Real-world evidence on the efficacy and safety of vonoprazanamoxicillin dual therapy for Helicobacter pylori treatment in elderly patients

BACKgROUND A dual therapy regimen containing amoxicillin is a common treatment option for the eradication of Helicobacter pylori(***).While substantial research supports the efficacy and safety of vonoprazan and amoxicillin(VA)dual therapy in the general population,there is still a lack of studies specifically focusing on its safety in elderly *** To evaluate efficacy and safety of VA dual therapy as first-line or rescue treatment for *** in elderly *** As a real-world retrospective study,data were collected from elderly patients aged 60 years and above who accepted VA dual therapy(vonoprazan 20 mg twice daily+amoxicillin 1000 mg thrice daily for 14 days)for *** eradication in the Department of gastroenterology at Peking university First Hospital between June 2020 and January *** status was evaluated by^(13)C-urease breath test 6 weeks after *** adverse events(Aes)during treatment were *** In total,401 cases were ***-one cases were excluded due to loss to follow-up,lack of re-examination,or unwillingness to take *** total of 380 included cases comprised 250 who received VA dual therapy as first-line treatment and 130 who received VA dual therapy as rescue *** was successfully eradicated in 239 cases(95.6%)in the first-line treatment group and 116 cases(89.2%)in the rescue treatment *** overall incidence of Aes was 9.5%for both ***,9.2%of patients experienced an Ae in the first-line treatment group and 10.0%in the rescue treatment *** patients discontinued treatment due to Ae,with a discontinuation rate of 1.3%.No serious Ae *** The VA dual therapy regimen as a first-line treatment and a rescue therapy was effective and safe for elderly patients aged 60 and older.

The brain's extracellular matrix(eCM),which is comprised of protein and glycosaminoglycan(gAg)scaffolds,constitutes 20%-40% of the human brain and is considered one of the largest influencers on brain cell functioning(Soles et al.,2023).Synthesized by neural and glial cells,the brain's eCM regulates a myriad of homeostatic cellular processes,including neuronal plasticity and firing(Miyata et al.,2012),cation buffering(Moraws ki et al.,2015),and glia-neuron interactions(Anderson et al.,2016).Considering the diversity of functions,dynamic remodeling of the brain's eCM indicates that this understudied medium is an active participant in both normal physiology and neurological diseases.

The outcomes of magnetic sphincter augmentation in patients with gastroesophageal reflux disease post bariatric surgery: A systemic review and meta-analysis

Objective: Although bariatric surgeries are widely performed around the world, patients frequently experience the recurrence of pre-existing gastroesophageal reflux disease (geRD) symptoms or develop new symptoms, some of which are resistant to medical treatment. This study investigates the effect and outcome of magnetic sphincter augmentation (MSA), a minimally invasive treatment for geRD, in this population. Methods: A thorough search of the PubMed, Cochrane, Scopus, Web of Science, and google Scholar databases from inception until June 6, 2024 was performed to retrieve relevant studies that evaluated the effects of MSA on the geRD health-related quality of life (geRD-HRQL) score and the reduction in proton pump inhibitor (PPI) use in patients who underwent bariatric surgery. The “meta” package in RStudio version 2023.12.0 + 369 was used. Results: A total of eight studies were included in the systematic review and seven studies were included in the meta-analysis. MSA significantly reduced the geRD-HRQL score (MD = −27.55 [95% CI: −30.99 to −24.11], p e (RR = 0.23 [95% CI: 0.16 to 0.33], p e treatment option for patients with geRD symptoms who undergo bariatric surgery. This approach showed promising results in terms of reducing the geRD-HRQL score and reducing the use of PPI. © 2025 Zhejiang university

Material basis and sleep-improving mechanisms of Lily-Ziziphi spinosae semen Decoction：Systematic evidence from LC-MS， network pharmacology and animal experiments

Lily-Ziziphi spinosae semen Decoction (LZ) is known for its blood nourishing， mind calming， body sedation， and sleep promoting effects in traditional Chinese medicine. However， its material basis and underlying mechanisms have not yet been clearly defined. This study applies liquid chromatography-mass spectrometry， network pharmacology， and animal studies to reveal the material basis and sleep-improving mechanisms of LZ. The mixed decoction (LZ-Me) and single decoction (LZ-Se) were prepared to study their chemical components. Network pharmacology was used to predict the sleep-improving targets and signaling pathways of LZ. ICR mice were intragastrically administered saline (NC)， melatonin (positive control group， 0.50 mg/kg)， low (12.90 g/kg)， medium (25.70 g/kg)， and high (38.60 g/kg) dose of LZ-Me and LZ-SH for 30 days.. The results showed that LZ-Me could prolong the sleep duration and shorten the sleep latency in sodium barbiturate induced mice model. The results of chemical composition showed that total polysaccharides， total flavonoids， total saponins， and total alkaloids in LZ-Me were significantly higher than those in LZ-Se (177.20%， 82.34%， 30.58%， and 11.66%， respectively). A total of 58 chemical components were identified by UPLC-QTOF-MS， and eight representative difference components of LZ-Me and LZ-Se were found. LZ-Me significantly increased tumor necrosis factor (TNF-α) in mice serum and neurotransmitter γ-aminobutyric acid (gABA) levels in mice hippocampus， decreased dopamine (DA) and glutamate (glu) levels in mice hippocampus (P ermore， LZ-Me up-regulated the abundance of the beneficial bacteria Lactobacillus and eubacterium＿R， down-regulated the abundance of the harmful bacteria Lachnospiraceae et al. The polysaccharides， flavonoids (spinosin and 6?-feruloylspinosin) and saponins (jujuboside A and jujuboside B) were the main material bases for the sleep-promoting effects of LZ.
