Purpose： This study was formulated to educate physicians and medical residents on procalcitonin （PCT）. The need for this study is based on lack of information and studies in the United States in the pediatric population regarding PCT, and also due to the lack of awareness by the Baylor Scott ＆ White McLane Children＇s residents in the implications and benefits of PCT. There are several instances where it can be valuable in the diagnosis and treatment of bacterial infections, which in turn, can decrease patient＇s length of stay, hospital costs, and patient exposure to unnecessary antibiotics, without increasing morbidity or mortality. Methods： An educational, qualitative analysis of PCT awareness is assessed utilizing pre and post surveys. These surveys will be given to pediatric and emergency medicine residents on paper before and after an educational presentation about PCT and its importance as a detection method for infection. Surveys consist of six closed-ended questions on the pre-survey and eight open and close-ended questions on the post-survey about PCT, how it works, general use, and how it can be beneficial. Results： Pediatric and emergency medicine residents were asked to participate in this study at a random monthly department meeting, selected by primary investigator and department head. Residents were asked to complete the pre-survey, listened to a short presentation, and completed the post survey. Comparing the pre-survey to post-survey, there was an overall improvement for both groups for questions 1-5 which showed statistical significance （P 〈 0.05）. No statistical difference was found between groups comparing emergency medicine vs. pediatric departments, interns vs. residents and each subgroup pre-survey vs. post-survey for questions 6 and 7. The open-ended post-survey question identified factors for obtaining PCT levels with attending physician＇s preference as the most common response. Hyperglycemia fluctuation is associated with diabetes mellitus(DM) complications when compared to persistent *** studi es have shown that paeoniflorin(PF),through its anti-apoptosis,anti-inflammati on,and anti-thrombotic properties,effectively protects against cardiovascular an d cerebrovascular ***,the mechanism underlying the protection fr om PF against vascular injuries induced by hyperglycemia fluctuations remains poorly ***,we investigated the potential protective role of PF o n human umbilical vein endothelial cells(HUVECs) subjected to intermittent gl ucose levels in vitro and in DM rats with fluctuating hyperglycemia in vivo.A remarkable increased apoptosis associated with elevated inflammation,increase d oxidative stress,and high protein level of PKCβ1 were induced in HUVECs by intermittently changing glucose for 8 days,and PF recovered those detrime ntal ***333531,a potent PKCβ1 inhibitor and metformin manifested si milar ***,in DM rats with fluctuating hyperglycemia,PF prote cted against vascular damage as what have been observed in *** toget her,PF attenuates the vascular injury induced by fluctuant hyperglycemia throu gh oxidative stress inhibition,inflammatory reaction reduction,and PKCβ1 prot ein level repression,suggesting its perspective clinic usage.

In this work, seismic refraction was used to obtain elastic properties (shear modulus (μ), Young modulus (E), Bulk modulus (K) and lithological information in Uyo and its environ as an aid to engineering foundation. Using seismic refraction method, the top and weathered layer of the engineering foundation in the study area was investigated to determine the elastic parameters of top soil and also assess the strength of engineering foundation based on the parameter distribution. A 24-channel signal enhancement seismograph, geophones, sledge hammer and a metal plate (source) for generating seismic waves were used. The study area lies between latitudes 4˚45' and 5˚15'N and between longitudes 7˚45' and 8˚30'E in the Niger Delta region of southern Nigeria. Geologically, the area is located in the Tertiary to Quaternary Coastal Plain Sands (CPS) (otherwise called the Benin Formation) and Alluvium environments of the Niger Delta region of southern Nigeria. Shear Modulus had average values of 0.43 × 108 N/m2 and 1.40 × 108 N/m2 for layers 1 and 2 respectively. The average values of the Young Modulus for layers 1 and 2 were determined as 2.32 × 108 N/m2 and 3.84 × 108 N/m2 respectively. The average values of the bulk Modulus for layers 1 and 2 were estimated as 1.52 × 108 N/m2 and 4.93 × 108 N/m2 respectively.

AIM： Minimal deviation carcinoma of the uterine cervix, otherwise known as extremely well-differentiated adenocarcinoma （EWDA）, is characterized by its benign microscopic appearance in contrast to its aggressive behavior. In order to elucidate the clinicopathological features and biological behavior of the gastric counterpart of EWDA, we, using immunohistochemistry, analyzed nine lesions for the phenotypic expression, proliferative activity, and the expression of oncogene-associated products. METHODS： Clinicopathological features, including preoperative biopsy diagnosis, were reviewed. Using immunohitstochemistry, Ki-67 labeling index and expression of p53 and c-erbB-2 protein in the gastric lesions were ***： Locations in the middle or upper third of the stomach and polypoid macroscopic features are characteristic of EWDA of the stomach. Although 4 of the 9 lesions showed only focal lymphatic or venous invasion, lymph node metastasis was not present and none of the patients died of the lesions （mean follow-up period, 56 too）. All 9 cases of EWDA could be classified into gastric phenotype （5 lesions） and intestinal phenotype （4 lesions）. The former resembled gastric foveolar epithelium, mucous neck cells or pyloric glands, but their papillary structures were frequently elongated and the tumor cellsand their nuclei were slightly larger and more hyperchromatic compared to normal epithelium. The latter resembled intestinal metaplasia with minimal nulcear atypia and irregular glands; two of these lesions demonstrated complete intestinal phenotype, while two demonstrated incomplete intestinal phenotype. Ki-67 labeling index was low and none of the cases revealed over-expression of p53 and c-erbB-2 protein. CONCLUSION： Unlike minimal deviation carcinoma of the cervix, these findings suggest that EWDA of the stomach is a lesion of low-grade malignancy. Summary: Many anomalies can be observed in the basilar trunk, such as aneurysms. Basilar trunk aneurysms are rare with a low reported prevalence. In this study, we describe a case of ischaemic stroke revealed by a basilar trunk aneurysm associated with venous thromboembolic disease. The patient was 60 years old and had no specific pathological history. She was seen for a sudden onset of right hemiplegia associated with dysarthria that had been present for 12 hours. The physical examination revealed a pyramidal syndrome and an altered general condition. An emergency cerebral computed tomography (CT) scan showed evidence of a cerebrovascular accident associated with a saccular aneurysm. We adopted the diagnosis of a deep left sylvian stroke associated with a superficial right sylvian stroke in the setting of an incidentally discovered basilar trunk aneurysm. On the 9ème day of hospitalisation, the patient experienced sudden onset of respiratory distress in conjunction with a warm, painful swelling of the right leg. TTE revealed signs of pulmonary embolism, with thrombi in the inferior vena cava and right atrium. Pulmonary embolism was highly probable, with a modified Geneva score of 15. We adopted the diagnosis of DVA complicated by pulmonary embolism in the setting of a basilar trunk aneurysm. The patient was transferred to the cardiology department, where she received oxygen therapy and thrombolysis with streptokinase. The immediate outcome 6 hours later was the sudden death of the patient due to the onset of cardiogenic shock.

The rejected specimens from the emergency department of the Center of Clinical Laboratory from January 1,2022 to January 1,2023 were analyzed to reduce the specimen rejection rates and to improve the quality of *** results showed that there were 1488 samples of rejected specimens and the non-conforming rate was 0.58%.The departments involved were mainly the emergency department,the Hematology department,the Cardiology department,the Intensive Care department,and the Brain Surgery *** the reasons for rejection,blood hemolysis accounted for 43.15%,blood coagulation accounted for 26.61%,and the rate of insufficient specimens was 17.14%.Among them,the sample rejection rate for arterial blood gas analysis was the highest,which accounted for 1.74%;followed by specimens for coagulation test,which was 1.18%.These results indicate the main reason for producing rejected specimens is mainly due to not following the standard operating *** rejection can largely be avoided if the standards for specimen collection are strictly followed.
