Cholangiocarcinoma refers to malignant tumors that develop in epithelial lining of biliary system, and it is divided into two categories according to tumor location, intrahepatic cholangiocarcinoma （ICC） and extrahepatic cholangiocarcinoma （ECC）. ICC occurs from the epithelial cells of the intrahepatic bile duct, its branches and interlobular biliary tree; and ECC is divided into hilar cholangiocarcinoma and distal cholangiocarcinoma by the circumscription at the confluence of cystic duct and the common hepatic duct.

BACKGROUND： The various combination of multiphase enhancement multislice spiral CT （MSCT） makes the diagno- sis of a small hepatocellular carcinoma （sHCC） on the back- ground of liver cirrhosis possible. This study was to explore whether the combination of MSCT enhancement scan and alpha-fetoprotein （AFP） level ficiency for sHCC. could increase the diagnostic ef- METHODS： This study included 35 sHCC patients and 52 cir- rhotic patients without image evidence of HCC as a control group. The diagnoses were made by three radiologists em- ploying a 5-point rating scale, with postoperative pathologic results as the gold standard. Receiver operating characteristic （ROC） curve analysis was performed to evaluate the diag- nostic value of the three MSCT combination modes （arterial phase＋portal-venous phase, arterial phase＋delayed phase, arterial phase＋portal-venous phase＋delayed phase） and AFP levels for sHCC on the background of liver cirrhosis. RESULTS： The area under ROC curve （AUC）, sensitivity, and specificity of the combination of arterial phase＋portal- venous phase＋delayed phase were 0.93, 93%, and 82%, respectively. The average AUC of the arterial phase＋portal- venous phase＋delayed phase combination was significantly greater than that of the arterial phase＋portal-venous phase （AUC=0.84, P=0.01） and arterial phase＋delayed phase （AUC=0.85, P=0.03）. Arterial phase＋portal-venous phase had a smaller AUC （0.84） than arterial phase＋delayed phase （0.85）, but the difference was insignificant （P=0.15）. After combining MSCT enhancement scan with AFP, the AUC, sensitivity, and specificity were 0.95, 94%, and 83%, respectively, indicating a greatly increased diagnostic efficiency for sHCC. CONCLUSIONS： The combination of AFP and 3 phases MSCT enhancement scan could increase the diagnostic efficiency for sHCC on the background of liver cirrhosis. The application of ROC curve analysis has provided a new method and reference in HCC diagnosis.

Background A high mortality rate of pancreatic cancer becomes a bottleneck for further treatment with long-term efficacy. It is urgent to find a new mean to predict the early onset of pancreatic cancer accurately. The authors hypothesized that genetic variants of cationic trypsinogen （PRSS1） gene could affect trypsin expression/function and result in abnormal activation of protease activated receptor-2 （PAR-2）, then lead to pancreatic cancer. The aim of this study was to elaborate some novel mutations of PRSS1 gene in the patients with pancreatic cancer. Methods Totally 156 patients with pancreatic cancer and 220 unrelated individuals as controls were enrolled in this study. The mutations of PRSS1 gene were analyzed by direct sequencing. K-ras Mutation Detection Kit was used to find the general k-ras gene disorder in the pancreatic cancer tissue. Then the clinical data were collected and analyzed simultaneously. Results There were two patients who carried novel mutations which was IVS 3 ＋157 G〉C of PFISSI gene in peripheral blood specimens and pancreatic cancer tissue. What＇s more, it was surprising to find a novel complicated mutation of exon 3 in PRSS1 gene （c.409 A〉G and c.416 C〉T） in another young patient. The complicated mutation made No.135 and No.137 amino acid transfer from Thr to Ala and Thr to Met respectively. No any mutation was found in the normal controls while no mutations of k-ras gene were detected in the three patients. Conclusion Mutations of PRSS1 gene may be an important factor of pancreatic cancer.

Hereditary gingival fibromatosis(HGF)is a rare inherited condition with fibromatoid hyperplasia of the gingival tissue that exhibits great genetic *** distinct loci related to non-syndromic HGF have been identified;however,only two diseasecausing genes,SOS1 and REST,inducing HGF have been identified at two loci,GINGF1 and GINGF5,***,based on a family pedigree with 26 members,including nine patients with HGF,we identified double heterozygous pathogenic mutations in the ZNF513(c.C748T,p.R250W)and KIF3C(c.G1229A,p.R410H)genes within the GINGF3 locus related to *** studies demonstrated that the ZNF513 p.R250W and KIF3C p.R410H variants significantly increased the expression of ZNF513 and KIF3C in vitro and in ***513,a transcription factor,binds to KIF3C exon 1 and participates in the positive regulation of KIF3C expression in gingival ***,a knock-in mouse model confirmed that heterozygous or homozygous mutations within Zfp513(p.R250W)or Kif3c(p.R412H)alone do not led to clear phenotypes with gingival fibromatosis,whereas the double mutations led to gingival hyperplasia *** addition,we found that ZNF513 binds to the SOS1 promoter and plays an important positive role in regulating the expression of ***,the KIF3C p.R410H mutation could activate the PI3K and KCNQ1 potassium ***513 combined with KIF3C regulates gingival fibroblast proliferation,migration,and fibrosis response via the PI3K/AKT/mTOR and Ras/Raf/MEK/ERK *** summary,these results demonstrate ZNF513+KIF3C as an important genetic combination in HGF manifestation and suggest that ZNF513 mutation may be a major risk factor for HGF.

There is a small amount of clinical data regarding the safety and feasibility of autologous peripheral blood mononuclear cell transplantation into the subarachnoid space for the treatment of amyotrophic lateral *** objectives of this retrospective study were to assess the safety and efficacy of peripheral blood mononuclear cell transplantation in 14 amyotrophic lateral sclerosis patients to provide more objective data for future clinical *** stem cell mobilization and collection,autologous peripheral blood mononuclear cells（1 × 109） were isolated and directly transplanted into the subarachnoid space of amyotrophic lateral sclerosis *** primary outcome measure was incidence of adverse *** outcome measures were electromyography 1 week before operation and 4 weeks after operation,Functional Independence Measurement,Berg Balance Scale,and Dysarthria Assessment Scale 1 week preoperatively and 1,2,4 and 12 weeks *** was no immediate or delayed transplant-related *** number of leukocytes,serum alanine aminotransferase and creatinine levels,and body temperature were within the normal *** evaluation showed no serious transplant-related adverse *** strength grade,results of Functional Independence Measurement,Berg Balance Scale,and Dysarthria Assessment Scale were not significantly different before and after *** findings suggest that peripheral blood mononuclear cell transplantation into the subarachnoid space for the treatment of amyotrophic lateral sclerosis is safe,but its therapeutic effect is not ***,a large-sample investigation is needed to assess its efficacy further.

ObjectiveTo induce the model of Mongolian gerbil infected by Helicobacter pylori (HP) and to observe the changes of the infiltration and the proliferation of inflammatory cells before and after the bactericidal treatment.MethodsThe animal model of HP infection was induced by inoculating the HP strain (American Type Culture Collection) to male Mongolian gerbil MGS/Sea (SPF). Half a month, 1 month and 3 months after inoculation, kill the animals and make HE staining and immunohistochemistry staining of the stomach to observe the pathological changes.ResultsThe infiltration of a great number of inflammatory cells was observed with the center of the mucosa of pylorus at the second week of HP infection. At the 3rd month of infection, ulcer appeared near the lesser curvature side of the pylorus. On the early stage of infection, the lesion was acute inflammation characterized by the neutrophilic infiltration, and then transformed into the chronic inflammation characterized by the lymphatic infiltration and the wide formation of lymphatic follicles.ConclusionMongolian gerbil is an ideal and standard animal model of HP infection. BACKGROUND Colonic duplication refers to a spherical or tubular cavity which shows similar properties with the native colon and is attached to the mesenteric side of the alimentary *** is the rarest in alimentary tract *** upon anatomic feature,colonic duplications can be classified as spherical(cystic)or tubular,with the latter being less common(approximately 20%).Symptoms of colonic duplication are dependent on the duplication site and extent,and patient age,***,patients with colonic duplication manifest typical intestinal obstruction,potentially accompanied by recurrent dark or bright red bloody stool,varying degrees of anemia-related symptoms,and body *** SUMMARY A young male patient was admitted to our hospital due to recurrent abdominal *** definite diagnosis was achieved by computed tomography(CT)or electronic colonoscopy,and the bowel preparation efficacy was *** disease was suspected,and thus laparoscopic exploration was *** approximately 60-cm-long inverted duplicated colon with severe edema and dilation was *** originated from the mesenteric side of the transverse colon and ended in the terminal part of the descending colon with a blind *** parallel native colon had a thickened colonic wall,became stiff,and was poor in *** patient then underwent subtotal colectomy and was discharged 7 d after the *** 3 mo post-surgery to date,the patient had regular bowel movement once daily and a steady increase in body *** Tubular colonic duplication is a rare type of alimentary tract duplication that can be detected by ultrasonography,CT,or magnetic resonance imaging based on the actual clinical *** resection of aberrant colon(including the duplicated colonic segment and other potentially involved colonic segments)is the only approach to cure this medical condition.
