To the Editor:Colorectal cancer(CRC)remains a leading cause of cancer-related death worldwide.[1]In addition to the classical adenoma-to-carcinoma model of colorectal carcinogenesis,the serrated neoplastic pathway accounts for 20%–30%of *** serrated lesions(SSLs),the most common premalignant serrated polyp subtype,share many of the genomic characteristics of post-colonoscopy CRCs and are thus recognized as their precursor ***,SSLs can be easily missed due to their flat shape and subtle endoscopic *** they can also be difficult to distinguish from other types of polyps,especially hyperplastic polyps(HPs),which are believed to have no malignant potential.[2]Approximately 10%of polyps initially diagnosed as HPs endoscopically are reclassified as SSLs after histopathological examination,reflecting diagnostic difficulties in colonoscopy.[3,4]Current guidelines recommend“diagnose-and-leave”and“resect-and-discard”strategies for diminutive polyps,which might lead to SSLs being left in situ or diagnoses being missed,[5]raising the risk of recurrence and post-colonoscopy ***,it remains debatable in the gastrointestinal community whether the risks of resecting SSLs outweigh the reduction in cancer *** strategy that improves the diagnostic accuracy of SSLs could have a meaningful clinical impact.

Dear editor,Amiodarone is a class Ⅲ antiarrhythmic medication commonly used in the emergency department (ED) and other critical care settings to treat several types of arrhythmias while also serving as a fixture of the Advanced Cardiac Life Support algorithm.[1] Its mechanism of action primarily involves blocking potassium channel currents during myocyte repolarization with additional effects on betaadrenergic receptors and both sodium and calcium channel blockade.[2] One of amiodarone’s less known complications is the development of acute hepatotoxicity with intravenous(IV) administration.[3] While discontinuation has been recommended, there is limited literature to help guide management when clinical factors warrant its continuation.

Rapid Preparation of Filamentous Fungal DNA for PCR Analysis by Ultrasonic Treatment

A simple method to prepare of DNA template suitable for PCR amplification from filamentous fungi will be valuable for improving experimental ***,a method was developed which just needed ultrasonic treatment of the mycelium at usual condition,and the produced solution could directly be used as DNA template for internal transcribed spacer(ITS)amplification *** PCR could be improved by additional treatment of 60℃water baths,but was not *** the template amount was 0.5-2μL and the ultrasonic time was 7-11 min,there was no distinctly influences on *** method was commonly used for ***,***,Lentinula sp.,Flammul *** Dictyophora *** detect target sequences of ITS,hygromycin resistance gene(Hyg),CRISPR-associated protein 9(Cas9),Citrinin gene C(CtnC),Citrinin gene D(CtnD),large subunit rRNA gene(NL),and so *** method could provide a simple,rapid,safe and economic approach to prepare the DNA template for large-scale PCR of the special filamentous fungi materials.

This review summarized the current controversies in the management of acute pancreatitis(AP).The controversies in management range from issues involving fluid resuscitation,nutrition,the role of antibiotics and antifungals,which analgesic to use,role of anticoagulation and intervention for complications in *** interventions vary from percutaneous drainage,endoscopy or *** research and emerging data are helping to formulate better *** available evidence favors crystalloids,although the choice and type of fluid resuscitation is an area of dynamic *** nutrition aspect does not have controversy as of now as early enteral feeding is preferred most often than *** empirical use of antibiotics and antifungals are gray zones,and more data is needed for conclusive *** choice of analgesic is being studied,and the recommendations are still *** position of using anticoagulation is still awaiting *** role of intervention is well established,although the modality is constantly changing and favoring endoscopy or percutaneous drainage rather than *** is evident that more multicenter randomized controlled trials are required for establishing the standard of care in these crucial management issues of AP to improve the morbidity and mortality worldwide.

Midazolam Impairs Learning and Memory Ability Through ERK/CREB Signaling Pathway and Oxidative Damage of Hippocampus in Rats

This study aimed to elucidate whether midazolam affected the learning and memory of rats through the extracellular signal-regulated kinase(ERK)/cyclic adenosine monophosphate response element-binding(CREB)signaling pathway and hippocampal oxidative *** 120 Wistar rats were randomly assigned to four groups,including one control and three midazolam-exposed groups(20,60 and 150 mg•kg^(-1)).After an intraperitoneal injection of midazolam/physiological saline for both 1 h(n=15)and 24 h(n=15),10 rats(five came from 1 h,and the remaining five came from 24 h)were randomly selected from each group for the Morris water maze *** hippocampus tissue samples were harvested for the assessment of superoxide dismutase(SOD)and catalase(CAT)activities as well as glutathione peroxidase(GPx),malonyl dialdehyde(MDA),nitric oxide(NO)and inducible nitric oxide synthase(iNOS)*** remaining 80 rats were euthanized,and the hippocampal tissue was *** expressions of ERK1,ERK2 and CREB mRNA were tested using *** protein expressions of p-ERK1/2 and p-CREB were tested using Western *** Morris water maze tests indicated that midazolam-treated rats have weaker learning and memory ability compared to the control *** increased MDA,NO,iNOS and CAT,and decreased GPx and SOD activities compared to the control *** expression levels of ERK1/2 and CREB in the hippocampus of rats in the midazolam treatment groups were significantly lower compared to the control group at 1 h after intraperitoneal injection of midazolam,and in a dose-dependent relationship but returning it to normal levels at 24 h after midazolam intraperitoneal ***,it was concluded that the learning and memory impairment of midazolam might be associated with the down-regulation of the ERK/CREB signaling pathway and oxidative damage in rat hippocampus.

Objective:Gram-negative bacterial infection is one of the causes of impaired ovarian function and fertility,which is closely related to the adverse effects of the endotoxin lipopolysaccharide(LPS).β-Carotene is a natural antioxidant with powerful anti-inflammatory and anti-apoptotic *** purpose of this study was to explore whether LPS exposure could exert adverse effects on porcine cumulus cells and whether β-carotene could relieve the stress caused by LPS ***:The porcine cumulus oocyte complexes(COCs) were treated with 2.5,5,10 and 20 μg/mL LPS alone or cotreated with 2.5,5 and 10 μMβ-carotene during in vitro maturation for 44 *** effect of LPS exposure on porcine cumulus expansion and oocyte maturation was evaluated by the cumulus expansion index and the first polar body(PB1) extrusion *** endoplasmic reticulum stress related protein localization of GRP78 was detected by immunofluorescence staining combined with scanning technology of laser confocal ***-V-FITC probe were used to evaluate the level of *** blotting and qPCR were performed to analyze the expression levels of apoptosis related proteins and ***:The results show that cumulus expansion was observably inhibited and that the PB1 extrusion rate was also significantly decreased when the concentration of LPS reached 10 μg/*** addition,higher expression levels of GRP78 and upregulated unfolded protein response(UPR)-related genes(PERK,IRE1,and ATF4) were detected in LPS-treated cumulus ***,the proportion of apoptotic cells significantly increased,the expression of the endoplasmic reticulum stress(ERS)-related apoptotic gene(CHOP) was upregulated,and the downstream apoptotic program was subsequently ***,the 5 μMβ-carotene cotreatment could effectively alleviate the adverse effects of LPS exposure,showing the improvement of cumulus expansion and PB1 ***,β-carotene supplementation significantly alleviated

Stroke persists as a global health and economic crisis,yet only two interventions to reduce stroke-induced brain injury *** the clinic,many patients who experience an ischemic stroke often further suffer from retinal ischemia,which can inhibit their ability to make a functional recovery and may diminish their overall quality of *** this,no treatments for retinal ischemia have been *** both cases,ischemia-induced mitochondrial dysfunction initiates a cell loss cascade and inhibits endogenous brain *** cells have the ability to transfer healthy and functional mitochondria not only ischemic neurons,but also to similarly endangered retinal cells,replacing their defective mitochondria and thereby reducing cell *** this review,we encapsulate and assess the relationship between cerebral and retinal ischemia,recent preclinical advancements made using in vitro and in vivo retinal ischemia models,the role of mitochondrial dysfunction in retinal ischemia pathology,and the therapeutic potential of stem cell-mediated mitochondrial ***,we discuss the pitfalls in classic rodent functional assessments and the potential advantages of laser Doppler as a metric of stroke *** studies evaluated in this review highlight stem cell-derived mitochondrial transfer as a novel therapeutic approach to both retinal ischemia and ***,we posit the immense correlation between cerebral and retinal ischemia as an underserved area of study,warranting exploration with the aim of these treating injuries together.

The status of drug resistance and ampC gene expression in Enterobacter cloacae

Objective To investigate the status of the drug resistance and the ampC gene expression of Enterobacter cloacae . Methods Disk diffusion tests were made for detecting the susceptibility of antimicrobial agents against Enterobacter cloacae . AmpC gene was amplified by polymerase chain reaction (PCR) and verified by DNA sequencing. AmpC gene expression was analyzed according to antimicrobial agent sensitive phenotype. Results The sensitivity rates of 144 strains to imipenam,cefepime and cefoperazone/sulbactam were 98.61%,65.97% and 63.89%,respectively. The sensitivity rates of 144 strains to other antimicrobial agents were lower. Among the 144 strains 120 were found to be positive by PCR for ampC. The PCR product showed high homology to the GenBank ampC sequence. Stably derepressed strains,hyperinducible strains and unexpressing or lower level expressing strains accounted for 30.0% (36/120),37.5% (45/120),and 32.5% (39/120),respectively. Fifty-six out of 120 strains (46.67%) also produced extended spectrum β-lactamases (ESBLs). The hyperinducible strains were highly sensitive to all the antimicrobial agents except amoxicillin/clavulanic acid and cefuroxime,while the stably derepressed strains were only sensitive to imipenam and cefepime. However,sensitivity to cefepime decreased if the strains also produced ESBLs. Conclusions The durg resistant status of Enterobacter cloacae is severe. Clearing out the expressive status of ampC gene will be helpful in selection of antimicrobial agents in the treatment of clinical infection.

Crystal Structure and Anticancer Properties of Cinobufagin 3-Hemisuberate Methyl Ester

The title compound cinobufagin 3-hemisuberate methyl ester（1） was isolated from the venom of Bufo bufo gargarizans CANTOR. The crystal structure of 1, C35H48O9, was determined by single-crystal X-ray diffraction analysis. It belongs to orthorhombic, space group P212121 with a = 8.9338（3）, b = 16.2970（4）, c = 22.4019（6） , V = 3261.59（16） 3, Mr = 612.73, Z = 4, Dc = 1.248 g/cm3, μ = 0.725 mm-1, F（000） = 1320, S = 1.040, the final R = 0.0374 and wR = 0.0412 for 4458 unique reflections, of which 4088 were observed（I 〉 2σ（I））. In the solid state, short intermolecular C-H...O interactions involving a methine and the ester carbonyl group in cinobufagin moiety and a methyl in the suberate moiety linked adjacent molecules into a three-dimensional network. Detailed analysis of the 1H-NMR data showed that X-ray structure of 1 would be expected to closely resemble the solution conformation in chloroform. Compound 1 was inactive for the inhibition of PC3 and HepG2 cancer cells, but the parent compound cinobufagin showed potent inhibition with IC50 values of 0.145 and 5.48 μM, respectively, indicating that esterification at C（3） decreased the cytotoxic effect of 1.

Neuroprotective effect of Naomaitong extract following focal cerebral ischemia induced by middle cerebral artery occlusion in rats

OBJECTIVE: To examine the neuroprotective effect of extract from Naomaitong following focal cerebral ischemia reperfusion induced by occlusion of middle cerebral artery(MCA), and to determine the biochemical alterations in urine using proton nuclear magnetic resonance spectroscopy and principal component ***: Wistar rats were randomly assigned tothree groups: sham-operated group, MCA focal cerebral ischemia reperfusion model group, and active extract of Naomaitong treatment group. The model was established by an improved MCA occlusion(MCAO) method. Sham-operated rats received the same surgical procedure, but without occlusion. The Naomaitong treatment group were treated with active extract from Naomaitong at a dose of3.0 g·kg-·1d-1. Brain tissues and urine samples were collected from all groups for histopathological assessment and proton nuclear magnetic resonance spectroscopy-based metabonomics, ***: Hematoxylin-eosin and triphenyl tetrazolium chloride staining of brain tissues showed a significant decrease in cerebral infarction area in the Naomaitong group. In model rats, metabonomic analyses showed increased urinary levels of glutamate, taurine, trimetlylamine oxide, betaine, and glycine, and reduced levels of creatinine and *** regulated the metabolic changes by acting on multiple metabolic pathways, including glycine metabolism, glutaminolysis, transmethylation metabolism and creatinine ***: These data demonstrate that extract from Naomaitong is neuroprotective against focal cerebral ischemia induced by MCAO, and can alleviate biochemical changes in urinary metabolism. Metabonomics may be a useful approach for assessing the biochemical mechanisms underlying the neuroprotective actions of extract from Naomaitong.
