traditional chinese medicine polysaccharides is a biologically active ingredient that is not easy to be digested.It is fermented by intestinal microflora to promote qualitative and selective changes in the composition of the intestinal microbiome,which often result in beneficial effects on the health of the host.People call it“prebiotics”.In this review,we systematically summarized the anti-diabetic effect of traditional chinese medicine polysaccharides.These polysaccharides regulate the metabolism of sugar and lipids by inter-influence with the intestinal microflora,and maintain human health,while improving type 2 diabetes-like symptoms such as high blood glucose,and abnormal glucose and lipid metabolism.

Nonalcoholic fatty liver disease(NAFLD)is characterized by excessive accumulation of hepatic lipids and metabolic stress-induced liver injury.There are currently no approved effective pharmacological treatments for NAFLD.traditional chinese medicine(TCM)has been used for centuries to treat patients with chronic liver diseases without clear disease types and mechanisms.More recently,TCM has been shown to have unique advantages in the treatment of NAFLD.We performed a systematic review of the medical literature published over the last two decades and found that many TCM formulas have been reported to be beneficial for the treatment of metabolic dysfunctions,including Potentilla discolor Bunge(PDB).PDB has a variety of active compounds,including flavonoids,terpenoids,organic acids,steroids and tannins.Many compounds have been shown to exhibit a series of beneficial effects for the treatment of NAFLD,including anti-oxidative and anti-inflammatory functions,improvement of lipid metabolism and reversal of insulin resistance.In this review,we summarize potential therapeutic effects of TCM formulas for the treatment of NAFLD,focusing on the medicinal properties of natural active compounds from PDB and their underlying mechanisms.We point out that PDB can be classified as a novel candidate for the treatment and prevention of NAFLD.

The global coronavirus disease 2019(COVID-19)pandemic has had a massive impact on global social and economic development and human health.By combining traditional chinese medicine(TCM)with modern medicine,the chinese government has protected public health by supporting all phases of COVID-19 prevention and treatment,including community prevention,clinical treatment,control of disease progression,and promotion of recovery.Modern medicine focuses on viruses,while TCM focuses on differential diagnosis of patterns associated with viral infection of the body and recommends the use of TCM decoctions for differential treatment.This differential diagnosis and treatment approach,with its profoundly empirical nature and holistic view,endows TCM with an accessibility advantage and high application value for dealing with COVID-19.Here,we summarize the advantage of and evidence for TCM use in COVID-19 prevention and treatment to draw attention to the scientific value and accessibility advantage of TCM and to promote the use of TCM in response to public health emergencies.

traditional chinese medicine(TCM)has been successfully applied worldwide in the treatment of coronavirus disease 2019(COVID-19),which is caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2).However,the pharmacological mechanisms underlying this success remain unclear.Hence,the aim of this review is to combine pharmacological assays based on the theory of TCM in order to elucidate the potential signaling pathways,targets,active compounds,and formulas of herbs that are involved in the TCM treatment of COVID-19,which exhibits combatting viral infections,immune regulation,and amelioration of lung injury and fibrosis.Extensive reports on target screening are elucidated using virtual prediction via docking analysis or network pharmacology based on existing data.The results of these reports indicate that an intricate regulatory mechanism is involved in the pathogenesis of COVID-19.Therefore,more pharmacological research on the natural herbs used in TCM should be conducted in order to determine the association between TCM and COVID-19 and account for the observed therapeutic effects of TCM against COVID-19.

Background Stroke is a major cause of death or long-term disability worldwide.Many patients with stroke receive integrative therapy consisting of Western medicine(WM)and routine rehabilitation in conjunction with chinese medicine(CM),such as acupuncture and chinese herbal medicine.However,there is no available evidence on the effectiveness of the combined use of WM and CM interventions in stroke rehabilitation.Aims The purpose of this meta-analysis is to evaluate the results of all individual studies to assess the combined use of CM and WM in stroke rehabilitation compared with WM only.Methods The Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines were followed.MEDLINE,EMBASE,Cochrane and China National Knowledge Infrastructure(CNKI)were searched.The included outcomes were dependency,motor function,depression and swallowing function.Subgroup analysis was performed,and publication bias was assessed using funnel plots.Summary of review 58 studies and 6339 patients were included in the meta-analysis.Subgroup analysis revealed that combined therapy comprising both acupuncture and WM had a superior effect on improving dependency and swallowing function compared with standard WM therapy alone.Potential superiority of combined therapy comprising CM and WM in improving depression compared with standard WM therapy was also found.Conclusions Our results indicate that the combined use of CM and WM could be more efficacious in stroke rehabilitation compared with the use of WM therapy alone.However,most studies were short in duration(2 to 4 weeks)and prone to different types of biases,which prevents making any conclusion regarding the long-term effects and raises concerns regarding true efficacy in context of high likelihood of Hawthorn bias.So,more randomised controlled trials with more rigorous design and longer duration of treatment and follow-up need to be conducted to compare WM alone versus WM and CM combined.

OBJECTIVE:To evaluate the efficacy and safety of activating blood circulation and removing blood stasis in terms of traditional chinese medicine(TCM) for managing renal fibrosis(RF) in patients with chronic kidney disease(CKD).METHODS:We searched randomized controlled trials(RCTs) from eight databases.RESULTS:Sixteen eligible studies with 1,356 participants were included in this study.Compared to treatment with Western medicine(WM) alone,the combined treatment with activating blood circulation and removing blood stasis in terms of TCM(ARTCM) and WM to manage RF in patients with CKD significantly ameliorated type Ⅳ collagen(C-Ⅳ)(SMD:-2.17,95% CI:3.01 to-1.34),type Ⅲ procollagen(PC-Ⅲ)(SMD:-1.08,95% CI:-1.64 to-0.53),laminin(LN)(SMD:-1.28,95% CI:-1.65 to-0.90),transforming growth factor β 1(TGF-β1)(SMD:-0.65,95% CI:-1.18 to-0.12),serum creatinine(Scr)(SMD:-1.36,95% CI:-1.85 to-0.87),blood urea nitrogen(BUN)(MD:-1.51,95% CI:-2.59 to-0.43),and 24 h urine protein(24h Upro)(SMD:-1.23;95% CI:-1.96 to-0.50).The level of hyaluronic acid(HA) was similar in both types of treatment(SMD:-0.74,95% CI:-1.91 to 0.44).The subgroup analysis showed that the duration of 8 weeks might affect the concentration of C-Ⅳ,PC-Ⅲ,and LN(P < 0.05).The effectiveness of the longer duration to C-Ⅳ,PC-Ⅲ,and LN was not certain.However,the result should be interpreted in care.The safety of the treatment using ARTCM and WM could not be evaluated because a few studies had reported adverse effects.The results of the Meta-analysis were not stable enough.There was publication bias for the reports on Scr(P = 0.001),C-Ⅳ(P = 0.001),PC-Ⅲ(P = 0.026),and LN(P = 0.030) and no publication bias for the reports on BUN(P = 0.293).The quality of evidence varied from low to very low.CONCLUSIONS:The combined treatment using ARTCM and WM to manage RF in patients with CKD has some advantages over treatment with WM alone.High-quality RCTs need to be conducted for the strong support.

Background traditional chinese medicine(TCM)is becoming a popular complementary approach in pediatric oncology.However,few or no meta-analyses have focused on clinical studies of the use of TCM in pediatric oncology.Objective We explored the patterns of TCM use and its efficacy in children with cancer,using a systematic review,meta-analysis and data mining study.Search strategy We conducted a search of five English(Allied and Complementary medicine Database,Embase,PubMed,Cochrane Central Register of Controlled Trials,and ClinicalTrials.gov)and four chinese databases(Wanfang Data,China National Knowledge Infrastructure,chinese Biomedical Literature Database,and VIP chinese Science and Technology Periodicals Database)for clinical studies published before October 2021,using keywords related to“pediatric,”“cancer,”and“TCM.”Inclusion criteria We included studies which were randomized controlled trials(RCTs)or observational clinical studies,focused on patients aged<19 years old who had been diagnosed with cancer,and included at least one group of subjects receiving TCM treatment.Data extraction and analysis The methodological quality of RCTs and observational studies was assessed using the six-item Jadad scale and the Effective Public Healthcare Panacea Project Quality Assessment Tool,respectively.Meta-analysis was used to evaluate the efficacy of combining TCM with chemotherapy.Study outcomes included the treatment response rate and occurrence of cancer-related symptoms.Association rule mining(ARM)was used to investigate the associations among medicinal herbs and patient symptoms.Results The fifty-four studies included in this analysis were comprised of RCTs(63.0%)and observational studies(37.0%).Most RCTs focused on hematological malignancies(41.2%).The study outcomes included chemotherapy-induced toxicities(76.5%),infection rate(35.3%),and response,survival or relapse rate(23.5%).The methodological quality of most of the RCTs(82.4%)and observational studies(80.0%)was rated as“moderate.”In studies of leukemia patient

OBJECTIVE:To investigate the clinical efficacy and safety of a diagnosis and treatment plan for moderate coronavirus disease 2019(COVID-19)that integrates traditional chinese(TCM)and western medicine.METHODS:One hundred twenty patients with moderate COVID-19 were randomized 1∶2 to the control group(n=40)and experimental group(n=80).Both groups received conventional western medicine treatment,and the experimental group also received TCM decoction.Over a 2-week period from diagnosis,we observed the time to clinical recovery(TTCR),rate of improvement on lung computed tomography(CT)imaging,time to defervescence,cough remission time,hospital discharge rate,average hospitalization stay,modified Medical Research Council(m MRC)scale score,clinical cure rate,laboratory findings,incidence of progression to severe or critical disease,and adverse events.RESULTS:Among 120 enrolled patients,108 completed the study.The baseline data did not differ between the experimental and control groups(all P>0.05).After treatment,the TTCR,rate of lung CT imaging improvement,time to defervescence,cough remission time,hospital discharge rate,average hospitalization stay(among discharged patients),m MRC scale score,clinical cure rate,and rates of normal values for laboratory findings were better in the experimental group than in the control group(P0.05).CONCLUSION:The diagnosis and treatment plan integrating chinese and western medicine showed improved clinical efficacy compared with western medicine alone for patients with moderate COVID-19 and is worthy of clinical promotion and application.

Dear Editor,The safety and modernization of traditional chinese medicine (TCM) are significant concerns for users’ lives and health. Recently, the adverse effects caused by TCM and traditional nontoxic TCM have been frequently reported (Joo et al., 2020;Liu et al., 2021). As the safety of TCM continues to attract widespread attention, many researchers have been investigating the toxicity of TCM in regard to the toxic ingredients, effects, mechanisms, and detoxification strategies in TCM (Figure S1 in Supporting Information). Although some TCMs cause toxic reactions, they also play key roles in treating certain unignorable diseases (Fang et al., 2022).Therefore, toxicity research is crucial and urgently needed to develop and safely use TCM.
