Overlap between headache, Depression, and Anxiety in General Neurological clinics： A Cross-sectional Study

Background： Many studies have reported that depression and anxiety have bidirectional relationship with headache. However, few researches investigated the roles of depression or anxiety in patients with headache. We surveyed the prevalence of depression and anxiety as a complication or cause of headache among outpatients with a chief complaint of headache at neurology clinics in general hospitals. Additional risk factors for depression and anxiety were also analyzed. Methods： A cross-sectional study was conducted at 11 general neurological clinics. All consecutive patients with a chief complaint of headache were enrolled. Diagnoses of depression and anxiety were made using the Chinese version of the Mini International Neuropsychiatric Interview, and those for headache were made according to the International Classification of headache Disorders, 2＇ld Edition. The headache impact test and an 11-point verbal rating scale were applied to assess headache severity and intensity. Logistic regression was used to analyze risk factors of patients with headache for depression or anxiety. Results： A total of 749 outpatients with headache were included. Among them, 148 （19.7%） were diagnosed with depression and 103 （13.7%） with anxiety. Further analysis showed that 114 （15.2%） patients complaining headache due to somatic symptoms of psychiatric disorders and 82 （10.9%） had a depression or anxiety comorbidity with headache. Most patients with depression or anxiety manifested mild to moderate headaches. Poor sleep and severe headache-related disabilities were predictors for either depression or anxiety. Conclusion： Clinicians must identify the etiology of headache and recognize the effects of depression or anxiety on headache to develop specific treatments.

Approximately 20% of patients with sleep apnea will have headaches and 25% of patients with other sleep disorders will also experience headaches. We aim to evaluate sleep apnea related symptom prevalence in patient with headache presenting to neurology outpatient clinics. Twenty four patients with headache were asked for sleep apnea symptoms. The prevalence of snoring, excessive daytime sleepiness and witnessed apnea was 33.3%, 20.8% and 4.2%, respectively. Among sleep apnea symptoms, the presence of at least one symptom, at least two symptoms and all three symptoms were 9 (37.5%), 4 (16.7%) and 1 (4.2%), respectively. The frequency of major symptoms of sleep apnea syndrome was high in patient with nonspecific headache. In the light of literature review, in case of a suspicion of sleep disorder in a headache patient, it would be appropriate to question the major symptoms of OSAS, and to refer the patient to a polysomnographic evaluation for an objective diagnosis. This approach would be helpful to improve the quality of life of patients with headache.

The clinical presentation of chronic post- traumatic headache in 53 patients from a highly specialized headache clinic was analysed and classified according to the diagnostic criteria of the primary headaches in The International headache Classification 2nd Edition, and compared with the 1st Edition. All patients fulfilled the criteria for both editions indicating that the restrictions in the 2nd Edition have no major influence on the prevalence in specialized clinics. We found the phenomenology to be very heterogeneous, but the vast majority (77% ) of patients presented with headache resembling chronic tension- type headache, either as the only manifestation or in combination with migraine symptoms. For the first time episodic tensiontype headache is described as occurring after head trauma. The prevalence of coexisting analgesic overuse was 42% and the treatment outcome for these patients was just as favourable as in primary headaches, whereas the time- consuming multidisciplinary treatment demonstrated only very modest results.

Calcified chronic subdural hematoma(CCSDH) is a rare disease that accounts for approximately 0.3%–2.7% of all chronic subdural hematomas(CSDHs). The clinical features of CCSDH are very similar to those of noncalcified CSDH and include headache,decreased alertness,weakness,numbness,gait disturbance,seizures,memory impairment,confusion,and unconsciousness. All symptomatic CCSDH should be treated surgically. Majority of these patients recover well following surgery. In this report,we present the case of a patient with CCSDH who developed severe cerebral edema following its removal,necessitating decompressive craniectomy. Although there were no abnormal findings in laboratory blood tests,and no signs of brain herniation or epilepsy was found the following day after surgery,the patient's family refused all treatment and a post-operative brain computed tomography(CT) scan. The patient was discharged and died at home. Cerebral hematoma and normal perfusion pressure breakthrough(NPPB) may cause severe cerebral edema following the total removal of a CCSDH.

Effect of Fasting during Ramadan on Migraine Sufferers

Background: Fasting during Ramadan in Muslim communities is distinct from regular voluntary or experimental fasting. Its consequent caffeine withdrawal, stress and low blood glucose level (hypoglycemia) may trigger headache in patients with migraine. Objectives: The main aim of this study was to evaluate the effect of the Ramadan fasting on the frequency of migraine attacks among observant Muslim migraine sufferers. Material and Methods: This research study was based on fasting Muslims;both genders were aged between 18 and 65 years. Fifty migraineurs were selected, participated and gave consent at Internal Medicine and neurology clinics in Riyadh National Hospital, KSA. Those patients were recruited over three consecutive months of Ragab, Shaban and Ramadan during the period of 2010 and 2013. The effect of fasting and special eating pattern during the month of Ramadan compared with the preceding two months (Ragab and Shaban) was studied. The primary parameters used for comparisons were migraine attack count, duration of migraine attacks in days, attack duration and severity of the attacks. Results: Only forty three migraine sufferers completed the current study to the end, 4 patients of them were males (9.3%) and 39 were females (90.7%). They were aged between 18 and 44 years (Mean ± SD, 30.9 ± 6.9 years). Most of the fasting migraineurs during Ramadan were caffeine drinkers (n = 36, 83.7%), cigarette smokers (n = 2, 4.6%), overweight or obese (with BMI?> 25) n = 27, 62.8%), under stress (n = 29, 67.4%) and with low to moderate physical activities (No exercise) (n = 31, 72%). The worst time for the fasting individuals was afternoon between 2:00 PM to 4:30 PM because majority of them had headache during these hours. This study showed a significant reduction in migraine attack count, number of headache days, number of attacks with acute medication and total medication intake among migraineurs during the month of Ramadan compared with the two pre-Ramadan months. Despite the high prevalence of headaches, multidisciplinary headache clinics are few and their efficacy still needs validation. We set out to describe the procedures, characterizethe patients and evaluate the treatment results in Danish headache Centre. All clinical records for patients discharged in 2002 were systematically reviewed. Diagnoses were classified in accordance with ICHD- II. Outcome results were analysed with respect to headache diagnoses, frequency, intensity, absence from work and medication use. Five hundred and five patients were included and 336 were eligible for the study. Mean age was 46 years and male/female ratio 1: 2.4. For patients without medication overuse headache (MOH) a reduction in headache frequency (P headache (TTH), migraine, cluster and other headaches. No reduction was seen in post-traumatic headache. Absence from work decreased significantly for migraine (P headache frequency was seen for TTH and migraine (P headache centre is valuable in treatment of patients with complex headache disorders.
