Intelligent diagnosis of atrial septal defect in children using echocardiography with deep learning

Background Atrial septal defect(ASD)is one of the most common congenital heart diseases.The diagnosis of ASD via transthoracic echocardiography is subjective and time-consuming.Methods The objective of this study was to evaluate the feasibility and accuracy of automatic detection of ASD in children based on color Doppler echocardiographic static images using end-to-end convolutional neural networks.The proposed depthwise separable convolution model identifies ASDs with static color Doppler images in a standard view.Among the standard views,we selected two echocardiographic views,i.e.,the subcostal sagittal view of the atrium septum and the low parasternal four-chamber view.The developed ASD detection system was validated using a training set consisting of 396 echocardiographic images corresponding to 198 cases.Additionally,an independent test dataset of 112 images corresponding to 56 cases was used,including 101 cases with ASDs and 153 cases with normal hearts.Results The average area under the receiver operating characteristic curve,recall,precision,specificity,F1-score,and accuracy of the proposed ASD detection model were 91.99,80.00,82.22,87.50,79.57,and 83.04,respectively.Conclusions The proposed model can accurately and automatically identify ASD,providing a strong foundation for the intelligent diagnosis of congenital heart diseases.

To the Editor:Probiotics are a promising treatment modality for functional constipation(FC);however,the factors affecting individual responses to probiotics remain unclear.Growing evidence has identified that there is a strong relationship among the gut microbiota and constipation and related gut-brain axis.[1]The gut microbiota may modulate the gut functions via gut metabolites or trigger the release of gut hormones,such as peptide YY,gastric inhibitory polypeptide,and 5-hydroxytryptamine.[2]In turn,gut hormones affect gut secretion,motility,and sensation through their receptors located on epithelial,enteric,and smooth muscle cells.[2]Based on these findings,exogenous probiotics have been used in patients with constipation;however,few have produced consistent results.[3]In this study,we investigated the efficacy and safety of several specific probiotics strains for constipation,and also examined the potential reasons for the individualized effects of probiotics,which may facilitate interventional decisionmaking for FC patients.

Background Magnetic resonance imaging(MRI)has played an important role in the rapid growth of medical imaging diagnostic technology,especially in the diagnosis and treatment of brain tumors owing to its non invasive characteristics and superior soft tissue contrast.However,brain tumors are characterized by high non uniformity and non-obvious boundaries in MRI images because of their invasive and highly heterogeneous nature.In addition,the labeling of tumor areas is time-consuming and laborious.Methods To address these issues,this study uses a residual grouped convolution module,convolutional block attention module,and bilinear interpolation upsampling method to improve the classical segmentation network U-net.The influence of network normalization,loss function,and network depth on segmentation performance is further considered.Results In the experiments,the Dice score of the proposed segmentation model reached 97.581%,which is 12.438%higher than that of traditional U-net,demonstrating the effective segmentation of MRI brain tumor images.Conclusions In conclusion,we use the improved U-net network to achieve a good segmentation effect of brain tumor MRI images.

Identification and differentiation of major components in three different “Sheng-ma” crude drug species by UPLC/Q-TOF-MS

Cimicifugae Rhizoma(Sheng ma) is a Ranunculaceae herb belonging to a composite family and well known in China,has been widely used in traditional chinese medicine.The Pharmacopoeia of the People's Republic of China contains three varieties(Cimicifuga dahurica(Turcz.),Cimicifuga foetida L.and Cimicifuga heracleifolia Kom.) which have been used clinically as "Sheng-ma".However,the chemical constituents of three components of "Sheng-ma" have never been documented.In this study,a rapid method for the analysis of the main components of "Sheng-ma" was developed using ultra-high performance liquid chromatography with quadrupole-time-of-flight mass spectrometry(UPLC/Q-TOF-MS).The present study reveals the major common and distinct chemical constituents of C.dahurica,C.foetida and C.heracleifolia and also reports principal component and statistical analyses of these results.The components were identified by comparing the retention time,accurate mass,mass spectrometric fragmentation characteristic ions and matching empirical molecular formula with that of the published compounds.A total of 32 common components and 8 markers for different "Sheng-ma" components were identified.These findings provide an important basis for the further study and clinical utilities of the three "Sheng-ma" varieties.

Eight new cytotoxic annonaceous acetogenins from the seeds of Annona squamosa

Eight new annonaceous acetogenins, squamotin A–D(1–4), annosquatin IV–V(5 and 6), muricin O(7) and squamosten B(8), together with four known ones(9–12) were isolated from the seeds of Annona squamosa. Their structures were elucidated by chemical methods and spectral data. The inhibitory activities of compound 1–9 against three multidrug resistance cell lines were evaluated. All tested compounds showed strong cytotoxicity.

Exercise fatigue is a physiological phenomenon where the body cannot maintain a specific level after being stimulated by excessive exercise.Its five main theories include“energy depletion”,“accumulation of metabolites”,“homeostasis disorder of the internal environment”,“free radicals”,and“central nervous system protection inhibition”.The imbalance of intestinal flora caused by vigorous exercise can further lead to fatigue.Therefore,maintaining healthy intestinal flora is crucial for athlete health and performance.Recent studies have demonstrated that probiotics can regulate intestinal flora and alleviate exercise fatigue;however,there are no systematic reviews on the anti-fatigue effects of probiotics.Based on the mechanisms of exercise-induced fatigue,this article summarizes the effects and mechanisms by which probiotics combat exercise fatigue,aiming to provide research ideas for the development of probiotics that prevent exercise fatigue.

Objective:The aim of this study was to develop a reliable approach to simultaneously quantify 11 markers and explore the quality variation in honey-processed licorice.Materials and Methods:A high-performance liquid chromatography-diode array detector method was developed for the simultaneous determination of 11 markers(nine flavonoids and two triterpenoid saponins)in honey-processed licorice.The changes to the 11 markers in honey-processed licorice were investigated using an orthogonal design with three input factors.Results:The established method was precise,accurate,and sensitive enough for the simultaneous quantitative evaluation of 11 markers in honey-processed licorice.Intuitive analysis and variance analysis revealed that(1)the soaking time of crude licorice,stir-frying temperature,and stir-frying time remarkably influenced the content of liquiritin apioside,signifying the decomposition of liquiritin apioside to liquiritin or transformation of liquiritin apioside to isoliquiritin apioside,(2)stir-frying temperature significantly influenced licorice-saponin G2,(3)stir-frying temperature was the most important factor of the three input factors,(4)in terms of composition,honey fried licorice had significant effects on two components,namely liquiritin apioside and licorice-saponin G2.Conclusions:Honey processing influenced the content of the 11 licorice analytes differently.This paper highlights the first report on how the quality of honey-processed licorice varies under different processing conditions and suggests the optimal levels of the investigated three factors as A2B2C3 according to the degrees of influence of these factors on the 11 components.Specifically,the soaking time of crude licorice with honey solution,stir-frying temperature,and stir-frying time were 40 min,100°C,and 20 min,respectively.

Optimization of the processing technology of Fructus Arctii by response surface methodology

The present study was designed to optimize the processing of Fructus Arctii by response surface methodology(RSM).Based on single factor studies,a three-variable,three-level Box-Behnken design(BBD) was used to monitor the effects of independent variables,including processing temperature and time,on the dependent variables.Response surfaces and contour plots of the contents of total lignans,chlorogenic acid,arctiin,and arctigenin were obtained through ultraviolet and visible(UV-Vis) monitoring and high performance liquid chromatography(HPLC).Fructus Arctii should be processed under heating in a pot at 311℃,medicine at 119 °C for 123 s with flipping frequently.The experimental values under the optimized processing technology were consistent with the predicted values.In conclusion,RSM is an effective method to optimize the processing of traditional chinese medicine(TCM).

Development and validation of a HPLC-UV-ESI-MS method for the simultaneous quantitation of ten bioactive compounds in Dahuang Fuzi Tang

AIM: To develop a high performance liquid chromatography(HPLC) coupled with electrospray ionization mass spectrometry(ESI-MS) and ultraviolet(UV) detector method for the acid-alkaline simultaneous determination of ten bioactive compounds, and analyze the effect of compatible medicinal plants on the concentration of components in Dahuang Fuzi Tang(DFT). METHOD: The chromatographic separation was performed on a Hypersil BDS C18 analytical column by gradient elution with acetonitrile and formate buffer(containing 0.15% formic acid, V/V) at 25 °C with a flow rate of 1.0 mL ·min–1 and UV detection at 280 nm. Four of the ten compounds in DFT were identified and their MS fragments were elucidated by HPLC-ESI-MS, and the contents of the six compounds were determined by HPLC-UV. RESULTS: All calibration curves showed good linear regression(r2 ≥ 0.9990). The limits of detection and limits of quantification were 0.021–0.155 μg·mL –1 and 0.076–0.520 μg·mL –1, respectively. Overall precision RSD(intra-day and inter-day) were less than 2.96%, and the average recoveries were 98.35%–101.45%, with RSD ranging from 1.54% to 3.01% for the analytes. CONCLUSION: The developed method can be applied for the quality control and provide analytical evidence on the chemical basis and combinational principles of DFT.

Background: Curcuma wenyujin rhizome(CWR) is a commonly used chinese herbal medicine for treating blood stasis in China for 1000 of years. However, the underlying mechanism of CWR remains unclear. Aims and Objectives: The purpose of this study is to clarify the bioactive mechanism of CWR in treating blood stasis. Materials and Methods: In this study, pharmacological indexes, including hemorheology and four blood coagulation indexes were tested. Bile and urine metabolomics were engaged by UPLC-Q/TOF-MS. Multivariate statistical analysis were used to screen out differential endogenous metabolites. Results: The results indicated that CWR significantly ameliorated the hemorheology and coagulation functions of acute blood stasis(ABS) model rats. Moreover, 27 endogenous metabolites between the CWR group and the ABS group were screened, and the levels were all improved to certain degrees by CWR preadministration. Metabonomics results indicated that ABS was mainly related to linoleic acid metabolism, arachidonic acid metabolism, glycerophospholipid metabolism, sphingolipid metabolism, pentose and glucuronate intercereasonversions, steroid hormone biosynthesis, and primary bile acid biosynthesis. Conclusion: In a word, the metabolomics method is consistent with the holistic view of traditional chinese medicine(TCM) that can be a powerful means to illustrate the biological activity mechanism of CWR in treating blood stasis and to offer research demonstration for further study on the effector mechanism of TCM.
