Colorectal cancer(CRC) is a common malignancy worldwide. In CRC patients, metastases are the main cause of cancer-related mortality. In a group of metastatic CRC patients, the metastases are limited to a single site(solitary organ); the liver and lungs are the most commonly involved sites. When metastatic disease is limited to the liver and/or lungs, the resectability of the metastatic lesions will dictate the management approach and the outcome. Less commonly, the site of solitary organ CRC metastasis is the peritoneum. In these patients, cytoreduction followed by hyperthermic intraperitoneal chemotherapy may improve the outcome. Rarely, CRC involves other organs, such as the brain, bone, adrenals and spleen, as the only site of metastatic disease. There are limited data to guide clinical practice in these cases. Here, we have reviewed the disease characteristics, management approaches and prognosis based on the metastatic disease site in patients with CRC with metastases to a single organ.

Prognostic model of survival outcomes in non-small cell lung cancer patients initiated on afatinib: pooled analysis of clinical trial data

Objective: Several predictors of survival have been identified in EGFR-positive non-small cell lung cancer(NSCLC) patients treated with first generation EGFR inhibitors. Prognostic models of survival outcomes with afatinib have not been evaluated.Methods: A prognostic tool for overall survival(OS)/progression free survival(PFS) based on pre-treatment clinicopathological factors was developed for EGFR-positive advanced NSCLC patients treated with first-line afatinib using penalised regression of individual-participant data from LUX-Lung 3 and 6(n = 468). Favourable, intermediate and poor risk groups were identified and externally validated using LUX-Lung 1(n = 390) and LUX-Lung 2(n = 129) trials that initiated afatinib following previous chemotherapy or EGFR inhibitor treatment.Results: Discriminative performance was good in the development and validation cohorts. For patients treated with first-line afatinib, the median OS for the favourable, intermediate and poor risk groups were > 47.7, 29.3 and 16.4 months, respectively, and the median PFS were 17.3, 13.2 and 8.3 months, respectively. The improvement in median OS with afatinib use compared to chemotherapy was > 12.4 months for the favourable risk group, whereas no OS benefit was apparent for the poor risk group. The improvement in median PFS with afatinib use compared to chemotherapy was 10.2 months for the favourable risk group and 3.2 months for the poor risk group.Conclusions: A prognostic tool was developed and validated to identify favourable, intermediate and poor risk groups for OS/PFS in EGFR-positive advanced NSCLC patients treated with afatinib. The prognostic groups can inform the likely absolute OS/PFS benefit expected from afatinib compared to chemotherapy in first-line treatment.

The Coorong estuary lies at the terminus of Australia's largest river system, the Murray-Darling; both are strongly influenced by human activities; including farming and extensive flow modification. Metagenomic approaches were used to determine the planktonic bacterial community composition and potential metabolic function at two extremes in the Coorong, the river mouth which exhibits marine-like salinity, and the hypersaline upper-reaches of the estuary. Significant shifts in taxa and metabolic function were seen between the two sites. The river mouth exhibited an increase in abundance of R hodobacteriaceae and Alteromonadaceae; families readily able to adapt to change in nutrient conditions; and the potentially pathogenic families B rucellaceae, Enterobacteriaceae and Vibrionaceae. Metabolisms over-represented include motility and chemotaxis, RNA metabolism and membrane transport, all of which are involved in actively searching for and obtaining nutrients. Also over-represented were metabolisms involved in population succession and stress response. An over-representation of taxa and metabolisms indicative of environmental change is reflective of anthropogenically af fected riverine input. In the hypersaline upper reaches of the estuary, the halophilic family Ectothiorhodospiraceae was over-represented, as were the families Flavobacteriaceae, Cytophagaceae and Nocardioidaceae, members of which are able to survive over a wide salinity range. Metabolisms over-represented here were reflective of increased bacterial growth, characteristic of hypersaline environments, and included DNA metabolism, nucleotide and nucleoside synthesis and cell cycle. Coorong metagenomes clustered taxonomically and metabolically with other planktonic metagenomes, but remained an outlier of this group with only 71% and 84% similarity, respectively. This indicates that the Coorong exhibits a unique planktonic bacterial community that is influenced by riverine input at the river mouth and salinity in the upper-reaches.

Apoptosis is an important process for maintaining tissue homeostasis and eliminating abnormal cells in multicellular organisms.Abnormality in apoptosis often leads to severe diseases such as cancers. Better understanding of its mechanisms and processes is therefore important. Accompanying molecular biology events of apoptosis is a series of cellular morphology changes: nucleus condensation, cell shrinkage and rounding, cell surface blebbing, dynamic blebbing, apoptotic membrane protrusions and nucleus fragmentations and finally, the formation and release of apoptotic bodies. It is difficult to detect cellular changes in the early phase of apoptosis due to the subtle changes at this phase. In the current study, we induced apoptosis in He La cells with H2 O2 and used nuclear dye Hoechst 33258, mitochondria, lysosome and cytoplasmic protein specific aggregation-induced emission fluorogens(AIEgens), TPE-Ph-In, 2 M-DABS and BSPOTPE to successfully perform live cell multiplexed imaging to investigate early apoptosis cellular events. We showed the gradual dissipation of mitochondria membrane potential until it is nondetectable by TPE-Ph-In. Increased mitophagy detected by TPE-Ph-In and 2 M-DABS, condensed nucleus detected by Hoechst33258, increased permeability and/or reduced integrity of nuclear membrane, and increased intracellular vesicles detected by2 M-DABS are some of the early events of apoptosis.

Objective: To assess the effect of providing free fruit and peer support in the workplace, on employees’ consumption of fruits and high fat snacks at work and home. Methods: Three worksites, including 75 employees, were randomly assigned to a free fruit condition (Group A), a free fruit and peer education and modelling condition (Group B), and a control group (Group C). Groups A and B had free fruit delivered to their workplace each morning for four weeks. Consumption of fruit and high fat snacks was measured pre- and post-intervention, and after a two week maintenance period. Results: Despite a small sample, the intervention increased employees’ fruit intake at work, decreased high fat snacks and was more successful in those who were not currently meeting the recommendations of two pieces of fruit per day. Peer support led to increased fruit consumption at work and sustained decreases in unhealthy snacks post-intervention. Conclusions: The provision of fruit in the workplace with peer support is a simple and effective method for improving fruit consumption at work in the short-term, particularly in those not meeting current recommendations. In addition, those participating in the intervention reduced their consumption of high fat snacks. Further research is necessary to determine whether a longer larger scale intervention can sustain dietary changes and thereby reduce risk for chronic disease in the Australian population.

目的：采用EQ-5D-5L量表对乳腺癌患者进行健康效用值的测量,为今后开展乳腺癌临床治疗的成本-效用分析提供方法借鉴和基础数据支持。方法：采用EQ-5D-5L量表对621名乳腺癌患者进行面对面问卷调查。结果：用EQ-5D-5L量表测量的乳腺癌患者的平均健康效用值为0.83±0.16（95%可信区间为0.82-0.85,中位数为0.87）;被调查患者的教育程度、家庭收入以及治疗方案会对健康效用值产生一定影响。结论：采用EQ-5D-5L量表对乳腺癌患者进行健康效用值的测量具有重要研究价值,应加快研制适用中国人群的EQ-5D-5L量表的效用值积分体系。

Androgens and esophageal cancer: What do we know?

Significant disparities exist between genders for the development and progression of several gastrointestinal(GI) diseases including cancer. Differences in incidence between men vs women for colon, gastric and hepatocellular cancers suggest a role for steroid sex hormones in regulation of GI carcinogenesis. Involvement of intrinsic gender-linked mechanisms is also possible for esophageal adenocarcinoma as its incidence is disproportionally high among men. However, the cause of the observed gender differences and the potential role of androgens in esophageal carcinogenesis remains unclear, even though the cancer-promoting role of androgen receptors(AR) shown in other cancers such as prostate and bladder suggests this aspect warrants exploration. Several studies have demonstrated expression of ARs in esophageal cancer. However, only one study has suggested a potential link between AR signaling and outcome- poorer prognosis. Two groups have analyzed data from cohorts with prostate cancer and one of these found a decreased incidence of esophageal squamous and adenocarcinoma after androgen deprivation therapy. However, very limited information is available about the effects of androgen and AR-initiated signaling on esophageal cancer cell growth in vitro and in vivo. Possible mechanisms for androgens/AR involvement in the regulation of esophageal cancer growth are considered, and the potential use of AR as a prognostic factor and clinical target is highlighted, although insufficient evidence is available to support clinical trials of novel therapies. As esophageal adenocarcinoma is a gender linked cancer with a large male predominance further studies are warranted to clarify the role of androgens and ARs in shaping intracellular signaling and genomic responses in esophageal cancer.
