Objective:To evaluate the detection of methicillin resistant Staphylococcus aureus(MRSA) and analyze the performance of Mastalex MRSA(Mast,UK).Methods:Two hundred and ten Staphylococcus aureus(***) strains were isolated from different clinical samples and were tested for methicillin resistance by Oxacillin(1μg) and Cefoxitin(30μg) disc diffusion, oxacillin agar screen,and minimum inhibitory concentration of oxacillin and *** isolates were grown on the blood agar and mannitol salt agar with(2 mg/L) and without oxacillin for the analysis of Mastalex ***:Out of 210 *** strains tested,103 strains were detected as methicillin resistant by Cefoxitin disk diffusion,Cefoxitin minimal inhibitory concentration(MlC) and Mastalex MRSA *** oxacillin disc diffusion and oxacillin agar screen detected 91 and 97 MRSA *** Cefoxitin MIC test performance was equivalent to Cefoxitin disc diffusion.103(100%) strains grown on blood agar without and with oxacillin,and 76(74%) and 93(91%) strains grown on mannitol salt agar without and with oxacillin shown positive agglutination with Mastalex MRSA test ***:The cefoxitin disk diffusion/ Mastalex MRSA is very suitable for detection of MRSA and the tests can be an alternative to PCR for detection of MRSA in resource constraint *** test would be particularly useful when confirmation of resistance is urgently required.

AIM: To improve our understanding of Iranian regional variation in Helicobacter pylori (H. pylori ) antibiotic resistance rates to find the best antibiotic therapy for eradication of H. pylori ***: A total of 266 patients undergoing endoscopy in Shiraz, Southern Iran, were included in this study. H. pylori strains were isolated from antral biopsies by culture and confirmed by the rapid urease-test and gram staining. Antibiotic susceptibility of H. pylori isolates was determined by ***: A total of 121 H. pylori strains were isolated, 50 from male and 71 from female patients. Data showed that 44% (n = 53), 20% (n = 24), 5% (n = 6), and 3% (n = 4) of all strains were resistant to the antibiotics metronidazole, amoxicillin, clarithromycin, and tetracycline, respectively. When the antibiotics were considered together we found 11 sensitivity patterns for the strains. Resistance to metronidazole was significantly higher in female than in male patients (P 256 mg/L) which was found in 71% of the isolates is considerable. In the case of amoxicillin, an increased resistance rate of 20% is worrying. Resistance to clarithromycin and tetracycline is also emerging among the H. pylori strains in our region.

Background: Extended-spectrum β-lactamases (ESBLs) are enzymes capable of hydrolyzing extended-spectrum cephalosporins, penicillins and monobactams but inactive against cephamycins and carbapenems. The ESBL-producing organisms are a breed of multidrug-resistant pathogens. Objectives: This study was aimed to determine the susceptibility pattern of ESBL-producing Escherichia coli to ciprofloxacin, amikacin and imipenem. Methods: A total of 75 ESBL-producing E. coli, were obtained from the tertiary care hospitals of Bangladesh and were studied for susceptibility pattern from October, 2010 to December, 2011. These isolates were identified by double disc synergy test (DDST) and were confirmed phenotypically as ESBL-producer by phenotypic confirmatory disc diffusion test (PCDDT). Minimum inhibitory concentrations (MICs) of ciprofloxacin, amikacin and imipenem among ESBL-producing E. coli were determined using agar dilution method. Results: Out of 75 DDST positive ESBL-producing E. coli, 71 (94.67%) were also positive by PCDDT. All ESBL-producing E. coli, were susceptible to imipenem. About 92.95% ESBL-producing E. coli were susceptible to amikacin but only 14.08% were susceptible to ciprofloxacin. Conclusion: In this study, ESBL-producing E. coli, showed high resistance to ciprofloxacin. Imipenem and amikacin were most effective against ESBL positive strains.

Organisms causing spontaneous bacterial peritonitis in children with liver disease and ascites in Southern Iran

AIM： To determine the causative agents of spontaneous bacterial peritonitis （SBP） in children with liver disease and ascites in our center. METHODS： During a 2.5 year period, from September 2003 to March 2006, 12 patients with 13 episodes of SBP were studied. In all cases at the time of admission serum albumin and glucose, urinalysis and urine culture was performed. Analysis [white blood cell （WBC） count with differential, albumin, glucose], gram stain, culture by BACTEC method and antibiogram was done on ascitic fluids. Abdominal paracentesis was repeated after 48 h of antibiotic therapy for bacteriologic assay. The patients were followed for at least three months in a gastroenterology clinic. RESULTS： There were 7 girls （58%） and 5 boys （42%） with a median age of 5.2 years （range, 6 mo to 16 years）. All cases had positive ascitic fluid culture. Gram stain was positive in 5 （38.5%） of them. The isolated organisms were S. pneumoniae in 5 （38.5%）, E. coli in 2 （15.3%）, S. viridans in 2 （15.3%）, and K. pneumoniae, H. influenza, Enterococci, and nontypable Streptococcus each in one （7.7%）. All of them except Enterococci were sensitive to ciprofloxacin and ceftriaxone. All ascitic fluid cultures were negative after 48 h of antibiotic therapy. CONCLUSION： S. pneumoniae is the most common cause of SBP in the pediatric age group and we recommend a third generation cephalosporine （e.g., Ceftriaxlone or Cefotaxime） for empirical therapy in children with SBP.

AIM: To study the antibiotic susceptibility patterns of gram-negative healthcare associated bacterial infections at two tertiary hospitals in the Sanandaj city, Kurdistan Province, ***: From January 2012 to December 2012, all positive cultures from potentially sterile body fluids were gathered. They sent to professor Alborzi clinical microbiology center in Shiraz for further analysis and susceptibility testing. The antibiotic susceptibility was determined using the Kirby-Bauer method(disk diffusiontechnique). The Results were interpreted according to Clinical and Laboratory Standards Institute guidelines against a series of antimicrobials. World Health Organization definitions for Healthcare associated infections were ***: Seven hundred and thirty-two positive cultures were reported from both hospitals. Seventynine isolates/patients fulfilled the study criteria for healthcare associated gram-negative infections. The most frequent bacterial cultures were from the pediatric wards(52%). Serratia marcescens(S. marcescens)(38%) Escherichia coli(E. coli)(19%), Klebsiella pneumoniae(K. pneumoniae)(19%), Acinetobacter baumannii(6%), Enterobacter species(6%), Serratia odorifera(4%) and Pseudomonas species(5%) were the most frequently isolated organisms. The susceptibility pattern of common isolates i.e., S. marcescens, E. coli and K. pneumoniae for commonly used antibiotics were as follows: Ampicillin 3.3%, 6.7%, 20%; gentamicin 73.3%, 73.3%, 46.7%; ceftazidim 80%, 73.3%, 33.3%; cefepim 80%, 86.7%, 46.7%; piperacillin/tazobactam 90%, 66.7%, 86.7%; ciprofloxacin 100%, 73.3%, 86.7%; imipenem 100%, 100%, 100%, respectively. CONCLUSION: The most effective antibiotics against gram-negative healthcare associated infections are imipenem followed by ciprofloxacin. The resistance rate is high against ampicillin and cephalothin. The high mortality rate(46.1%) associated with S. marcescens is alarming.

Induced systemic resistance(ISR)is a mechanism by which certain plant beneficial rhizobacteria and fungi produce immunity,which can stimulate crop growth and resilience against various phytopathogens,insects,and *** beneficial rhizobacteria and fungi improve plant performance by regulating hormone signaling,including salicylic acid(SA),jasmonic acid(JA),prosystemin,pathogenesis-related gene 1,and ethylene(ET)pathways,which activate the gene expression of ISR,the synthesis of secondary metabolites,various enzymes,and volatile compounds that ultimately induce defense mechanisms in *** protect themselves from disease,plants have various advanced defense mechanisms in which local acquired resistance,systemic gene silencing,systemic wound response,systemic acquired resistance(SAR),and ISR are *** rhizobacteria activate the SA-dependent SAR pathway by producing SA at the root’s *** contrast,other rhizobacteria can activate different signaling pathways independent of SA(SA-independent ISR pathways)such as those dependent on JA and ET *** main objective of this review is to provide insight into the types of induced resistance utilized for plant *** to this,the genetic approaches used to suppress disease-causing genes,i.e.,RNA interference and antisense RNA,which are still underutilized in sustainable agriculture,along with the current vision for virus-induced gene silencing are also discussed.
