Objective Current commercially available immunological tests cannot be used for discriminating active tuberculosis(TB)from latent TB *** evaluate the value of biomarker candidates in the diagnosis of active TB,this study aimed to identify differentially expressed genes in peripheral blood mononuclear cells(PBMCs)between patients with active TB and individuals with latent TB infection by transcriptome *** The differentially expressed genes in unstimulated PBMCs and in Mycobacterium tuberculosis(Mtb)antigen-stimulated PBMCs from patients with active TB and individuals with latent TB infection were identified by transcriptome *** candidate genes were evaluated in cohorts consisting of 110 patients with TB,30 individuals with latent TB infections,and 50 healthy controls by quantitative real-time *** operating characteristic(ROC)curve analysis was performed to calculate the diagnostic value of the biomarker *** Among the differentially expressed genes in PBMCs without Mtb antigen stimulation,interferon-induced protein with tetratricopeptide repeats 3(IFIT3)had the highest area under curve(AUC)value(0.918,95%CI:0.852-0.984,P<0.0001)in discriminating patients with active TB from individuals with latent TB infection,with a sensitivity of 91.86%and a specificity of 84.00%.In Mtb antigen-stimulated PBMCs,orosomucoid 1(ORM1)had a high AUC value(0.833,95%CI:0.752-0.915,P<0.0001),with a sensitivity of 81.94%and a specificity of 70.00%.Conclusion IFIT3 and ORM1 might be potential biomarkers for discriminating active TB from latent TB infection.

hiv testing among persons with tuberculosis (TB) results in high-yield identification of persons infected with hiv. To evaluate differences in hiv testing among children versus adults with TB in Vietnam, we collected and analyzed age-disaggregated facility and aggregated provincial data from the National tuberculosis Program. hiv testing was incompletely documented for >70% of children, whereas adult testing data were >90% complete. Standardized training of personnel for universal hiv testing and documentation for children with TB could improve hiv case-detection and permit linking of children with hiv to antiretroviral treatment to prevent morbidity and mortality.

Objective To investigate the risk factors attributable to tuberculosis-related deaths in areas with human immunodeficiency virus（hiv） infection epidemics. Methods A prospective cohort study of newly registered patients in tuberculosis（TB） dispensaries in six representative Chinese provinces was conducted from September 1, 2009 to August 31, 2011. Risk factors for TB-associated death were identified through logistic regression analysis. Results Of 19,103 newly registered pulmonary TB patients, 925（4.8%） were found to be hiv-positive. Miliary TB and acid-fast bacillus smear-negative TB were more common among these patients. Out of a total of 322（1.7%） deaths that occurred during TB treatment, 85（26%） of the patients were co-infected with hiv. Multivariate analysis revealed that hiv infection was the strongest predictor of death [adjusted odds ratio（aO R） 7.86]. Other significant mortality risk factors included presentation with miliary TB（aO R 4.10; 95% confidence interval： 2.14-7.88）, ≥35 years of age（aO R 3.04）, non-Han ethnicity（aO R 1.67）, and farming as an occupation（aO R 1.59）. For patients with TB/hiv co-infection, miliary TB was the strongest risk factor for death（aO R 5.48）. A low CD4 count（≤ 200 cells/μL）（aO R 3.27） at the time of TB treatment initiation and a lack of antiretroviral therapy（ART） administration（aO R 3.78） were also correlated with an increased risk of death. Conclusion Infection with hiv was independently associated with increased mortality during TB treatment. Offering hiv testing at the time of diagnosis with TB, early TB diagnosis among hiv/acquired immunodeficiency syndrome patients, and the timely provision of ART were identified as the key approaches that could reduce the number of hiv-associated TB deaths.

After coronavirus disease 2019(COVID-19),previously known as pneumonia of unknown etiology(PUE)and 2019 novel coronavirus(2019-nCoV),was first discovered in Wuhan City,Hubei Province,the Chinese government took measures to stop the spread of the epidemic nationwide and interrupted local transmission in China(1–2).Although there have been several localized outbreaks that were caused by overseas importation of the virus—including Heilongjiang,Jilin,Beijing,Liaoning,and other provincial-level administrative divisions(PLADs)—these outbreaks have been stopped within two to four weeks and were limited in scope to less than ten cases or a few hundred cases for each *** the COVID-19 pandemic still uncontrolled globally,the threat of importing the virus continues unabated and still threatens China’s population,virtually all of which is susceptible.

Rationale: Excess alcohol use (EAU) is associated with adverse TB treatment outcomes. Objective: We investigated the relationship between EAU and death among TB patients 15 years and older prescribed anti-TB treatment in the United States. Design: Using data reported to the National tuberculosis Surveillance System for 1997-2012, we calculated adjusted odds ratios and excess attributable risk percent for death among TB patients with reported EAU. Results: EAU was associated with death among patients younger than 65. The excess attributable risk percent for death among those with reported EAU for those younger than 65 was >35%. Conclusions: Interventions to reduce EAU in patients <65 years may reduce deaths.
