Background:We investigated the criteria that hospitalized patients in intensive care units(ICUs)deem important when designating relatives who are best qualified to interact with the caregiving ***:We conducted an exploratory,observational,prospective,multicenter study between March 1,2018,and October 31,2018,within two ICUs.A 12-item questionnaire was distributed to patients in the ICUs by the investigating *** were considered eligible if they had a good understanding of the French language and if they had not officially designated surrogates before ICU ***:Seventy-one patients whose average age was 63.9±17.3 years,of whom 21(29.5%)were females,completed the *** average Charlson comorbidity score was 2.5±2.4,and the average Simplified Acute Physiology Score(SAPS II)was 39.8±*** main etiology was respiratory infection(40.8%),followed by sepsis(23.9%).The most important criteria identified by patients when selecting reference persons were a good knowledge of the patient’s wishes and values,an emotional attachment to the patient,and being a family ***:Our findings reveal that ICU patients considered the following criteria to be critical when designating reference persons:knowledge of their wishes and the existence of emotional and family attachments.

Background:Populations of French Polynesia(FP),where france performed atmospheric tests between 1966 and 1974,experience a high incidence of differentiated thyroid cancer(DTC).However,up to now,no sufficiently large study of DTC genetic factors in this population has been performed to reach definitive *** research aimed to analyze the genetic factors of DTC risk among the native FP ***:We analyzed more than 300000 single nucleotide polymorphisms(SNPs)genotyped in 283 DTC cases and 418 matched controls born in FP,most being younger than 15 years old at the time of the first nuclear *** analyzed the genetic profile of our cohort to identify population *** then completed a genome-wide analysis study on the whole ***:We identified a specific genetic structure in the FP population reflecting admixture from Asian and European *** identified three regions associated with increased DTC risk at 6q24.3,10p12.2,and *** lead SNPs at these loci showed respective p-values of 1.66×10^(−7),2.39×10^(−7),and 7.19×10^(−7) and corresponding odds ratios of 2.02,1.89,and ***:Our study results suggest a role of the loci 6q24.3,10p12.2 and 17q21.32 in DTC ***,a whole genome sequencing approach would be better suited to characterize these factors than genotyping with microarray chip designed for the Caucasian ***,the functional impact of these three new loci needs to be further explored and validated.

Methotrexate,etoposide,dexamethasone,and pegaspargase(MESA)with sandwiched radiotherapy is known to be effective for early-stage extranodal natural killer/T-cell lymphoma,nasal type(NKTCL).We explored the efficacy and safety of reduced-intensity,non-intravenous etoposide,dexamethasone,and pegaspargase(ESA)with sandwiched *** multicenter,randomized,phase III trial enrolled patients aged between 14 and 70 years with newly diagnosed early-stage nasal NKTCL from 27 centers in *** were randomly assigned(1:1)to receive ESA(pegaspargase 2,500 IU/m^(2)intramuscularly on day 1,etoposide 200 mg orally,and dexamethasone 40 mg orally on days 2–4)or MESA(methotrexate 1 g/m^(2)intravenously on day 1,etoposide 200 mg orally,and dexamethasone 40 mg orally on days 2–4,and pegaspargase 2,500 IU/m^(2)intramuscularly on day 5)regimen(four cycles),combined with sandwiched radiotherapy.

目的评价信迪利单抗对比安慰剂联合化疗(顺铂加紫杉醇或顺铂加5-氟尿嘧啶)作为不可切除的局部晚期、复发或转移性食管鳞癌一线治疗的效果。设计多中心、随机、双盲、Ⅲ期临床研究。背景2018年12月14日至2021年4月9日,国内66家研究中心,国外13家研究中心。受试者659例未曾接受过系统性治疗的成年(≥18岁)晚期或转移性食管鳞癌患者。干预受试者按1∶1的比例随机分组,每3周接受1次信迪利单抗或安慰剂(体重60 kg者200 mg)与顺铂75 mg/m2和紫杉醇175 mg/m2的联合治疗。本研究经修订,允许研究者选择顺铂联合紫杉醇或顺铂联合5-氟尿嘧啶(800 mg/m2,第1~5天连续输注给药)化疗方案。主要结局指标所有患者和程序性细胞死亡配体1(PD-L1)表达联合阳性评分≥10的患者总生存期。结果659例患者随机分配至信迪利单抗(n=327)或安慰剂(n=332)联合化疗组。659例患者中有616例(93%)接受了信迪利单抗或安慰剂与顺铂和紫杉醇联合治疗,有43例(7%)接受了信迪利单抗或安慰剂与顺铂和5-氟尿嘧啶联合治疗。期中分析显示,在所有患者(中位总生存期16.7 vs 12.5个月,风险比0.63,95%可信区间0.51~0.78;P<0.001)和联合阳性评分≥10的患者(17.2 vs 13.6个月,0.64,0.48~0.85;P=0.002)中,信迪利单抗联合化疗组的总生存期均优于安慰剂联合化疗组。与安慰剂联合化疗相比,信迪利单抗联合化疗显著延长了所有患者(7.2 vs 5.7个月,0.56,0.46~0.68;P<0.001)和联合阳性评分≥10患者(8.3 vs 6.4个月,0.58,0.45~0.75;P<0.001)的无进展生存期。信迪利单抗联合化疗组出现治疗相关不良事件的患者人数为321/327例(98%),安慰剂联合化疗组为326/332例(98%)。在信迪利单抗联合化疗组和安慰剂联合化疗组中,3级及以上治疗相关不良事件的发生率分别为60%(196/327)和55%(181/332)。结论与安慰剂相比,信迪利单抗联合顺铂加紫杉醇作为一线治疗显著改善了晚期或转移性食管鳞癌患者的总生存期和无进展生存期。信迪利单抗联合顺铂加5-氟尿嘧啶也显示出类似的获益。临床试验注册*** NCT03748134。

Background:Genetic testing for breast cancer susceptibility is widely used,but for many genes,evidence of an association with breast cancer is weak,underlying risk estimates are imprecise,and reliable subtype-specific risk estimates are ***:We used a panel of 34 putative susceptibility genes to perform sequencing on samples from 60,466 women with breast cancer and 53,461 *** separate analyses for protein-truncating variants and rare missense variants in these genes,we estimated odds ratios for breast cancer overall and tumor *** evaluated missense-variant associations according to domain and classification of pathogenicity.
