目的系统评价和整合社区居家老年人跌倒经历的看法与体验,为后续发展跌倒预防及管理的干预提供信息,为健康照护实践提供建议。方法依据乔安娜布里格斯研究所(Joanna Briggs Institute,JBI)质性系统评价方法论进行质性研究meta整合,在PubMed、CINAHL、PsycInfo、Embase、Web of science等数据库进行检索,发表时间限定为2010年1月至2023年5月,仅纳入经同行评审的英文文献。使用JBI质性研究质量评价标准评价纳入文献的方法学质量,按照ConQual评分准则检验整合结果的信度。本综述方案已在PROSPERO注册(CRD 42023421789)。结果共纳入10项研究,方法学质量评价得分为60%至90%。提炼59个完好明确的结果,将相似结果归纳组合形成7个类别,并综合成3个整合结果,在ConQual评分上,综合结果2评为中等,综合结果1和3评为低等。整合结果1:跌倒后老年人经历了身体的损伤和疼痛、日常活动受限、社交受限、独立性降低或丧失、并有恐惧和无助感。整合结果2:经历跌倒后,老年人反思跌倒的原因,并识别、解释风险因素。整合结果3:老年人对跌倒原因和后果有正面和负面的看法,其采取的应对跌倒的措施包括使用辅助器具、纠正风险因素、寻求医疗帮助、关注身体和心理状况等。结论医疗卫生提供者应重视老年人跌倒后的感受、经历,以及对跌倒原因和影响的反思,进行特定的干预。今后还需要进行纵向研究,以检测跌倒对老年人的长期影响,深入了解他们的反思、感知、态度和预防行为随时间的改变情况。

Thermal energy storage(TES) is of great importance in solving the mismatch between energy production and *** this regard,choosing type of Phase Change Materials(PCMs) which are widely used to control heat in latent thermal energy storage systems,plays a vital role as a means of TES ***,this field suffers from lack of a comprehensive investigation on the impact of various PCMs in terms of *** address this issue,in this study,in addition to indicating the melting temperature and latent heat of various PCMs,the exergy destruction and exergy efficiency of each material are estimated and compared with each ***,in the present work the impact of PCMs mass and ambient temperature on the exergy efficiency is *** results proved that higher latent heat does not necessarily lead to higher exergy ***,to obtain a suitable exergy efficiency,the specific heat capacity and melting temperature of the PCMs must also be *** to the results,LiF-CaF_(2)(80.5%:19.5%,mass ratio) mixture led to better performance with satisfactory exergy efficiency(98.84%) and notably lower required mass compared to other ***,the highest and lowest exergy destruction are belonged to GR25 and LiF-CaF_(2)(80.5:19.5) mixture,respectively.

Newborn hearing screening(NHS) programs are essential to identify hearing loss early in life and to improve outcomes in children. In Saudi Arabia, the national NHS program has been operational since 2016;however, few studies have evaluated its status, and none have covered all provinces across the country. This cross-sectional retrospective study provides an overview of the program's status across all provinces, focusing on screening coverage rates, referral/fail rates, and follow-up procedures. In 2021, 199,034 newborns were screened, with a coverage rate of 92.6% and an overall referral/fail rate of 1.87%. These performance measures provide a foundation for future progress and improvements. This study highlights the importance of ongoing efforts to enhance the program's effectiveness and sustainability.

Assessment of Oral Anticoagulation with Vitamin K Antagonists in Patients Living in a Low-Income Country of West Africa

Introduction: Despite the rise of direct oral anticoagulants (DOACs), vitamin K antagonists (VKA) remain the most widely used oral anticoagulants in developing countries. The aim of this study was to estimate the prevalence of good anticoagulation in patients treated with VKA in Lomé and describe associated factors. Methods: This was a cross-sectional study conducted from November 2019 to October 2020 in the cardiology departments of two University teaching hospitals in Lomé (CHU Sylvanus Olympio and CHU Campus), involving patients on VKA for ≥3 months, with a target international normalized ratio (INR) of 2.5 and a therapeutic margin between 2 and 3. The quality of anticoagulation was assessed by the time in therapeutic range (TTR) which was assessed by the Rosendaal method. Good anticoagulation was defined by a TTR > 70%. Results: A total of 344 patients were included (mean age = 58 ± 13.8 years, women = 56.1%). Indications for VKA treatment were represented by venous thromboembolic disease (43.3%), supraventricular arrhythmia (28.2%), severe left ventricular systolic dysfunction (19.8%) and pulmonary hypertension (8.7%). The average TTR was 47.6 ± 20.8%. The rate of good anticoagulation was 17.7%. Factors associated with good anticoagulation were the use of fluindione vs acenocoumarol (OR = 11.17;95% CI: 3.2 - 39.6;p = 0.0002), concomitant low-dose aspirin (OR 4.44;95% CI: 1.4 - 13.9;p = 0.01) and INR monitoring exclusively by the patient himself (OR = 4.92;95% CI: 1.5 - 16.3;p = 0.008). The rate of thromboembolic and hemorrhagic complications was each 2.6% and was not correlated with the quality of anticoagulation. Quality of anticoagulation by VKAs was poor in our practice. Factors associated with good anticoagulation were the use of fluindione vs acenocoumarol, concomitant low-dose aspirin and monitoring of INR exclusively by the patient himself. Purpose:To examine the effects of a school-based karate intervention on academic achievement,psychosocial functioning,and physical fitness in children aged 7-8 ***:Twenty schools in 5 different European countries(2 second-grade classrooms per school)participated in a cluster randomized controlled trial(sport at School trial).Participants were assigned to either a control group,which continued with their habitual physical education lessons,or to an intervention group,which replaced these lessons with a 1-year karate intervention(Karate Mind and Movement program).A total of 721 children(344 girls and 377 boys,7.4±0.5 years old,mean±SD)completed the study,of which 333 and 388 were assigned to the control group and intervention group,*** included academic performance(average grade),psychosocial functioning(Strengths and Difficulties Questionnaire for parents),and different markers of physical fitness(cardiorespiratory fitness,balance,and flexibility).Results:The intervention provided small but significant benefits compared to the control group for academic achievement(d=0.16;p=0.003),conduct problems(d=-0.28;p=0.003),cardiorespiratory fitness(d=0.36;p0.05).Conclusion:A 1-year school-based karate intervention was effective in improving academic achievement,conduct problems,and physical fitness in primary school *** results support the inclusion of karate during physical education lessons.

Introduction The application of large language models such as generative pre-trained transformers(GPTs)has been promising in medical education,and its performance has been tested for different medical *** study aims to assess the performance of GPTs in responding to a set of sample questions of short-answer management problems(SAMPs)from the certification exam of the College of Family Physicians of Canada(CFPC).Method Between August 8th and 25th,2023,we used GPT-3.5 and GPT-4 in five rounds to answer a sample of 77 SAMPs questions from the CFPC *** independent certified family physician reviewers scored AI-generated responses twice:first,according to the CFPC answer key(ie,CFPC score),and second,based on their knowledge and other references(ie,Reviews’score).An ordinal logistic generalised estimating equations(GEE)model was applied to analyse repeated measures across the five *** According to the CFPC answer key,607(73.6%)lines of answers by GPT-3.5 and 691(81%)by GPT-4 were deemed ***’s scoring suggested that about 84%of the lines of answers provided by GPT-3.5 and 93%of GPT-4 were *** GEE analysis confirmed that over five rounds,the likelihood of achieving a higher CFPC Score Percentage for GPT-4 was 2.31 times more than GPT-3.5(OR:2.31;95%CI:1.53 to 3.47;p<0.001).Similarly,the Reviewers’Score percentage for responses provided by GPT-4 over 5 rounds were 2.23 times more likely to exceed those of GPT-3.5(OR:2.23;95%CI:1.22 to 4.06;p=0.009).Running the GPTs after a one week interval,regeneration of the prompt or using or not using the prompt did not significantly change the CFPC score *** In our study,we used GPT-3.5 and GPT-4 to answer complex,open-ended sample questions of the CFPC exam and showed that more than 70%of the answers were accurate,and GPT-4 outperformed GPT-3.5 in responding to the *** language models such as GPTs seem promising for assisting candidates of the CFPC exam by pr

In this review,we focus on providing basics and examples for each component of the protein therapeutic specifications to interested pharmacists and biopharmaceutical scientists with a goal to strengthen understanding in regulatory science and *** specifications comprise a list of important quality attributes for testing,references to use for test procedures,and appropriate acceptance criteria for the tests,and they are set up to ensure that when a drug product is administered to a patient,its intended therapeutic benefits and safety can be rendered *** of drug substance or drug product to the specifications is achieved by testing an article according to the listed tests and analytical methods and obtaining test results that meet the acceptance *** attributes are chosen to be tested based on their quality risk,and consideration should be given to the merit of the analytical methods which are associated with the acceptance criteria of the *** criteria are set forth primarily based on efficacy and safety profiles,with an increasing attention noted for patient-centric *** in this work are related guidelines that support the biopharmaceutical specification setting,how to set the acceptance criteria,and examples of the quality attributes and the analytical methods from 60 articles and 23 pharmacopeial *** are also explored on process analytical technologies and other orthogonal tools which are on-trend in biopharmaceutical characterization and quality control.
