Assessment of cerebral oxygenation response to hemodialysis using near-infrared spectroscopy (NIRS):Challenges and solutions

To date,the clinical use of functional near-infrared spectroscopy(NIRS)to detect cerebral ischemia has been largely limited to surgical settings,where motion artifacts are *** this study,we present novel techniques to address the challenges of using NIRS to monitor ambu-latory patients with kidney disease during approximately eight hours of hemodialysis(HD)*** with end-stage kidney disease who require HD are at higher risk for cognitive impairment and dementia than age-matched *** studies have suggested that HD-related declines in cerebral blood flow might explain some of the adverse outcomes of HD ***,there are currently no established paradigms for monitoring cerebral per-fusion in real-time during HD *** this study,we used NIRS to assess cerebral hemo-dynamic responses among 95 prevalent HD patients during two consecutive HD *** observed substantial signal attenuation in our predominantly Black patient cohort that required probe *** also observed consistent motion artifacts that we addressed by devel-oping a novel NIRS methodology,called the HD cerebral oxygen demand algorithm(HD-CODA),to identify episodes when cerebral oxygen demand might be outpacing supply during HD *** then examined the association between a summary measure of time spent in cerebral deoxygenation,derived using the HD-CODA,and hemodynamic and treatment-related *** found that this summary measure was associated with intradialytic mean arterial pressure,heart rate,and volume *** studies should use the HD-CODA to implement studies of real-time NIRS monitoring for incident dialysis patients,over longer time frames,and in other dialysis modalities.

Hepatitis B virus and micro RNAs:Complex interactions affecting hepatitis B virus replication and hepatitis B virusassociated diseases

Chronic infection with the hepatitis B virus(HBV) is the leading risk factor for the development of hepatocellular carcinoma(HCC). With nearly 750000 deaths yearly, hepatocellular carcinoma is the second highest cause of cancer-related death in the world. Unfortunately, the molecular mechanisms that contribute to the development of HBV-associated HCC remain incompletely understood. Recently, micro RNAs(mi RNAs), a family of small non-coding RNAs that play a role primarily in post-transcriptional gene regulation, have been recognized as important regulators of cellular homeostasis, and altered regulation of mi RNA expression has been suggested to play a significant role in virus-associated diseases and the development of many cancers. With this in mind, many groups have begun to investigate the relationship between mi RNAs and HBV replication and HBV-associated disease. Multiple findings suggest that some mi RNAs, such as mi R-122, and mi R-125 and mi R-199 family members, are playing a role in HBV replication and HBV-associated disease, including the development of HBV-associated HCC. In this review, we discuss the current state of our understanding of the relationship between HBV and mi RNAs, including how HBV affects cellular mi RNAs, how these mi RNAs impact HBV replication, and the relationship between HBV-mediated mi RNA regulation and HCC development. We also address the impact of challenges in studying HBV, such as the lack of an effective model system for infectivity and a reliance on transformed cell lines, on our understanding of the relationship between HBV and mi RNAs, and proposepotential applications of mi RNA-related techniques that could enhance our understanding of the role mi RNAs play in HBV replication and HBV-associated disease, ultimately leading to new therapeutic options and improved patient outcomes.

Genotype-Phenotype Correlations in Ornithine Transcarbamylase Deficiency: A Mutation Update

Ornithine transcarbamylase （OTC） deficiency is an X-linked trait that accounts for nearly half of all inherited disorders of the urea cycle. OTC is one of the enzymes common to both the urea cycle and the bacterial arginine biosynthesis pathway; however, the role of OTC has changed over evolution. For animals with a urea cycle, defects in OTC can trigger hyperammonemic episodes that can lead to brain damage and death. This is the fifth mutation update for human OTC with previous updates reported in 1993, 1995, 2002, and 2006. In the 2006 update, 341 mutations were reported. This current update contains 417 disease-causing mutations, and also is the first report of this series to incorporate information about natural variation of the OTC gene in the general population through examination of publicly available genomic data and examination of phenotype/genotype correlations from patients participating in the Urea Cycle Disorders Consortium Longitudinal Study and the first to evaluate the suitability of systematic computational approaches to predict severity of disease associated with different types of OTC mutations.

Science Mapping:A Systematic Review of the Literature

Purpose： We present a systematic review of the literature concerning major aspects of science mapping to serve two primary purposes： First, to demonstrate the use of a science mapping approach to perform the review so that researchers may apply the procedure to the review of a scientific domain of their own interest, and second, to identify major areas of research activities concerning science mapping, intellectual milestones in the development of key specialties, evolutionary stages of major specialties involved, and the dynamics of transitions from one specialty to ***/methodology/approach： We first introduce a theoretical framework of the evolution of a scientific specialty. Then we demonstrate a generic search strategy that can be used to construct a representative dataset of bibliographic records of a domain of research. Next, progressively synthesized co-citation networks are constructed and visualized to aid visual analytic studies of the domain＇s structural and dynamic patterns and trends. Finally, trajectories of citations made by particular types of authors and articles are presented to illustrate the predictive potential of the analytic ***： The evolution of the science mapping research involves the development of a number of interrelated specialties. Four major specialties are discussed in detail in terms of four evolutionary stages： conceptualization, tool construction, application, and codification. Underlying connections between major specialties are also explored. A new and improved RRT∗algorithm has been developed to address the low efficiency of obstacle avoidance planning and long path distances in the electric vehicle automatic charging robot *** algorithm enables the robot to avoid obstacles,find the optimal path,and complete automatic charging *** maintains the global completeness and path optimality of the RRT algorithmwhile also improving the iteration speed and quality of generated paths in both 2D and 3D path *** finding the optimal path,the B-sample curve is used to optimize the rough path to create a smoother and more optimal *** comparison experiments,the new algorithmyielded reductions of 35.5%,29.2%,and 11.7%in search time and 22.8%,19.2%,and 9%in path length for the 3D ***,experimental validation of the automatic charging of electric vehicles was conducted to further verify the effectiveness of the *** simulation experimental validation was carried out by kinematic modeling and building an experimental *** error between the experimental results and the simulation results is within 10%.The experimental results show the effectiveness and practicality of the algorithm.

BACKGROUND The long-term impact of vitamin D deficiency and metabolic syndrome(MetS)on cardiovascular disease(CVD)and all-cause mortality are still a matter of *** To test the hypotheses that lower serum 25 hydroxyvitamin D[25(OH)D]concentrations(a marker of vitamin D level)and MetS have a long-term impact on the risk of CVD and all-cause mortality,and individuals with vitamin D deficiency can be identified by multiple *** A sample of 9094 adults,20 to 90 years of age,who participated in the Third National Health and Nutrition Examination Survey(NHANES III,1988 to 1994)were followed through December 2015 was *** associations of serum 25(OH)D concentrations and MetS with CVD and all-cause mortality were analyzed longitudinally using Cox regression *** and regression tree(CART)for machine learning was applied to classify individuals with vitamin D *** Of 9094 participants,30%had serum 25(OH)D concentrations<20 ng/mL(defined as vitamin D deficiency),39%had serum 25(OH)D concentrations between 20 to 29 ng/mL(insufficiency),and 31%had serum 25(OH)D concentrations≥30 ng/mL(sufficiency).Prevalence of MetS was 28.4%.During a mean of 18 years follow-up,vitamin D deficiency and MetS were significantly associated with increased risk of CVD and all-cause *** with both vitamin D deficiency and MetS had the highest risk of CVD mortality(HR=1.77,95%CI:1.22-2.58)and all-cause mortality(HR=1.62,95%CI:1.26-2.09),followed by those with both vitamin D insufficiency and MetS for CVD mortality(HR=1.59,95%CI:1.12-2.24),and all-cause mortality(HR=1.41,95%CI:1.08-1.85).Meanwhile,vitamin D sufficiency significantly decreased the risk of CVD and all-cause mortality for those who even had *** the total study sample,CART analysis suggests that being non-Hispanic Black,having lower serum folate level,and being female were the first three predictors for those with serum 25(OH)D *** Vitamin D defic

“No Need to Get Salty” Or Is There? Sodium Functionality as a Consideration in Nutrition Public Health Policy

Aim: Public health professionals focus on both downstream (individual) and upstream (population-level) interventions to reduce sodium consumption and prevent sodium-related chronic diseases. Effective upstream interventions specifically aimed at reducing dietary sodium intake include the implementation of comprehensive nutrition standards that restrict the amount of sodium contained in foods available for purchase. The aim of this work was to identify sought-after foods that did not meet the philadelphia Nutrition Standards’ sodium limits and reformulate those foods to be standards-compliant and consumer-acceptable. Subject and Methods: Two foods were reformulated for compliancy with the philadelphia Nutrition Standards’ sodium limits and consumer acceptability: the hoagie roll and soft pretzel. Reformulation included sensory testing and engaging potential manufacturing partners to investigate products’ commercial potential. Results: While hoagie roll reformulation led to a local company manufacturing and selling the reformulated product, soft pretzel reformulation stalled due to lack of consumer acceptability of the reformulated product. Salt contributes desirable characteristics in the texture, taste, and appearance of the soft pretzel, the absence of which consumers found unacceptable. Conclusion: Product reformulation holds great potential to create lower-sodium foods that otherwise have all of the characteristics of the higher-sodium “original” products but requires an understanding of the role of salt in product recipes. Reducing salt without considering its multiple functions in food may result in a product that is unacceptable or even unsafe. A simple four-step tool can help public health practitioners evaluate the extent to which products are suitable for reformulation.

Return and Volatility Connectedness between Stock Markets and Macroeconomic Factors in the G-7 Countries

We examine the relationship between return and volatility of the stock markets and macroeconomic fundamentals for the G-7 countries by using monthly data ranging from July 1985 to June 2015. To meet this end, we apply the spillover index approach based on the generalized VAR framework developed by Diebold and Yilmaz (2012, 2014). The empirical analysis shows strong interactions between the returns and volatilities of the G-7 stock markets and the considered set of corresponding macroeconomic factors including industrial production, money supply, interest rates, inflation, oil prices and exchange rates. The return and volatility spillover transmission/reception dynamics of the relationships between these stock markets and the macroeconomic fundamentals have changed after the global financial crisis of 2008. Our findings provide useful insights for investors and policy makers concerned with the unprecedented swings in the stock markets of G-7 countries.
