Minimal perisigmoidal adhesions in cases with normally appearing peritoneum as a sure sign of superficial peritoneal endometriosis

Objective To test for the presence of perisigmoidal adhesions in cases with suspected endometriosis（EMS）.Methods A prospective cohort controlled study was performed. Seventy-four women with no clinical or ultrasonic evidence of ovarian cysts or EMS were included in the study. All cases were admitted to undergo laparoscopy for different indications. In all cases, thorough inspection of pelvic peritoneum was done. Then these patients were divided into two groups. Group A had no perisigmoidal adhesions（15 patients） and group B had mild filmy perisigmoidal adhesions. In both groups, documentations of visible endometriotic lesions, bipolar cauterization of selected parts of uterosacral ligaments, ovarian beds and sides of douglas’ s pouch and histopathology assessment were done in all cases from suspicious, positive endocoaggulation and perisigmoid *** Twenty-seven patients with visible EMS had perisigmoidal adhesions. Thirtytwo patients with normal appearing peritoneum with perisigmoidal adhesions had positive electrocoagulation test in one or more pre-selected parts of the pelvic *** patients with normally appearing peritoneum without perisigmoidal adhesions had negative electrocoagulation test（bipolar cauterization turned white）.Conclusion Presence of mild filmy perisigmoidal adhesions causing tenting of the sigmoid colon could be a strong evidence of the presence of at least minimal or subtle EMS and should warrant careful inspection of the peritoneum cavity before exclusion of EMS.

AIM:To evaluate the long-term treatment outcomes of entecavir monotherapy in treatment naive patients in an Australian tertiary care setting. METHODS:A retrospective analysis of treatment naive patients receiving entecavir monotherapy through Westmead hospital was *** were excluded if they had received previous treatment with another nucleoside or nucleotide analogue,were pregnant or less than 18 years old. RESULTS:Out of 336 patients,163 patients fulfilled the selection *** of follow up was 3-46 mo (mean 26 mo).134 patients(82.2%)had pre-treatment biopsies,with 26 patients(16.0%)demonstrating F3-4 *** total,153 patients(93.9%)achieved at least Partial Virological Suppression(PVS),with 134 patients (82.2%)achieving complete virological suppression. The cumulative CVS and PVS rates at 36 mo were 82.1%and 96.4%,respectively.3 patients(1.8%)failed to achieve PVS,while 5 patients(3.0%)developed virological rebound.128 patients(78.5%)maintained CVS throughout follow *** of CVS included lower baseline DNA level(P=0.001),hepatitis B virus e antigen negative status(P=0.001)and increasing age at treatment(log rank 0.001).No significant adverse effects were reported necessitating cessation of entecavir. Colorectal cancer (CRC) is the 3rd most common cancer in the United States with more than 10000 new cases diagnosed annually. Approximately 20% of patients with CRC will have distant metastasis at time of diagnosis, making them poor candidates for primary surgical resection. Similarly, 8%-25% of patients with CRC will present with bowel obstruction and will require palliative therapy. Emergent surgical decompression has a high mortality and morbidity, and often leads to a colostomy which impairs the patient’s quality of life. In the last decade, there has been an increasing use of colonic stents for palliative therapy to relieve malignant colonic obstruction. Colonic stents have been shown to be effective and safe to treat obstruction from CRC, and are now the therapy of choice in this scenario. In the setting of an acute bowel obstruction in patients with potentially resectable colon cancer, stents may be used to delay surgery and thus allow for decompression, adequate bowel preparation, and optimization of the patient’s condition for curative surgical intervention. An overall complication rate (major and minor) of up to 25% has been associated with the procedure. Long term failure of stents may result from stent migration and tumor ingrowth. In the majority of cases, repeat stenting or surgical intervention can successfully overcome these adverse effects.

目的确定来自于F-脱氧葡萄糖(FDG) PET/CT (PET)显像的肿瘤代谢性参数和放射组学特征是否能预测侵袭性B细胞淋巴瘤患者的疗效和预后。方法经作者单位伦理委员会批准,该回顾性研究纳入82例侵袭性B细胞淋巴瘤患者,行PET显像分期。分析使用不同阈值获得的全身具有代表性的肿瘤部位的代谢肿瘤体积(MTV)和肿瘤放射组学特征,对获取的特征与疗效、无病生存时间(DFS)和总生存时间(OS)进行相关性分析。结果治疗结束后,66例(80.5%)患者完全缓解。与疗效相关的参数是治疗前肿瘤大于6 cm(P=0.026)、治疗后无大于1.5 cm的残余肿瘤(P=0.028)及标准摄取值(SUV)阈值取3和6时的全身MTV(P=0.015和0.009)。纹理特征均未能预测一线治疗方案的疗效,而某些纹理特征如灰阶非均匀性(GLNU)与DFS相关(P=0.013),峰度与OS相关(P=0.035)。结论侵袭性B细胞淋巴瘤患者的全身MTV与疗效相关,而肿瘤的纹理特征不能预测疗效,尽管有几个纹理特征与治疗后残余肿瘤的出现有关,也有某些纹理特征与DFS和OS有关。尚需在更多患者中进行前瞻性研究以确定这些参数的临床预测价值。
