Use of tightrope fixation in ankle syndesmotic injuries

Objective： Conventional fixation of syndesmotic injuries with screws remains problematic. A novel fibrewire device （Tightrope） has suggested advantages. However, small case series have reported high soft tissue complication rates. The purpose of our study was to quantify complication rates and further procedures in patients treated with Tightropes. A secondary objective was to determine incidence of complications and further procedures in those treated with syndesmotic screws over the same period. Methods： All patients undergoing syndesmotic fixa- tion for ankle fracture between May 2008 and October 2009 were retrospectively reviewed. Incidence of complications, secondary procedures, maintenance of syndesmotic reduc- tion and time spent on non-weight bearing were recorded. Family doctors were contacted for those treated with Tight- ropes to check for any complications managed elsewhere. Results： Thirty-five patients required syndesmoticfixation, in which 12 were treated with Tightropes. They were followed up in clinic for a mean of 12.4 weeks. Family doctors were contacted at mean 14.6 months after treatment to determine any complications suffered. There were no complications attributable to method of fixation. In this series, 12 patients underwent 13 procedures and no patient had recurrent diastasis at discharge; 23 patients treated with screw fixation underwent 45 procedures （19 were screw removals）. There was 1 case ofsyndesmotic diastasis. Screw removal resulted in 2 minor complications. Conclusion： Tightrope fixation provideds effective syndesmotic fixation that is maintained at discharge. We do not experience soft tissue complications reported elsewhere.

Deep brain stimulation (DBS) has an increasing role in the treatment of idiopathic Parkinson’s disease. Although, the subthalamic nucleus (STN) is the commonly chosen target, a number of groups have reported that the most effective contact lies dorsal/dorsomedial to the STN (region of the pallidofugal fibres and the rostral zona incerta) or at the junction between the dorsal border of the STN and the latter. We analysed our outcome data from Parkinson’s disease patients treated with DBS between April 2002 and June 2004. During this period we moved our target from the STN to the region dorsomedial/medial to it and subsequently targeted the caudal part of the zona incerta nucleus (cZI). We present a comparison of the motor outcomes between these three groups of patients with optimal contacts within the STN (group 1), dorsomedial/medial to the STN (group 2) and in the cZI nucleus (group 3). Thirty five patients with Parkinson’s disease underwent MRI directed implantation of 64 DBS leads into the STN (17), dorsomedial/medial to STN (20) and cZI (27). The primary outcome measure was the contralateral Unified Parkinson’s Disease Rating Scale (UPDRS) motor score (off medication/off stimulation versus off medication/on stimulation) measured at follow-up (median time 6 months). The secondary outcome measures were the UPDRS III subscores of tremor, bradykinesia and rigidity. Dyskinesia score, L-dopa medication reduction and stimulation parameters were also recorded. The mean adjusted contralateral UPDRS III score with cZI stimulation was 3.1 (76%reduction) compared to 4.9 (61%reduction) in group 2 and 5.7 (55%reduction) in the STN (P-value for trend < 0.001). There was a 93%improvement in tremor with cZI stimulation versus 86%in group 2 versus 61%in group 1 (P-value = 0.01). Background: Few papers have addressed the psychological impact of strabismus in adults, with none comparing preoperative and postoperative data using standardised questionnaires relating surgical results and psychosocial outcomes. Methods: 46 participants were seen at their 6 week preoperative and 3 month postoperative appointments. Standardised measures of anxiety and depression (hospital Anxiety and Depression Scale), social anxiety (Derriford Appearance Scale), and quality of life (WHOQoLBref) were completed. Results: Preoperatively, levels of depression were comparable to relevant population norms; however, levels of general anxiety were slightly raised and levels of social anxiety and social avoidance were significantly poorer than population norms. Surgery resulted in significant improvements in psychosocial adjustment with improvements on all study variables for the participant group as a whole. The nondiplopic group made more significant gains than the diplopic group. Approximately one third of study measures were significantly correlated with the objective measure of eye misalignment preoperatively dropping to only one variable postoperatively. Calculations involving the subjective measure of eye misalignment and study variables showed the opposite pattern with five variables achieving significance postoperatively. There were no clear sex or age effects apparent in the data. Conclusion: Strabismus surgery offers significant improvements to psychological and physical functioning.

Idiopathic or primary angiitis of the CNS (PACNS) and cerebral amyloid angiopa thy (CAA) are unusual vasculopathies generally regarded as unrelated disorders. A few case reports have,however, described granulomatous angiitis in patients wi th sporadic,amyloid βpeptide (Aβ)-related CAA. Here we describe the clinical, neuroradiological and neuropathological features of nine patients with Aβ-rel ated angiitis (ABRA). Combining these with the individual case reports drawn fro m the literature has allowed us to define ABRA as a clinical entity and to compa re its features with those of PACNS. The mean age of presentation of ABRA (67 ye ars) is higher than that of PACNS but lower than that of sporadic non-inflammat ory Aβ-related *** in mental status (59%), headaches (35%), seiz ures and focal neurological deficits (24%) are common. Hallucinations are a pre senting manifestation in 12 %of cases. Most patients have white matter hyperint ensities on MRI but these are of similar appearance to those in PACNS. Cerebrosp inal fluid usually shows modest elevation of protein and *** ogy reveals angiodestructive inflammation,often granulomatous, and meningeal lym phocytosis. Aβis consistently present in abundance in affected blood vessels bu t usually scanty within the parenchyma of the cerebral ***, the corte x includes numerous activated microglia,occasionally in a plaque-like distribut ion and containing cytoplasmic Aβ. The cerebral white matter shows patchy glios is and rarefaction, in some cases marked. Our findings (i) help to dissect one s eparate clinicopathological entity from what is likely to be a spectrum of prima ry angiitides of the CNS;(ii) have important therapeutic implications for one ca tegory of patients with amyloid-related vasculopathy; and (iii) may provide val uable insights into the development of amyloid-associated inflammation, of rele vance not only to ABRA but also to Aβ-immunization-related encephalitis and t

The standard therapy for episodes of severe acute inflammatory demyelinating disease of the central nervous system is high dose intravenous corticosteroids. A small proportion of patients fail to improve with this regime and their prognosis can become grave. A recent sham controlled double blind crossover trial in this group of patients demonstrated a significant benefit from plasma exchange. We report six patients with severe acute steroid- insensitive inflammatory demyelinating disease of the central nervous system treated with plasma exchange. We observed a clear improvement in five of these six patients. Whilst complications of plasma exchange occurred these did not outweigh the benefits. Our study supports the use of plasma exchange in severe acute steroid- insensitive inflammatory disease of the central nervous system.

Clinical study on tri-tongue acupuncture combined with lowfrequency electrical stimulation for treating post-stroke dysarthria

BACKGROUND Few studies have investigated low-frequency electrical stimulation combined with tri-tongue acupuncture for the treatment of post-stroke *** randomized clinical study assessed the correlation between the clinical efficacy of low-frequency electrical stimulation combined with tri-tongue acupuncture in patients with post-stroke *** To investigate the clinical effects of tri-tongue acupuncture combined with lowfrequency electrical stimulation for treating post-stroke *** Ninety patients with post-stroke dysarthria,who were admitted to our hospital from December 2019 to June 2021,were selected and equally divided into two groups(n=45/group)according to the random number table ***-tongue acupuncture was administered in the control *** treatment group received both tri-tongue acupuncture and low-frequency electrical *** clinical efficacy,Western Aphasia Battery(WAB)score,general quality of life inventory(GQOLI-74)score,frenchay Dysarthria Assessment score,and speech function grades were compared and analyzed between both *** The overall efficacy in the treatment group was better than that in the control group(Pfrenchay Dysarthria Assessment,or GQOLI-74 scores(P>0.05)did not differ between the *** therapy,the WAB,frenchay Dysarthria Assessment,and GQOLI-74 scores in both groups increased significantly(P0.05),whereas significant improvements were observed in both groups after treatment(P<0.05).The degree of improvement in the treatment group was greater than that in the control group(P<0.05).CONCLUSION Low-frequency electrical stimulation,in conjunction with tri-tongue acupuncture,exhibits a good clinical effect on post-stroke dysarthria.

We have shown previously that intraparenchymal infusion of glial cell line- derived neurotrophic factor (GDNF) continuously into the posterior putamen in five Parkinson’ s disease patients is safe and may represent a new treatment option. Here, we report a continuation of this phase I study. After 2 years of continual GDNF infusion, there were no serious clinical side effects and no significant detrimental effects on cognition. Patients showed a 57% and 63% improvement in their off- medication motor and activities of daily living subscores of the Unified Parkinson’ s Disease Rating Scale, respectively, and health- related quality- of- life measures (Parkinson’ s Disease Questionnaire- 39 and Short Form- 36) showed general improvement over time.

Background The effects of active video game play on healthy individuals remain uncertain.A person＇s functional health status constitutes a dynamic interaction between components identified in the International Classification of Functioning,Disability,and Health （ICF）.The aim of this study was to investigate the short-term effects of active video game play on community adults using the *** Sixty community adults with an average age of 59.3 years and without physical disabilities were *** 2 weeks,each adult participated in six sessions of active video game play lasting 20 minutes *** were assessed before and af^er the *** were collected using sources related to the ICF components,including the hospital Anxiety and Depression Scale,Multidimensional Fatigue Inventory,Biodex Stability System,chairrising time,frenchay Activity Index,Rivermead Mobility Index,Chronic Pain Grade Questionnaire,Work Ability Index,and World Health Organization Quality of Life-Brief *** Compared to baseline data,significantly reduced risk of a fall measured by Biodex Stability System and improvements in disability scores measured by the Chronic Pain Grade Questionnaire were *** was no significant change in the other variables *** Short-term,active video game play reduces fall risks and ameliorates disabilities in community adults.
