Aim: To investigate the effect of melatonin on ischemia–reperfusion (I/R) injury in rat ovary. Material and Method: A total of 24 Wistar rats were divided into four groups. The rats were first numbered randomly and then randomly divided into four equal groups: sham, torsion, detorsion and melatonin groups. In group I (n = 6) sham, as well as in group II ovary torsion was not performed and no drug was administered. In group III, 1 hour of ischemia and 2 hours of reperfusion were performed and no drug was given. In group IV, melatonin was dissolved in 1% ethanol just before use. Melatonin was injected 1 hour before torsion to the torsion plus melatonin group. Right ovaries were surgically removed in all groups. Result: Malondialdehyde (MDA) levels were measured. We suggested that administration of melatonin would be in treating ovaries from torsion/detorsion induced damage in humans. The histopathological changes such as vascular congestion, edema, hemorrhage, and follicular degeneration were found to be decreased in the melatonin + ischemia-reperfusion group. Ischemia-reperfusion group: PECAM-I expression is positive in vessel of the stromal area. The follicles and interfolicular area infiltrated with CD68 positive cells were increased in a time of ischemia-reperfusion exposure dependent manner. Conclusions: Melatonin after ischemia-reperfusion CD31 and CD68 expressions is weak, due to the reduction of the inflammatory effects and hemorrhage is thought that melatonin is effective.

Background:This study was undertaken to evaluate demographics,clinical manifestations,laboratoryfi ndings and outcomes of children with inflammatory bowel disease(IBD)in ***:We analyzed the medical records of 127 children diagnosed with IBD(under 18 years old)between January 2004 and January 2012 in 8 pediatric gastroenterology ***:Of the 127 patients,90(70.9%)suffered from ulcerative colitis(UC),29(22.8%)from Crohn's disease(CD),and 8(6.3%)from IBD *** mean age of the 127 patients was 11.6±4.1 years,and 11.8%of the patients were below 5 years *** the patients,49.6%were male,and males were more predominant in patients with CD than in those with UC(72.4%vs.42.2%,P=0.008;a male/female ratio of 2.62 in CD,P=0.0016).Approximately one fifth of the patients had extra-intestinal manifestations and 13.3%of the patients had associated *** manifestations and associated diseases were more common in early onset disease[P=0.017,odds ratio(OR)=4.02;P=0.03,OR=4.1].Of the patients,15%had normal laboratory parameters including anemia,high platelet count,hypoalbuminemia,hypoferritinemia,and high sedimentation *** under receiver operation characteristics was used to predict pancolitis in patients with *** values of C-reactive protein,sedimentation rate and pediatric ulcerative colitis activity were 0.61(P=0.06),0.66(P=0.01)and 0.76(P=0.0001),***(4.4%)patients with UC underwent colectomy,andfi nally two(1.5%,95%confidence interval:0-3.7%)patients died from primary disease or ***:IBD is an increasing clinical entity in *** of IBD are similar to those in other populations,but prospective multicenter studies are needed to analyze the true incidence of IBD in Turkish children.

Single-Incision Laparoscopic Cholecystectomy with Conventional Instruments: A Surgeon’s Initial Experience

Purpose: Since the early 1990s, laparoscopic cholecystectomy has become the gold standard for cholecystectomy. Single-incision laparoscopic surgery (SILS) is a rapidly evolving field as a bridge between traditional laparoscopic surgery and natural orifice transluminal endoscopic. The results are presented here of a single surgeon’s initial experience with single-incision laparoscopic cholecystectomy with conventional laparoscopic instruments through his first 11 cases. Materials and Methods: A single curved intra-umbilical 25-mm incision was made by pulling out the umbilicus. A 12-mm trocar was placed through an open approach, and the abdominal cavity was explored with a 10-mm laparoscope. One 10-mm and one 5-mm port were inserted laterally from the laparoscope port. Dissection was performed using a dissector, which was not articulated. The gallbladder was investigated with an Endograsper, which was not articulated either. The hilum was dissected, and the cystic duct and artery were clipped and divided. Results: The patients are comprised of 9 females and 2 males with a mean age of 43.3 years and mean body mass index (BMI) of 27.6 kg/m2. Open cholecystectomy was not required. The mean operative time was 69.9 min. Length of stay was only one day. All procedures were completed successfully without any perioperative or postoperative complications. In all cases, there was no need to extend the skin incision. Postoperative follow-up did not reveal any umbilical wound complications. The cosmetic results were scored as excellent by all patients. Conclusion: These results suggest that single-incision laparoscopic cholecystectomy is feasible, safe and effective and a promising alternative method to four-port and SILS-port laparoscopic cholecystectomy and as scarless abdominal surgery for the treatment of some patients with gallbladder disease with standard laparoscopic instruments.

AIM: To assess the diagnostic accuracy,of aminotransferase-to-platelet ratio index(APRI) alone and with antischistosomal antibody(Ab) in patients with hepatitis C virus(HCV) and schistosomiasis coinfection. METHODS: This retrospective study included medical records of three hundred and eighty three Egyptianmen patients who had undergone percutaneous liver biopsy between January 2006 to April 2014 in tertiary care hospital in Qatar for diagnosis or monitoring purpose were selected. Data of patients > 18 years of age were included in the study. The values of HCV RNA titer and antischistosomal antibody titer were also taken into consideration. Patients were excluded from the study if they had any other concomitant chronic liver disease,including; history of previous antiviral or interferon therapy,immunosuppressive,therapy,chronic hepatitis B infection,human immunodeficiency virus co-infection,autoimmune hepatitis,decompensated liver disease,hepatocellular carcinoma,prior liver transplantation,and if no data about the liver biopsy present. RESULTS: Median age of patients was 46 years. About 7.1% had no fibrosis,whereas 30.4%,37.5%,20.4%,and 4.6% had fibrosis of stage Ⅰ,Ⅱ,Ⅲ,and Ⅳ respectively. In bivariate analysis,APRI score,levels of AST,platelet count and age of patient showed statistically significant association with liver fibrosis(P < 0.0001); whereas antischistosomal antibody titer(P = 0.52) and HCV RNA titer(P = 0.79) failed to show a significant association. The respective AUC values for no fibrosis,significant fibrosis,severe fibrosis and cirrhosis of APRI score were 63%,73.2%,81.1% and 88.9% respectively. This showed good sensitivity and specificity of APRI alone for grading of liver fibrosis. But the inclusion of anti-Schistosoma antibody did not improve the prediction of fibrosis stage. CONCLUSION: The study results suggest that noninvasive biochemical markers like APRI are sensitive and specific in diagnosing the degree of fibrosis and cirrhosis in patient

To the editor： Psoriasis is a chronic inflammatory, immune-mediated skin disease. Current research suggests that the inflammatory mechanisms are immune-based and maintained primarily by T cells in the dermis） The more common disorders associated with psoriasis are psoriatic arthropathy and inflammatory bowel diseases. It is reported that more than 30% of patients with psoriasis have arthritis which may be accompanied by microscopic inflammatory changes of colonic mucosa even in absence of bowel symptoms.2

Introduction: Oncocytic neoplasms of the adrenal cortex are extremely rare and are usually nonfunctioning. Only a little more than fifty cases have been described in the literature. Case Presentation: We report the case of an adrenocortical oncocytic neoplasm with malignant potential in a 21-year-old female with co-secrete testosterone and cortisol. The patient had been operated of adrenal mass. Fallowing operation, the Cushing’s syndrome and high testosterone levels resolved. Conclusion: In our knowledge this is the first case in the l?terature which co-secret cortizol and testosterone in malign oncystoma.

Double balloon enteroscopy(DBE) is an advanced type of endoscopic procedure which brings the advantage of reaching the whole small bowel using anterograde or the retrograde route. This procedure is both diagnostic and interventional for a variety of small intestinal diseases, such as vascular lesions, tumors, polyps and involvement of inflammatory bowel *** indication is the diagnosis and treatment of mid-gastrointestinal bleeding according to the recent published data all over the world. The complication rates seem to be higher than conventional procedures but growing experience is lowering them and improving the procedure to be safe and well tolerated. This review is about the technique, indications, diagnostic importance and complications of DBE according to the literature growing since 2001.
