Urethral stricture disease and stenotic complications of their repair remain challenging problems for urological surgeons.In recent years,there has been a drive to offer definitive reconstructive urethroplasty to men with recurrent urethral stricture disease to improve quality of life and avoid repeated dilatation and catheterization,the latter of which is also associated with recurrent urinary tract infections.

Malignant melanoma cell-intrinsic PD-1:PD-L1 interaction thrusts tumorigenesis,angiogenesis,and radioresistance via mTOR hyperactivation to aggravate circumjacent aggression.Interdicting melanoma intrinsic growth signals,including the blockade of PD-L1 and mTOR signaling concurrently,cooperative with radiotherapy may provide a vigorous repertoire to alleviate the tumor encumbrance.Thence,we design a three-pronged platinum@polymer-catechol nanobraker to deliver mTOR inhibitor TAK228 and anti-PD-L1 antibody(aPD-L1)for impeding the melanoma-PD-1-driven aggression and maximizing the melanoma eradication.The aPD-L1 collaborated with TAK228 restrains melanoma cell-intrinsic PD-1:PD-L1 tumorigenic interaction via blocking melanoma-PD-L1 ligand and the melanoma-PD-1 receptor-driven mTOR signaling;corresponding downregulation of mTOR downstream protumorigenic cellular MYC and proangiogenic hypoxia-inducible factor 1-alpha is conducive to preventing tumorigenesis and angiogenesis,respectively.Further,high-Z metal platinum sensitizing TAK228-enhanced radiotherapy confers the nanobraker on remarkable tumoricidal efficacy.Hereto,the customized three-pronged nanobrakers efficiently suppress melanoma tumorigenesis and angiogenesis concomitant with the amplification of radiotherapeutic efficacy.Such an ingenious tactic may provide substantial benefits to clinical melanoma patients.

Large scale exome sequencing studies have ＇revealed regions of the genome, which contribute to the castrate resistant prostate cancer （CRPC） phenotype.1-3 Such studies have identified mutations in genes, which may have diagnostic/prognostic potential, or which may be targeted therapeutically. Two of these genes include the androgen receptor （AR） and speckle-type POZ protein （SPOP） genes. However, the findings from these exome sequencing studies can only be translated therapeutically once the functional consequences of these mutations have been determined. Here, we highlight the recent study by An et al.4 which investigated the functional effects of mutations in the SPOP gene that were identified in the aforementioned exome sequencing studies, particnlarly in the context of SPOP-mediated degradation of the AR.

Despite the concern of within-tumor genetic diversity,this diversity is in fact limited by the kinship among cells in the tumor.Indeed,genomic studies have amply supported the ’Nowell dogma’ whereby cells of the same tumor descend from a single progenitor cell.In parallel,genomic data also suggest that the diversity could be> 10-fold larger if tumor cells are of multiple origins.We develop an evolutionary hypothesis that a single tumor may often harbor multiple cell clones of independent origins,but only one would be large enough to be detected.To test the hypothesis,we search for independent tumors within a larger one(or tumors-in-tumor).Very high density sampling was done on two cases of colon tumors.Case 1 indeed has 13 independent clones of disparate sizes,many having heavy mutation burdens and potentially highly tumorigenic.In Case 2,despite a very intensive search,only two small independent clones could be found.The two cases show very similar movements and metastasis of the dominant clone.Cells initially move actively in the expanding tumor but become nearly immobile in late stages.In conclusion,tumors-in-tumor are plausible but could be very demanding to find.Despite their small sizes,they can enhance the within-tumor diversity by orders of magnitude.Such increases may contribute to the missing genetic diversity associated with the resistance to cancer therapy.

Background: Complete resection of locally advanced sigmoid colon cancer(LASCC) is sometimes difficult. Patients with LASCC have a dismal prognosis and poor quality of life, which has encouraged the evaluation of alternative multimodality treatments. This prospective study aimed to assess the feasibility and efficacy of neoadjuvant chemora?diotherapy(neo CRT) followed by surgery as treatment of selected patients with unresectable LASCC.Methods: We studied the patients with unresectable LASCC who received neo CRT followed by surgery between October 2010 and December 2012. The neoadjuvant regimen consisted of external?beam radiotherapy to 50 Gy and capecitabine?based chemotherapy every 3 weeks. Surgery was scheduled 6–8 weeks after radiotherapy.Results: Twenty?one patients were included in this study. The median follow?up was 42 months(range, 17–57 months). All patients completed neo CRT and surgery. Resection with microscopically negative margins(R0 resection) was achieved in 20 patients(95.2%). Pathologic complete response was observed in 8 patients(38.1%). Multivisceral resection was necessary in only 7 patients(33.3%). Two patients(9.5%) experienced grade 2 postopera?tive complications. No patients died within 30 days after surgery. For 18 patients with pathologic M0(yp M0) disease, the cumulative probability of 3?year local recurrence?free survival, disease?free survival and overall survival was 100.0%, 88.9% and 100.0%, respectively. For all 21 patients, the cumulative probability of 3?year overall survival was 95.2% and bladder function was well preserved.Conclusion: For patients with unresectable LASCC, preoperative chemoradiotherapy and surgery can be performed safely and may result in an increased survival rate.

Colorectal cancer(CRC)is among the leading causes of cancer mortality.The lifetime risk of developing CRC is about 5%in adult males and females.CRC is usually diagnosed at an advanced stage,and at this point therapy has a limited impact on cure rates and long-term survival.Novel and/or improved CRC therapeutic options are needed.The involvement of microRNAs(miRNAs)in cancer development has been reported,and their regulation in many oncogenic pathways suggests their potent tumor suppressor action.Although miRNAs provide a promising therapeutic approach for cancer,challenges such as biodegradation,specificity,stability and toxicity,impede their progression into clinical trials.Nanotechnology strategies offer diverse advantages for the use of miRNAs for CRC-targeted delivery and therapy.The merits of using nanocarriers for targeted delivery of miRNA-formulations are presented herein to highlight the role they can play in miRNA-based CRC therapy by targeting different stages of the disease.

BACKGROUND Percutaneous radiofrequency ablation(RFA)is an effective treatment for unresectable hepatocellular carcinoma(HCC)and a minimally invasive alternative to hepatectomy for treating tumour recurrence.RFA is often performed using contrast-enhanced computed tomography(CECT)and/or ultrasonography.In recent years,angiographic systems with flat panel image detectors and advanced image reconstruction algorithms have broadened the clinical applications of cone-beam computed tomography(CBCT),including RFA.Several studies have shown the effectiveness of using CBCT for immediate treatment assessments and follow-ups.AIM To assess the treatment response to RFA for HCC using CBCT.METHODS Forty-eight patients(44 men;aged 37-89 years)with solitary HCC[median size:3.2(1.2-6.6)cm]underwent RFA and were followed for 25.6(median;13.5-35.2)mo.Image fusion of CBCT and pre-operative CECT or magnetic resonance imaging(MRI)was used for tumour segmentation and needle path and ablation zone planning.Real-time image guidance was provided by overlaying the threedimensional image of the tumour and needle path on the fluoroscopy image.Treatment response was categorized as complete response(CR),partial response(PR),stable disease(SD),or progressive disease(PD).Disease progression,death,time to progression(TTP),and overall survival(OS)were recorded.Kaplan-Meier and Cox regression analyses were performed.RESULTS Initial post-RFA CECT/MRI showed 38 cases of CR(79.2%),10 of PR(20.8%),0 of SD,and 0 of PD,which strongly correlated with the planning estimation(42 CR,87.5%;6 PR,12.5%;0 SD;and 0 PD;accuracy:91.7%,P<0.01).Ten(20.8%)patients died,and disease progression occurred in 31(35.4%,median TTP:12.8 mo)patients,resulting in 12-,24-,and 35-mo OS rates of 100%,81.2%,and 72.2%,respectively,and progression-free survival(PFS)rates of 54.2%,37.1%,and 37.1%,respectively.The median dose-area product of the procedures was 79.05 Gy*cm2(range 40.95-146.24 Gy*cm2),and the median effective dose was 10.27 mSv(range 5.32-19.01 mSv).Tumour size<2 cm(P=0.00

RNA binding motif proteins(RBMs)have been widely implicated in the tumorigenesis of multiple human cancers but scarcely studied in nasopharyngeal carcinoma(NPC).Here,we compare the m RNA levels of 29 RBMs between 87 NPC and 10 control samples.We find that RBM47 is frequently upregulated in NPC specimens,and its high expression is associated with the poor prognosis of patients with NPC.Biological experiments show that RBM47 plays an oncogenic role in NPC cells.Mechanically,RBM47 binds to the promoter and regulates the transcription of BCAT1,and its overexpression partially rescues the inhibitory effects of RBM47-knockdown on NPC cells.Moreover,transcriptome analysis reveals that RBM47 regulates alternative splicing of pre-m RNA,including those cancer-related,to a large extent in NPC cells.Furthermore,RBM47 binds to hnRNPM and cooperatively regulates multiple splicing events in NPC cells.In addition,we find that knockdown of hnRNPM inhibits proliferation and migration of NPC cells.Our study,taken together,shows that RBM47 promotes the progression of NPC through multiple pathways,acting as a transcriptional factor and a modulator of alternative splicing in cooperation with hnRNPM.Our study also highlights that RBM47 and hnRNPM could be prognostic factors and potential therapeutic targets for NPC.

Chemodynamic therapy(CDT)offers a promising alternative to conventional cancer treatment.However,the limited acidity and H_(2)O_(2) concentration in tumor microenvironment(TME)severely impair the anticancer effects of CDT.In this study,we report a microemulsion-assisted coassembly method to prepare iron(III)tetraphenylporphyrin(FeTPP)and magnetic(Fe_(3)O_(4))nanocomposite material(FeTPP@Fe_(3)O_(4)),using photoactive FeTPP and Fe_(3)O_(4) nanocrystals as building blocks.The selfassembling nature of FeTPP results in disordered aggregation and fluorescence quenching,leading to a high light-to-heat conversion efficiency.Continuously,the photo-thermal effect enhances the catalytic decomposition of hydrogen peroxide(H_(2)O_(2))in the Fenton reaction on Fe_(3)O_(4) nanocrystals to generate highly toxic hydroxyl radicals(·OH)to destroy cancer cells.This cascade reaction produces a synergistic therapeutic effect between CDT and photothermal therapy(PTT),which significantly amplifies the therapeutic effect and enhances the treatment outcome of cancer patients.The highly efficient tumor catalytic therapy in vivo results confirmed that this nanomedicine treatment is an excellent biocompatible catalytic nanomedicine therapy achieved through a photo-enhanced Fenton reaction activity approach.
