases. This study aimed to investigate the effects of combined application of extended-release niacin and atorvastatin on lipid profile modification and the risks of adverse events in patients with coronary artery disease. Methods Consecutive 108 patients with coronary artery disease and serum total cholesterol （TC）〉 3.5 mmol/L were randomized into two groups： group A using atorvastatin and group B using extended-release niacin （niacin ER） and atorvastatin. Plasma lipid profile, glucose, and adverse events were assessed at the hospitalization, and 6 and 12 months after treatment. In addition, clinical cardiovascular events were evaluated after 12 months of treatment. Results The levels of TC, low density lipoprotein cholesterol （LDL-C） were significantly decreased （P 〈0.05） in groups A and B, but the levels of high density lipoprotein cholesterol （HDL-C） and ApoA increased by 29.36% and 40.81% respectively after 12 months of treatment in group B （P 〈0.01）. The medications were generally well tolerated in the two groups. No significant difference of adverse events was found between the two groups （group A： 3.2% vs group B 5.1%, P 〉0.05）. Conclusions Combined use of extended-release niacin with atorvastatin was superior to atorvastatin monotherapy alone in lipid profile regulation. Combination therapy with niacin ER and atorvastatin was well tolerated and safe in patients with coronary artery disease.

Clinical and Microbiological Characteristics of Patients with Complicated Intra-abdominal Infections in Intensive care Unit

In order to investigate the clinical and microbiological characteristics of patients with complicated intra-abdominal infections(cIAIs)in intensive care unit(ICU),the clinical data of 612 cIAIs patients from January 2016 to December 2018 were retrospectively *** characteristics,distribution of pathogens and drug resistance were statistically *** was found that patients with community-acquired intra-abdominal infections(CA-IAIs)made up a majority of cIAIs *** positive rate of abdominal drainage fluid culture was 55.56%.Gramnegative bacteria accounted for the majority,the most commonly isolated bacteria of which were Escherichia coli(20.96%),Klebsiella pneumoniae(10.20%)and Pseudomonas aeruginosa(5.57%).The most commonly isolated gram-positive bacteria were Enterococcus(16.88%)and Methicillinresistant staphylococcus aureus(MRSA,3.90%).Enterobacter isolates showed high resistance rate to most cephalosporins and low resistance rate to piperacillin/tazobactam and *** spectrum beta-lactamase(ESBL)screen positive isolates from CA-IAIs patients showed an increasing trend in past three *** and MRSA showed high resistance rate to clindamycin,quinolone,erythromycin and tetracycline,while they showed high sensitivity rate to linezolid,tegacycline,teicoplanin and *** results indicate that isolated bacteria from abdominal drainage fluid show high resistance rates to commonly used antibiotics in ICU patients with *** curative effects on diseases should be monitored continuously when antibiotics are ***,we should always keep eyes on drug-resistant bacteria,especially when the treatment efficacy is not good.

Examining older patient preferences for quality of care in postacute transition care and day rehabilitation programs

Background: Quality in health care has traditionally been determined based on clinical or health outcomes. However, these factors may not be the only aspects of health care that are important to patients. Within rehabilitation factors related to the process of care, the way in which rehabilitation services are delivered, may also be important to patients when defining quality of care. Objective: The purpose of this study was to examine and compare the preferences of older people receiving post-acute outpatient rehabilitation or residential intermediate (transition) care for alternative configurations of rehabilitation programs. Methods: A discrete choice experiment (DCE) was designed to elicit the preferences of older people for the design and delivery of post-acute rehabilitation programs. The participants were older adults (≥65 years) receiving post-acute outpatient rehabilitation or residential intermediate (transition) care in South Australia. Each participant was presented with a series of choice questions involving two hypothetical programs, the characteristics of which varied in every choice. Participants were then asked to select their preferred program. Results: Despite marked differences in case-mix and dependency levels, the preferences of the two groups were very similar, focusing on relationships and communication with health care professionals. Both groups demonstrated very strong preferences for the use of an electronic medical record and for receiving information about their treatment and progress via a meeting with a specialist physician and nurse. The outpatient rehabilitation group also exhibited a strong preference for a shared decision making model in relation to their future care needs. Conclusions: The findings highlight the commonality of preferences of older patients receiving post-acute services for the optimal configuration of rehabilitation services. The Impact of a Multidisciplinary Nursing Model on the Quality of Life of COPD Patients in Remission in the Elderly

Objective:To study the effects of multidisciplinary nursing model on self-care ability,lung function,exercise endurance,negative emotion and quality of life of elderly patients with chronic obstructive pulmonary disease(COPD).Methods:80 patients with COPD hospitalized in the Aerospace Center Hospital from August 2019 to August 2020 were *** were randomly divided into control group and experimental group,with 40 patients in each *** patients in the control group received routine nursing,and the patients in the experimental group received multidisciplinary nursing intervention on the basis of routine *** nursing observation time of the two groups was 8 ***:after 8 weeks of nursing intervention,it was found that FEV1/FVC and 6MWD in the observation group were significantly higher than those in the control group(P<0.05).There was no significant difference in SAS score and SDS score between the two groups before nursing,but it was significantly lower in the observation group after *** difference between the above data was statistically significant(P<0.05).The SGRQ score of the observation group was significantly lower than that of the control group(P<0.05).Conclusion:the application of multidisciplinary nursing model can effectively improve the exercise endurance of elderly patients with COPD,eliminate the negative emotions of patients,and improve the quality of life of *** comprehensive application value is significant.

Chronic intermittent hypoxia induces cardiac inflammation and dysfunction in a rat obstructive sleep apnea model

Chronic intermittent hypoxia is considered to play an important role in cardiovascular pathogenesis during the development of obstructive sleep apnea（OSA）.We used a well-described OSA rat model induced with simultaneous intermittent *** Sprague Dawley rats were individually placed into plexiglass chambers with air pressure and components were electronically *** rats were exposed to intermittent hypoxia 8 hours daily for *** changes of cardiac structure and function were examined by *** cardiac pathology,apoptosis,and fibrosis were analyzed by H＆E staining,TUNNEL assay,and picosirius staining,*** expression of inflammation and fibrosis marker genes was analyzed by quantitative real-time PCR and Western *** intermittent hypoxia/low pressure resulted in significant increase of left ventricular internal diameters（LVIDs）,endsystolic volume（ESV）,end-diastolic volume（EDV）,and blood lactate level and marked reduction in ejection fraction and fractional *** intermittent hypoxia increased TUNNEL-positive myocytes,disrupted normal arrangement of cardiac fibers,and increased Sirius stained collagen *** expression levels of hypoxia induced factor（HIF）-l α,NF-κB,IL-6,and matrix metallopeptidase 2（MMP-2） were significantly increased in the heart of rats exposed to chronic intermittent *** conclusion,the left ventricular function was adversely affected by chronic intermittent hypoxia,which is associated with increased expression of HIF-lα and NF-κB signaling molecules and development of cardiac inflammation,apoptosis and fibrosis.

Satisfaction of Patients Examined with Mobile X-Ray vs. X-Ray at the Hospital—A Randomized Controlled Trial

Background: In the Municipality of Aarhus, Denmark, mobile X-ray is offered to selected patients when a transfer to the department of Radiology may be an obstacle. To our knowledge, no studies have examined patient’s satisfaction with mobile X-ray in a randomized controlled trial, but international qualitative and quantitative studies report a high level of patient satisfaction with mobile X-ray. Purpose: The purpose of the study was to investigate whether patients in aged care facilities who were offered mobile X-ray were more satisfied with the X-ray examination compared to patients examined with X-ray at the department of Radiology, Aarhus University Hospital. Design: A part of a randomized controlled trial. Methods: Satisfaction was measured using a self-developed questionnaire, containing 13 questions measuring satisfaction from different perspectives. Participants: Due to patient’s fragility, healthcare staff members answered questions concerning satisfaction on behalf of the patients, who had been examined with mobile X-ray (n = 66) or X-ray at the hospital (n = 63). The patients were living in nursing homes and homes for the elderly in Aarhus Municipality. Data: Data were collected and stored using the computer program REDCap. Data were statistically analyzed using Fisher’s exact test. Results: Patients examined with mobile X-ray had a significantly higher satisfaction rate than those examined with X-ray at the hospital. Conclusion: Satisfaction of patients examined with X-ray was reported by healthcare staff to be in favor of mobile X-ray.
