[目的]观察瓜蒌薤白半夏汤合桃红四物汤联合西药治疗痰阻血瘀型胸痹疗效。[方法]使用随机平行对照方法,将62例住院及门诊患者按病志号抽签方法简单随机分为两组。对照组31例:阿司匹林、单硝酸异山梨酯、美托洛尔等扩血管、抗血小板、调脂、降压、抗心绞痛等。治疗组31例瓜蒌薤白半夏汤合桃红四物汤(瓜蒌12g,薤白、法半夏各9g,熟地15g,川芎8g,当归15g,赤芍10g,桃仁9g,红花6g),水煎400m L,1剂/d,饭后30min口服,2次/d;西药治疗同对照组。连续治疗7d为1疗程。观测临床症状、心绞痛发作、心电图、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组显效18例,有效11例,无效2例,总有效率93.55%。对照组显效6例,有效16例,无效9例,总有效率70.97%。治疗组疗效优于对照组(P<0.01)。心电图改善治疗组优于对照组(P<0.05)。[结论]瓜蒌薤白半夏汤合桃红四物汤联合西药治疗痰阻血瘀型胸痹,疗效满意,无严重不良反应,值得推广。

AIM: To check the safety and efficacy of boceprevir/telaprevir with peginterferon/ribavirin for hepatitis C virus(HCV) genotype 1 in the real-world settings. METHODS: This study was a non-randomized, observational, prospective, multicenter. This study involved 47 centers in Italy. A database was prepared for the homogenous collection of the data, was used by all of the centers for data collection, and was updated continuously. All of the patients enrolled in this study were older than 18 years of age and were diagnosed with chronic infection due to HCV genotype 1. The HCV RNA testing was performed using COBAS-Taq Man2.0(Roche, LLQ 25 IU/m L). RESULTS: All consecutively treated patients were included. Forty-seven centers enrolled 834 patients as follows: Male 64%; median age 57(range 18-78), of whom 18.3% were over 65; mean body mass index 25.6(range 16-39); genotype 1b(79.4%); diagnosis of cirrhosis(38.2%); and fibrosis F3/4(71.2%). The following drugs were used: Telaprevir(66.2%) and PEG-IFN-alpha2a(67.6%). Patients were na?ve(24.4%), relapsers(30.5%), partial responders(14.8%) and null responders(30.3%). Overall, adverse events(AEs) occurred in 617 patients(73.9%) during the treatment. Anemia was the most frequent AE(52.9% of cases), especially in cirrhotic. The therapy was stopped for 14.6% of the patients because of adverse events or virological failure(15%). Sustained virological response was achieved in 62.7% of the cases, but was 43.8% in cirrhotic patients over 65 years of age. CONCLUSION: In everyday practice, triple therapy is safe but has moderate efficacy, especially for patients over 65 years of age, with advanced fibrosis, nonresponders to peginterferon + ribavirin.

Background: Vitamin B12 (cobalamin) is an essential micronutrient necessary for DNA methylation and plays role in lipid metabolic reactions. Metformin is the first therapeutic choice for T2DM management. Prolonged use of metformin causes vitamin B12 deficiency due to poor absorption by interfering with calcium-based vitamin B12 absorption. Vitamin B12 deficiency leads to elevated homocysteine levels. The aim of this study was to evaluate serum vitamin B12 and homocysteine levels in type 2 diabetic patients with and without metformin therapy. Methods: A cross-sectional study was conducted on two hundred and thirty diabetic patients (180 males and 50 females). Their ages ranged from (30 - 60 years) living in Saudia Arabia at Al-Madinah Al-Monawarah. Patients were selected at outpatients clinics of Islamic University Medical Center during follow up at internal medicine and endocrinology clinic. The included patients were diagnosed with type 2 diabetes mellitus according to American Diabetes Association (ADA) Criteria. The included patients were categorized into two groups according to treatment with metformin drug. Laboratory measurements included serum level of vitamin B12, serum total homocysteine, serum fasting glucose and serum folate. Blood EDTA samples were used to measure HbA1c and MCV. Neurological examinations were performed to detect presence of peripheral neuropathy using Toronto Clinical Neuropathy Score (TCSS), which is a validated and reliable scale for the diagnosis and staging of diabetic polyneuropathy. Results: There were no statistical differences between the two groups as regard (age, sex, smoking, weight, BMI, systolic blood pressure, diastolic blood pressure, fasting blood glucose, Folate and MCV). There were statistical differences between the two groups as regard (duration of diabetes, duration of metformin therapy, dose of metformin, Serum homocystein and HbA1c). The basic attributes of urban built environment are important factors affecting mental state of older *** and perception are expressions of built environment from objective and subjective perspectives,and their comprehensive effects on older adults’ mental health have not yet been explored and *** with the ecological model of active living,the social-ecological model for health and environmental psychology for health,this paper took Dalian as an example and integrated multi-source spatio-temporal data,to compare and examine the internal influence mechanism of built environment on older adults’ mental health from objective and perceptual perspectives through structural equation *** model was further validated by excluding the samples of residential *** study showed that the proximity and aggregation degree of basic service facilities,such as supermarkets,restaurants,and barbershops,affected older adults’ mental health through direct and perceived mediating paths,in which the high degree of aggregation had a negative effect on the mental health of older adults through perceived paths of noise,insecurity,and poor environmental sanitation;the density of walking path and the local integration,depth,and connectivity of vehicle path had limited interventions on mentality dimension of older adults;the diversity of leisure,landscape,and exercise facilities in community positively affected older adults’ mental health through the direct and perceived mediating paths;underground parking in community and lifts in high-rise buildings had a more positive impact on older adults’ mental health than ground parking and stairs in multi-story ***,compared with objective built environment,perceived built environment had a mediating effect and played a more significant role in the mental health of older adults.

脓毒症是一种由感染引起的异质性疾病,感染触发了一系列复杂的局部或者全身的免疫炎症反应,引起多器官功能衰竭,发病率和病死率显著升高。由于至今仍然没有诊断脓毒症的金标准,所以脓毒症的临床诊断仍是一个难题。因此,脓毒症的临床诊断需要不断改变来满足临床和研究的要求。然而,尽管有许多新型的生物标记和筛选工具去预测脓毒症发生的风险,但是这些措施的诊断价值和有效性不足以让人满意,并且没有充分的证据去建议临床使用这些新技术。因此,脓毒症的临床诊断标准需要定期更新去适应不断产生的新证据。这篇综述旨在呈现当前脓毒症的诊断和早期识别方面的最新研究证据。临床运用不同的诊断方法的推荐意见依赖于推荐、评价、发展和评估分级体系(Grades of Recommendation Assessment,Development and Evaluation,GRADE),因为大部分的研究是观察性研究,并没有对这些方法进行可靠评估,采用的是两步推理方法。未来需要更多研究来确认或者反驳某一特殊的指标检测,同时应该直接采用相关病人的结果数据。

Background: Anambra state in south-east Nigeria is one of the high TB burden states in the country. Despite recent improvements in TB case notification, estimates from the National Prevalence survey suggest that there is still a significant pool of missed TB cases in the state. Although active TB case finding interventions are needed at community level, information on local TB transmission hotspots is lacking. The objective of this study was to map the geo-spatial location of all TB cases detected in the state in 2019. Findings from this secondary data analysis will help to target interventions appropriately with a view to achieving better program efficiency. Method: A de-identified dataset containing descriptive physical addresses of registered TB cases in 2019 was developed. The dataset was then deconstructed and restructured using Structured Query Language in a relational data base environment. The validated dataset was geocoded using ArcGIS server geocode service and validated using python geocoding toolbox, and Google geocoding API. The resultant geocoded dataset was subjected to geo-spatial analysis and the magnitude-per-unit area of the TB cases was calculated using the Kernel Density function. TB case notification rates were also calculated and Choropleth maps were plotted to portray the TB burden as contained in the dataset. Results: Five local government areas
