BACKGROUND Neovascular glaucoma(NVG)is likely to occur after pars plana vitrectomy(PPV)for diabetic retinopathy(DR)in some patients,thus reducing the expected *** the risk factors for NVG occurrence and building effective risk prediction models are currently required for clinical *** To develop a visual risk profile model to explore factors influencing DR after *** We retrospectively selected 151 patients with DR undergoing *** patients were divided into the NVG(NVG occurrence)and No-NVG(No NVG occurrence)groups according to the occurrence of NVG within 6 months after *** risk factors for postoperative NVG were screened by logistic regression.A nomogram prediction model was established using R software,and the model’s prediction accuracy was verified internally and externally,involving the receiver operator characteristic curve and correction *** After importing the data into a logistic regression model,we concluded that a posterior capsular defect,preoperative vascular endothelial growth factor≥302.90 pg/mL,glycosylated hemoglobin≥9.05%,aqueous fluid interleukin 6(IL-6)≥53.27 pg/mL,and aqueous fluid IL-10≥9.11 pg/mL were independent risk factors for postoperative NVG in patients with DR(P<0.05).A nomogram model was established based on the aforementioned independent risk factors,and a computer simulation repeated sampling method was used to internally and externally verify the nomogram *** area under the curve(AUC),sensitivity,and specificity of the model were 0.962[95%confidence interval(95%CI):0.932-0.991],91.5%,and 82.3%,*** AUC,sensitivity,and specificity of the external validation were 0.878(95%CI:0.746-0.982),66.7%,and 95.7%,*** A nomogram constructed based on the risk factors for postoperative NVG in patients with DR has a high prediction *** study can help formulate relevant preventive and treatment measures.

AIM:To explore the prognostic factors for lacrimal gland adenoid cystic carcinoma(LGACC)in Chinese ***:Clinical and histopathological data were reviewed in patients with pathologically confirmed *** recurrence,metastasis,and disease-specific death were the main outcome *** and multivariate analyses were performed by the Kaplan-Meier method and a Cox proportional hazard ***:This retrospective cohort study included 45 patients with pathologically confirmed LGACC between January 2008 and June ***(T)classification(P=0.005),nodal metastasis(N)classification(P=0.018)and positive margin(P=0.008)were independent risk factors of recurrence;T(P=0.013)and N(P=0.003)classification and the basaloid tumor type(P=0.032)were independent risk factors for metastasis;T classification(P<0.001)was an independent factor of death of *** the further analysis,the durations from first surgery to radiotherapy is correlated with metastatic risk in LGACC patients with basaloid component(P=0.022).CONCLUSION:Histological subtype should be emphasized when evaluating prognosis and guiding *** radiotherapy may reduce the risk of metastasis in patients with basaloid component.

Utility of real-time 3D visualization system in the early stage of phacoemulsification training

●AIM:To determine the teaching effects of a real-time three dimensional(3D)visualization system in the operating room for early-stage phacoemulsification training.●METHODS:A total of 10 ophthalmology residents of the first-year postgraduate were *** the residents were novices to cataract ***-time cataract surgical observations were performed using a custom-built 3D visualization *** training lasted 4wk(32h)in all.A modified International Council of Ophthalmology’s Ophthalmology Surgical Competency Assessment Rubric(ICO-OSCAR)containing 4 specific steps of cataract surgery was *** self-assessment(self)and expert-assessment(expert)were performed through the microsurgical attempts in the wet lab for each participant.●RESULTS:Compared with pre-training assessments(self 3.2±0.8,expert 2.5±0.6),the overall mean scores of posttraining(self 5.2±0.4,expert 4.7±0.6)were significantly improved after real-time observation training of 3D visualization system(P<0.05).Scores of 4 surgical items were significantly improved both self and expert assessment after training(P<0.05).●CONCLUSION:The 3D observation training provides novice ophthalmic residents with a better understanding of intraocular microsurgical *** is a useful tool to improve teaching efficiency of surgical education.

A deep learning based fine-grained classification algorithm for grading of visual impairment in cataract patients

Recent advancements in artificial intelligence(AI)have shown promising potential for the automated screening and grading of ***,the different types of visual impairment caused by cataracts exhibit similar phenotypes,posing significant challenges for accurately assessing the severity of visual *** address this issue,we propose a dense convolution combined with attention mechanism and multi-level classifier(DAMC_Net)for visual impairment ***,the double-attention mechanism is utilized to enable the DAMC_Net to focus on lesions-related ***,a hierarchical multi-level classifier is constructed to enhance the recognition ability in distinguishing the severities of visual impairment,while maintaining a better screening rate for normal *** addition,a cost-sensitive method is applied to address the problem of higher false-negative rate caused by the imbalanced *** results demonstrated that the DAMC_Net outperformed ResNet50 and dense convolutional network 121(DenseNet121)models,with sensitivity improvements of 6.0%and 3.4%on the category of mild visual impairment caused by cataracts(MVICC),and 2.1%and 4.3%on the category of moderate to severe visual impairment caused by cataracts(MSVICC),*** comparable performance on two external test datasets was achieved,further verifying the effectiveness and generalizability of the DAMC_Net.

Outcomes and variables that impact pneumatic retinopexies

●AIM:To evaluate the efficacy of pneumatic retinopexy(PR)in patients undergoing PR as primary treatment for rhegmatogenous retinal detachment(RRD)and analyze the factors associated with success and failure in the studied population.●METHODS:A retrospective chart review was done of patients with RRD treated with PR as primary management method treated at New York eye and Ear Infirmary of Mount Sinai between January 2017 and December *** outcome measured success or failure of *** outcome measured best corrected visual acuity(BCVA)after PR.A separate risk analysis was done to identify and stratify risks associated with success and failure of PR.●RESULTS:A total of 179 eyes from 179 patients were included for final *** 83 patients(46.37%)achieved anatomical reattachment of the retina after primary PR with no need for additional *** 96 patients(53.63%)had a failed primary PR and required a PPV and 6 of them required pars plana vitrectomy(PPV)with scleral buckle(SB).In total,19 cases(10.61%)were done as temporizing pneumatics,18(94.74%)underwent PPV,and 1(5.26%)did not require further *** visual acuities at postoperative month 1(POM1)for patients who underwent primary PR successfully and for those that underwent PPV after,were 0.58(20/80)and 1.03(20/200)*** who met Pneumatic Retinopexy Versus Vitrectomy for the Management of Primary Rhegmatogenous Retinal Detachment Outcomes Randomized Trial(PIVOT)criteria had a statistically significant decreased risk of primary PR failing(hazard ratio 0.29,P=0.00).Majority of missed or new breaks were found superotemporally.●CONCLUSION:PR is a good treatment option for treating RRDs in patients that meet PIVOT criteria and can be conducted as a temporizing *** criteria and fovea on status decrease the risk of PR failure.
