A microgravity environment has been shown to cause ocular damage and affect visual acuity,but the underlying mechanisms remain ***,we established an animal model of weightlessness via tail suspension to examine the pathological changes and molecular mechanisms of retinal damage under *** 4 weeks of tail suspension,there were no notable alterations in retinal function and morphology,while after 8 weeks of tail suspension,significant reductions in retinal function were observed,and the outer nuclear layer was thinner,with abundant apoptotic *** investigate the mechanism underlying the degenerative changes that occurred in the outer nuclear layer of the retina,proteomics was used to analyze differentially expressed proteins in rat retinas after 8 weeks of tail *** results showed that the expression levels of fibroblast growth factor 2(also known as basic fibroblast growth factor)and glial fibrillary acidic protein,which are closely related to Müller cell activation,were significantly *** addition,Müller cell regeneration and Müller cell gliosis were observed after 4 and 8 weeks,respectively,of simulated *** findings indicate that Müller cells play an important regulatory role in retinal outer nuclear layer degeneration during weightlessness.

Effect of initial stent position on patency of transjugular intrahepatic portosystemic shunt

AIM To evaluate the effect of initial stent position on transjugular intrahepatic portosystemic shunt(TIPS).METHODS We studied 425 patients from January 2004 to January 2015 with refractory ascites or variceal bleeding who required TIPS placement. Patients were randomly divided into group A(stent in hepatic vein, n = 57), group B(stent extended to junction of hepatic vein and inferior vena cava, n = 136), group C(stent in left branch of portal vein, n = 83) and group D(stent in main portal vein, n = 149). Primary unassisted patency was compared using Kaplan-Meier analysis, and incidence of recurrence of bleeding, ascites and hepatic encephalopathy(HE) were *** The mean primary unassisted patency rate in group B tended to be higher than in group A at 3, 6 and 12 mo(P = 0.001, 0.000 and 0.005), and in group D it tended to be lower than in group C at 3, 6 and 12 mo(P = 0.012, 0.000 and 0.028). The median shunt primary patency time for group A was shorter than for group B(5.2 mo vs 9.1 mo, 95%CI: 4.3-5.6, P = 0.013, logrank test), while for group C it was longer than for group D(8.3 mo vs 6.9 mo, 95%CI: 6.3-7.6, P = 0.025, log-rank test). Recurrence of bleeding and ascites in group A was higher than in group B at 3 mo(P = 0.014 and 0.020), 6 mo(P = 0.014 and 0.019) and 12 mo(P = 0.024 and 0.034. Recurrence in group D was higher than in group C at 3 mo(P = 0.035 and 0.035), 6 mo(P = 0.038 and 0.022) and 12 mo(P = 0.017 and 0.009). The incidence of HE was not significantly different among any of the groups(P = 0.965).CONCLUSION The initial stent position can markedly affect stent patency, which potentially influences the risk of recurrent symptoms associated with shunt stenosis or occlusion.

Bladder tumors:dynamic contrast-enhanced axial imaging,multiplanar reformation,three-dimensional reconstruction and virtual cystoscopy using helical CT

Background There have been few studies to evaluate the effects of helical CT on bladder tumor. This study was to evaluate the clinical applications of helical CT dynamic contrast-enhanced axial imaging,multiplanar reformation (MPR),three-dimensional (3D) reconstruction and virtual cystoscopy (CTVC) in bladder tumors. Methods The precontrast and four-phase postcontrast helical CT scans were performed in 42 patients with bladder tumors confirmed by conventional cystoscopy and pathology. MPR,3D and CTVC images were generated from the volumetric data of the excretory phase. The results were then compared with the findings of conventional cystoscopy and surgery in a double-blinded mode. Results The sensitivity of the axial,3D and CTVC images in detecting the bladder tumors were 90.8%,76.9% and 95.4% respectively. The dynamic contrast-enhanced axial images could provide excellent intramural and extravesical information,and the accuracy in preoperative tumor staging was 87.7%. MPR could directly demonstrate the origin and extravesical invasions of the tumors and their relation to the ureter. 3D and CTVC images were useful for displaying the surface morphology of the tumor and the relationship between the tumor and the ureteric orifices,whereas CTVC could depict the tumors smaller than 5 mm that were not seen on the axial images. Conclusions The combination of axial,MPR,3D and CTVC images with helical CT can provide comprehensive information on bladder tumor.

Idiopathic abdominal cocoon syndrome with unilateral abdominal cryptorchidism and greater omentum hypoplasia in a young case of small bowel obstruction

Abdominal cocoon syndrome(ACS) is a rare cause of intestinal obstruction due to total or partial encapsulation of the small intestine by a fibrocollagenous membrane. Idiopathic ACS with abdominal cryptorchidism and greater omentum hypoplasia is even rarer clinically. We successfully treated a 26-year-old male case of small bowel obstruction with acute peritonitis. He was finally diagnosed with idiopathic ACS with unilateral abdominal cryptorchidism and greater omentum hypoplasia during exploratory laparotomy. He then underwent enterolysis, cryptorchidectomy, and appendectomy. He recovered gradually from the operations and early postoperative inflammatory ileus. There has been no recurrence of intestinal obstruction since the operation, and he is still in follow-up. We analyzed his clinical data and retrospectively reviewed the literature, and our findings may be helpful for the clinical diagnosis and treatment on ACS.

Must Peutz-Jeghers syndrome patients have the LKB1/STK11 gene mutation?A case report and review of the literature

Peutz-Jeghers syndrome(PJS) is an autosomal dominant inherited disease, which is characterized by mucocutaneous pigmentation and multiple gastrointestinal hamartoma polyps. The germline mutation of LKB1/STK11 gene on chromosome 19 p13.3 is considered to be the hereditary cause of PJS. However, must a patient with PJS have the LKB1/STK11 gene mutation? We here report a case of a male patient who had typical manifestations of PJS and a definite family history, but did not have LKB1/STK11 gene mutation. By means of high-throughput sequencing technology, only mutations in APC gene(c.6662 T > C: ***2221 Thr) and MSH6 gene(c.3488 A > T: ***1163 Val) were detected. The missense mutations in APC and MSH6 gene may lead to abnormalities in structure and function of their expression products, and may result in the occurrence of PJS. This study suggests that some other genetic disorders may cause PJS besides LKB1/STK11 gene mutation.

Using LaserSight Astrapro Planner 2.2 Z software in corneal topography-guided laser in situ keratomileusis for myopia with asymmetric corneal shape

Angiographic embolization in the treatment of intrahepatic arterial bleeding in patients with blunt abdominal trauma

BACKGROUND: Angiographic embolization (AE) as an adjunct non-operative treatment of intrahepatic arterial bleeding has been widely used. The present study aimed to evaluate the efficacy of selective AE in patients with hepatic ***: Seventy patients with intrahepatic arterial bleeding after blunt abdominal trauma who had undergone selective AE in 10 years at this institution were retrospectively *** criteria for selective AE included active extravasation on contrast-enhanced CT, an episode of hypotension or a decrease in hemoglobin level during the non-operative treatment. The data of the patients included demographics,grade of liver injuries, mechanism of blunt abdominal trauma,associated intra-abdominal injuries, indications for AE,angiographic findings, type of AE, and AE-related hepatobiliary complications. RESULTS: In the 70 patients, 32 (45.71%) had high-grade liver injuries. Extravazation during the early arterial phase mainly involved the right hepatic segments. Thirteen (18.57%) patients underwent embolization of intrahepatic branches and the extrahepatic trunk and these patients all developed AE-related hepatobiliary complications. In 19 patients with AE-related complications, 14 received minimally invasive treatment and recovered without severe sequelae. CONCLUSIONS: AE is an adjunct treatment for liver *** and/or super-selective AE should be advocated to decrease the incidence and severity of AE-related hepatobiliary complications.
