Natural history of benign prostate hyperplasia

Background Benign prostate hyperplasia is one of the most common diseases affecting the health of the aging males. Watchful waiting is an acceptable management strategy for benign prostate hyperplasia in which the patient is monitored by the physician but receives no active intervention. The epidemiological data on this are lacking in China. Our study was designed to evaluate the changes of signs and symptoms of patients with benign prostate hyperplasia during management by watchful waiting in China. Methods One hundred and forty-five patients with benign prostate hyperplasia aged 〉 50 years were enrolled in management by watchful waiting. All the patients were visited every 6 months and were given an International Prostate Symptom Score and Quality of Life questionnaire to complete. They also had uroflowmetry and were assessed using ultrasonography to get the volume of prostate, transition zone and amount of residual urine. The Student＇s t test, the Chi-square test, and variance analysis were used in the statistical analysis. Results All patients were visited after 6 months, the mean volume of transitional zone was found to have increased by 1.6 ml （P〈0.01）, International Prostate Symptom Score was increased by 0.8 （P〈0.01） and Quality of Life was increased by 0.2 （P〈0.01）, and there was no statistical change in other data. Among these patients, 17.9% （26/145） visited again after 12 months when the data failed to show a statistically significant difference among the three groups （0, 6, and 12 months）. Conclusions After one year＇s follow-up, the progression of benign prostate hyperplasia was slow and the clinical data did not undergo much change.

Trends in the prevalence of heart diseases over a ten-year period from single-center observations based on a large echocardiographic database

There is a paucity of data regarding trends in the incidence of heart disease in China during recent years. Using a large echocardiography database in our center, we analyzed trends in the prevalence of several common heart diseases from Dec. 2003. This study retrospectively analyzed the echocardiographic database in our Department from 2003 to 2012. A total of 385 682 cases were included in the study. The prevalence of rheumatic heart disease decreased over the 10-year period, from 4.04% in 2003 to 3.06% in 2012 （P〈0.01）. Infective endocarditis also decreased, from a mean prevalence of 0.37% in July 2003 to 0.27% in Dec. 2008 （P〈0.001）. The prevalence of hy- pertrophic cardiomyopathy, which includes 20% apical hypertrophic cardiomyopathy and 20% hypertrophic obstructive cardiomyopathy, was about 1.8%. The prevalence of the three most common adult congenital heart diseases （CHDs） decreased by about 10% from July 2003 to Dec. 2008 （all P〈0.001）. The prevalence of moderate pulmonary arterial hypertension （PAH） or left ventricular systolic dysfunction （LVSD） decreased during the 10-year period （P〈0.001）, but there was no change in the prevalence of severe PAH or LVSD （P〉0.05）. The present study indirectly demonstrates that the prevalence of several common heart diseases in China has declined in recent years.

Relationship between XRCC1 polymorphisms and susceptibility to prostate cancer in men from Han, Southern China

Aim： To investigate the association among XRCC1 polymorphisms, smoking, drinking and the risk of prostate cancer （PCa） in men from Han, Southern China. Methods： In a case-control study of 207 patients with PCa and 235 cancerfree controls, frequency-matched by age, we genotyped three XRCC1 polymorphisms （codons 194, 280 and 399） using the polymerase chain reaction-restriction fragment length polymorphism （PCR-RELP） method. Results： Among the three polymorphisms, we found that the XRCC1 Arg399Gln variant allele was associated with increased PCa risk （adjusted odd ratio [OR]： 1.67, 95% confident interval [CI]： 1.11-2.51）, but the XRCC1 Arg 194Trp variant allele had a 38% reduction in risk of PCa （adjusted OR： 0.62, 95% CI： 0.41-0.93）. However, there was no significant risk of PCa associated with Arg280His polymorphism. When we evaluated the three polymorphisms together, we found that the individuals with 194Arg/Arg wild-type genotype, Arg280His and Arg399Gln variant genotypes had a significantly higher risk of PCa （adjusted OR： 4.31; 95% CI： 1.24-14.99） than those with three wild-type genotypes. In addition, we found that Arg399Gln variant genotypes had a significant risk of PCa among heavy smokers （adjusted OR： 2.04; 95% CI： 1.03-4.05）. Conclusion： These results suggest that polymorphisms of XRCC1 appear to influence the risk of PCa and may modify risks attributable to environmental exposure.

Predicting responses to platin chemotherapy agents with biochemically-inspired machine learning

The selection of effective genes that accurately predict chemotherapy responses might improve cancer *** compare optimized gene signatures for cisplatin,carboplatin,and oxaliplatin responses in the same cell lines and validate each signature using data from patients with *** support vector machine learning is used to derive gene sets whose expression is related to the cell line GI 50 values by backwards feature selection with *** genes and functional pathways distinguishing sensitive from resistant cell lines are identified by contrasting signatures obtained at extreme and median GI 50 *** of gene signatures at different thresholds are combined to reduce the dependence on specific GI 50 values for predicting drug *** most accurate gene signatures for each platin are:cisplatin:BARD1,BCL2,BCL2L1,CDKN2C,FAAP24,FEN1,MAP3K1,MAPK13,MAPK3,NFKB1,NFKB2,SLC22A5,SLC31A2,TLR4,and TWIST1;carboplatin:AKT1,EIF3K,ERCC1,GNGT1,GSR,MTHFR,NEDD4L,NLRP1,NRAS,RAF1,SGK1,TIGD1,TP53,VEGFB,and VEGFC;and oxaliplatin:BRAF,FCGR2A,IGF1,MSH2,NAGK,NFE2L2,NQO1,PANK3,SLC47A1,SLCO1B1,and *** from The Cancer Genome Atlas(TCGA)patients with bladder,ovarian,and colorectal cancer were used to test the cisplatin,carboplatin,and oxaliplatin signatures,resulting in 71.0%,60.2%,and 54.5%accuracies in predicting disease recurrence and 59%,61%,and 72%accuracies in predicting remission,*** cisplatin signature predicted 100%of recurrence in non-smoking patients with bladder cancer(57%disease-free;N=19),and 79%recurrence in smokers(62%disease-free;N=35).This approach should be adaptable to other studies of chemotherapy responses,regardless of the drug or cancer types.

Periodontitis is a chronic infectious disease characterized by plaque biofilm as its main pathogenic factor;its clinical manifestations include gingival inflammation,alveolar bone resorption,attachment loss,and periodontal pocket formation,all of which eventually lead to tooth loss and seriously affect the quality of life of patients[1].This illness has a close relationship with blood sugar levels in patients with diabetes and is one of the most important complications of diabetes[2].Macrophages are an important part of host immune responses and exhibit powerful functions of identifying,phagocytizing,and removing bacteria;macrophage polarization is involved in the pathogenesis of periodontitis[3].

背景术中知晓的长期影响差异巨大，部分患者没有任何长期影响，而另一些患者则可导致严重和持久的心理障碍。本研究比较了随机化B—Aware研究中，发生明确术中知晓患者与未发生明确术中知晓患者创伤后应激障碍（postlraumaticstressdisorder，PTSD）的发生率。方法我们采用配对群体设计，旨在随访发生明确术中知晓的13例患者。每1例发生术中知晓的患者均按年龄、性别、手术类型、手术日期及手术医院配比4例对照。以临床术后应激障碍评分对每1例术中知晓及对照患者进行面对面的访谈。结果本研究收集数据资料的时间从2006年6月到2007年3月，随访的中位数时间5．3年（范围4．3-5．7年）。其间发生明确术中知晓的13例患者中6例去世，7例患者中的5例（71％）与25例对照中的3例（12％）在随访时点表现符合PTSD的诊断标准（校正的比值比=13．3[95％的可信区间为1．4-650]，P=0．02）。症状出现时间中位数时间为术后14天（范围7-243天），症状持续时间的中位数为4．7年（范围4．4-5．6年）。结论PTSD在B-Aware研究中有明确术中知晓的患者中常见而且持久。全麻下应强化预防患者术中知晓的措施。

Assessing Biomedical Solid and Liquid Waste Management in university Hospital Centers (CHU) in Togo, 2021

Introduction. Biomedical waste represents an environmental concern and a risk to healthcare workers, users of healthcare services, and the surrounding population. This study aimed to assess the management of solid and liquid biomedical waste in university Hospitals Centers (UHC) in Togo in 2021. Methods. This is a cross-sectional, evaluative and analytical study undertaken in 2021. It involved 3 UHCs, 25 departments, 340 care providers and departments randomly selected, 72 directors or deputies, supervisors and heads of departments, 27 collection and incineration agents selected by a reasoned choice technique, and 44 patients and attendants selected by an accidental choice technique. Data analysis was done using Pearson’s Chi2 statistical test for comparing proportions and logistic regression. Results. Solid and liquid waste management was “poor” due to non-use of waste management guidelines (ORa = 3.50;p = 0.0000), insufficient training of healthcare providers and collection agents (ORa = 6.55;p = 0.0000 and ORa = 6.08;p = 0.0000 respectively), insufficient user awareness sessions (ORa = 4.04;p = 0.0001), insufficient coordination of activities (ORa = 5.07;p = 0.0002), insufficient supervision of service providers and collection agents (ORa = 2.34;p = 0.0000), insufficient monitoring and follow-up of activities (ORa = 20.40;p = 0.0000). The sorting was not systematic (74.1%), and the Biochemical Oxygen Demand (BOD) and Chemical Oxygen Demand (COD) of the effluents were relatively high. Conclusion. Managing solid and liquid biomedical waste is insufficient in Togo’s university hospitals and represents a potential risk to human health and the environment.
