背景：以前的血管内超声（intravascular ultrasound，IVUS）试验证实，他汀治疗可减缓或阻止动脉粥样硬化的进展，但是迄今尚无应用动脉粥样斑块体积百分比（percent atheroma volume，PAV）证实粥样硬化消退的确切证据。PAV是最严格的评价病变进展和消退的IVUS测量指标。目标：评价极高强度他汀治疗是否能逆转IVUS确定的冠状动脉粥样硬化。设计和地点：于美国、加拿大、欧洲和澳大利亚53个社区和3级保健中心进行前瞻性开标盲法终点试验（A Study to Evaluate the Effect of Rosuvastatin on Intravascular Ultrasound-Derived Coronary Atheroma Burden，ASTEROID）。应用马达驱动回撤IVUS评价基线和治疗24个月时冠状动脉粥样斑块负荷。每对基线和随访IVUS测量结果均进行盲法分析。病例：从2002年11月到2003年10月，507例患者有基线IVUS检查结果，并接受至少1个剂量的研究药物。在24个月后，349例患者具有可用于评估的系列IVUS检查结果。干预：所有患者均接受瑞舒伐他汀40ms／d强化治疗。主要观测指标：预先设定了两个一级疗效指标：PAV变化和基线最严重病变10min节段动脉粥样斑块体积变化。二级疗效指标为整个动脉标准化总斑块体积的变化。结果：平均（SD）LDL—C水平由基线时的130．4（34．3）ms／dL降至60．8（20．0）ms／扎，平均下降了53．2％（P〈0．001）。平均（SD）HDL-C水平从基线时的43．1（11．1）ms／dL升至49．0（12．6）ms／dL，平均增加了14．7％（P〈0．001）。整个血管PAV平均（SD）变化为-0．98％（3．15％），中位数为-0．79％（97．5％CI，-1．21％～-0．53％）（与基线比较，P〈0．001）。最严重病变10min节段斑块体积平均（SD）变化为-6．1（10．1）mm^3，中位数为-5．6mm^3（97．5％CI，-6．8～-4．0mm^3）（与基线比较，P〈0．001）。总斑块体积变化中位数降低了6．8％，平均减少了-14．7（25．7）mm^3，中位数为-12．5mm^3（95％CI，-15．1～-10．5mm^3）（与基线比较，P〈0．001）。不良事件少见，与其他他汀试验相似。结论：应用瑞舒伐他汀40ms／d进行极高强度他汀治疗可使LDL-C平均水平达到60．8ms／dL，使HDL—C增加14．7％。这导致所有3个预先设定的IVUS斑块负荷指标均显示动脉粥样硬化消退。因此，将LDL-C降至低于目前指南规定的水平，同时显著提高HDL-C，可以使冠心病患者动脉粥样硬化斑块消退。这些变化对临床预后的影响尚需进一步研究确定。

背景：关于不同脂质测定、非高密度脂蛋白胆固醇（non-HDL-C）、脂质比值、载脂蛋白和C-反应蛋白（CRP）的临床应用，现行心血管危险检测指南仍有争议。目的：直接比较总胆固醇（totalcholesterol，TC）、低密度脂蛋白胆固醇（LDL-C）、HDL-C、non-HDL-C、载脂蛋白A-I（apoA-I）及B100（apoB100）、高敏C-反应蛋白（高敏CRP）以及TC与HDL-C、LDL-C与HDL-C、apoB100与apoA-I、apoB100与HDL-C比值，在妇女作为将来心血管事件预测指标的临床用途。设计、地点及参试者：于15 632例起始健康美国妇女进行的前瞻性队列研究。1992年11月至1995年7月人组时年龄≥45岁（4分位范围48～59岁）。所有参试者均经10年随访，观察将来心血管事件的发生情况。主要观测指标：按照各项生化指标不同基线值而确定的首次主要心血管事件（n=4 764）的风险比（hazard ratios，HRs）及95％可信区间（confidence intervals，CIs）。结果：在校正年龄、吸烟状况、血压、糖尿病及体重指数后，测量结果在最高5分位者将来发生心血管事件的风险比LDL-C为1．62（95％CI，1．17～2．25），apoA-I为1．75（95％CI，1．30～2．38），TC为2．08（95％CI，1．45～2．97），HDL-C为2．32（95％CI，1．64～3．33），apoB100为2．50（95％CI，1．68～3．72），non-HDL-C为2．51（95％CI，1．69～3．72），高敏CRP为2．98（95％CI，1．90～4．67）（P＜0．001，趋势贯穿所有5分位）。脂质比值风险比，apoBl00与apoA-I为3．01（95％CI，2．01～4．50），LDL-C与HDL-C为3．18（95％CI，2．12～4．75），apoB100与HDL-C为3．56（95％CI，2．31～5．47），TC与HDL-C为3．81（95％CI，2．47～5．86）（P＜0．001，趋势贯穿所有5分位）。高敏CRP与脂质参数的相关系数范围为-0．33到0．15，以CRP＜1、1～3和＞3 ms／L为临床分割点，可对每项脂质测量及脂质比值水平增加提供预后危险信息。结论：non-HDL-C以及TC与HDL-C比值对将来心血管事件的预测效果与载脂蛋白相同或者更好。校正年龄、血压、吸烟、糖尿病和肥胖后，高敏CRP提供的预后信息超越各项脂质测量。

背景：他汀治疗的时机和强度会对急性冠脉综合征(acute coronary syndrome，ACS)的临床结局产生何种影响?目前尚缺乏相关评估资料。目的：在ACS患者中比较早期强化他汀治疗与迟发低强度他汀治疗的疗效与安全性。设计、地点和参试者：国际化随机双盲试验。于1999年12月29日～2003年1月6日参与A to Z试验Z阶段研究的ACS患者中，2265例接受辛伐他汀40mg／d，1个月后剂量增至80mg／d；另2232例接受安慰剂，4个月后服用辛伐他汀20mg／d。二者进行比较。主要观察指标：一级研究终点包括心血管死亡、非致死性心肌梗死、ACS再入院以及卒中。随访6～24个月。结果：安慰剂+辛伐他汀组患者服用安慰剂1个月后低密度脂蛋白(low-density ***)胆固醇中值水平为122mg／dL(3．16mmol／L)，服用辛伐他汀20mg／d8个月后LDL胆固醇中值水平为77mg／dL(1．99mmol／L)。单用辛伐他汀组患者服用辛伐他汀40mg／d1个月后LDL胆固醇平均水平为68mg／dL(1．76mmol／L)，服用辛伐他汀80mg／d8个月后LDL胆固醇平均水平为63mg／dL(1．63mmol／L)。安慰剂+辛伐他汀组共有343例患者(占16．7％)出现一级终点事件，而单用辛伐他汀组(40mg／80mg)有309例患者(占14．4％)出现一级终点事件(风险比[HR]，0．89；95％可信区间[CI]，0．76～1．04；P=0．14)。两组中分别有109例(占5．4％)和83例(占4．1％)患者出现心血管死亡(HR，0．75；95％ CI 0．57～1．00；P=0．05)。其他各一级终点事件无差异。前4个月，两组一级终点事件无显著差异(HR，1．01；95％CI，0.83～1.25；P=0．89)，从第4个月起至研究结束，单用辛伐他汀组一级终点事件明显减少(HR，0．75；95％CI，0．60～0．95；P=0．02)。9例(占0．4％)接受辛伐他汀80mg／d治疗的患者发生肌病(肌酸激酶高于正常值上限10倍，同时伴有肌肉症状)，服用低剂量辛伐他汀的患者未出现肌病，1例接受安慰剂治疗的患者发生肌病(P=0．02)。结论：尽管试验未能达到预定终点，但是早期启用辛伐他汀强化治疗有助于减少ACS患者主要心血管事件的发生率。

背景：对于非ST段抬高急性冠脉综合征（acute coronary syndromes，ACS）患者而言，与单独应用普通肝素相比，依诺肝素或者血小板糖蛋白Ⅱb／Ⅲa受体拮抗剂替罗非班与普通肝素联合应用都显示出较好的疗效。目前，尚不清楚依诺肝素和替罗非班联合应用是否像普通肝素和替罗非班标准联合方案一样安全有效。目的：在非ST段抬高的ACS患者中评价依诺肝素和替罗非班联合疗法与普通肝素和替罗非班联合方案的疗效及安全性。设计、地点及参试者：国际前瞻性、开标（open—label）、随机、非劣势（noninferiority）试验。在接受替罗非班和阿斯匹林治疗的非ST段抬高ACS患者中分别给予依诺肝素1mg／kg（n=2026）每12小时一次或经体重校正的静脉普通肝素（n=1961），然后进行比较。这项A to Z试验的A阶段试验是于1999年12月至2002年5月进行的。主要观察指标：意向治疗人群7天时死亡、再发心肌梗死和难治性缺血的发生情况，该意向治疗人群是根据优效性和非劣势原则确定的。应用“心肌梗死溶栓治疗试验”（Thrombolysis in Myocardial Infarction，TIMI）中之出血分级系统，通过监测出血发生率判定用药安全性。结果：在治疗第7天时，随机分配至依诺肝素组的2018例患者中有169例（8．4％）发生死亡、心肌梗死或者难治性缺血，而普通肝素组的1952例患者中有184例（9．4％）发生上述事件（风险比[hazard ratio，HR]为0．88，95％可信区间[confidence interval，CI]为0．71～1．08）。该结果符合预先设定的非劣势标准。除死亡外，所有一级和二级复合终点事件分析结果均提示，依诺肝素更为有益。死亡仅见于1％的患者（依诺肝素组23例，普通肝素组17例）。任何TIMI分级的出血发生率都很低（依诺肝素组为3．0％，普通肝素组为2．2％；P=0．13）。最差情况分析（合并了两种独立的出血判定策略）表明，采用依诺肝素每治疗200例患者就会发生一次TIMI严重出血。结论：对于接受替罗非班和阿斯匹林治疗的非ST段抬高ACS患者而言，依诺肝素是除普通普通肝素之外的另一种适当选择。依诺肝素可使一级终点相对减少12％，绝对减少1％，符合非劣势标准。该结果与以往未联用糖蛋白Ⅱb／Ⅲa受体拮抗剂的试验一致。

Elevated plasma levels of B-type natriuretic peptide(BNP) and N-terminal pro-BNP(NT-pro-BNP) are seen in the setting of cardiac ischemia and are associated with adverse outcomes in patients with coronary artery disease. The mechanisms leading to natriuretic peptide elevation in patients with coronary artery disease, including the contribution of coronary atherosclerosis itself, have not been fully elucidated. Measurement of NT-pro-BNP, electron beam computed tomography, and cardiac magnetic resonance imaging were performed in 2,445 subjects from the Dallas Heart Study who were free of heart failure and renal insufficiency. Electron beam computed tomography determined coronary artery calcium scores were categorized as none(with higher log NT-pro-BNP levels(p=0.03). This association persisted in similar models excluding patients with low LV ejection fractions, LV hypertrophy, angina pectoris, and a history of myocardial infarction. In conclusion, these findings support the hypothesis that coronary atherosclerosis may directly influence the activation of the cardiac neurohormonal system.

AIM To assess the association of resting heart rate with allcause and cardiovascular disease(CVD) mortality in the Diabetes Heart Study(DHS).METHODS Out of a total of 1443 participants recruited into the DHS, 1315 participants with type 2 diabetes who were free of atrial fibrillation and supraventricular tachycardia during the baseline exam were included in this analysis. Heart rate was collected from baseline resting electrocardiogram and mortality(all-cause and CVD) was obtained from state and national death registry. Kaplan-Meier(K-M) and Cox proportional hazard analyses were used to assess the *** The mean age, body mass index(BMI) and systolic blood pressure(SBP) of the cohort were 61.4 ± 9.2 years, 32.0 ± 6.6 kg/m2, and 139.4 ± 19.4 mmHg respectively. Fiftysix percent were females, 85% were whites, 15% were blacks, 18% were smokers. The mean ± SD heart rate was 69.8(11.9) beats per minute(bpm). After a median follow-up time of 8.5 years(maximum follow-up time is 14.0 years), 258 participants were deceased. In K-M analysis, participants with heart rate above the median had a significantly higher event rate compared with those below the median(log-rank P = 0.0223). A one standard deviation increase in heart rate was associated with allcause mortality in unadjusted(hazard ratio 1.16, 95%CI: 1.03-1.31) and adjusted(hazard ratio 1.20, 95%CI: 1.05-1.37) models. Similar results were obtained with CVD mortality as the outcome of *** Heart rate is an independent predictor of all-cause mortality in this population with type 2 diabetes. In this study, a 1-SD increase in heart rate was associated with a 20% increase in risk suggesting that additional prognostic information may be gleaned from this ubiquitously collected vital sign.

AIM:To study if impaired renal function is associated with increased risk of peri-infarct heart failure (HF) in patients with preserved ejection fraction (EF).METHODS:Patients with occluded infarct-related arteries (IRAs) between 1 to 28 d after myocardial infarction (MI) were grouped into chronic kidney disease (CKD) stages based on estimated glomerular filtration rate (eGFR).Rates of early post-MI HF were compared among eGFR *** regression was used to explore independent predictors of ***:Reduced eGFR was present in 71.1% of 2160 patients,with significant renal impairment (eGFR was higher with worsening renal function:15.5%,17.8% and 29.4% in patients with CKD stages 1,2 and 3 or 4,respectively (P was again higher with worsening renal function among patients with preserved EF:10.1%,13.6% and 23.6% (P was not significant among patients with depressed EF:27.1%,26.2% and 37.9% (P= 0.071).Moreover,eGFR was an independent correlate of HF in patients with preserved EF (P=0.003) but not in patients with depressed EF (P=0.181).CONCLUSION:A significant proportion of post-MI patients with occluded IRAs have impaired renal *** renal function was associated with an increased rate of early post-MI HF,the association being strongest in patients with preserved *** findings have implications for management of peri-infarct HF.
