Early Kinetics of Procalcitonin in Predicting Surgical Outcomes in Type A Aortic Dissection Patients

Background： In cardiac surgery, elevation of procalcitonin （PCT） could be observed postoperatively in the absence of any evidence of infection and also seems to be a prognostic marker. PCT levels measured in patients undergoing Type A aortic dissection （TAAD） were used to determine prognostic values for complications and surgical outcomes. Methods： Measurements of PCT, C-reactive protein （CRP）, and leukocyte count were observed in TAAD surgery patients （n = 251 ; average age： 49.02 ± 12.83 years; 78.5% male） at presurgery （TO） and 24 h （T1）, 48 h （T2）, and 7 days （T3） postsurgery. PCT clearance （PCTc） oll days 2 and 7 was calculated：（PCTd - PCTd,±,, d,,v）/PCTdx 100%. Endotracheal intubation duration, length of stay （LOS） in the Intensive Care Unit （1CU）/hospital, and complications were recorded± Results： PCT peaked 24 h postsurgery （median 2.73 ng/ml） before decreasing, Correlation existed between PCT levels at T1 and duration of cardiopulmonary bypass （P = 0.001, r = 0.278）. Serum PCT concentrations were significantly higher in nonsurvivor and multiple organ dysfunction syndrome groups on all postoperative days. PCT levels at T1 correlated with length of time of ventilation support and ICU/hospital LOS. Comparing PCT values of survivors versus nonsurvivors, a PCT cutoff level of 5.86 ng/ml at T2 had high sensitivity （70.6%） and specificity （74.3%） in predicting in-hospital death. PCTc-day 2 and 7 were significantly higher in survivor compared with nonsurvivor patients （38% vs. 8%, P = 0.012, 83% vs. -39%, P 〈 0.001）. A PCTc-day 7 cutoff point of 48.7% predicted survival with high sensitivity （77.8%） and specificity （81.8%）. Conclusions： PCT level and PCTc after TAAD surgery might serve as early prognostic markers to predict postoperative outcome. PCT measurement may help identify high-risk patients.

Phthalate Exposure and Human Semen Quality in Shanghai:A Cross-sectional Study

Objective To monitor the level of phthalates in human semen samples and to analyze the relationship between phthalate levels and semen parameters. Methods Concentrations of three kinds of commonly used phthalates （di-ethyl phthalate, DEP; di-n-butyl phthalate, DBP; di-2-ethylhexyl phthalate, DEHP） were measured using reversed-phase HPLC. Semen parameters were measured by computer aided sperm analysis （CASA）. Results The three phthalates were detected in most of the biological samples, with median levels of 0.30 mg/L （0.08-1.32 mg/L） in semen specimens. There was a significant positive association between liquefied time of semen and phthalate concentrations of semen. The correlation coefficient was 0.456 for DEP, 0.475 for DBP, and 0.457 for DEHP, respectively. There was no significant difference between phthalate concentrations of semen and sperm density or livability, though the correlation coefficients were negative. Conclusion These results suggest that people who reside in Shanghai are exposed to phthalates, especially to DBP and DEHP. Although the level of phthalates is relatively mild, an association of phthalate levels and reduced quality of human semen has been shown in the present study.

The air quality guidelines (AQGs) issued by the World health Organization (WHO) in 2005 are key references for the National Ambient Air Quality Standards (NAAQSs) in China.On 22September 2021,the WHO issued new AQGs (NAQGs)[1],which propose more stringent targets to control exposure to multiple air pollutants,including fine particulate matter (PM2.5) and ozone (O3),to which,in 2019,451 million premature deaths,globally,were attributable [2].

Neonatal care is of great significance to health capabil-ity.Since neonatal intensive care units(NICUs)started to be established in the late 1980s in China,neonatal mortal-ity has decreased each year to reach 3.4‰in 2020[1,2].However,the improvement for neonatal care is still on the way.More than three-fifths of deaths included critically ill neonates occur in their first 28 days[3].Critically ill neo-nates usually have lower gestational age and body weight,leading to high mortality or requiring specific treatment and monitoring[4].It is a common practice to transfer a critically ill newborn patient to a tertiary children's hospi-tal with an excellent NICU.Therefore,it is crucial to pro-mote patients'safety by improving the neonatal transport network services[5].Owing to the complexity and rapid changes in neonatal conditions,especially for those requir-ing multi-disciplinary treatment,there is a high reliance on a specialized newborn transport team[6,7].

Inhibitory Effect of Isoflavones on Prostate Cancer Cells and PTEN Gene

Objective To explore the mechanisms by which genistein and daidzein inhibit the growth of prostate cancer cells. Methods LNCaP and PC-3 cells were exposed to genistein and daldzein and cell viability was determined by MTF assay and cytotoxicity of the drugs by LDH test. Flow cytometry （FCM） was used to assess the cell cycle in LNCaP and PC-3 cells. Reverse transcription-polymerase chain reaction （RT-PCR） was applied to examine the expression of PTEN gene （a tumor suppressor gene）, estrogen receptor alpha gene （ERα）, estrogen receptor beta gene （ERβ）, androgen receptor gene （AR） and vascular endothelial growth factor gene （VEGF）. Results The viability of PC-3 and LNCaP cells decreased with increasing concentrations and exposure time of genistein and daidzein. Genistein increased G2/M phase cells in PC 3 cells while decreased S phase cells in LNCaP cells in a dose-dependent manner. Daidzein exerted no influence on the cell cycle of LNCaP and PC-3 cells, but the apoptosis percentage of LNCaP cells was elevated significantly by daidzein, Genistein induced the expression of PTEN gene in PC-3 and LNCaP cells. Daidzein induced the expression of PTEN gene in LNCaP but not in PC-3 cells. The expression of VEGF, ERα and ERβ genes decreased and AR gene was not expressed after incubation with genistein and daldzein in PC-3 cells. In LNCaP cells, the expression of VEGF and AR gene decreased but there was no change in the expression of ERα and ERβ gene after incubation with genistein and daidzein. Conclusion Genistein and daidzein exert a timeand dose-dependent inhibitory effect on PC-3 and LNCaP cells. The down-regulation of ER gene by daidzein influences the growth of PC-3 ceils directly, The inhibition of PC-3 cells by genistein and that of LNCaP cells by genistein and daidzein may be via Alct pathway that is repressed by PTEN gene, which subsequently down-regulates the expression of AR and VEGF genes. The implementation and evaluation of HIV symptom management guidelines: A preliminary study in China

Objective:The overarching objective of this study was to examine the effectiveness of HIV symptom management guidelines in China in reducing the incidence and severity of symptoms and improving patients'quality of life.Methods:We conducted a controlled,pre-and post-implementation design in the HIV/AIDS inpatient unit in Shanghai.Patients recruited from November 2014 to February 2015 were in the intervention group and those from October 2013 to February 2014 were in the control group.There were 74 patients in each group.Participants in the intervention group received interventions based on the HIV symptom management guidelines.Overall symptom severity,depression,and quality of life were measured in two groups at baseline,week 4,and week 8.Results:Totally 126 patients completed the research,65 in the intervention group and 61 in the control group.The total symptom severity scores showed a statistically significant difference between groups across time(P0.05)in the intervention group were lower than those in the control group in week 8 without statistical significance.The multilevel growth mixture model indicated a greater increase in the total score of quality of life for the group treated according to the symptom management guidelines(P=0.04).Conclusion:The evidence-based HIV symptom management guidelines can improve a patient's quality of life and relieve negative symptoms.The guidelines can be applied in a similar context to other HIV/AIDS units or clinics.

Association of the genetic polymorphism of EPHX1 and EPHX2 with the susceptibility to chronic benzene poisoning

The aim of this study was to explore the association of the genetic polymorphism of EPHX1 and EPHX2 with the susceptibility to chronic benzene poisoning(CBP).A case-control study of 268 patients with CBP and 268 healthy workers matched by age and sex,all of whom were occupationally exposed to benzene,was conducted.The single nucleotide polymorphisms(SNPs,rs2854451,rs3738047,rs2234922 and rs1051741)of EPHX1 gene and the SNP(rs751141)of EPHX2 gene were tested by the TaqMan PCR method.In the subjects carrying the genotype of EPHX1 rs3738047 GG,the risk of CBP was decreased in the individuals simultaneously carrying EPHX1 rs2234922 G(P=0.02).Alternatively,in the subjects carrying the genotype of EPHX1 rs2234922 AA,the risk of CBP was increased in the individuals simultaneously carrying the allele of EPHX2 rs751141A(P=0.03).It was also found that there were potential interactions between alcohol consumption and the polymorphism of EPHX1 rs1051741(x_(H)^(2)=5.28,P=0.02)or rs2234922(x_(H)^(2)=6.71,P=0.01).Compared to individuals with EPHX1 rs1051741 CC or rs2234922 AA genotype in the drinkers,the risk of CBP in those carrying genotypes of EPHX1 rs1051741 CT+TT or rs2234922 AG+GG was decreased,respectively(P=0.04,P<0.01).Haplotype analysis of polymorphisms in EPHX1 showed that the risk of CBP was increased in the subjects with haplotype 2(rs2854451-A,rs3738047-G,rs2234922-A,rs1051741-C)or haplotype 4(rs2854451-G,rs3738047-A,rs2234922-G,rs1051741-T),but decreased in those with haplotype 6(rs2854451-G,rs3738047-G,rs2234922-G,rs1051741-T)or haplotype 10(rs2854451-A,rs3738047-A,rs2234922-G,rs1051741-T),respectively.Logistic regression analysis revealed that smoking might play a role in modifying the risk of CBP(OR=0.313,95%CI:0.123–0.794,P=0.015).The genetic polymorphism in EPHX1 may be associated with the risk of CBP in the Chinese occupational population and further research is needed for the association between the genetic polymorphism in EPHX2 and the susceptibility to CBP.

Combined Effect of Fluoride and Arsenate on Gene Expression of Osteoclast Differentiation Factor and Osteoprotegerin

Objective To study the combined effect of fluoride and arsenate on the expression of SD rat osteoblastic osteoclast differentiation factor （ODF） mRNA and osteoprotegerin （OPG） mRNA. Methods Osteoblasts were obtained by enzymatic isolation from newborn SD rats. A factorial experiment was performed. Osteoblasts were exposed to NaF （0.5 mmolF/L, 4 molF/L） and Na3AsH2 （12.5 μmolAs/L and 200 μmolAs/L） separately or F plus As and cultured for 48 h. The gene expression of osteoblastic ODF and OPG was observed by RT-PCR. Results The expression ofODF mRNA increased in F0.5, F4 groups compared with control group and two groups of F0.As200, F,As200 compared with As200 group, and decreased significantly in groups of F4As12.5, F0.5As200, and F4As200. The expression of OPG rnRNA decreased in groups of F4, As200, F4As12.5, F0.5As200, and F4As200. Conclusion The joint effect of fluoride and arsenate on the gene expression of ODF is antagonistic, while the combined effect on the gene expression of OPG is synergistic. F4, F4As12.5, and F0.5As200 promote bone resorption of rat osteoclasts, whereas F0.5As12.5 inhibits osteolytic effect of rat osteoclasts.

Association of Dietary Pattern during Pregnancy and Gestational Diabetes Mellitus： A Prospective Cohort Study in Northern China

Objective To examine the association of maternal dietary patterns during pregnancy with gestationa diabetes mellitus （GDM） in northern China. Methods The dietary intakes of pregnant women were recorded twice by 24-hour dietary recalls for three days prior to having been diagnosed with GDM, at 5-15 and 24-28 gestational weeks, respectively. GDM was diagnosed, and serum glycosylated hemoglobin （HbAlc） was measured at 24-28 weeks. Dietary patterns were assessed by factor analysis. The association of the dietary pattern with GDM and HbAlc was examined by multiple logistic models. Results Of 753 participants, 64 （8.5%） were diagnosed with GDM. Four dietary patterns were identified： Western pattern （dairy, baked/fried food and white meat）, traditional pattern （light-colored vegetables, fine grain, red meat and tubers）, mixed pattern （edible fungi, shrimp/shellfish and red meat） and prudent pattern （dark-colored vegetables and deep-sea fish）. Compared with the prudent pattern, both the Western pattern and the traditional pattern were associated with an increased risk of GDM （aOR = 4.40, 95% CI： 1.58-12.22; aOR = 4.88, 95% Ch 1.79-13.32） and a high level of HbAlc （aOR = 12.37, 95% Ch 1.47-103.91; aOR = 26.23, 95% CI： 2.54-270.74）. Compared to the lowest quartile （Q）, 0.3 of the Western pattern scores and Q3-Q4 of the traditional pattern scores were associated with a higher risk of GDM. Conclusion The consumption of the Western pattern or the traditional pattern during pregnancy may increase the risk of GDM.
