Background Primary open-angle glaucoma (POAG) is characterized by optic nerve damage and consists of a group of genetically heterogeneous disorders. This study was to investigate the associations of genetic and environmental factors with POAG in a hospital-based Chinese population Methods Thirty-two adult onset POAG patients and 96 age-sex matched control subjects were studied by multivariable logistic regression analysis for the relationships between POAG and its risk factors including family history, diabetes, hypertension, cardiovascular diseases, cigarette smoking, alcohol consumption and polymorphisms of the myocilin and the optineurin genes Results Univariate analysis showed that POAG was related to family history, cardiovascular disease, alcohol consumption and a myocilin sequence alteration (T353I) ( P <0 04) Multivariable logistic regression analysis confirmed that POAG was significantly associated with family history ( OR =20 2), hypertension ( OR =3 58), cigarette smoking ( OR =10 8), alcohol consumption ( OR =0 028) and T353I ( OR =6 03, all P <0 05) Conclusions Family history, hypertension, cigarette smoking and T353I in the myocilin gene are risk factors for POAG Alcohol consumption, however, has a protective effect

AIM：To estimate the overall prevalence of diabetic retinopathy（DR） in Iran by a systematic review and ***：We conducted a search of all published literature on diabetic patients for the prevalence of DR using Web of Sciences, Pub Med, Scopus, Google Scholar, and national electronic databases SID, Magiran, and Iranmedex from their inception until September 2016 with standard keywords. Pooled estimates of the DR prevalence and the corresponding 95% confidence intervals（CI） were calculated using random effects ***：Thirty-one studies involving 23 729 patients with type Ⅰ and Ⅱ diabetes were included. The publication bias assumption for prevalence of DR was rejected by Begg and Egger tests（P=0.825, P=0.057, respectively）. The results of Cochran test and I^2 statistics showed considerable heterogeneity for prevalence of DR（Q=1278.21, d.f.=30, P〈0.001 and I^2=97.7%）. The prevalence of DR, non-proliferative diabetic retinopathy（NPDR） and proliferative diabetic retinopathy（PDR） in Iranian diabetic patients were 41.9%（95% CI：35.6-48.2）, 32.2%（95% CI：28.7-35.8）, and 13.2%（95% CI：8.3-18.1）, respectively. CONCLUSION：The prevalence of DR in Iran appears a little high. NPDR was more common. This study highlights the necessity for DR screening and management in diabetic patients in Iran.

This study examined whether exposure to nitrate in drinking water is associated with increased risk for bladder cancer by conducting a comprehensive literature research. A meta-analysis was performed with and without adjustment for confounding factors. Three groups (reference, intermediate and high groups) were established in terms of different nitrate concentrations in each included study. Separate relative risk measures were calculated for intermediate and high groups. Heterogeneity was assessed by using the Q statistics. Publication bias was evaluated by Egger’s and Begg’s test. Quality assessment for studies was performed by using the Newcastle-Ottawa scale. Two cohorts, two case-controls, and one ecological study were included in this study. The adjusted data showed that the combined risk ratios (RRs) were 1.13 (95% CI: 0.81 to 1.57) and 1.27 (95% CI: 0.75 to 2.15) for inter-mediate and high groups respectively. For unadjusted data, the corresponding RRs were 1.18 (95% CI: 0.89 to 1.57) and 1.29 (95% CI: 0.81 to 2.07). Sensitivity test indicated that results were significantly underestimated when Ward’s study was included. No significant publication bias was found. There was heterogeneity among studies. The results suggested that there was no sufficient evidence that nitrate in drinking water is associated with increased risks for bladder cancer.

Effectiveness of colorectal cancer screening integrating non-genetic and genetic risk: a prospective study based on UK Biobank data

Objective: Few studies have evaluated the benefits of colorectal cancer(CRC) screening integrating both non-genetic and genetic risk factors. Here, we aimed to integrate an existing non-genetic risk model(QCancer-10) and a 139-variant polygenic risk score to evaluate the effectiveness of screening on CRC incidence and ***: We applied the integrated model to calculate 10-year CRC risk for 430,908 participants in the UK Biobank, and divided the participants into low-, intermediate-, and high-risk groups. We calculated the screening-associated hazard ratios(HRs) and absolute risk reductions(ARRs) for CRC incidence and mortality according to risk ***: During a median follow-up of 11.03 years and 12.60 years, we observed 5,158 CRC cases and 1,487 CRC deaths, respectively. CRC incidence and mortality were significantly lower among screened than non-screened participants in both the intermediateand high-risk groups [incidence: HR: 0.87, 95% confidence interval(CI): 0.81±0.94;0.81, 0.73±0.90;mortality: 0.75, 0.64±0.87;0.70, 0.58±0.85], which composed approximately 60% of the study population. The ARRs(95% CI) were 0.17(0.11±0.24) and 0.43(0.24±0.61), respectively, for CRC incidence, and 0.08(0.05±0.11) and 0.24(0.15±0.33), respectively, for mortality. Screening did not significantly reduce the relative or absolute risk of CRC incidence and mortality in the low-risk group. Further analysis revealed that screening was most effective for men and individuals with distal CRC among the intermediate to high-risk ***: After integrating both genetic and non-genetic factors, our findings provided priority evidence of risk-stratified CRC screening and valuable insights for the rational allocation of health resources.

AIM： To evaluate the efficacy and safety of vitrectomy with internal limiting membrane（ILM） peeling for diabetic macular edema（DME）. METHODS： The PubMed, Embase, Web of Science, Cochrane, SionMed, ***, CNKI databases and Wanfang databases, published until Oct. 2017, were searched to identify studies comparing the clinical outcomes following vitrectomy with and without ILM peeling, for treating DME. Pooled results were expressed as odds ratios（ORs） with corresponding 95% confidence intervals（CI） for vitrectomy with and without ILM peeling with regard to best corrected visual acuity（BCVA）, central macular thickness（CMT）, and complication incidents. RESULTS： A total of 14 studies involving 857 eyes were included of which three studies were Chinese and the rests were English literatures. Meta-analysis indicated that compared with vitrectomy alone, vitrectomy with ILM peeling could improve BCVA more obviously（OR=1.66, 95%CI： 1.12-2.46, P=0.01） and had higher rate of CMT reduction（OR=3.89, 95%CI： 1.37-11.11, P=0.01）. There were significant statistical differences between the two surgical methods for both BCVA and CMT（P〈0.05）. For the incidence of intraoperative and postoperative complications, the incidence of epiretinal membrane（ERM） was slightly lower in the ILM peeling group than the group without ILM peeling（OR=0.38, 95%CI： 0.07-2.00, P=0.25）, although insignificant statistically. Other incidences of overall complications, iatrogenic peripheral retinal break and increased intraocular pressure indicated no significant difference between two groups（OR=1.19, 95%CI： 0.82-1.73, P=0.36; OR=1.21, 95%CI： 0.66-2.21, P=0.53; OR=1.34, 95%CI： 0.75-2.40, P=0.32）. CONCLUSION： Vitrectomy is effective for DME and the effect can be improved by additional ILM peeling, especially for anatomical efficacy, without increasing the incidence of intraoperative and postoperative complications. Fish from the Great Lakes contain polychlorinated biphenyls（PCBs）which have been shown to disrupt endocrine function and mimic thyroid hormones,but they also contain beneficial omega-3 fatty acids that may offer protection against endocrine *** purpose of this study was to examine the effects of Lake Ontario fish consumption and the estimated consumption of PCBs and omega-3 fatty acids on the risk of thyroid cancer in a group of sport *** from the New York State Angler Cohort Study were followed for cancer incidence from 1991–***-seven cases of incident thyroid cancer and 108 controls were included in the *** estimated fish consumption,estimated omega-3 fatty acid consumption,and estimated PCB consumption from Lake Ontario fish were examined for an association with the incidence of thyroid cancer,while matching on sex,and controlling for age and smoking *** from logistic regression indicate no significant associations between fish consumption,short-term estimated omega-3 fatty acids,or estimated PCB consumption from Great Lakes fish and the development of thyroid cancer,but it was suggested that long-term omega-3 fatty acid from Great Lakes fish may be protective of the development of thyroid *** conclusion,fish consumption,with the possible concomitant PCBs,from the Great Lakes does not appear to increase the risk of thyroid cancer in New York *** research is needed in order to separate the individual health effects of PCBs from omega-3 fatty acids contained within the fish.
