Cost-effectiveness analysis of malaria rapid diagnostic tests:a systematic review

Background:Rapid diagnostic tests(RDT)can effectively manage malaria cases and reduce excess costs brought by misdiagnosis.However,few studies have evaluated the economic value of this technology.The purpose of this study is to systematically review the economic value of RDT in malaria diagnosis.Main text:A detailed search strategy was developed to identify published economic evaluations that provide evidence regarding the cost-effectiveness of malaria RDT.Electronic databases including MEDLINE,EMBASE,Biosis Previews,Web of Science and Cochrane Library were searched from Jan 2007 to July 2018.Two researchers screened studies independently based on pre-specified inclusion and exclusion criteria.The Consolidated health Economic Evaluation Reporting Standards(CHEERS)checklist was applied to evaluate the quality of the studies.Then cost and effectiveness data were extracted and summarized in a narrative way.Fifteen economic evaluations of RDT compared to other diagnostic methods were identified.The overall quality of studies varied greatly but most of them were scored to be of high or moderate quality.Ten of the fifteen studies reported that RDT was likely to be a cost-effective approach compared to its comparisons,but the results could be influenced by the alternatives,study perspectives,malaria prevalence,and the types of RDT.Conclusions:Based on available evidence,RDT had the potential to be more cost-effective than either microscopy or presumptive diagnosis.Further research is also required to draw a more robust conclusion.

Infuence of urbanization on schistosomiasis infection risk in Anhui Province based on sixteen year’s longitudinal surveillance data:a spatio-temporal modelling study

Background Urbanization greatly afects the natural and social environment of human existence and may have a multifactoral impact on parasitic diseases.Schistosomiasis,a common parasitic disease transmitted by the snail Oncomelania hupensis,is mainly found in areas with population aggregations along rivers and lakes where snails live.Previous studies have suggested that factors related to urbanization may infuence the infection risk of schistosomiasis,but this association remains unclear.This study aimed to analyse the efect of urbanization on schistosomiasis infection risk from a spatial and temporal perspective in the endemic areas along the Yangtze River Basin in China.Methods County-level schistosomiasis surveillance data and natural environmental factor data covering the whole Anhui Province were collected.The urbanization level was characterized based on night-time light data from the Defense Meteorological Satellite Program Operational Linescan System(DMSP-OLS)and the National Polar-Orbiting Partnership’s Visible Infrared Imaging Radiometer Suite(NPP-VIIRS).The geographically and temporally weighted regression model(GTWR)was used to quantify the infuence of urbanization on schistosomiasis infection risk with the other potential risk factors controlled.The regression coefcient of urbanization was tested for signifcance(α=0.05),and the infuence of urbanization on schistosomiasis infection risk was analysed over time and across space based on signifcant regression coefcients.Variables studied included climate,soil,vegetation,hydrology and topography.Results The mean regression coefcient for urbanization(0.167)is second only to the leached soil area(0.300),which shows that the urbanization is the most important infuence factors for schistosomiasis infection risk besides leached soil area.The other important variables are distance to the nearest water source(0.165),mean minimum temperature(0.130),broadleaf forest area(0.105),amount of precipitation(0.073),surface temperature(0.066),soil bulk density(0.037)and gr

China’s population has rapidly aged over the recent decades of social and economic development as neurodegenerative disorders have proliferated,especially Alzheimer’s disease(AD)and related dementias(ADRD).AD’s incidence rate,morbidity,and mortality have steadily increased to make it presently the fifth leading cause of death among urban and rural residents in China and magnify the resulting financial burdens on individuals,families and society.The‘healthy China Action’plan of 2019-2030 promotes the transition from disease treatment to health maintenance for this expanding population with ADRD.This report describes related epidemiological trends,evaluates the economic burden of the disease,outlines current clinical diagnosis and treatment status and delineates existing available public health resources.More specifically,it examines the public health impact of ADRD,including prevalence,mortality,costs,usage of care,and the overall effect on caregivers and society.In addition,this special report presents technical guidance and supports for the prevention and treatment of AD,provides expertise to guide relevant governmental healthcare policy development and suggests an information platform for international exchange and cooperation.

Long-term case-fatality rate of nontuberculous mycobacterial disease in people living with HIV

Background:Few data are available regarding the long-term case-fatality rate(CFR)among people living with HIV(PLWH)with nontuberculous mycobacteria(NTM)disease.The aim of this study is to analyze the long-term CFR in patients with NTM disease and to identify risk factors for their death.Methods:A retrospective cohort study of 379 cases of microbiologically confirmed NTM disease in PLWH was conducted from January 1,2012,to December 31,2020,in Shanghai,China.We used Kaplan-Meier survival analysis and the log-rank test to compare the long-term CFR in patients with disseminated NTM(DNTM)and localized NTM disease.Univariate Cox proportional hazards regression analysis and a stepwise Cox proportional hazards regression model were used to estimate the predictors of long-term CFR.Results:The cohort was followed up for a median of 26 months.The total CFR was 15.7%by one year and increased to 22.6%at 5 years after the diagnosis of NTM disease.The 5-year CFR of PLWH with DNTM was significantly higher than that of PLWH with localized NTM(26.7%vs 19.6%for DNTM and localized NTM disease,respectively).Older age[hazard ratio(HR)=1.04,95%confidence interval(CI):1.02-1.06,P<0.001],comorbidity(HR=2.05,95%CI:1.21-3.49,P<0.01),DNTM(HR=2.08,95%CI:1.17-3.68,P<0.05),and HIV viral load(HR=1.32,95%CI:1.12-1.55,P<0.001)were all independent risk factors for long-term CFR.In the subgroup analysis,time to culture positivity was negatively correlated with CFR in patients with DNTM(HR=0.90,95%CI:0.82-0.98,P<0.05).Conclusions:NTM was associated with a high long-term CFR in PLWH.Further approaches to prevent NTM disease in PLWH are urgently needed.

Impact of immunization programs on 11 childhood vaccine-preventable diseases in China: 1950–2018

To evaluate the achievements of China’s immunization program between 1950 and 2018,we chose 11 vaccine-preventable diseases(VPDs)as representative notifiable diseases and used annual surveillance data obtained between 1950 and 2018 to derive disease incidence and mortality trends.Quasi-Poisson and polynomial regression models were used to estimate the impacts of specific vaccine programs,and life-table methods were used to calculate the loss of life expectancy,years of life lost,and loss of working years.The total notification number for the 11 VPDs was 211,866,000 from 1950 to 2018.The greatest number occurred in 1959,with a total incidence of 1,723 per million persons.From 1978 to 2018,a substantial decline was observed in the incidence of major infectious diseases.The incidence of pertussis fell 98%from 126.35 to 1.58 per million,and the incidences of measles,meningococcal meningitis,and Japanese encephalitis fell 99%,99%,and 98%,respectively.The regression models showed that most of the 11 diseases exhibited dramatic declines in morbidity after their integration into the Expanded Program on Immunization(EPI),while varicella and paratyphoid fever,which were not integrated into the EPI,showed increased morbidity.From 1978 to 2018,the total life expectancy for the 11 VPDs increased by 0.79 years,and similar results were obtained for different age groups.China has had great success in controlling VPDs in recent decades,and improving vaccination coverage is a key aspect of controlling VPDs in China.

Impact of Ambient Air Pollution on public health under Various Traffic Policies in Shanghai,China

Objective To investigate the potential impact of ambient air pollution on public health under various traffic policies in Shanghai. Methods The exposure level of Shanghai residents to air pollution under various planned traffic scenarios was estimated, and the public health impact was assessed using concentration-response functions derived from available epidemiological studies. Results Our results showed that ambient air pollution in relation to traffic scenarios had a significant impact on the future health status of Shanghai residents. Compared with the base case scenario, implementation of various traffic scenarios could prevent 759-1574, 1885-2420, and 2277-2650 PM10-related avoidable deaths （mean-value） in 2010, 2015, and 2020, respectively. It could also decrease the incidence of several relevant diseases. Conclusion Our findings emphasize the need to consider air pollution-related health effects as an important impact of traffic policy in Shanghai.

Association between Ambient Air Pollution and Outpatient Visits for Acute Bronchitis in a Chinese City

Objective To investigate the short-term association between outdoor air pollution and outpatient visits for acute bronchitis,which is a rare subject of research in the mainland of China.Methods A time-series analysis was conducted to examine the association of outdoor air pollutants with hospital outpatient visits in Shanghai by using two-year daily data（2010-2011）.Results Outdoor air pollution was found to be associated with an increased risk of outpatient visits for acute bronchitis in Shanghai.The effect estimates of air pollutants varied with the lag structures of the concentrations of the pollutants.For lag06,a 10 μg/m3 increase in the concentrations of PM10,SO2,and NO2 corresponded to 0.94%（95% CI：0.83%,1.05%）,11.12%（95% CI：10.76%,11.48%）,and 4.84%（95% CI：4.49%,5.18%） increases in hospital visits for acute bronchitis,respectively.These associations appeared to be stronger in females（P〈0.05）.Between-age differences were significant for SO2（P〈0.05）,and between-season differences were also significant for SO2（P〈0.05）.Conclusion Our analyses have provided the first evidence that the current air pollution level in China has an effect on acute bronchitis and that the rationale for further limiting air pollution levels in Shanghai should be strengthened.

Effect of the new maternity insurance scheme on medical expenditures for caesarean delivery in Wuxi, China： a retrospective pre/post-reform case study

Aiming to control rising medical expenditures and help improve China＇s healthcare systems, this study examined whether a cap-based medical insurance scheme with shared financial interest between the insurance and healthcare providers is effective in containing hospitals＇ C-section medical expenditures. We used 6547 caesarean delivery case records from a teaching tertiary-level general public hospital located in Wuxi, China （2004-2013）, and used the Chow test to investigate the possibility of significant variation in mean medical expenditures for caesarean deliveries pre- and post-reform. We also used paired sample t-tests and linear regression models to compare the mean medical expenditures between insured and uninsured women undergoing caesarean delivery during the post-reform period. After the scheme＇s implementation, medical expenditures for caesarean deliveries declined and the medical expenditures of women covered by the scheme were significantly lower than those of uninsured patients. These findings indicated the scheme＇s effectiveness in minimizing caesarean delivery expenditures. The cap-based medical insurance scheme with shared financial interest between insurance and healthcare providers would likely steer healtbcare providers＇ behaviors in a more cost-effective direction.

Diurnal Temperature Range as a Novel Risk Factor for Sudden Infant Death

Objective To assess the relationship between diurnal temperature range （DTR） and sudden infant death （SID） between 2001 and 2004 in Shanghai,China.Methods We conducted a time‐stratified case‐crossover analysis to estimate the percent increase of SID associated with changes in DTR after adjustment for daily weather conditions （temperature and relative humidity） and outdoor air pollution.Results DTR was significantly associated with daily SID.An increase of 1 °C in the current‐day （L0） and in the 2‐day moving average （L01） DTR corresponds to a 1.56% （95% CI：0.97%,2.15%） and a 1.89% （95% CI：1.17%,2.60%） increase in SID,respectively.Conclusion An increased DTR was associated with an increased risk of SID in Shanghai.More studies are needed to understand the effect of DTR on infant deaths.
