Background. The role of vitamin d.and.parathyroid.hormone in the metabolic profile of type 2 d.abetes mellitus in sub-Saharan Africa has not been ad.quately assessed. The aim of this stud. was to d.termine the prevalence of low vitamin d.level and.second.ry hyperparathyroid.sm and.their association with insulin sensitivity and.β-cell secretory function among Congolese type 2 d.abetics. Method.logy: Fasting glycaemia, fasting insulin, 25OH d. and.human parathyroid.hormone (hPTH) were measured.in one hundr.d.and.eighty-four type 2 d.abetic patients followed.as outpatients in South Kivu. Levels of 25OH d. 65 pg/ml d.fined.low vitamin d.and.elevated.parathyroid.hormone levels, respectively. The HOMA mod.l was used.to measure insulin sensitivity and.β-cell secretory function. Results: Med.ans (IQR) were 25.3 (20.4 - 32.4) ng/ml for 25OH d. and.53.7 (38.4 - 115.7) pg/ml for hPTH. 58.7% of d.abetics had.insulin resistance, 126 (68.5%) had.low vitamin d.and.80 (43.5%) had.hyperparathyroid.sm. In multivariate analysis, hPTH (partial r = −0.28;p = 0.0002) and.25OH d. (partial r = 0.16;p = 0.03) showed.an ind.pend.nt association with insulin sensitivity after ad.ustment for bod. mass ind.x and.waist circumference. Finally, hPTH (partial r = 0.27;p = 0.0002) was the sole d.terminant of β-cell secretory function. Conclusions: This stud. confirms the high prevalence of low vitamin d.level and.second.ry hyperparathyroid.sm and.their association with insulin resistance and.impaired.islet β-cell secretory function among Congolese with type 2 d.abetes mellitus. Vitamin d.and.calcium supplementation should.be envisaged.for cases of d.ficiency in this region.

With increasing incid.nce of d.abetes, use of d.abetes specific nutrition supplements (d.NS) is common for better management of the d.sease. To stud. effect of 12-week d.NS supplementation on glycemic markers, anthropometry, lipid.profile, SCFAs, and.gut microbiome in ind.vid.als with d.abetes. Markers stud.ed.were glycemic [Fasting Blood.Glucose (FBG), Post Prand.al Glucose (PPG), HbA1c, Incremental Area und.r curve (iAUC), Mean Amplitud. of Glycemic Excursions (MAGE), Time in/above Range (TIR/TAR)], anthropometry [weight, Bod. Mass Ind.x (BMI), waist circumference (WC)], lipid.profile, d.et and.gut health [plasma short chain fatty acid. (SCFAs)]. N = 210 ad.lts were rand.mized.to receive either d.NS with stand.rd.care (d.NS + SC;n = 105) or stand.rd.care alone (SC alone;n = 105). After 12 weeks, significant d.fferences between d.NS + SC versus SC alone was observed.in FBG [−3 ± 6 vs 14 ± 6 mg/d.;p = 0.03], PPG [−35 ± 9 vs −3 ± 9 mg/d.;p = 0.01], weight [−0.6 ± 0.1 vs 0.2 ± 0.1 kg;p = 0.0001], BMI [−0.3 ± 0.1 vs 0.1 ± 0.1 kg/m2;p = 0.0001] and.WC [−0.3 ± 0.2 vs 0.2 ± 0.2 cm;p = 0.01]. HbA1C and.low-d.nsity lipoprotein (Ld.) were significantly red.ced.in d.NS + SC [−0.2 ± 0.9;p = 0.04 and.−5 mg/d.;p = 0.03] respectively with no change in control. Continuous Glucose Monitoring (CGM) reported.significant d.fferences between d.NS + SC versus SC alone for mean glucose [−12 ± 65 vs 28 ± 93 mg/d.;p d.;p = 0.04], TAR 250 [−3 ± 27 vs 9 ± 38 mg/d.;p = 0.05], iAUC [−192 (1.1) vs −48 (1.1) mg/d.;p = 0.03]. Land.Use Land.Cover Analysis for God.vari Basin in Maharashtra Using Geographical Information System and.Remote Sensing

The d.namic transformation of land.use and.land.cover has emerged.as a crucial aspect in the effective management of natural resources and.the continual monitoring of environmental shifts. This stud. focused.on the land.use and.land.cover (LULC) changes within the catchment area of the God.vari River, assessing the repercussions of land.and.water resource exploitation. Utilizing LANd.AT satellite images from 2009, 2014, and.2019, this research employed.supervised.classification through the Quantum Geographic Information System (QGIS) software’s SCP plugin. Maximum likelihood.classification algorithm was used.for the assessment of supervised.land.use classification. Seven d.stinct LULC classes—forest, irrigated.cropland. agricultural land.(fallow), barren land. shrub land. water, and.urban land.are d.lineated.for classification purposes. The stud. revealed.substantial changes in the God.vari basin’s land.use patterns over the ten-year period.from 2009 to 2019. Spatial and.temporal d.namics of land.use/cover changes (2009-2019) were quantified.using three Satellite/Land.at images, a supervised.classification algorithm and.the post classification change d.tection technique in GIS. The total stud. area of the God.vari basin in Maharashtra encompasses 5138175.48 hectares. Notably, the built-up area increased.from 0.14% in 2009 to 1.94% in 2019. The proportion of irrigated.cropland. which was 62.32% in 2009, d.clined.to 41.52% in 2019. Shrub land.witnessed.a noteworthy increase from 0.05% to 2.05% over the last d.cad.. The key find.ngs und.rscored.significant d.clines in barren land. agricultural land. and.irrigated.cropland. juxtaposed.with an expansion in forest land. shrub land. and.urban land. The classification method.logy achieved.an overall accuracy of 80%, with a Kappa Statistic of 71.9% for the satellite images.
