AIM:To assess the relationship between dietary inflammatory index(DII)and prevalence of glaucoma among individuals aged 40y and above in the United ***:Participants were drawn from 2 cycles of the National Health and Nutrition Examination Survey(NHANES,2005-2008)for a cross-sectional *** was calculated from 24-hour dietary recall questionnaire conducted by experienced researchers and data analyzed in R according to the NHANES user guide,“Stratified Multi-stage Probability Sampling”.The relationship between DII and glaucoma was evaluated by multi-factor logistic regression analysis and the existence of a non-linear association examined by restricted cubic spline(RCS)***:A total of 5359 subjects were included and the cross-sectional analysis weighted to represent the US population of 109 *** was elevated in glaucoma patients(P<0.001)and smoking and alcohol use contributed to significant differences(P<0.001).DII correlated negatively with Healthy Eating Index(HEI)-2015(Spearman rank correlation coefficient,r=-0.49).RCS analysis showed a linear relationship between DII and glaucoma risk(P of non-linear relationship=0.575).CONCLUSION:An increased DII is strongly associated with high risk of glaucoma and diet-induced inflammation should be controlled to delay glaucoma progression.

OBJECTIVE: To screen the target population with symptomatic bradyarrhythmia for pharmacotherapy. METHODS: This study use database from clinical trial that use Yuanjiang decoction(元姜方), a traditional chinese medicinal prescription. Eligible participants were recruited and treated with Yuanjiang decoction(composed of 6 chinese herbal medicines), 200 m L twice daily. Cross-contingency analysis, logistic regression analysis, interaction analysis, discriminant analysis and 10-fold cross validation methods were used to establish discriminant model. RESULTS: This study concluded that the clinical treatment of bradyarrhythmia has a clear effect. Low minimum heart rate, high maximum heart rate are risk factors that affect the efficacy. Patients with only one comorbid disease did not significantly affect the efficacy, but patient with two or more diseases of coronary heart disease, hypertension, paroxysmal atrial fibrillation, premature ventricular contraction and premature atrial contraction at the same time did not have a good effect. Using the discriminant analysis method to establish a efficacy prediction model, y = 0.07X1 + 0.16X2-0.65X3-1.12X4-0.71X5-0.75X6-0.91X7(X1 = 24 h mean heart rate, X2 = minimum heart rate, X3 = coronary heart disease, X4 = paroxysmal atrial fibrillation, X5 = premature ventricular contraction, X6= sinus block, X7 = atrioventricular block). CONCLUSION: Our model based on the clinical features of patients with bradyarrhythmia. Should be useful aid for predicting pharmacotherapy response and could screen the optimal pharmacotherapy target.

Chronic atrophic gastritis(CAG)is an important stage of precancerous lesions of gastric *** treatment and regulation of CAG are essential to prevent its progression to *** chinese medicine(TCM)has shown multi-targeted efficacy in CAG treatment,with advantages in enhancing gastric mucosal barrier defense,improving microcirculation,modulating inflammatory and immune responses,and promoting lesion healing,*** studies and meta-analyses indicate that TCM provides significant benefits,with specific chinese herbal compounds and monomers demonstrating protective effects on the gastric mucosa through mechanisms including anti-inflammation,antioxidation,and regulation of cellular proliferation and apoptosis,***,it is pointed out that the efficacy of TCM in the treatment of CAG requires standardized research and unified standards,and constantly clarifies and improves the evaluation criteria of each dimension of gastric mucosal barrier function.

OBJECTIVE: To enhance the understanding of identifying personalized pharmacotherapy options in traditional chinese Medicine(TCM), and further support the registration of new TCM drugs. METHODS: Generalized Boosted Models and XGBoost were employed to construct a classification model to identify the bad prognosis factors in resistant hypertension(RH) patients. Furthermore, we used association analysis to explore the rules of "symptomsyndrome" and "symptom-herb" for the major influencing factors, in order to summarize prescription pattern and applicable patients of TCM. RESULTS: Patients with major adverse cardiac events mostly have complex symptoms of phlegm, stasis, deficiency and fire intermingled with each other, and finally summarized the human experience of using chinese herbal medicine to precisely intervene in some symptoms of RH patients on the basis of conventional Western medical treatment. CONCLUSIONS: Machine learning algorithms can make full use of human use experience and evidence to save clinical trial resources and accelerate the development of TCM varieties.

Mechanisms and Efficacy of chinese Herbal Medicines in Benign Prostatic Hyperplasia

Benign prostatic hyperplasia(BPH) is one of the most common diseases in elderly men, the incidence of which gradually increases with age and leads to lower urinary tract symptoms(LUTS), which seriously affects the quality of life of patients. chinese herbal medicines(CHMs) are widely used for the treatment of BPH in china and some other countries. To explore the molecular mechanisms of CHMs for BPH, we conducted a review based on peer-reviewed English-language publications in Pub Med and Web of Science databases from inception to December 31, 2023. This article primarily reviewed 32 papers on the use of CHMs and its active compounds in the treatment of BPH, covering animal and cell experiments, and identified relevant mechanisms of action. The results suggest that the mechanisms of action of CHMs in treating BPH may involve the regulation of sex hormones,downregulation of cell growth factors, anti-inflammatory and antioxidative effects, inhibition of cell proliferation, and promotion of apoptosis. CHMs also exhibit α-blocker-like effects, with the potential to relax urethral smooth muscle and alleviate LUTS. Additionally, we also reviewed 4 clinical trials and meta-analyses of CHMs for the treatment of BPH patients, which provided initial evidence of the safety and effectiveness of CHMs treatment. CHMs treatment for BPH shows advantages as a multi-component, multi-target, and multi-pathway therapy, which can mitigate the severity of the disease, improve LUTS, and may become a reliable treatment option in the future.

OBJECTIVE: To investigate the difference in gut microbiota between population with damp-heat constitution(DHC) and balanced constitution(BC). METHODS: A multi-centered cross-sectional casecontrol study was conducted, which included 249 participants with damp-heat constitution or balanced constitution. Baseline information of participants was collected, and stool samples were collected for gut microbiota analysis. Principal coordinate analysis, linear discriminant analysis effect size analysis, receiver operating characteristic, random forest model, and phylogenetic investigation of communities by reconstruction of unobserved states methods were used to reveal the relationship between gut microbiota and the damp-heat constitution. RESULTS: Compared to those in the BC group, the richness and diversity of the microbiota, specifically those of several short-chain fatty acid producing genera such as Barnesiella, Coprobacter, and Butyricimonas, were significantly decreased in the DHC group. Regarding biological functions, flavonoid biosynthesis, propanoate metabolism, and nucleotide sugar metabolism were suppressed, while arachidonic acid metabolism and glutathione metabolism were enriched in the DHC group. Finally, a classifier based on the microbiota was constructed to discriminate between the DHC and BC populations. CONCLUSION: The gut microbiota of the DHC population exhibits significantly reduced diversity and is closely related to inflammation, metabolic disorders, and liver steatosis, which is consistent with clinical observations, thus serving as a potential diagnostic tool for traditional chinese medicine constitution discrimination.

OBJECTIVE: To illustrate the effect of add-on therapy of traditional chinese Medicine(TCM) on the long-term survival of anemic lower-risk myelodysplastic syndromes(LR-MDS) patients. METHODS: In this study, we conducted a retrospective analysis of the data of patients with LR-MDS who were identified from Shuguang hospital between January 2006 and June 2020. The Kaplan-Meier method was used to calculate overall survival(OS), while the Cox model was employed to analyze prognostic factors of long-term survival. A total of 162 patients who met the inclusion criteria were divided into two groups: the TCM group(n = 78) or the non-TCM group(n = 84). RESULTS: There were no significant differences in baseline characteristics. The cumulative survival rate of the TCM group was significantly higher than that of the non-TCM group [log-rank P = 0.029, hazard ratio(HR) = 0.484, 95% confidence interval(CI)(0.249, 0.942), P 0.05). CONCLUSIONS: The integration of TCM and Western Medicine has the potential to extend the OS of LR-MDS patients when compared to the use of Western Medicine alone.

AIM:To describe the anatomic characteristics of the lacrimal sac and its adjacent bone structures and to provide surgical recommendations for endoscopic dacryocystorhinostomy(EN-DCR).METHODS:This retrospective comparative study involved 118 sides with complete nasolacrimal duct obstruction and 83 unaffected sides from 126 *** tomographic-dacryocystography(CT-DCG)scans were performed before lacrimal surgery,and image reconstruction was used to obtain continuous 0.75-mm axial,coronal,and segmental sections for *** morphology of the lacrimal sac and its relationship with adjacent bony structures were determined for measurement,as well as the obstructed ***:The height of the lacrimal sac was 12.99±2.10 mm in this *** operculum of the middle turbinate(OMT)was located vertically in the lower third of the lacrimal ***,the junction between the maxillary bone and the lacrimal bone(MB-LB)was close to,mostly(60.2%)posterior to,the lacrimal *** uncinate process was more frequently attached to the lacrimal bones(75.1%).The obstructions were generally located around the entrance of the nasolacrimal duct(NLD).However,some were placed higher,with 7.63%blocked not lower than the *** was a negative correlation between the diameter of the lacrimal sac and the level of obstruction(r=-0.35,P<0.01).CONCLUSION:In this study,the OMT and MB-LB can be served as the landmarks in *** uncinectomy should be performed in most *** obstructions were generally located around the entrance of the NLD,but some extreme individual variations strongly implies the importance of CT-DCG scanning before surgery.

A recent paper published in Nature[1]by Bradley and colleagues reports that zosurabalpin,an optimized tethered macrocyclic peptide(MCP)antibiotic,can kill Acinetobacter baumannii by inhibiting Lpt B2FGC transport of lipopolysaccharide(LPS)from the bacterial plasma membrane to its outer membrane(Fig.1).This breakthrough in the search for new antibiotics,like petrichor after a long drought,breathes new life into the prospect of treating drug-resistant bacterial like carbapenem-resistant Acinetobacter baumannii(CRAB).
