The limitations of current drug-eluting stent technologies in selectively inhibiting vascular smooth muscle cell proliferation,which often leads to inflammation,call for innovative approaches in coronary artery disease *** the present work,we propose a revolutionary solution:a three-in-one platform for vascular stents,combining arsenic trioxide(ATO)and tacrolimus(TAC)to address anti-proliferation,pro-endothelialization,and anti-inflammation *** findings demonstrate that the synergistic action of ATO and TAC effectively suppresses aberrant vascular smooth muscle cell proliferation and mitigates endothelial cell ***,the combination treatment of TAC/ATO enhances endothelial cell migration and adhesion ***,our TAC/ATO-eluting stent exhibits superior re-endothelialization and anti-restenosis effects in a rabbit and porcine stent implantation *** in vitro and in vivo results solidify the notion that the TAC/ATO-eluting stent ensures rapid re-endothelialization and significantly reduces the incidence of in-stent ***,this study represents a promising and novel multifunctional platform with immense potential in the therapy of coronary artery disease.

Accelerating Pneumococcal Conjugate Vaccine introductions in Indonesia:key learnings from 2017 to 2022

Despite high pneumococcal disease and economic burden in Indonesia and interest to introduce pneumococcal conjugate vaccine (PCV), there were challenges in establishing a comprehensive strategy to accelerate and enable the introduction in country in the early 2010s. Starting in 2017, Clinton Health Access initiative and partners supported the government of Indonesia with evidence-based decision-making and implementation support for introducing PCV into the routine immunization program. Indonesia has since accelerated PCV roll out, with nationwide reach achieved in 2022. On the path to PCV introduction, several challenges were observed that impacted decision making on whether and on how to optimally roll out PCV, resulting in significant introduction delays;including (1) a complex country context with a devolved government structure, fragmented domestic funding streams, and an imminent transition out of major immunization donor (Gavi) support;(2) strong preference to use domestically sourced products, with limited experience accessing global pooled procurement mechanism including for vaccines;and (3) concerns around programmatic feasibility and sustainability. This case study documents key insights into the challenges experienced and how those were systematically addressed to accelerate new vaccine introduction in Indonesia, with support from local and global stakeholders over time. The learnings would be beneficial for other countries yet to introduce critical new vaccines, in particular those with similar archetype as Indonesia e.g., middle-income countries with domestic manufacturing capacity and/or countries recently transitioning out of Gavi support.

Common psychiatric disorders(CPDs)and depression contribute significantly to the global epidemic of type 2 diabetes(T2D).We postulated a possible pathophysiological mechanism that through Bridge-Symptoms present in depression and CPDs,promotes the establishment of emotional eating,activation of the reward system,onset of overweight and obesity and,ultimately the increased risk of developing *** plausibility of the proposed pathophysiological mechanism is supported by the mechanism of action of drugs such as naltrexonebupropion currently approved for the treatment of both obesity/overweight with T2D and as separate active pharmaceutical ingredients in drug addiction,but also from initial evidence that is emerging regarding glucagon-like peptide 1 receptor agonists that appear to be effective in the treatment of drug *** hope that our hypothesis may be useful in interpreting the higher prevalence of CPDs and depression in patients with T2D compared with the general population and may help refine the integrated psychiatric-diabetic therapy approach to improve the treatment and or remission of T2D.

Birth Preparedness and Complication Readiness in Uttar Pradesh, India

The objective of this study is to investigate the subject of birth preparedness and complication readiness to tackle the obstetric complications. Birth preparedness and complication readiness is studied based on husband’s response. Emergency birth preparedness has been promoted to address delays in seeking skilled care at birth. However, little evidence is available for birth preparedness and its key covariates. Therefore, to examine the level of birth preparedness, this study has used primary data collected from the selected district of Uttar Pradesh. The information in this study may be valuable to Indian health policy makers and programme implementers who wish to lower the prevailing high maternal mortality in the country. This paper is based on the primary data collected from 33 husbands of deceased women and 150 husbands of surviving mothers from one selected district of the state of Uttar Pradesh, India. Birth preparedness and complication readiness is measured using series of questions. The husbands were asked whether they followed the following key desired steps while their wife was pregnant with last child: 1) thinking about the measures to be taken in case of life danger;2) being aware about the delivery date of wife;3) saving money for meeting delivery expenses;4) discussing plans with close family members;5) keeping ready clean cotton and other necessities;6) identifying a hospital to go in case of emergency;7) arranging transportation and 8) arranging dai/doctor. Taking at least five steps were considered being well-prepared. Frequency distribution and cross tabulation analysis were done and logistic regression model was applied to understand independent effect of key covariates. Proportion test and t-test were also applied to check the statistical significance in two groups. It was found that different steps of birth preparedness were not planned in case of all pregnancies. On February 1, 2016, the world Health Organization declared that the cluster of microcephaly cases and other neurological disorders constitute public health emergency of international concern. Furthermore, few studies demonstrated that there was an increased evidence of causal relationship of Zika virus(ZIKAV) infection and microcephaly, birth abnormalities and neurological disorders such as Guillain–Barre′ *** transmission occurs mainly by the bite of infected mosquitos(Aedes species), but there are also reports that infections could occur via the placenta, breast milk, saliva,blood transfusion and sex. This article reviews the global efforts, progress in scientific research to understand the pathogenesis of ZIKAV infection & disease, clinical presentations, congenital transmission and autoimmune neurological disorders. The paper further explores the knowledge gaps, future priority research agenda for strategic response including vector control and prevention. We conducted a systematic literature review to synthesise available evidence on ZIKAV infection and its vector and host interaction from electronic databases.

Veterinary Laboratory Diagnosis: A Missed Opportunity in the Continuum of Care for Veterinarians in Selected Countries in Sub-Sahara Africa

In Africa, sustainable livestock production must address food security, as well as animal disease concerns simultaneously in addition to social aspects. Livestock disease challenges seem to be increasing with one of them being lack of good animal diagnostics services with notable emerging zoonotic diseases which if not correctly diagnosed in animals pose risks to humans. The major livestock hubs in sub-Sahara Africa (SSA) include Uganda, Ethiopia, Tanzania and Nigeria and utilization of diagnostic services by veterinarians in these countries could inform further action by different stakeholders in the SSA region. A cross sectional survey with veterinarians was conducted between January 2022 and February 2022 to determine the number of veterinarians who have submitted samples for veterinary diagnosis, laboratory utilization rates and challenges faced by veterinarians using veterinary labs in sub-Sahara Africa between the period of 2018 and 2022. Methods: This was a cross sectional study, where an online questionnaire was shared with veterinarians through their online social group networks in different countries and data was captured and analyzed. Results: 74% (64/87) of veterinarians reported having submitted a sample to a lab, Government labs were the most utilized at 54%, followed by private labs at 37% and the human health labs recorded 9% utilization rate. The most faced challenge by veterinarians was failing to get samples to the lab because the labs where far (52%) with the least challenge reported being failure to understand the lab results at 3%. Conclusion: Government labs were the most utilized in this period, however notable challenges like failing to get samples to the lab still exist. Veterinary diagnosis is still an underutilized service in sub-Sahara Africa and more measures need to be worked on in terms of sample logistics, capacity building and trainings of both lab personnel and veterinarians in interpretation of results to ensure improved utilizati
