Major gastrointestinal bleeding and antithrombotics: Characteristics and management

BACKGROUND There are few reports on major gastrointestinal(GI)bleeding among patients receiving an *** To describe clinical characteristics,bleeding locations,management and inhospital mortality related to these *** Over a three-year period,we prospectively identified 1080 consecutive adult patients admitted in two tertiary care hospitals between January 1,2013 and December 31,2015 for major GI bleeding while receiving an *** bleeding events were medically *** characteristics,causative lesions,management and fatalities were *** distribution of antithrombotics prescribed was compared across the bleeding lesions *** Of 576 patients had symptoms of upper GI bleeding and 504 symptoms of lower GI *** cause was identified for 383(35.5%)***-duodenal ulcer was the first causative lesion in the upper tract(209 out of 408)and colonic diverticulum the first causative lesion in the lower tract(120 out of 289).There was a larger proportion of direct oral anticoagulant use among patients with lower GI than among those with upper GI lesion locations(P=0.03).There was an independent association between gastro-duodenal ulcer and antithrombotic use(P=0.03),taking account of confounders and proton pump inhibitor *** wise comparisons pointed to a difference between vitamin K antagonist,direct oral anticoagulants,and antiplatelet agents in monotherapy vs dual antiplatelet *** We showed a higher rate of bleeding lesion identification and suggested a different pattern of antithrombotic exposure between upper and lower GI lesion locations and between gastro-duodenal ulcer and other identified upper GI causes of *** was similar across antithrombotics and in-hospital mortality was low(5.95%).

Pricing strategies in inelastic energy markets： can we use less if we can＇t extract more？

Limited supply of nonrenewable energy resources under growing energy demand creates a situation when a marginal change in the quantity supplied or demanded causes non-marginal swings in price levels. The situation is worsened by the fact that we are currently running out of cheap energy resources at the global scale while adaptation to climate change requires extra energy costs. It is often argued that technology and alternative energy will be a solution. However, alternative energy infrastructure also requires additional energy investments, which can further increase the gap between energy demand and supply. This paper presents an explorative model that demonstrates that a smooth transition from an oil-based economy to alternative energy sources is possible only if it is started well in advance while fossil resources are still abundant. Later the transition looks much more dramatic and it becomes risky to rely entirely on technological solutions. It becomes increasingly likely that in addition to technological solutions that can increase supply we will need to find ways to decrease demand and consumption. We further argue that market mechanisms can be just as powerful tools to curb demand as they have traditionally been for stimulating consumption. We observe that individuals who consume more energy resources benefit at the expense of those who consume less, effectively imposing price externalities on the latters. We suggest two transparent and flexible methods of pricing that attempt to eliminate price externalities on energy resources. Such pricing schemes stimulate less consumption and can smooth the transition to renewable energy.

Numerical modeling and parametric sensitivity analysis of heat transfer and two-phase oil and water flow characteristics in horizontal and inclined flowlines using OpenFOAM

Estimating the oil-water temperatures in flowlines is challenging especially in deepwater and ultra-deepwater offshore applications where issues of flow assurance and dramatic heat transfer are likely to occur due to the temperature difference between the fluids and the surroundings. Heat transfer analysis is very important for the prediction and prevention of deposits in oil and water flowlines, which could impede the flow and give rise to huge financial losses. Therefore, a 3D mathematical model of oil-water Newtonian flow under non-isothermal conditions is established to explore the complex mechanisms of the two-phase oil-water transportation and heat transfer in different flowline inclinations. In this work, a non-isothermal two-phase flow model is first modified and then implemented in the InterFoam solver by introducing the energy equation using OpenFOAM® code. The Low Reynolds Number (LRN) k-ε turbulence model is utilized to resolve the turbulence phenomena within the oil and water mixtures. The flow patterns and the local heat transfer coefficients (HTC) for two-phase oil-water flow at different flowlines inclinations (0°, +4°, +7°) are validated by the experimental literature results and the relative errors are also compared. Global sensitivity analysis is then conducted to determine the effect of the different parameters on the performance of the produced two-phase hydrocarbon systems for effective subsea fluid transportation. Thereafter, HTC and flow patterns for oil-water flows at downward inclinations of 4°, and 7° can be predicted by the models. The velocity distribution, pressure gradient, liquid holdup, and temperature variation at the flowline cross-sections are simulated and analyzed in detail. Consequently, the numerical model can be generally applied to compute the global properties of the fluid and other operating parameters that are beneficial in the management of two-phase oil-water transportation.

Strong, lightweight, and highly conductive CNT/Au/Cu wires from sputtering and electroplating methods

In this study, we present a 2-step deposition method via sputtering and electroplating that uses carbon nanotube(CNT) wires synthesized from a wet-spinning technique to produce high-performance CNT/Au/Cu composite wires. After the Au sputtering pre-treatment, the deposition of Cu on the CNT wires was found to be much more homogeneous due to improved wettability and reactivity of the wire surface. At different electrodeposition time, the mechanical strength of the CNT/Au/Cu composite wires could be as high as 0.74 GPa(~2 times stronger than metal wires) while their electrical conductivity could reach 4.65 × 10~5 S/cm(~80% of that for copper). More importantly, the CNT/Au/Cu composite wires with high CNT volume fraction are expected to be lightweight(up to 42% lower than Cu mass density), suggesting that our high-performance composite wires are a promising candidate to substitute conventional heavy metal wires in the future applications.

450 nm diode laser:A new help in oral surgery

AIM To describe the performance of 450 nm diode laser in oral surgery *** The case described consisted of the removal of a lower lip fibroma through a blue diode laser(λ = 450 nm). RESULTS The efficacy of this device, even at very low power(1W, CW), allows us to obtain very high intra and postoperative comfort for the patient, even with just topical anaesthesia and without needing suture. The healing process was completed in one week and, during the follow-up, the patient did not report any problems, pain or discomfort even without the consumption of any kind of drugs, such as painkillers and antibiotics. The histological examination performed by the pathologist showed a large area of fibrous connective tissue with some portions of epithelium-connective detachments and a regular incision with very scanty areas of *** The 450 nm diode laser proved of being very efficient in the oral soft tissue surgical procedures, with no side effects for the patients.

Travelling Wave Solutions to a Special Type of Nonlinear Evolution Equation

A unified approach is presented for finding the travelling wave solutions to one kind of nonlinear evolution equation by introducing a concept of 'rank'. The key idea of this method is to make use of the arbitrariness of the manifold in Painlevé analysis. We selected a new expansion variable and thus obtained a rich variety of travelling wave solutions to nonlinear evolution equation, which covered solitary wave solutions, periodic wave solutions, Weierstrass elliptic function solutions, and rational solutions. Three illustrative equations are investigated by this means, and abundant travelling wave solutions are obtained in a systematic way. In addition, some new solutions are firstly reported here.

Benefits of heterospecific aggregation on necromass: in fluence of temperature, group density, and compositi on on fitness-related traits

Necrophagous blowflies(Diptera:Calliphoridae)such as Calliphora vicina,a cold-tolerant species,and L ucilia serie ata,a warm-adapted species,are pioneer carrion・*** these two species have different temperature preferences,larvae aggregate actively and often feed simultaneously on *** hypothesis to be tested was that *** benefits from the association with *** at lower temperatures(i.e.,15℃)and that *** derives greater benefits from this association at higher temperatures(i.e.,28℃).Therefore,both species were raised at these two constant temperatures from first instars to adults under three different conditions:monospecific low-density,monospecific high-density,and heterospecific *** time until larval migration,surface area of puparia,and survival rates were determined for each *** between these fitness-related traits were found between species,temperatures,group densities,and species *** 28°C,*** larvae bred in heterospecific groups migrated significantly earlier and in higher numbers than that under same density conspecific con・ditions,with a lower mortality *** 15°C,both species benefited from high-density heterospecific associations,expressed by faster development and larger *** conclusion,necrophagous larvae benefited from heterospecific aggregations at suboptima 1 temperatures by adapting their migration time to that of the faster *** temperature changes throughout the day and over the year,the beneficiary of such a collective association also *** costs involved and deviations to the temperature-size rule highlight the complexity of the carrion ecosystem.
