Gestational diabetes mellitus(GDM),defined as any degree of glucose intolerance with onset or first recognition during pregnancy,is a common pregnancy outcome.[1]GDM carries risks for the mothers,and fetus,neonate,and childhoodin the offspring.Women with GDM mayincrease risks of high blood pressure and preeclampsia during pregnancy,primary cesarean section,and future obesity,hypertension,dyslipidemia,metabolic syndrome,and cardiovascular disease.

Background: Given the general unavailability, common adverse effects, and complicated administration of tetracycline, the clinical application of classic bismuth quadruple therapy (BQT) is greatly limited. Whether minocycline can replace tetracycline forHelicobacter pylori (H.pylori) eradication is unknown. We aimed to compare the eradication rate, safety, and compliance between minocycline- and tetracycline-containing BQT as first-line regimens.Methods: This randomized controlled trial was conducted on 434 na?ve patients withH.pylori infection. The participants were randomly assigned to 14-day minocycline-containing BQT group (bismuth potassium citrate 110 mg q.i.d., esomeprazole 20 mg b.i.d., metronidazole 400 mg q.i.d., and minocycline 100 mg b.i.d.) and tetracycline-containing BQT group (bismuth potassium citrate/esomeprazole/metronidazole with doses same as above and tetracycline 500 mg q.i.d.). Safety and compliance were assessed within 3 days after eradication. Urea breath test was performed at 4–8 weeks after eradication to evaluate outcome. We used a noninferiority test to compare the eradication rates of the two groups. The intergroup differences were evaluated using Pearson chi-squared or Fisher’s exact test for categorical variables and Student’st-test for continuous variables.Results: As for the eradication rates of minocycline- and tetracycline-containing BQT, the results of both intention-to-treat (ITT) and per-protocol (PP) analyses showed that the difference rate of lower limit of 95% confidence interval (CI) was >?10.0% (ITT analysis: 181/217 [83.4%]vs. 180/217 [82.9%], with a rate difference of 0.5% [?6.9% to 7.9%]; PP analysis: 177/193 [91.7%]vs. 176/191 [92.1%], with a rate difference of ?0.4% [?5.6% to 6.4%]). Except for dizziness more common (35/215 [16.3%]vs. 13/214 [6.1%],P = 0.001) in minocycline-containing therapy groups, the incidences of adverse events (75/215 [34.9%]vs. 88/214 [41.1%]) and compliance (195/215 [90.7%]vs. 192/214 [89.7%]) were similar between the two groups.Conclusion:

Summary What is already known about this topic?A coronavirus disease 2019(COVID-19)vaccine booster is planned for administration to eligible individuals.Understanding the factors that influence attitudes towards the booster shot will help to identify groups that will most readily accept a booster dose.What is added by this report?Of the individuals polled,75.2%reported they would receive a booster shot.Sociodemographic characteristics influencing booster vaccine acceptance included age,gender,occupation,and education.Moreover,those who had been vaccinated against influenza,who believed herd immunity would be effective against severe acute respiratory syndrome coronavirus 2,and who reported reduced anxiety after vaccination were more likely to accept a booster dose.What are the implications for public health practice?A booster shot of the COVID-19 vaccine could be widely accepted.Communicating about the effectiveness of the COVID-19 vaccine and the impact of infection on people’s health could help increase public willingness to get a booster dose.

AIM： To estimate overall prevalence of retinopathy of prematurity （ROP） in Iran using a systematic review and Meta-analysis. METHODS： A systematic review and Meta-analysis was performed of all published studies pertaining to prevalence of ROP using international and national electronic databases （ISI Web of Sciences, PubMed, Scopus, Google Scholar, SlD, Maglran, and IranMedex） from their inception until May 2016 with standard keywords. Begg and Egger tests were used to examine the publication bias and Cochran test and f statistics were used to evaluate the statistical heterogeneity. Pooled estimate of the prevalence of ROP were calculated using random effects Meta- analysis. RESULTS： The publication bias assumption was rejected by Egger tests with P-value equal to 0.024. The results of Cochran test and 12 statistics revealed substantial heterogeneity （Q=1099.02, df=25, f=97.7%, P=-0.001）. The overall prevalence of ROP using the random effect model in Iran was 26.1% （95% Ch 20.3%-31.8%）. CONCLUSION： The prevalence of ROP is relatively high in Iran. Low birth weight and gestational age are significant risk factors for the disease. Improved care, including oxygen delivery and monitoring, for preterm babies in all facility settings would reduce the number of babies affected with ROP.

Dear Editor,Since it first appeared in humans in China in 2013,a novel H7N9influenza virus has caused 1,568 cases of infection,with a mortality rate of 39.2%(Yin et al.,2021).The early H7N9 virus strains had low pathogenicity and did not cause avian disease(Zhang et al.,2013).However,they caused concern about poultry consumption and caused huge economic losses to the poultry industry.Since mid-2016,some H7N9 viruses mutated.

AIM To investigate the relationship between the colors of vegetables and fruits and the risk of colorectal cancer in Korea.METHODS A case-control study was conducted with 923 colorectal cancer patients and 1846 controls recruited from the National Cancer center in Korea. We classified vegetables and fruits into four groups according to the color of their edible parts(e.g., green, orange/yellow, red/purple and white). Vegetable and fruit intake level was classified by sex-specific tertile of the control group. Logistic regression models were used for estimating the odds ratios(OR) and their 95% confidence intervals(CI).RESULTS High total intake of vegetables and fruits was strongly associated with a reduced risk of colorectal cancer in women(OR = 0.32, 95%CI: 0.21-0.48 for highest vs lowest tertile) and a similar inverse association was observed for men(OR = 0.60, 95%CI: 0.45-0.79). In the analysis of color groups, adjusted ORs(95%CI) comparing the highest to the lowest of the vegetables and fruits intake were 0.49(0.36-0.65) for green, and 0.47(0.35-0.63) for white vegetables and fruits in men. An inverse association was also found in women for green, red/purple and white vegetables and fruits. However, in men, orange/yellow vegetables and fruits(citrus fruits, carrot, pumpkin, peach, persimmon, ginger) intake was linked to an increased risk of colorectal cancer(OR = 1.61, 95%CI: 1.22-2.12).CONCLUSION Vegetables and fruits intake from various color groups may protect against colorectal cancer.

Objective: Plant-based diets have multiple health benefits for cancers;however, little is known about the association between plant-based dietary patterns and esophageal cancer(EC).This study presents an investigation of the prospective associations among three predefined indices of plant-based dietary patterns and the risk of EC.Methods: We performed endoscopic screening for 15,709 participants aged 40-69 years from two high-risk areas of China from January 2005 to December 2009 and followed the cohort until December 31, 2022. The overall plant-based diet index(PDI), healthful plant-based diet index(h PDI), and unhealthful plant-based diet index(u PDI), were calculated using survey responses to assess dietary patterns. We applied Cox proportional hazard regression to estimate the multivariable hazard ratios(HRs) and 95% confidence intervals(95% CIs) of EC across 3plant-based diet indices and further stratified the analysis by subgroups.Results: The final study sample included 15,184 participants in the cohort. During a follow-up of 219,365person-years, 176 patients with EC were identified. When the highest quartile was compared with the lowest quartile, the pooled multivariable-adjusted HR of EC was 0.50(95% CI, 0.32-0.77) for h PDI. In addition, the HR per 10-point increase in the h PDI score was 0.42(95% CI, 0.27-0.66) for ECs. Conversely, u PDI was positively associated with the risk of EC, and the HR was 1.80(95% CI, 1.16-2.82). The HR per 10-point increase in the u PDI score was 1.90(95% CI, 1.26-2.88) for ECs. The associations between these scores and the risk of EC were consistent in most subgroups. These results remained robust in sensitivity analyses.Conclusions: A healthy plant-based dietary pattern was associated with a reduced risk of EC. Emphasizing the healthiness and quality of plant-based diets may be important for preventing the development of EC.

Background:Empiric therapy for patients with unexplained recurrent pregnancy loss(URPL)is not precise.Some patients will ask for assisted reproductive technology due to secondary infertility or advanced maternal age.The clinical outcomes of URPL patients who have undergonein vitro fertilization-embryo transfer(IVF-ET)require elucidation.The IVF outcome and influencing factors of URPL patients need further study.Methods:A retrospective cohort study was designed,and 312 infertile patients with URPL who had been treated during January 2012 to December 2015 in the Reproduction center of Peking University Third Hospital were included.By comparing clinical outcomes between these patients and those with tubal factor infertility(TFI),the factors affecting the clinical outcomes of URPL patients were analyzed.Results:The clinical pregnancy rate(35.18%vs.34.52%in fresh ET cycles,P=0.877;34.48%vs.40.27%in frozen-thawed ET cycles,P=0.283)and live birth rate(LBR)in fresh ET cycles(27.67%vs.26.59%,P=0.785)were not significantly different between URPL group and TFI group.URPL group had lower LBR in frozen-thawed ET cycles than that of TFI group(23.56%vs.33.56%,P=0.047),but the cumulative LBRs(34.69%vs.38.26%,P=0.368)were not significantly different between the two groups.The increased endometrial thickness(EMT)on the human chorionic gonadotropin day(odds ratio[OR]:0.848,95%confidence interval[CI]:0.748-0.962,P=0.010)and the increased number of eggs retrieved(OR:0.928,95%CI:0.887-0.970,P=0.001)were protective factors for clinical pregnancy in stimulated cycles.The increased number of eggs retrieved(OR:0.875,95%CI:0.846-0.906,P<0.001),the increased two-pronucleus rate(OR:0.151,95%CI:0.052-0.437,P<0.001),and increased EMT(OR:0.876,95%CI:0.770-0.997,P=0.045)in ET day were protective factors for the cumulative live birth outcome.Conclusion:After matching ages,no significant differences in clinical outcomes were found between the patients with URPL and the patients with TFI.A thicker endometrium and more retrieved oocytes increase the

Objective Anemia is a common public health concern in patients with type 2 diabetes worldwide.This study aimed to identify the prevalence of anemia among patients with diabetes.Methods Electronic databases,including PubMed,Scopus,Web of Sciences,and Google Scholar,were searched systematically for studies published between 2010 and 2021.After removing duplicates and inappropriate reports,the remaining manuscripts were reviewed and appraised using theNewcastleOttawa Scale(NOS)tool.A random-effects model was used to calculate the pooled estimates of the extracted data using Stata version 17.Heterogeneity of the studies was assessed using the Q statistic.Results A total of 51 articles containing information on 26,485 patients with diabetes were included in this study.The articles were mainly from Asia(58.82%)and Africa(35.29%).The overall prevalence of anemia was 35.45%(95%CI:30.30–40.76),with no evidence of heterogeneity by sex.Among the two continents with the highest number of studies,the prevalence of anemia in patients with diabetes was significantly higher in Asia[40.02;95%CI:32.72–47.54]compared to Africa[28.46;95%CI:21.90–35.50](P for heterogeneity=0.029).Moreover,there has been an increasing trend in the prevalence of anemia in patients with diabetes over time,from[15.28;95%CI:9.83–22.21]in 2012 to[40.70;95%CI:10.21–75.93]in 2022.Conclusion Globally,approximately 4 in 10 patients with diabetes suffer from anemia.Therefore,routine anemia screening and control programs every 3 months might be useful in improving the quality of life of these patients.

Long-term outcomes of additional surgery versus non-gastrectomy treatment for early gastric cancer after non-curative endoscopic submucosal dissection: a meta-analysis

Background:Endoscopic resection is increasingly used in the treatment for early gastric cancer(EGC);however,about 15%of endoscopic submucosal dissection(ESD)cases report non-curative resection.The efficacy of different remedial interventions after non-curative ESD for EGC remains controversial.This meta-analysis aimed to compare the long-term outcomes of additional surgery and non-gastrectomy treatment for EGC patients who underwent non-curative ESD.Methods:All relevant studies published up to October 2021 were systematically searched in the PubMed,Web of Science,and Embase databases.The medical subject headings terms"early gastric cancer,""gastrectomy,""endoscopic submucosal dissection,"and their related free keywords were used to search relevant articles without restrictions on regions,publication types,or languages.The Newcastle–Ottawa Quality Assessment Scale was used to evaluate the quality of the included studies.Odds ratios(ORs)with 95%confidence intervals(CIs)of 5-year overall survival(OS),disease-specific survival(DSS),disease-free survival(DFS)and hazard ratios(HRs)with 95%CIs of OS were calculated using a random-or fixed-effects model.Results:This meta-analysis included 17 retrospective cohort studies with 5880 patients,of whom 3167 underwent additional surgery and 2713 underwent non-gastrectomy.We found that patients receiving additional gastrectomy had better 5-year OS(OR=3.63,95%CI=3.05–4.31),DSS(OR=3.22,95%CI=2.22–4.66),and DFS(OR=4.39,95%CI=1.78–10.82)outcomes than those receiving non-gastrectomy treatments.The pooled HR also showed that gastrectomy following non-curative ESD significantly improved OS(HR=0.40,95%CI=0.33–0.48).In addition,elderly patients benefited from additional surgery in consideration of the 5-year OS(HR=0.54,95%CI=0.41–0.72).Conclusions:Compared with non-gastrectomy treatments,additional surgery offered better long-term survival outcomes for patients with EGC who underwent non-curative ESD.
