Long-Term Postoperative Perfusion Indices in Surgically Resolved Myopic Traction Maculopathy

Background: Pathological myopia can be complicated by the presence of posterior staphyloma, macular atrophy, ruptures in Bruch’s membrane, pathologic choroidal neovascularization, and different degrees of myopic traction maculopathy. Purpose: To report the structural, functional and perfusional outcomes in patients underwent surgery for different stages of myopic traction maculopathy (MTM). Methods: A retrospective, consecutive, comparative, interventional, one-surgeon, case-control study was conducted in 46 eyes of 34 individuals between April 2015 and May 2021. Participants included normal emmetropic eyes (Control emmetropia, n = 25), healthy myopic eyes (Control high myopia, n = 20), and operated and structurally fully resolved myopic eyes with different stages of MTM (Surgically treated group, n = 46). Long-term postoperative functional and perfusional follow-up evaluations were performed with spectral domain-optical coherence tomography (SD-OCT) and OCT angiography. The primary outcome measure included long-term functional, structural and perfusion macular status across groups. Results: Forty-six eyes in 34 patients were included in the study group, with both eyes affected in 12 (26.3%) patients. The mean axial length was 29.89 ± 1.67 mm. The preoperative logMAR was 1.29 ± 0.54 and the postoperative logMAR was 0.60 ± 0.52 (P Conclusion: Compared to emmetropic and healthy myopic eyes, surgically-resolved MTM eyes generally have larger superficial foveal avascular zone area, lower vessel density, smaller choriocapillaris flow area, thinner central subfoveal thickness, and more macular defects. Eyes with stage III or IV MTM had larger deviation compared to eyes at earlier stages. Visual function change after surgery was associated with superficial foveal avascular zone area. Better functional, structural and perfusion index outcomes were observed when highly myopic eyes underwent early surgery.

The relatively high percentage of people with disabilities in Europe combined with the facts of ageing population, strong relation of impairment to age, and as State of the Art shows, dissatisfaction or even unawareness of people with disabilities of available assistive technology are revealing the necessity to incorporate a user-centric approach that beyond 2nd generation practices will achieve to provide embedded and built-in accessibility solutions, as well as toolkits for developers, for ＂engraving＂ accessibility in existing and emerging mass-market ICT-based products, aiming to make accessibility open, plug and play, personalised and configurable, realistic and applicable in various contexts, keeping always the user in the loop. The AEGIS （Accessibility Everywhere： Groundwork, Infrastructure, Standards） IP （Integrated Project） of the 7th European Framework Programme seeks to determine whether 3rd generation access techniques will provide a more accessible, more exploitable and deeply embeddable approach in mainstream ICT （information and communication technologies）. This paper presents the holistic UCD （user-centered design） implementation plan, upon which AEGIS has been based in order to achieve its targets, starting from modelling its target users, in the most efficient way possible.

Objective:Long-term survivors(LS)of non-small cell lung cancer(NSCLC)without driver alterations,displaying an overall survival(OS)of more than 3 years,comprise around 10%of cases in several series treated with *** are classical prognosis factors for these cases[stage,Eastern Cooperative Oncology Group(ECOG),etc.],but more data are required in the *** this multi-center study,we focused on LS of advanced NSCLC with OS above 36 months to perform a clinical-pathological and molecular ***:In the first step,we conducted a clinical-pathological characterization of the ***,we carried out a genetic analysis by comparing LS to a sample of short-term survivors(SS)(with an OS less than 9 months).We initially used whole-genome RNA-seq to identify differentiating profiles of LS and SS,and later confirmed these with reverse transcription-polymerase chain reaction(RT-PCR)for the rest of the ***:A total of 94 patients were included,who were mainly men,former smokers,having adenocarcinoma(AC)-type NSCLC with an ECOG of *** obtained an initial differential transcriptome expression,displaying 5 over-and 33 under-expressed genes involved in different pathways:namely,the secretin receptor,surfactant protein,trefoil factor 1(T FF1),serpin,Ca-channels,and Tolllike receptor(TLRs)***,RT-PCR analysis of 40(20 LS/20 SS)samples confirmed that four genes(surfactant proteins and SFTP)were significantly down-regulated in SS compared to LS by using an analysis of covariance(ANCOVA)model:SFTPA1(P=0.023),SFTPA2(P=0.027),SFTPB{P=0.02),and SFT PC(P=0.047).Conclusions:We present a sequential genetic analysis of a sample of NSCLCLS with no driver alterations,obtaining a differential RNA-seq/RT-PCR profile showing an abnormal expression of SF genes.

Geometrical Evaluation of Cervical Cells. Fractal and Euclidean Diagnostic Methodology of Clinical Application

Background: The concomitant use of fractal and Euclidian measurements has led to the development of new methodologies of cell evaluation, including a diagnosis of cervical cells that set up differences between normality and various degrees of lesion, to carcinoma. Aim: To confirm the diagnostic capacity of the methodology based on fractal and Euclidian geometry for the mathematical diagnosis through a blind study of normal cells and with different types of lesion, as atypia of undetermined significance (ASCUS), low grade squamous intra-epithelial lesion (LGSIL) and high grade squamous intra-epithelial lesion (HGSIL). Methods: 100 cells of Papanicolaou tests were analyzed and divided into 4 groups according to conventional parameters: 25 normal, 25 ASCUS, 25 LGSIL and 25 HGSIL. By means of the Box-counting Fractal Space, we calculated the fractal dimension and occupying spaces of the border and surface in pixels of the cell nucleus and cytoplasm. The diagnostic parameters of the previously developed methodology were applied and compared with the conventional diagnosis, setting up sensibility, specificity, negative likelihood ratio and Kappa coefficient. Results: The values of the occupation of the border and surface of the cell nucleus and cytoplasm were consistent with the values found by the diagnostic methodology previously found. The subtraction of the nucleus and cytoplasm frontiers presented values between: 189 and 482 for normality;159 and 432 for ASCUS;126 to 401 for LGSIL and 39 to 122 for HGSIL. A sensitivity and specificity of 100%, and a Kappa coefficient of 1 were obtained. Conclusions: The capacity of the methodology to diagnose quantitatively the different stages in the evolution of the cervical cells observed in Papanicolaou tests was confirmed, from normality to HGSIL.

Objective:To determine the incidence of preeclampsia(PE)and preterm PE in Spain and to identify the risk factors for developing the ***:This is a multicenter prospective cohort study performed at six maternity units across *** with singleton pregnancies attending their first-trimester routine visit at the hospital were offered *** and pregnancy characteristics,including mean arterial pressure,as well as ultrasound findings were *** and multivariate logistic regression analyses were performed to identify independent risk factors for subsequent development of ***:A total of 5868 pregnancies were recruited for this study,including 174(3.0%)cases of PE,47(0.8%)cases of preterm PE and 127(2.2%)cases of term *** maternal age was 33.9 years(interquartile range:30.1 to 36.9)and median gestational age at the routine visit was 12.7 weeks(interquartile range:12.3 to 13.0).However,293(5.0%)of the women were on aspirin treatment during pregnancy,likely reducing the true incidence of the *** expected,increasing body mass index(P<0.001),uterine artery pulsatility index(P=0.011)and mean arterial pressure(P<0.001),assisted conception(P=0.013),previous personal(P<0.001)or family history of PE(P=0.024)and chronic hypertension(P=0.001)were identified as independent risk factors for developing subsequent PE during *** for PE by maternal factors alone leads to a detection rate of 36.8%(64/174)at 10.0%(587/5868)screen positive ***:In Spain,3.0%of singleton pregnancies are complicated by PE and 0.8%require delivery before term due to its *** of PE by risk factors alone is only able to detect about 40%of total PE at 10%screen-positive rate.

Attitudes towards prescribing psychiatric medicines: Do the views of psychology and medical students differ?

Concordance has been suggested as a process of the consultation in which prescribing process is based on partnership. The aim of this cross-sectional study was to compare attitudes towards psychiatric medication and concordance in medicine taking between medical and psychology students, as they represent future members of mental health teams. Two hundred and sixteen medical students and 222 psychology students completed the Leeds Attitudes toward Concordance scale (LATCon) and the Beliefs about Medication Questionnaire (BMQ) (both adapted for psychiatric medication). Psychology students saw psychiatric medicines as more harmful and were also most likely to believe that psychiatrists overprescribed these medicines. They also scored higher than medical students on attitudes towards concordance, although this difference remained at the limit of statistical significance.
